2.CT Findings of Cervical Lymphadenopathy: Differential Diagnosis.

PURPOSE: To evaluate the characteristics of CT findings in differential diagnosis of cervical lymphadenopathy. MATERIALS AND METHODS: We evaluated CT findings of histopathologically proved 32 cases of tuberculous lymphadenitis, 12 cases of nodal lymphoma, 29 cases of metastasis from March 1986 to Dec. 1992, retrospectively. We analyzed age and sex distribution, location of lymphadenopathy, internal nodal density, feature of contrast enhancement, extracapsular spread and nodal calcification. RESULTS: Tuberculous lymphadenitis was more preponderant in young female (mean;32) and CT showed internal low density(91%) with irregular thick marginal enhancement(46.9%), predilection for spinal accessory node(71.9%), and nodal calcification(5/32). Nodal lymphoma showed predilection for internal jugular chain (100%) and had conglomerated homogeneous internal node structures(83%). Metastasis showed old male preponderance (mean :52 years), predilected at internal jugular chain(89.7%),internal low density(93.3%) with regular thin marginal enhancement(44.8%), extracapsular spread(41.4%), and nodal calcification(2 cases of nodal metastasis of papillary type thyroid cancer). CONCLUSION: We concluded that CT evaluation of the chracteristics of lymphadenopathy is helpful for differential diagnosis of cervical lymphadenopathy.

3.Intervention effects of tiaozhi jiangtang tablet on insulin resistance in rats with diabetes mellitus type 2.

OBJECTIVETo observe the effect of Tiaozhi Jiangtang Tablet (TJT) on insulin resistance (IR) in rats with diebetes mellitus type 2.
METHODSThe model rats of diebetes mellitus were induced by intraperitoneal injection of streptozotocin (30mg/kg) and feeding with high lipid forage. The rats in the TJT group were treated with TJT and those in the metformin group treated with metformin as positive controls. The glucose infusion rate (GIR) was detected by glucose clamp technique after treatment for 8 weeks. At the same time, fasting blood glucose ( FBG), fasting insulin ( FINS), free fatty acids ( FFA), total cholesterol ( TC), triglyceride (TG), and high-density lipoprotein cholesterol (HDL-C) were measured respectively, and insulin sensitivity index (ISI) and HDL-C/TC calculated. The changes of insulin sensitivity and lipid metabolism were evaluated.
RESULTSTC, TG, HDL-C/TC, FINS, and FFA significantly reduced in the TJT group as compared with those in the control group, while ISI and GIR significantly increased, the effects of TJT were similar to those of metformin.
CONCLUSIONTJT is effective in increasing insulin sensitivity and improving glucose and lipid metabolisms in rats with diebetes mellitus type 2.

4.A STUDY ON THE EFFECT OF THERMOCYCLING TO THE PHYSICAL PROPERTIES OF DENTURE LINERS.

For the improvement of denture fitness of changed residual ridge, denture liner can be used. Denture liner should be very stable physically in various environments of the mouth as well as be bonded strongly with denture resin. In this study, the specimens bonded with four kinds of soft denture liner and three kinds of hard denture liner were used to test the physical properties of the liners. All experimental groups were stored in 37+/-1degrees C distilled water for 24hours, followed by thermocycling between 15degrees C and 45degrees C with 15 second dwell time. 1000, 2000, 3000 cycles of thermocycling were excuted and physical properties were measured by Instron Universal Testing Machine. The obtained results were as follows: 1. In tensile bond test of soft liners, it was shown that both of Molloplast-B(R) specimens before and after thermocycling had the highest tensile strength, and in case of hard liners, Dura-liner II(R)speimen had the highest tensile strength before and after thermocycling. Depending on thermocycling, Soft-Relining(R), Denture-Relining(R),Molloplast-B(R), Coe-soft(R) and Kooliner(R) specimen showed significant difference(p<0.05). 2. In strain test of soft liners, it was shown that Molloplast-B(R) specimen before thermocycling and the Coe-soft(R) after thermocycling showed highest results, and in case of hard liners, the Dura-Liner II(R) speimen before and after thermocycling had the highest result. Depending on thermocycling. Denture-Relinig(R). Molloplast-B(R) and Dura-Lner II(R) specimen showed significant difference(p<0.05). 3. In Maximum distance test of soft liners, the Molloplast-B(R) specimen before thermocycling and the Coe-soft(R) after thermocycling showed highest results, and in case of hard liners, the Dura-Liner II(R) specimen before and after thermocycling showed highest results. Depending on thermocycling, Denture-Relining(R), Molloplast-B(R) specimens showed significant difference(p<0.05). 4.In elasticity test of soft liners , the Molloplast-B(R) specimen before and after thermocycling showed highest results, and in case of hard liners, the Dura-Liner II(R) specimen before thermocycling and the Tokuso-Rebase (R) after thermocycling showed highest results. Depending on thermocycling, Soft-Relining(R) , Molloplast-B(R) specimens showed significant difference(p<0.05).

5.Familial Lichen Planus.

Familial lichen planus is an uncommon disease, and there are 104 cases reported in the English literature. Several theories regarding the etiopathogenesis of lichen planus have been proposed. However, immunological mechanisms and genetic susceptibility have been emphasized the most. Several reports showed HLA analysis in lichen planus patients, and recently 2 cases have revealed an increased frequency of DR1 antigen in patients with lichen planus compared to a control group. We observed two brothers with typical skin lesions of lichen planus of almost simultaneous onset. We confirmed lichen planus by skin biopsies in both brothers and analyzed HLA typing. We also observed the occurrence of DR1 antigen in both of them.

6.The Factors Influencing Glass Particles in Single Dose Glass Ampules upon Opening.

PURPOSE: The purpose of this study was to examine the factors influencing glass particle contamination in single dose glass ampules upon opening. METHOD: The study was single case experimental design. Different methods of opening ampule (hand, wooden stick), different sizes of ampules(1cc, 2cc) and different sizes of needles(17gauge, 23gauge, 5micrometer filter) were evaluated. Eighteen ampules were randomly assigned in each group. The number of glass ampule particles > or =10micrometer was counted by microscope. RESULTS: There was no significant difference in the number of particles aspirated by opening methods. But number of glass particles was much lower when using 1cc ampules rather than 2cc ampules and was also much lower when using smaller size needles and needles which include a 5micrometer filter rather than larger size of needles. CONCLUSION: We suggest that larger bore or unfiltered needles increase the risk of aspirating more glass particles than smaller bore or filter needles. In addition, these data show that a wooden stick can be used as a method opening glass ampules.

8.Feasibility of Laparoscopic Surgery for Intussusception in Pediatric Patients and Risk of Bowel Resection.

PURPOSE: Intussusception is a common cause of intestinal obstruction in children. While most patients can be treated by enema reduction, about 20% require surgery. We investigated the usefulness and feasibility of laparoscopic surgery and the intraoperative risk of bowel resection. METHODS: We retrospectively reviewed pediatric patients who underwent surgery for intussusception from 2010 to 2017. We collected data for age, gender, body weight, associated symptoms, duration of symptoms, white blood cell count, operating time, and postoperative complications. RESULTS: Of 155 patients, 37 (23.8%) underwent surgery due to enema reduction failure in 29 (78.3%), recurrence in 6 (16.3%), a suspicious lead point in 1, and suspicious ischemic change observed on ultrasonography in 1. The mean age was 26.8±18.9 months (range, 3.5~76.7 months), and the mean body weight was 12.9±3.9 kg (range, 5.4~22.2 kg). Laparoscopic surgery was successful in 29 patients (78.4%), and 7 (18.9%) needed bowel resection and anastomosis. The mean operating time was 56.7±32.8 min. A lead point was found in 3 patients in the bowel resection group (p=0.005); in addition, the operating time and hospital stay were longer in this group. There were no intra- or postoperative complications. CONCLUSION: Laparoscopic surgery was successful in 78.4% of the patients with a short hospital stay and early oral intake. The only predictive factor for bowel resection was the presence of a lead point. Laparoscopic surgery may be an optimal treatment intervention for children with intussusception, except for those who show initial peritonitis.

9.Multicentric randomized double blinded clinical study of Yiqi Tongmai Oral Liquid against angina pectoris in patients with coronary heart disease

OBJECTIVE: To study the efficacy and safety of Yiqi Tongmai Oral Liquid (YQTM), a traditional compound Chinese herbal medicine, in treating angina pectoris in patients with coronary heart disease. METHODS: A multicentric, randomized, double blinded and paralleled controlled trial was conducted on 110 patients in trial group treated with YQTM, and 109 patients in control group treated with Shuxin Oral Liquid (SX). Cure and effective rates in both groups were evaluated. Frequency and duration of angina attack were counted and measured. Coronary angiography (CAG), electrocardiogram (ECG) and flat exercise test were taken in both groups. Blood lipid indexes, such as cholesterol (CH), triglyceride (TG), low-density lipoprotein (LDL), high density lipoprotein (HDL), were determined at pre- and post-treatment. The hemodynamic indexes, such as whole blood viscosity (J2), high-shear reduced viscosity (Eh), low-shear reduced viscosity (Ei), red cell aggregation index (Lb), red cell rigidity index (Rh), fibrinogen (Fb), blood sedimentation rate (BSR) and hematocrit (HCT), were determined at pre-and post-treatment. The indicated scores of symptoms and signs of traditional Chinese medicine (TCM) pattern, such as chest pain, chest constriction, breath shortness, palpitation, fatigue, dim complexion, spontaneous perspiration and tongue proper, tongue coating were evaluated in week 0, 1, 2, 3, 4 during the treatment course. The safety indexes, such as body temperature, pulse, respiration and blood pressure were observed. Routine tests of blood, urine and stool, hepatic function test and renal function test were taken at pre- and post-treatment. RESULTS: There was no significant difference between the total effective rate of the trial group and that of the control group, which were 91.82% and 85.32%, respectively (P>0.05). Trial groups percentile of cure rate is significantly higher than that of the control group (P<0.01). The frequency and duration of angina attack, the positive ratio of CAG and flat exercise test of both groups were lowered, while the effect of the trial group on frequency and duration of angina attack was better. No significant difference was found in ECG features between the two groups (P>0.05). The levels of CH, TG and LDL of both groups were lowered significantly (P<0.05). The effect of lowering CH, TG and LDL of the trial group was stronger than that of the control group (P<0.05). The hemodynamic indexes, such as J2, Eh, Ei, Lb, Rh, Fb, BSR and HCT were improved significantly in both groups (P<0.05). The improvements of J2, Eh, Ei, Lb, Rh, Fb and SR in the trial group were greater than those of control group (P<0.05). The TCM symptoms and signs, such as chest pain, chest constriction, breath shortness, palpitation, fatigue, dim complexion, spontaneous perspiration were improved significantly in both groups (P<0.05). The improvements of chest constriction, palpitation, fatigue and spontaneous perspiration in the trial group were greater than those of the control group (P<0.05). The total indicated score of TCM symptoms and signs was lowered more significantly than that of the control group (P<0.01). No significant changes were found at pre- and post-treatment in safety indexes, such as routine tests for blood, urine and stool, hepatic function test and renal function test. There was no significant difference in safety features of both groups (P>0.05). CONCLUSION: Yiqi Tongmai Oral Liquid bears good therapeutic effect on angina pectoris without adverse reaction, and is superior to Shuxin Oral Liquid. Yiqi Tongmai Oral Liquid is a new effective and safe medicine for the treatment of angina pectoris in patients with coronary heart disease.
10.Kimura's Disease Involving the Ipsilateral Face and Extraocular Muscles.

Kimura's disease (KD) is a rare, chronic inflammatory disorder, which is characterized by tumor-like masses mainly located in the head and neck region. Extraocular muscle involvement in KD is uncommon. We report a case of KD that involved both the extraocular muscles and buccal area. A 13-year-old male presented to our clinic with a two-year history of exophthalmos of the left eye and facial swelling. Facial CT and MRI showed a 1.5 x 1.5 cm2 soft tissue mass located at the left masticator and buccal area, exophthalmos of the left eye, and diffuse thickening of the left extraocular muscles. We performed a lateral rectus muscle incisional biopsy of the left eye. Oral methylprednisolone therapy was initiated and tapered following the incisional biopsy.
