1.A Case of Embryonal Carcinoma Developed in a Cryptorchid Testis.

A case of testicular embryonal carcinoma developed in s cryptorchid testis is presented with a review of the literature. According to Campbell's series, the incidence of malignant cryptorchid testis was 11.6%. and his statistical evaluation was similar to Gilbert and Hamilton's series of 7,000 cases of testicular neoplasms where 840 tumors were developed in undescended testes(12%). Both authors agree that the chance of neoplastic development in a cryptorchid testis is approximately 48 times greater then that developing in a normally descended testis. Several cases of testicular tumor were reported sporadically in Korea, but malignant tumors in cryptorchism were rarely seen in the literatures Only two cases of malignant cryptorchid testis were already reported in Korea.

3.A Follow up Study of the Reconstructive Surgery for Leprous Wrist Drop

Twenty three leprous wrist drop hands had been operated for reconstructive surgery at the department of orthopedic surgery Presbyterian Hospital during the period from 1965 to 1972. The follow up study was carried out on twelve hands from six months to 82 months and was evaluated by the following assessments. 1. Function 2. Appearance 3. Environmental use and patients satisfaction in result The results were as follows 1. Satisfactory result was 84.1% in function, 66.7% in appearance, 75% in environmental use and patients satisfaction, and 75.3% in overall total assessment. 2. As complication, a case of adhesion of the transfered tendon was noted. 3. Preoperative and postoperative physiotherapy contributed an important roll in the end results.

4.The Treatment of Acromioclavicular separation

There are many procedures for treatment of injuries of acromioclavicular separation, but still controversies concerning the best management of these injuries. From March 1981 to August 1984 at Masan Korea Hospital, twelve cases were treated by technique of modified Phemister method, two cases by Dewar and Barrington method, two cases by Stewart method, one case by Neviaser method, three cases by skillful neglect method and two cases by shoulder harness immobilization. The following results were obtained. 1. These injuries were more prevalent in male and more in right side. 2. The most common cause of injuries was traffic accident and followed by falling from the height. 3. Three cases were grade 2 and nineteen cases grade 3 by Allman's classification. 4. We treated these injuries by operative method in seventeen cases and non-operative method in five cases. 5. There were good functional results of treatment in operative method. 6. There were two cases of superficial wound infection, two cases of migration of K-wire and one case of recurrence of deformity in operative method, and two cases of shoulder stiffness and 2 cases of recurrence of deformity in non-operative method.

6.Multiple Meningiomas: Case Report.

Multiple meningiomas are a seldom encountered neoplasm. Most authors report an incidence ranging from 1 to 3% of all meningioma prior to the introduction of computerized tomography(CT). The routine use of CT has resolved diagnostic problems involved with multiple meningiomas and the incidence was 8% since the introduction of CT scan. In fact, it may be difficult to differentiate "true multiple meningioma" from simple recurrences or the special variant(forme fruste) of Von Recklinghausen's disease, and the pathogenetic factors related to true multiple meningioma is unknown, but various theroies of the etiology are explored by literature reports. We report the rare case of multiple meningiomas which diagnosed by CT scan and confirmed by operation.

7.Vaginal Erosion and Pelvic Abscess after Anterior Intravaginal Slingplasty (IVS) in Patient with Stress Urinary Incontinence.

Midurethral slings are increasingly popular for the surgical treatment of stress urinary incontinence. They are relatively simple to implant and shorten operating time, hospital stay, and recovery time. Tension free vaginal tape (TVT) and Intravaginal slingplasty (IVS) mesh are both polypropylene mesh. TVT is a type I totally macroporous mesh which, because of pores larger than 75 microns, admits bacteria, macrophages, and fibroblasts into the pores. IVS is a type III mesh with microporous components which admits bacteria but not macrophages. So anterior IVS appear to be more at risk of infection or vaginal erosion than other monofilament mesh. We report our experience with a patient who underwent anterior IVS and developed erosion of the mesh through vagina, creating pelvic abscess.

8.Cerebral Infarction Size According to the Duration of the Middle Cerebral Artery Occlusion in the Reversible and Irreversible Ischemic Infarction Models in the Rat.

To establish a simple recirculation model in rats without craniectomy and to determine the recirculation time necessary to benefits the infarction area with restoration of the blood flow in a series of 30 adult rats, the authors induced irreversible ischemia(group I) in 15 rats through unilateral occlusion of the middle cerebral artery(MCA) by inserting a 16mm length of 4-0 nylon surgical thread through the internal carotid carotid artery(ICA). Reversible ischemia(group II) was induced by pullin the thread that occluded the MCA with hours recirculation in 5 rats. The author again divided the rats into 4 hours(Ia and Iia), 6 hours(Ib and Iib) and 12 hours(Ic and Iic) groups designated as according to the sacrifice time of rats in group I and the starting time of recirculation in group II following the MCA occlusion. The percentage of the infarction area to the total brain was calculated by a computer image analysis system, and the infarction size between reversible and irreversible groups were compared. The results showed that although the total average infarction sizes of irreversible ischemic groups were larger than those of reversible ischemic groups, the difference of the infarction size between each group o irreversible and reversible ischemia was not significant(group Ia Vs. Iia:7.78+/-8.37% Vs. 3.28+/-1.98%; group Ib Vs. Iib: 9.07+/-4.72% Vs. 6.61+/-3.19;group Ic Vs. Iic: 8.35+/-4.47% Vs. 6.90+/-6.07%. p>0.05, one-way ANOVA test). However, the degree of difference of the infarction size between irreversible and reversible ischemia decrease along with the prolongation of ischemic duration(difference between group Ia and IIa:4.5%;group Ib and Iib:2.46%;group Ic and Iic:.45%. The author concludes that ischemic brain injury may be decreased by making a restoration of the blood flow within 4 hours after occlusion of a blood vessel in rats, and this kind of reversible ischemic model may be useful in various studies on the focal cerebral ischemia without manipulation of the brain tissue and blood vessel.

9.Predicting Factors for Spontaneous Passage of Ureteral Calculi Based on Unenhanced Helical CT Findings.

PURPOSE: We performed a prospective study to determine whether unenhanced helical computed tomography(UHCT) findings predict the spontaneous passage of ureteral calculi. MATERIALS AND METHODS: Between April 2006 and June 2007, 175 patients with a single ureteral calculus <1cm in diameter were enrolled in this study, and a UHCT was performed. All of the patients were managed conservatively for 2 weeks. Patients without spontaneous passage of ureteral calculi within 2 weeks were treated by ureteroscopy or extracorporeal shock wave lithotripsy(ESWL). The secondary signs (hydronephrosis, perinephric edema, and the tissue rim sign) were graded on a scale of 0-3. We evaluated whether spontaneous passage of ureteral calculi was associated with stone diameter, location, Hounsfield units(HU), and the degree of secondary signs. RESULTS: Ninety-two patients(52.6%) had spontaneous passage of ureteral calculi. The mean stone diameter was significantly smaller in the passage group than the non-passage group(4.28mm vs. 6.73mm, p=0.002). The rate of spontaneous passage was significantly higher involving distal ureteral calculi(66.1%) than proximal ureteral calculi(30.3%, p<0.001). The incidences of hydronephrosis and perinephric edema were significantly lower in the spontaneous passage group than the non-passage group(8.7% vs. 73.5% and 5.4% vs. 69.9%, respectively). The grades of hydronephrosis and perinephric edema were significantly lower in the spontaneous passage group than the non-passage group(p=0.001). Although there was a tendency toward increasing grades of hydronephrosis and perinephric edema with increasing stone size, the grades were more frequent and severe in the non-passage group in patients with similarly sized stones. CONCLUSIONS: The degree of hydronephrosis and perinephric edema are useful ancillary signs in predicting the likelihood of spontaneous passage of ureteral calculi.

10.Effects of Diospyros kaki peel, Momordica charantia, and Canavalia gladiata extracts on the cariogenic traits of Streptococcus mutans

OBJECTIVES: The purpose of this study is to determine methods of dental caries prevention by investigating the use of compounds of Diospyros kaki (D. kaki) peel, Momordica charantia (M. charantia), and Canavalia gladiata (C. gladiata) extracts to limit the cariogenic traits of Streptococcus mutans (S. mutans), such as their ability to proliferate and adhere to the tooth surface. METHODS: Broth microdilution and the agar spreading assay were used to determine the antimicrobial effect and minimum inhibitory concentration (MIC) of S. mutans extracts. In order to identify the adhesive ability of S. mutans at varying concentrations, culture plates were first stained with 1 ml of 0.01% crystal violet for 15 minutes at room temperature, and then eluted with 1 ml of EtOH:Acetone (8:2) solution for 15 minutes in a 37℃ incubator. Eluted solutions were then evaluated by use of a spectrophotometer at 575 nm. RESULTS: Experiments were conducted in order to investigate the effectiveness of D. kaki peel, M. charantia, and C. gladiata extracts on limiting the proliferation of S. mutans. The MIC was measured as an indication of whether the antibacterial activity of D. kaki peel, M. charantia, and C. gladiata extracts had a significant bacteriostatic effect on S. mutans. M. charantia extract was effective for growth inhibition on S. mutans at a minimum concentration of 0.25%. From the adhesion ability assay, M. charantia extract had an anti-adhesive effect. CONCLUSIONS: These results indicate that M. charantia extract demonstrates antibacterial activity and has an anti-adhesive effect on S. mutans. Due to these properties, M. charantia extract may be used to prevent dental caries.
