1.Case Report: Ocular sareoidoasis.

Sarcoidosis is a multisystem granulomatous disorder of unknown etiology. The authors experienced a case of sarcoidosis in 22-year-old female who had palpable mass on left eyelid. Ocular examination revealed enlargement of lacrimal gland, anterior uveitis, iris nodule and abnormal fundus findings such as hyperemic disc with blurred margin, perivasculitis, vitreous himorrhage, and choroidal nodule. Systemic investigation yielded bilateral hilar lymphadenopathy and pulmonary infiltration, enlargement of multiple supraclavicular lymphnodes. Histologic findings of lacrimal gland revealed non-caseating epitheloid granuloma. In the course of corticosteroid therpy, above signs are subsided gradually, but exacerbated when she discontinued steroid because of the development of acne on the face. And so, we confirmed the sarcoidosis.

3.Effect of irradiation on the periodontal tissues in streptozotocin-induced diabetic rats.

PURPOSE: To observe the histopathological changes in the periodontal tissues of mandibular molars in streptozotocin-induced diabetic rats after irradiation. MATERIALS AND METHODS: The male Sprague-Dawley rats weighing approximately 250 gm were divided into four groups: control, diabetes, irradiation, and diabetes- irradiation groups. Diabetes mellitus was induced in the rats by injecting streptozotocin. Rats in the control and irradiation groups were injected with citrate buffer only. After 5 days, the head and neck region of the rats in irradiation and diabetes-irradiation groups were irradiated with a single absorbed dose of 10 Gy. All the rats were sacrificed at 3, 7, 14, 21, and 28 days after irradiation. The specimen including the mandibular molars were sectioned and observed using a histopathological method. RESULTS: In the diabetes group, osteoclastic activity was observed in the alveolar bone and the root throughout the period of experiment. Also, osteoblastic and fibroblastic activities were markedly decreased. In the irradiation group, the osteoclasts were observed in the alveolar bone and the dilated capillaries were increased in the early experimental phases. However, vigorous osteoblastic activity was noted in the late experimental phases. In the diabetes-irradiation group, osteoclastic activity in the alveolar bone and the root was observed in the early experimental phases. However, there were no resorption and osteoblastic activity in the alveolar bone and the root in the late experimental phases, and obvious atrophic change of fibrous tissues was noted. CONCLUSION: This experiment suggests that osteoblastic activity was caused by irradiation in the late experimental phases, but atrophic change of the periodontal ligament tissues was induced after irradiation in diabetic state.

4.Comparison of Posterior Cruciate Retaining & Substituting Total Knee Arthroplasty in Varus Deformity of Knee.

PURPOSE: To evaluate the effect of posterior cruciate ligament retaining total knee arthroplasty on its clinical results in patients with varus deformity. MATERIALS AND METHODS: We analyzed clinical and radiologic results of the 61 cases (39 patients) with varus deformity more than 15 degrees retrospectively. The average follow up period was 6.1 years (range, 5~8 years old). The cases were divided into posterior cruciate ligament retaining group A (42 cases) and substituted group B (19 cases). RESULTS: There was no statistically significant difference between two groups in Knee Society knee score (A group: 88.1, B group: 88.9), functional score (A group: 79.3, B group: 75.6) activity level (A group: 67.5, B group: 62.5), flexion contracture (A group: 4.5 degree, B group: 4.9 degree), tibiofemoral angle (A group: 4.7 degree, B group: 5.2 degree) and roentgenographic findings. Patients with PCL retaining knees showed significantly increased activity levels in going up and down stairs, and walking. CONCLUSION: It is considered that the preservation of the posterior cruciate ligament is better than the substitution of the ligament if technical problems are solved.

5.Cemented Total Knee Arthroplasty with Posterior Cruciate Ligament Retention: The Comparision of Rheumatoid Arthritis with Osteoarthritis.

PURPOSE: To analyze the clinical and roentgenographic results of rheumatoid arthritis and osteoarthritis after cemented total knee arthroplasty with posterior cruciate retention. MATERIALS AND METHODS: Since 1989, 53 cases of rheumatoid arthritis and 454 cases of osteoarthritis were followed up for average 7.3 years(range 5-14 years) after cemented total knee arthroplasty with posterior cruciate retention. RESULTS: There was no statistically difference in KSS knee score(A: 88.5, B: 92.2), functional score(A: 73.2, B: 79.3), activity level(A: 76, B: 80), flexion contracture(A: 4.5 degree, B: 4.3 degree), tibiofemoral angle(A: 4.8 degree, B: 4.7 degree) and roentgenographic findings. There were hyperextension of 4 cases in group A and 3 cases in group B, but instability was not found. CONCLUSION: There was no significant difference in clinical and radiological results between the osteoarthritic and rheumatoid arthritic group after cemented TKR with PCL retention. Therefore, cemented TKR with PCL retention could be used as a valuable method in treating rheumatoid arthritis.
6.Moderate to Severe Varus Deformity in Total Knee Arthroplasty: PFC Sigma Fixed Bearing Versus Rotating Platform.

PURPOSE: To compare the performance of a fixed bearing with a PFC Sigma with that of a rotating platform in a total knee arthroplasty performed on patients with a moderate to severe varus deformity. MATERIALS AND METHODS: Among the patients who had received a total knee arthroplasty with either a PFC sigma fixed bearing or a rotating platform between October 1998 to June 2002, 77 cases (53 patients) with a > 15degrees varus deformity were examined. The surgery time and the clinical ratings and the radiographic images for each patient were investigated. RESULTS: There were no significant differences in the knee score, functional score, activity level, or radiological findings. There were seven cases where friction sounds were produced from the insertions in the rotating platform whilst walking and there were three cases of lateral instability. CONCLUSION: The PFC sigma fixed bearing was more effective for stabilizing the knees with a moderate to severe varus deformity because the rotating platform had a friction sound from the insertions while walking and lateral instability.

7.Statistical Analysis of 1,000 Cases of Kawasaki Disease Patients Diagnosed at a Single Institute.

PURPOSE: To find the risk factors associated with coronory artery lesions, non-responsiveness to intravenous immunoglobulin(IVIG) treatment, and recurrences in Kawasaki disease patients. METHODS: We retrospectively analyzed 1,000 Kawasaki disease patients who were admitted to Yonsei University Medical Center from September 1990 to December 2003. We compared between responder and non-responder groups to IVIG treatment as well as between relapsed and non-relapsed groups, and as to the relapsed group, we also compared variables between patients in their first and second attack states. Finally, factors associated with longer-fever duration from disease onset were evaluated. RESULTS: Longer fever durations before and after IVIG treatment, male sex, lower Hgb and Hct level, higher WBC count and segmented WBC proportion, and higher CRP and Harada's score were related with coronary artery lesions. Non-responsiveness was related to higher WBC count, segmented WBC proportion, CRP, SGPT, Harada's score, and pyuria. Moderate-to-severe coronary artery dilatations and recurrences were more commonly seen among the non-responder group. No significant predictive factors for recurrence were found. In the relapsed group, lower WBC count, CRP, and shorter fever duration from disease onset were observed in their second attack state. Fever duration from disease onset showed positive correlation with WBC count, CRP, and Harada's score and negative correlation with Hgb levels. CONCLUSION: Higher WBC count, CRP, and higher Harada's score were related to both higher incidences of coronary artery lesions and non-responsiveness to IVIG treatment, and these factors were also related with longer fever duration. Non-responders to IVIG treatment showed higher recurrence rate and more moderate-to-severe coronary artery dilatations than responders.

8.Effect of smoking on pulmonary function in ex\smokers and current smokers.

BACKGROUND: Smoking is closely related to pulmonary diseases, especially pulmonary function. Past studies were defective in that pulmonary function was not included for ex-smokers and the study population being too small. This study attempted to show the change of pulmonary function parameters according to post-smoking years of ex-smokers and pack-years of current smokers. METHODS: We analyzed the results of parameters derived from the forced expiratory spirogram in 3,713 adults who visited Kyungpook National University Hospital Health Screening Center between May 1997 and March 1998. Independent variables used were age, sex, height, weight and smoking status. Dependent variables were pulmonary function parameters. Multiple regression analysis was used. RESULTS: 1.Overall, there was a significant change in pulmonary function parameters among the subjects studied. Significant variables shown were age, sex, height and pack-years. Weight and post-smoking years turned out to be relatively less significant variables. 2. Positive correlation was seen in both FEV1/FVC and FEF25-75% in ex-smokers with longer post-smoking years. 3. There was negative correlation in all the parameters of current smokers with longer pack-years. CONCLUSION: The results of this test show that smoking slowly reduce pulmonary function and even those smokers who quit smoking can not be expected to have normal pulmonary function as healthy nonsmokers. However, ex-smokers can expect some improvement in certain pulmonary function parameters as the post-smoking years become longer.

10.Combined Therapy of Paroxetine and Tricyclic Antidepressant in Depression of Schizophrenic Patients.

Depression is well-known to comorbid with several psychiatric disorders. Many schizophrenics also suffer from depression in the course of their illness. Combined therapy of SSRI and tricyclic antidepressants were reported to have benefits in some depressed patients. Paroxetine, a potent CYP2D6 inhibitor, increases the blood levels of tricyclic antidepressant markedly, Using paroxetine, we tried this combined therapy in the treatment of depressive symptoms in 10 chronic schizophrenic inpatients and evaluated its efficacy and drug interactions between paroxetine and tricyclic antidepressants. The following results were obtained: 1) The mean score of Hamilton's Depression Rating Scale(HDRS) was reduced significantly after 6 weeks-trials of this combined therapy for the mild depressive symptoms in 10 chronic schizophrenics. In four patients, 50% or more reductions in the scores of HDRS were noticed at final evaluation. 2) Two among our 10 subjects experienced severe toxic behavioral problems. Anticholinergic crisis with toxic confusion due to high blood levels of tricyclics was fecund in one patient and the other showed rapid clinical deterioration in his psychotic symptoms such as delusion and hallucination without any consciousness alternation. 3) Baseline plasma levels of tricyclics before adding paroxetine were higher or than expected in our chronic schizophrenic subjects maintained with their antipsychotic medications. Several antipsychotics were also known as a potent CYF2D6 inhibitors and to increase the blood levels of tricyclics. Because the blood levels of tricyclics had already increased significantly by the use of antipsychotics, adding paroxetine to antipsychotics and tricyclic antidepressant In our subjects could increase the blood levels of tricyclics not so much as previously reported in the literatures.
