2.Changes of the Renal Arteries Accordding to Various Embolic Materials.

The transarterial embolization has been widely used to control bleeding. It has a variety of clinical utility; to reduce bleeding on the surgical field, to reduce the size of malignant tumor as a preopearative treatment, to treat arteriovenous malformation or arterial aneurysm as a curative method and to promote life qua' ity of patient with diffuse or multiple hepatocellular carcinoma as a palliative treatment, etc. With the advance of modem technology, various embolic materials have been also developed. IIowever, it has not- been -fully investigated of histopathologic changes of the embolized organs according to the embolic materials used. This study was undertaken to investigate the histopathologic changes of embolized renal artery in rabbit by various embolic materials, according to each embolic material and to time passed by after embolization. Of the 5 arteries embolized by ethylene vinyl alcohol copolymer(EVAL), one showed abscess formation in embolized kidney. The other 4 allowed to perform further pathologic study: within a week after embolization there was no any specific change in vessels, however, r,unimal endothelial hypertrophy was observed following 2 weeks of embolization. Of the 8 renal arteries embolized by N-buthyl-2-cyanoacrylate(Histoacryl), 4 showed total occlusion of the main renal arteries as well as renal infarction, which reflects the strong adhesiveness of Histoacryl to vascular wall. 3.Endoscopic Variceal Ligation for Treatment of Bleeding Esophageal Varices.

The endoscopic variceal ligation(EVL) has been adopted as a new treatment for acute hemorrhage from esophageal varices that ranks the highest mortality rate in upper gastrointestinal tract bleedings. This treatment method has good effects for the urgent treatment and eradication of varices from acute variceal bleeding in repeated sessions. We enrolled 34 patients with an acute or chronic variceal bleeding episode at the time of admission in this study from Apr. 8, 1992 to June. 2, 1994. Among 34 patients, there were 31 males and 3 females, at ages between 45-66(mean: 52 years). The incidence of symptoms on admission was 10 in tarry stool, 9 in hematemesis 8 in ascites, 4 in hepatic encephalopathy and 3 in nonspecific symptoms. Varices were eradicated or reduced to grade I in 30(88.2%) of the 34 patients by 4-25 bands (mean: 10.8 bands) in 1-7 EVL sessions(mean: 3.1 sessions). After EVL, there are complicated by active bleeding in 3 patients, dysphagia in 3 patienta and transient chest discomfort in 5 patients but subsided during 24 hours. These results indicated that EVL is a safe method for treatment of bleeding from esophageal varices.

4.Experiences with a New Technique of Nephrolithotomy in Removing Renal Caliceal Calculi.

A new technique of nephrolithotomy was used to remove calyceal stone in seven cases. Key points of this technique are direct removal of stone through the calicopuncture by forceps without renal vascular clamping and Gelfoam Packing followed by compression with hands for control of bleeding. The results were s follow : 1. With the exception of a single small stone all the targeted calculi were removed (94% in completeness of stone removal). 2. This method did not have any significant renal parenchymal damage and there was no episode of postoperative urine leakage of delayed bleeding. 3. Postoperative complications were a case of urinary obstruction caused by slipped Gelfoam into calyx accidently, which was delivered spontaneously during the voiding on 2nd postoperative day and a case of wound infection. This new technique nephrolithotomy was seemed to be an useful and rapid surgical method of calyceal stone surgery in some situations such as in case of marked adhesion present around the pelvis, narrow infundibulum and failed stone removal during the performing pyelolithotomy.

5.Morphometric Study on Cervical Intraepithelial Neoplasia.

Cervical intraepithelial neoplasia in human consist of dysplasia of various developmental stages and squamous cell carcinoma in situ of various types. These lesions can be diagnosed cytologically on cervico-vaginal smears, although the diagnostic reproducibility is limited. To obtain the objects morphologic distinction between normal squamous epithelial cell in different maturation, different stages of dysplastic cells and varieties of in situ carcinoma cells, Kontron IBAS-1 imaging analyzer was applied for the measurement of nuclear and cytoplasmic areas of each categorised cells. The followings are results obtained: 1) Nuclear and cytoplasmic areas of superfical (36.9 micrometer2, 2319.9 micrometer2) intermedicate (45.7 micrometer2, 2989.7 micrometer2) and parabasal cells (50.8 micrometer2, 432.7 micrometer2) of normal squamous epithelium origin are mostly distinctive between cell types. However, cytoplasmic areas of both superficial and intermediate cells and nuclear areas of both intermediate and parabasal cells are not significantly different. 2) Normal squamous cells and various dysplastic cells show obvious difference in their nuclear and cytoplasmic areas, while difference between cytoplasmic areas of both parabasal (432.7 micrometer2) and severe dysplastic cells (409.7 micrometer2) are not statistically significant. 3) No statistical difference is observed in between nuclear areas of both moderate dysplastic (112.3 micrometer2) and severe dysplastic cell (117.6 micrometer2). 4) Varieties of carcinoma in situ cells and severe dysplastic cells are in difference in their nuclear and cytoplastic areas, whereas nuclear areas from both in situ carcinoma cells (95.3 micrometer2) of large cell type and severe dysplasia (117.6 micrometer2) are not distinctive. The results lead the author to consider that the morphometric analysis for various parameters of cell constituents are of value in making objective distinction between cells from cervical intraepithelial neoplasia in human.

BACKGROUND AND OBJECTIVES: Although there have been some reports that measured the size of the mastoid pneumatization, only a few studies have reported the age-related variations in the mastoid air cell system using three-dimensional (3D) reconstruction techniques of computed tomography (CT) images. This study presents that 3D reconstruction techniques of CT images can be used to measure the volume of mastoid pneumatization. In adddition, normal development of the mastoid air cell system in Koreans was evaluated. SUBJECTS AND METHOD: Retrospective study. A 3D reconstruction based on the image data obtained from CT was performed in the 205 ears of 105 patients without otologic diseases in order to measure the volume of mastoid pneumatization by a surface-rendering algorithm on a personal computer. RESULTS: The mastoid pneumatization continued to grow until the third decade. Thereafter, it declined slowly, and then rapidly after the seventh decade. The mean volume of mastoid pneumatization was 7095.2 mm3 in Korean adults. No statistically significant difference was found between males and females or between right and left sides. There was a significant difference between the larger and smaller sides of individuals. CONCLUSION: The volume measurement technique based on the 3D reconstruction technique reported here is widely available, highly accurate, and easy to perform. The results of this study may be helpful in evaluating the mastoid air cell system and for managing mastoid diseases.

10.A Case of Amelanotic Melanoma Developing in a Burn Scar.

Carcinomatous degeneration of chronic scars, known as Marjcinulcer, are relatively rare. The majority of them are quamous cell carcinoma or other skin cancers including basal cell carcinoma. But malignant melanone, especially amelanotic melanoma oveloping from a chronic burn scar is extremely rare. We have experiencedl a case of amelanotic melanoma arising ron a chronic burn scar in a patient, 53 year-old female, who had a painful erythematous ulcra edonodule on the left heel for about 20 years since she had sustained a burn there at the age of 1 year. Although there are no melanin pigments in hematoxylinsin(H & E) stain as well a Fontana Masson stain, a biopsy specimen from the skin lesion revealed cytologic atypia and junctional activity in the dermoepidermal junction arcl positive immunohistochemical staier, for S-100 protein, which is consistent with amelanotic melanoma. lesion was widely excised and a skin graft was dure She was also treated with intralesional BCG injection and recurrence or metastasis was ot detected during the follow up period of 6 months.
