6.Genetic Polymorphism of a Platelet Glycoprotein IIIa as anInherited Risk Factor for Coronary Artery Disease in Koreans.

BACKGROUND: Platelet aggregation and thrombus formation within the coronary artery are major factors in acute coronary syndrome. The platelet glycoproteinIIb/IIIa receptor is a pivotal mediator of platelet aggregation. Recently there have been reports that the genetic polymorphism of GPIIIa is an inherited risk factor for coronary artery thrombosis. This study investigated the relation between the Pl(A) polymorphism and coronary artery disease in Korean patients. METHODS: One hundred patients with acute myocardial infarction and unstable angina were enrolled. Coronary angiogram was performed in eighty-one cases. Genomic DNA from peripheral blood was amplified by polymerase chain reaction. Allele-specific restriction digestion was used to determine the Pl(A) genotype. RESULTS AND CONCLUSIONS: The prevalence of Pl(A2) genotype was zero percent in our study group. All patients had the Pl(A1/A1) genotype. These results suggest that Pl(A) genetic polymorphism of GPIIIa is not an inherited risk factor for coronary artery diseases in Koreans.

7.Effect of Leptin on the Steroidogenesis of Cultured Human Granulosa Cells.

OBJECTIVE: To elucidate the location of leptin and receptors of ovary specimens obtained from patients undergoing hysterectomy by immunohistochemical staining and to determine the effect of leptin on the steroidogenesis of cultured granulosa cells. METHOD: In the culturing process of the granulosa cells, FSH (1 IU/ml)and leptin (50 ng/ml), IGF-I (50 ng/ml) was administered to each study group (Group I: FSH; Group II: FSH, leptin; Group III: FSH, IGF-I, leptin), and the levels of estradiol, progesterone, androstenedione in the culture media was measured by radioimmunoassay. Statistical analysis was conducted by one-way ANOVA with Scheffe test. RESULTS: The results showed that leptin and leptin receptors were both found to be strongly stained in granulosa and theca cells, and also in some interstitial cells. Leptin receptors were also observed in cultured granulosa cells. While there was no statistically significant difference in the androstnedione concentrations between the groups, estradiol concentrations was significantly decreased in Group IV (2202.0+/-151.14 pg/ml) compared to Group III (2859.0+/-122.6 pg/ml), and progesterone concentrations were also significantly decreased in Group II(4696.3+/-190.6 ng/ml) and Group IV (4517+/-206.78 ng/ml) compared to Group III(5546.0+/-179.5 ng/ml). CONCLUSTION: The study result of this study suggest that leptin is directly involved in the regulation of ovarian functions, in particular steroidogenesis.

8.Limb-Body Wall Malformation Complex with Absence of External Genitalia.

The limb-body wall malformation complex is a sporadic congenital anomaly characterized by protean manifestations. The diagnosis is based upon the presence of at least two out of craniofacial anomaly, body wall defect, and limb abnormalities. We present a case of limb-body wall malformation complex. This case shows abdominal and pelvic wall defects with eventration of the viscera. It also shows an absence of right kidney, polycystic left kidney, absence of external genitalia and anus, and lower extremity abnormalities. The right lower extremity is absent and the deformed left leg shows malformed foot-like structure attached to the shin in addition to a normally positioned left foot. Our patient is the first case of complete absence of the external genitalia associated with limb-body wall malformation complex in Korean publications and the seventh in English publications.
9.Clinical Analysis of Glaucoma Triple Procedure of Extracapsular Cataract Extraction and Posterior Chamber Intraocular Lens Implantation Combined with Trabeculectomy.

Glaucoma triple procedure of extracapsular cataract extraction and posterior chamber intraocular lens implantation combined with trabeculectomy which was undergone in 21 eyes(20 cases) since May 1984, were reviewed. The results as follows. 1. In the distribution of sex and age of 20 patients, 16 patients(75%) were male, and seventies, which were most common, were 9 patients(45.0%); fifties, 5; forties, 1; thirties, 1. The average age was 63.9 years old. 2. In type of glaucoma, chronic open angle glaucoma which was most common, were 14 eyes(66.7%); chronic angle closure glaucoma, 5 eyes(23.8%); aphakic glaucoma, 1 eye(4.8%); secondary glaucoma, 1 eye(4.8%). 3. Visual acuity improved in 20 eyes but unchanged in 1 eye, and 14 eyes(66.7%) achieved visual acuity 0.5 or better. 4. Preoperative mean intraocular pressure(IOP)was 20.9mmHg and postoperative mean IOP was 6.1, 4.5mmHg lower than preoperative level at 2 and 6 months res pectively. 5. 2.1 glaucoma medications was needed preoperatively and 0.7, 0.8 was needed at postoperative 2 and 6 months respectively. 6. There were no serious complications that affected the visual acuity. However minor complications were as follows. Hyphema were 4 eyes(19.0%); mydriasis, 3 eyes(14.3%); flat chamber, 1 eye(14.3%); flat chamber, 1 eye(4.8%); cystoid macular edema, 1 eye(4.8%).
