Large cell neuroendocrine carcinoma (LCNEC) of the lung is a newly recognized entity of pulmonary neuroendocrine carcinoma. Histologically, it is very difficult to differentiate LCNEC from other pulmonary carcinomas and the prognosis is significantly poor. The cutaneous metastasis of LCNEC of the lung shares some features with Merkel cell carcinoma of the skin in light microscopy and yet it is negatively stained with cytokeratin 20. We report a case of cutaneous metastasis of LCNEC of the lung, previously misdiagnosed as squamous cell carcinoma. Our patient showed a poor response to the chemotherapy and also revealed a brain metastasis on follow-up brain CT scan.

3.The Past, Present and Future of the Blood Pre-Deposit System in Korea.

The certificate of blood donation was introduced in 1975. Blood donor receives the certificate after donation. The person who has this certificate can receive one unit of blood component transfusion without charge in the hospital when he needs transfusion. The donor can use it for himself or anyone else whom it may concern, if necessary. This blood pre-deposit system in Korea is regulated by Transfusion Law. The government health insurance budget and pre-deposit foundation budget support the reimbursement fee for blood. Whenever blood centers collect blood from donors, they deposit 2,500 KRW into the pre-deposit foundation budget. The return rate of certificates was 15-28% and recently showed a gradual decrease to 13% due to the extension of coverage by government insurance for cancer patients, who are the main transfusion candidates. This system has contributed to the shift from paid donation to voluntary donation for the past 40 years. However, because this certificate imposes financial incentives for donors, the spirit of voluntary non-remunerated donation can be abused. If the transfusion fee is fully covered by government budget, it might be possible to discontinue the use of the certificate in order to implement true voluntary donation.

5.Improving capability of local public hospital and health center against newly emerging infectious diseases after Middle East respiratory syndrome epidemic in Korea.

Healthcare system in Korea was found to be very vulnerable to public health emergency preparedness and response as demonstrated in the recent outbreak of Middle East Respiratory Syndrome Coronavirus. We need to redefine the function of and improve the capability of local district public hospital and local health center to cope with potential threats by newly emerging infectious disease in the nearer future. While central government may be responsible for early detection of newly emerging infectious disease transmitted from outside of the country, local government and its district-level public healthcare agencies need to primarily control over spread of the disease among the local residents. Governance setting for rapid response required in public health crisis situation appears to be possible based on strong local public health infrastructure for health promotion and disease prevention at si-gun-gu district level. Proper and sustainable investment is also needed for local public hospital with high standard facilities and skilled healthcare manpower since there seems little economic incentives to maintain such facilities in private hospitals. In conclusion, it would be urgent task to improve prevention activities for infectious disease of local health centers and clinical activities of local public hospital. Especially, role and competencies of public health physicians working at the public health center need to be specified as an essential component of public health infrastructure.

6.Crystalline Retinopathy without Corneal Dystrophy.

Bietti's crystalline retinopathy is a rare form of tapetoretinal degeneration characterized by yellow, polygonal, glistening intraretinal crystals in the posterior pole and in the superficial paralimbal cornea, which may be due to a systemic abnormality of lipid metabolism, and has been to have a autosomal recessive pattern.Also lots of the reports described similar cases without any corneal changes. 56 years-old female with complaint of progressive visual decrease had corrected visual acuity of 0.1 in her right eye and 1.0 in her left eye.Yellow intraretinal crystals with retinal pigment epithelial (RPE)dystrophy and choroidal sclerosis were noticed without any corneal changes.During follow-up for thirty months, visual acuity gradually decreased to counting fingers in her right eye and 0.7 in her left eye, and RPE degeneration and choroidal sclerosis worsened with no change in intraretinal crystals.

8.A Case Report of Surgical Management of Tricuswpid Valve Endocarditis.

Surgical treatment is recommended in tricuspid valve endocarditis not responding to antibiotics or presenting severe heart failure. However, risk of early prosthetic valve endocarditis especially in drug addictors is principal concern in the treatment. A 37 year-old man with tricuspid valve endocarditis underwent staged operation of primary tricuspid valvulectomy and secondary bioprosthetic valve implantation successfully. We report it with references of literature.
