1.Clinical Analysis of Diabetic Gangrene

The angiopathy and neuropathy is the basic mechanism causing diabetic gangrene, but the exact mechanism is still unknown. The foot is especially susceptible to diabetic complication of angiopathy and neuropathy. In such a vulnerable foot, trivial trauma may quickly lead the foot to ulceration, infection, gangrene, and to the cataclysmic amputation. Twenty nine patients with diabetic gangrene were studied, the conclution were as follow. 1. Overall incidence is 1.8%, most common age over 50 years of age, and the most common disease durstion is from 10 to 14 years. 2. The most common site is toe (56% of cases). 3. The most common predisposing factor was local pressure (41% of cases), the radiographic bone change was seen in 14% of cases. 4. The FBS level is from 200 to 300 in 35% of cases. 5. Bacterial infection was seen in 80%, and staphylococcal aureus was most common. 6. The surgical or conservative treatment were effective in 80% of cases and mortality was 14%. 7. Diabetic retinopathy was most common associated complication (47% of cases).

2.Development and Evaluation of a Web-based Education Program for Nursing Students on Control of Vancomycin-resistant Enterococcus Infection.

PURPOSE: The purpose of this study was to develop a web-based education program on control vancomycin-resistant enterococci (VRE) infections and to identify the effects of the program on knowledge and performance of nursing students. METHODS: The web-based VRE infection control education program was developed by using the network-based instructional systems design model. The nursing students in the experimental group could access this web-based education program at any time, and as many times as they wanted, during the clinical training period. Effects were evaluated by assessing knowledge and performance of VRE infection control measures during the clinical training period. RESULTS: The contents of the education program included diagnosis, transmission, and treatment of VRE, contact precautions, hand washing, personal protective equipment, environment management, and quizzes. The lecture portion was filmed in a virtual screen studio using flash animation, video, and sound effects, and it was uploaded on an internet site. The knowledge and performance scores of the experimental group after using the education program were significantly higher than those of the control group. CONCLUSION: The results suggest that the web-based VRE infection control education program is an effective educational method to enhance knowledge and performance of VRE infection control measures.

4.Quercetin ameliorates glutamate toxicity-induced neuronal cell death by controlling calcium-binding protein parvalbumin

Background:
Glutamate is the main excitatory neurotransmitter. Excessive glutamate causes excitatory toxicity and increases intracellular calcium, leading to neuronal death. Parvalbumin is a calcium-binding protein that regulates calcium homeostasis. Quercetin is a polyphenol found in plant and has neuroprotective effects against neurodegenerative diseases.
Objectives:
We investigated whether quercetin regulates apoptosis by modulating parvalbumin expression in glutamate induced neuronal damage.
Methods:
Glutamate was treated in hippocampal-derived cell line, and quercetin or vehicle was treated 1 h before glutamate exposure. Cells were collected for experimental procedure 24 h after glutamate treatment and intracellular calcium concentration and parvalbumin expression were examined. Parvalbumin small interfering RNA (siRNA) transfection was performed to detect the relation between parvalbumin and apoptosis.
Results:
Glutamate reduced cell viability and increased intracellular calcium concentration, while quercetin preserved calcium concentration and neuronal damage. Moreover, glutamate reduced parvalbumin expression and quercetin alleviated this reduction. Glutamate increased caspase-3 expression, and quercetin attenuated this increase in both parvalbumin siRNA transfected and non-transfected cells. The alleviative effect of quercetin was statistically significant in non-transfected cells. Moreover, glutamate decreased bcl-2 and increased bax expressions, while quercetin alleviated these changes. The alleviative effect of quercetin in bcl-2 family protein expression was more remarkable in non-transfected cells.
Conclusions
These results demonstrate that parvalbumin contributes to the maintainace of intracellular calcium concentration and the prevention of apoptosis, and quercetin modulates parvalbumin expression in glutamate-exposed cells. Thus, these findings suggest that quercetin performs neuroprotective function against glutamate toxicity by regulating parvalbumin expression.

7.A Survey on Smoking of Adolescence.

PURPOSE: This study is to investigate the smoking experience and its related factors among adolescence. METHOD: The subject for this study consisted of 2711 students of middle, high school located in youngam-koon, chonnam. The data were collected by an anonymous questionnaire during the period from March. 1, 2001 to June. 20, 2001. RESULT: The frequency of cigaret smoker was 8%. The frequency of smoker in the high school was higher than that of smokers at the middle school. The motivation of smoking was curiosity and solicitation by friends, and the level of smoking was on average less than 10 cigarettes a day. The 72.2% of smokers started during the middle school period. Students who smoke had more positive attitude toward smoking than students who don't smoke. There were significant differences in there knowledge about the harmful effect of smoking between nonsmoker and smoker. Most smoking students thought that nonsmoking education is necessary for adolescents, even though they don't want to quit the smoking. The frequency of smoker was different between genders. CONCLUSION: An understanding of the characteristics of smoking of adolescence may provide guidance for preventive interventions with adolescents.

8.Protecitve Effect of Breast Feeding for Infection.

A Comparative study was retrospectively performed with 374 infants in order to find protective effect of breast feeding for infection. This study shows that breast feeding protect infant against gastrointestinal and respiratory infection. Breast feeding offers advantage by reducing gastraintestinal and respiratory infection when they should maintain breast feeding for at least three months. 1) First year morbidity increased with extent of artificial feeding and was nearly two fold for babies not breast-fed. 2) Prescence of older sibling, low birth weight and male sex are associated with increased morbidity. 3) Infant illness from 0~3 months, 4~6 months, 7~10 months, 11~12 moths was analyzed. Compared with the 108 bottle fed babies, the babies who had been partially (40) or fully (136) breast fed had significantly lower rate of gastrointestinal illness at 7~10 month (p<0.05), 11~12 month (p<0.005) and respiratory illness at 7~10 month (p<0.05). 4) Between the early weaning group (90) and the bottle group (108) showed no siggnificant difference. 5) Among the babies observed throughout the first year of life, 35 (9%) and 46 (12) were admitted to hospital with gastrointestinal and respiratory infection. Babies who were breast fed for more than 3 month had significant lower rate for hospital admission due to gastrointestinal and respiratory infection when compared with bottle fed babies. Breast feeding durning more than 3 month of life confers protection against gastrointestinal and respiratory infection.

9.Use of Tc-99m Diisopropyl Iminodiacetic Acid (Tc-99m DISIDA) Scintigraphy for a Noninvasive Estimate of Bile Reflex after Gastric Operations.

BACKGROUNDS:Bile reflux gastritis can occur when pylorus ablation is associated with bile stasis in the stomach. It can also occur with a gastrojejunostomy when bile is continuously poured into the gastric remnant after a vagotomy and an antrectomy. The diagnosis of bile reflux gastritis can be made only when the patient has bile gastritis documented on biopsy; the simple observation of a bile-stained mucosa in a gastric remnant is not sufficient to make the diagnosis of bile reflux gastritis. METHODS: Technetium-99m diisopropyl iminodiacetic acid (Tc-99m DISIDA) scintigraphy was used to study bile reflux into the gastric remnant in 31 patients with gastric operations. All patients had gastrofibroscopic biopsies in order to identify the bile reflux gastritis. RESULTS: Tc-99m DISIDA Scintigraphy identified bile reflux in 15 (83.2%) of 18 patients after a subtotal gastrectomy and a Billroth II gastrojejunostomy. Hewever, no bile reflux occured in either the 10 patients with a hemigastrectomy plus Billroth I gastoduodenostomy or the 3 patients with a truncal vagotomy plus pyloroplasty. Also, gastrofibroscopic biopsies identified bile reflux gastritis in only 3 patients (9.7%) with a subtotal gastrectomy plus Billroth II reconstruction. CONCLUSIONS: The patients who underwent a subtotal gastrectomy and Billroth II reconstruction showed higher bile reflux rates than did the patients who underwent a hemigastrectomy plus Billroth I reconstruction and a truncal vagotomy plus pyloroplasty (p<0.05). Also, only 9.7% of the postgastrectomy patients developed bile reflux gastritis.

10.One Case of Tracheopathia Osteoplastica.

Tracheopathia osteoplastica is a rare, benign pathologic condition chahacterized the occurrence of numerous cartilage and bony nodules within the tissue of mucosa. We report a case of tracheopathia osteoplastica that involved both larynx and trachea. The patient was 73 years old male and had hoarseness. The neck CT showed thickened and irregular wall of the trachea extending to the level of the carina with multiple calcified nodules and plaques of bony density projecting into the lumen.
