1.Sequence Diversity of E2/NSl of Hepatitis C Virus from Koreans.

BACKGROUND: E2/nonstructural protein 1 (E2/NSl) glycoprotein of hepatitic C virus (HCV) is most variable region of HCV and appears to cause positive selection of escape mutants, which may play a role in the mechanism of persistent viral infection. We analyzed the sequence diversity of E2/NSl of HCV from twelve Koreans. METHODS: The nucleotide and deduced amino acid sequences of the putative E2/NSl region of HCV were determined by analysis of twelve independent amplified CDNA clones obtained from Korean patients with chronic hepatitis and primary hepatocellular carcinoma, using dideoxynucleotide chain termination method. RESULTS: High level of variations in both nucleotide and amino acid sequences were observed in the hypervariable region 1 and 2 (HVR1 and HVR2). Sequences of HVR1, situated at the nucleotide positions from 1478 to 1552 (amino acid positions : from 383 to 407), showed from 27.8 to 94.4% diversity at the nucleotide level and from 25.0 to 87.5% diversity at the amino acid level. 4 few amino acid residues (amino acid positions: 384, 388, 389 405, 406) within HVR1 were highly conserved in most cases. Sequences of HVR2, situated at the nucleotide positions from 1736 to 1774 (amino acid positions from 469 to 481). Two groups of highly conserved amino acid sequences were observed in the HVR2 of twelve Korean cases, indicating that there are type specific conservation in the HVR2 region. 4 large part of the eave lope region located at the nucleotide positions from 1832 to 1892 (amino acid positions : from 501 to 521) was highly conserved, to show 100% Identity of amino acid sequences. The positions of six putative N-glycosylation sites were highly conserved. The percent diversities of nucleotide of E2/NSl was from 60.4% to 92.7% and the percent diversities of amino acid of E2/NSl was from 57.6% to 95.5% among twelve Korean HCV clones. CONCLUSION: Extensive genetic variations including two hypervariable regions were clustered and interspersed with highly conserved nucleotides sequences in E2/NSl region of HCV isolated from Koreans.

3.Alveolar Soft Part Sarcoma (Malignant Granular Cell Myoblastoma) in Axilla (Pectoral Muscle): Report of a Case

Since that first description of the so-called granular cell myoblastoma by Abrikossoff in 1926, it has been reported as occuring in many sites. This tumor is an unusual but not rare tumor. From the number of reviewed case reports(550) upto now, it appears that the occurence of multiple granular cell tumors (36) is much more common (7–16%) than traditionally believed. The precise histogenesis of this tumor is open to question. Most of this tumor is benign, but malignant ones have been reported. A case of malignant granular cell myoblastoma which was histologically proven and metastasized to the coracoid process of left scapula, the lung, and the skull, is presented with a brief of the literature.

4.Fracture and Dislocation of the Cervical Spine: Clinical Study of 16 patients

Reviewing 16 patients of fractures and dislocations of the cervical spine treated and managed at Department of Orthopedic Surgery, Kyungpook National University Hospital from January 1963 to August 1970 We came to the following conclusions: 1. The incidence of trauma was high in the young men, most frequent in the 3rd and 4th decades. 2 Most of the injuries were the results of automobile accidents or of falls on the head and neck. 3. Twelve of sixteen patients had dislocations or subluxations associated with the fractures and the remainder of 16 had dislocations or subluxations without demonstrable fractures. These occurred most often at the level of the 5th and 6th cervical vertebra. 4. Twelve of sixteen patients had a total of demonstrable fractures, and compression of a vertebral body was the most common type of fracture. 5. Trauma to the head was the commonest associated injury. 6. Mild or severe cord damage signs had developed in 12 of 16 patients; a case who was assocated with severe cerebral damage was dead, 7 were quadriplegic and 4 returned to former professional life. The remainder of 16 had a good recovery without disability.

5.MRI findings of acute disseminated encephalomylitis.

Acute disseminated encephalomyelitis (ADEM) is a demyelinating disease of probable autoimmune etiology. The MR images of patients with clinically suspected ADEM were retrospectively reviewed. The clinical symptoms occurred 5 days to 1 month after viral upper respiratory infection (4) and Coxsakie viral infection(1). The symptoms had begun with fever(3), headache(3), sore throat(1), and drowsy mental state (1), which progressed with monophasic course to altered mental change(2), extremity weakness(2), seizure(1) and/or cerebellary symptom(I) MRI findings of ADEM showed patchy(4), nonhemorrhagic(5), asymmetric(5) high signal intensity lesions on T2-weighted images. The number of the lesions was mostly multiple(4). The lesions mainly involved the brain stem(3) and subcortical white matter(3). Follow-up MR images of 13 days to 20 days after high dose steroid therapy showed marked improvement in two of three, which well correlated with clinical manifestations. MR findin of multiple, patchy, nonhemorrhagic and asymmetric lesions in subcortical white matter and brain stem on T2-weighted images seem to be characteristic features of ADEM, but nonspecific. Therefore, clinical correlation is required in evaluating ADEM.

7.A Case of Embryonal Carcinoma Developed in a Cryptorchid Testis.

A case of testicular embryonal carcinoma developed in s cryptorchid testis is presented with a review of the literature. According to Campbell's series, the incidence of malignant cryptorchid testis was 11.6%. and his statistical evaluation was similar to Gilbert and Hamilton's series of 7,000 cases of testicular neoplasms where 840 tumors were developed in undescended testes(12%). Both authors agree that the chance of neoplastic development in a cryptorchid testis is approximately 48 times greater then that developing in a normally descended testis. Several cases of testicular tumor were reported sporadically in Korea, but malignant tumors in cryptorchism were rarely seen in the literatures Only two cases of malignant cryptorchid testis were already reported in Korea.

8.A Case of Right Ovarian Vein Syndrome.

A case of right ovarian vein syndrome is reported with a review of literature. The patient presented was a 29 year-old multipara and eighth month of pregnancy. The extirpation of entire right ovarian vein, including it's all tributaries produced a good result and has not caused interruption of pregnancy. The clinical situation due to aberrant right ovarian vein is probably more frequent than heretofore suspected, by reason of a frown of X-rays during pregnancy and also an unawareness of the condition. Therefore, if clinical observations and X-ray studies are performed, more cases of such a condition. will be expected.

10.Evaluation of Muscle Strength Using Isokinetic Testing and Functional Result after Total Knee Arthroplasty.

PURPOSE: Objective measurements of knee of flexor and extensor strength using isokinetic equipment and function after total knee arthroplasty (TKA) were evaluated until postoperative 12 months. MATERIALS AND METHODS: The control group constituted of 15 persons who were of same age and weight without knee problems. Most of the patients (70%) were operated on both knees simultaneously. The patient group consisted of constituted with 20 patients (34 cases). Isokinetic testing (Cybex) of knee flexor and extensor strength of knee and functional evaluation by HSS score was performed preoperatively and at 3, 6, 12 months postoperatively. RESULTS: The peak torque of the knee extensor and flexor muscle in the patient group was decreased by 17-41% compared to the control group. Especially, the peak torque of extensor was more decreased than flexor muscles. At postoperative 12 months, the peak-torque values of hamstring and quadriceps were able to attain the same strength levels of the preoperative knee. According to the grade of the Hospital for Special Surgery knee rating scales by Insall, 28 cases (82 %) in the patient group could obtain good result at postoperative one year. CONCLUSIONS: Exercises to increase the strength of extensor should be emphasized in the rehabilitation programs after TKA
