1.A Peterson's hernia and subsequent small bowel volvulus: surgical reconstruction utilizing transverse colon as a new Roux-en-Y limb - 1 case.

Peterson's hernia is an internal hernia that can occur after Roux-en-Y anastomosis. It often accompanies small bowel volvulus and is prone to strangulation. Reconstruction of intestinal continuity after massive small bowel resection in a patient who undergoes near total gastrectomy and Roux-en-Y anastomosis can be difficult. A 74-year-old man who had undergone a near total gastrectomy and Roux-en-Y gastrojejunostomy for stomach cancer presented with abdominal pain. The preoperative computed tomography showed strangulated small bowel volvulus. During the emergent laparotomy, we found a strangulated Peterson's hernia with small bowel volvulus. After resection of the necrotized intestine, we made a new Roux-en-Y anastomosis connecting the remnant stomach and the jejunum with a transverse colon segment. We were safely able to connect the remnant stomach and the jejunum by making a new Roux-en-Y anastomosis utilizing a transverse colon segment as a new Roux-limb by two stage operation.

2.Appendiceal Mucocele with Lower Gastrointestinal Bleeding.

The appendiceal mucocele is very rare disease of 0.2% incidence. About 24% of patients are asymptomatic and symptomatic patients present with pain in the right lower quadrant of abdomen in 64%, plapable maas in the right lower quadrant of abdomen in 50%, and rarely, melena, hematochezia, anemia, diarrhea, malaise, and abdominal distension. The gastrointestinal bleeding may be presented in the patient with intussusception, but the massive bleeding is generally absent. We report a case of appendiceal mucocele accompanying with gastrointestinal bleeding and review of literature.

3.Concurrent Malignant Lymphoma of the Colon and Small Bowel as the Primary Origin.

Primary gastrointestinal lymphoma primarily confined to gastrointestinal tract is relatively rare diaease. Although the lymphoma primarily involved colorectum or small bowel was commonly reported than primary gastric lymphoma in Korea, the concurrent primary lymphoma of colon and small bowel is very rare in reported cases. We report a case of combined primary malignant lymphoma of the jejunum and cecum, who was admitted due to melena and anemia. She had right hemicolectomy and end-to-side ileotransverse colostomy at 2 months ago, due to primary colon lymphoma. The laparotomy was performed and identified hard, 2 x 3 cm sized masses on distal 25 cm, 80 cm and 150 cm from the Treitz ligament. The histology of these small ma showed histiocytic lymphoma as same as cecal specimen. In general, the small bowel lymphoma shaws poor prognosis than gastric or colorectal lymphoma due to diffieulty in diagnosis and late symptoms, but this problem could be resolved through the knowledge about primary gastrointestinal lymphoma and the development of diagnostic methods.

5.A Clinicopathologic Characteristics and Survival Analysis of Ovarian Tumors.

OBJECTIVE: The purpose of this study was to identify the clinicopathologic characteristics, treatments and overall 5-year survival and to analyze the prognostic factors affecting the survival in patient with ovarian tumors. METHODS: We reviewed retrospectively the outcome of 116 patients with ovarian cancer and 64 patients with borderline malignant ovarian tumor, who had been treated at Department of Obstetrics and Gynecology, Korea University Anam Hospital from Jan. 1991 to Dec. 2001. Univariate and multivariate analysis for survival were conducted to test the prognostic significance of several clinicopathologic factors. RESULTS: The mean age of the patients with borderline malignant ovarian tumor was 39.9 years old, and nulliparity was most common (42.2%). Mucinous tumor was the most common histologic subtype (68.8%). The FIGO stage distribution of borderline malignancy were 89.1%, 1.6%, 9.3% for stage I, II, and III, respectively. The overall 5-year survival rate of patients with borderline malignancy was 98.4%. The mean age of the patients with ovarian cancer was 50.1 years old. The incidence of ovarian cancer according to histologic type were 74.1%, 12.9%, 11.2%, 0.9%, 0.9%, in epithelial ovarian cancer, Sex-cord stromal tumor, malignant germ cell tumor, metastatic carcinoma of the ovary, and small cell carcinoma, respectively. The FIGO stage distribution of ovarian cancer were 40.5%, 12.1%, 42.2%, 5.2% for stage I, II, III, and IV, respectively. The overall 5-year survival rate of patients with ovarian cancer was 60.9%. In multivariate analysis, FIGO stage was identified as a significant independent prognostic factor in this study. CONCLUSION: In this study, patients with borderline malignancy of the ovary have good prognosis and the overall 5-year survival rate was 98.4%. Otherwise, the overall survival rate of patients with ovarian cancer was 60.9% and the stage was most important prognostic factor.

6.Mutagenic Activity of Organic Pollutans in Drinking Water in Seoul.

To measure the mutagenic activity of micro-organic pollutants in drinking water, mutagenic test was conducted using Salmnella typhimurium TA 98 strain on the water sample taken from three water supply station and six tap water in Seoul in July and November 1987. The results were as follows : 1. The average amounts of organic matters in raw, treated, and tap water sampled in July were 0.38mg/l, 0.28mg/l, and 0.45mg/l, respectively, and sampled in November were 0.34mg/l, 0.24mg/l, and 0.22mg/l, respectively. The amount of organic matters of tap water sampled in November did not increase while that of tap water sampled in July increased compare to those of raw or treated water. 2. The amount of organic matters is the highest in neutral fraction compare to acidic and basic fractions. 3. In the five out of six tap water and raw water of Paldang and Kuui station sampled in July, the mutagenic ratios were greater than two (both direct and indirect mutagenicity). 4. In the three out six tap water and raw and treated water of Kuui station sampled in November, the mutagenic ratios were greater than two. 5. While mutagenic were low in acidic and basic fraction, they were high in neutral fraction. The samples which had high mutagenic activity in the total amount also showed high mutagenic activity in neutral fraction. 6. While mutagenic activity was decreased after the treatment of water, it was increased in tap water as the distance from the water supply station increases.

9.The Echocardiographic Estimation of Left Ventricle Function in the Congestive Heart Failure after Vasodilator(Prazosin) Therapy.

We studied the left ventricular function with the use of M-mode and 2-D Echocardiography to 32 patients with congestive heart failure before and 2 weeks after medication of prazosin 2 mg tid during July 1979 and Dec. 1980 in Heart Center, Paik Hospital, In-Je Medical College, Busan, Korea. The results were following; 1) LVDeD before and after vasodilaor therapy was each 5.64+/-0.76 and 5.04+/-0.57.cm, LVSeD before and after therapy each 4.52+/-0.51 and 3.70+/-0.57cm, and F.S. before and after therapy 19.87+/-3.66 and 26.59+/-6.75%. The difference of LVSeD, LVDeD and F.S. was each statistically significant. 2) LVDeV before and after therapy was each 182.2+/-58.07 and 132.80+/-47.01 ml LVSeV before and after therapy each 92.35+/-23.60 and 50.65+/-27.21ml, and E.F. before and after therapy each 49.31+/-7.07 and 61.86+/-11.74%. The difference of LVDeV, LVSeV and E.F. was each statisitically significant. 3) LVPWTh before and after therapy was each 0.89+/-0.11 and 0.77+/-0.08cm, and Vcf before and after therapy 0.83+/-0.11 and 1.03+/-0.27 cir./sec. The difference of LVPWTh and vcf before and after therapy was each statistically significant. 4) The difference of VSTh, RVD, LAD, and Aorta Dimension before and after therapy was each statisitically significant.
