1.Factors Influencing on the Cognitive Function in Type 2 Diabetics

OBJECTIVES: The aims of this study were to know the frequency and the nature of cognitive dysfunction in type 2 diabetics, and to reveal influencing variables on it. METHODS: From eighty type 2 diabetics (42 males and 38 females), demographic and clinical data were obtained by structured interviews. Cognitive functions were measured using the MMSE-K (Korean Version of the Mini-Mental State Examination) and the Korean Version of the Montreal Cognitive Assessment (MoCA-K) tests. Severity of depression was evaluated by the Korean Version of the Hamilton Depression Rating Scale (K-HDRS). RESULTS: 1) Among eighty type 2 diabetics, 13.75% were below 24 on the MMSE-K, while 38.8% were below 22 on the MoCA-K. 2) The total scores and subtest scores of the MoCA-K including visuospatial/executive, attention, language, delayed recall and orientation were significantly lower in type 2 diabetics with cognitive dysfunction (N=31) than those without cognitive dysfunction (N=49) (p < 0.001, respectively). 3) There were significant difference between type 2 diabetics with and those without cognitive dysfunction in age, education, economic status, body mass index, duration of diabetes, total scores of the K-HDRS, the MMSE-K and the MoCA-K (p < 0.05, respectively). 4) The total scores of the MoCA-K had significant correlation with age, education, body mass index, family history of diabetes, duration of diabetes, total scores of the K-HDRS (p < 0.05, respectively). 5) The risks of cognitive dysfunction in type 2 diabetics were significantly influenced by sex, education, fasting plasma glucose and depression. CONCLUSIONS: The cognitive dysfunction in type 2 diabetics seemed to be related to multiple factors. Therefore, more comprehensive biopsychosocial approaches needed for diagnosis and management of type 2 diabetes.

2.Changes of Ear Canal with Mandibular Movement.

BACKGROUND AND OBJECTIVES: Although the necessity of hearing aids as one of the rehabilitation methods is increasing, many hearing impaired persons are avoiding the use of behind-the-ear (BTE) hearing aids and prefer the complete-in-the-canal (CIC) hearing aids. CIC hearing aids have cosmetic and acoustic advantages but have problems of acoustic feedback and discomfort in wearing. Changes in the external auditory canal (EAC) by mandibular movements are a primary factor for the acoustic feedback and discomfort. The purpose of this research is to study the pattern of changes in the EAC by mandibular movement using the Korean population as subjects and to get basic data concerning the manufacture of impressions and shells for CIC hearing aids. MATERIALS AND METHODS: Subjects were 19 men with a fit external ear diameter to make CIC hearing aid. Impressions were made by taking 5 different conditions of jaw into consideration : fully opened state, 1/2 opened state, biting state, chewing state and resting state. Impressions were scanned at intervals of O.1mm using a 3D Laser digitizing system. We measured anterior to posterior (AP) width and superior to inferior (SI) height of each impression at the first bend, interbend and the 2nd bend of 5 conditions. RESULTS: Compared with the resting state, statistically significant changes in the AP width of the ear canal were observed at the interbend in the following order : fully opened jaw, 1/2 opened jaw (p <0.05), at 1st bend: 1/2 opened jaw (p<0.05). The 2nd bend biting state showed maximal AP width of the ear canal (p<0.05). The SI height of ear canal revealed statistically insignificant changes. CONCLUSION: We recommend making of ear impressions in the chewing state and shells to be supported at the interbend of the ear canal. The SI dimension of a shell tip should be more ground than AP dimension.

3.Morphological and Acoustic Change of Autologous Fat Injection for Unilateral Vocal Cord Paralysis.

BACKGROUND AND OBJECTIVES: Glottic incompetence resulting from the unilateral vocal cord paralysis can be improved by medialization of the paralyzed vocal cord. Recently, it has been thought that the autologous fat could be used as a good material for augmenting paralytic vocal cord. There were reports about long-term follow-up results concerning voluminous, histological and acoustic changes after autologous fat injection. Authors observed periodically the morphological and acoustic changes of autologous fat deposits which were injected into iatrogenically paralyzed vocal cord. MATERIALS AND METHODS: Eight mongrel dogs were injected with autologous fat into the iatrogenically paralyzed vocal cords, and were sacrificed at 1, 3, 6 months after the fat injection and observed histological and acoustic changes. RESULTS: The bulk of the fat graft lasted at least 3 months in this experiment. There were minimal evidence of inflammatoy changes, foreign body reaction and improvement of voice quality was noted in spectrography. CONCLUSION: The autologous fat injection appears to be a relatively safe and effective method for medializing the paralyzed vocal cord.

BACKGROUND/AIMS: A recent study reported that entecavir had inferior efficacy in nucleos(t)ide analogue (NA)-experienced chronic hepatitis B (CHB) patients compared to NA-naïve patients. We sought to compare the efficacy of tenofovir disoproxil fumarate (TDF) in NA-experienced and NA-naïve CHB patients. METHODS: We retrospectively enrolled 252 consecutive patients who had a serum hepatitis B virus (HBV) DNA level greater than 2,000 IU/mL at the initiation of TDF treatment and who received TDF for at least 6 months. Complete virologic suppression (CVS) was defined as undetectable serum HBV DNA. We generated a multivariate Cox proportional-hazard model to examine predictive factors that were independently associated with time to CVS. RESULTS: The mean age of patients was 48.2 years, and the cohort included 181 NA-naïve patients and 71 NA-experienced patients. The median duration of TDF treatment was 14.4 (interquartile range, 9.5-17.8) months. A total of 167 (92.3%) of 181 NA-naïve patients achieved CVS, and 60 (84.5%) of 71 NA-exposed patients achieved CVS. Forty-nine (89.1%) of 55 patients who previously took an NA aside from adefovir and 11 (68.8%) of 16 adefovir-experienced patients achieved CVS. In multivariable analysis, previous adefovir exposure significantly influenced time to CVS (hazard ratio, 0.37; 95% confidence interval, 0.19-0.72; P=0.003), after adjusting for HBeAg positivity, baseline HBV DNA level and cirrhosis. CONCLUSIONS: Tenofovir had inferior efficacy in adefovir-experienced CHB patients compared to NA-naïve patients. The response of patients with previous adefovir exposure to TDF monotherapy should be monitored closely.

5.Sub-classification of Advanced-Stage Hepatocellular Carcinoma: A Cohort Study Including 612 Patients Treated with Sorafenib.

PURPOSE: Advanced hepatocellular carcinoma (HCC) is associated with various clinical conditions including major vessel invasion, metastasis, and poor performance status. The aim of this study was to establish a prognostic scoring system and to propose a sub-classification of the Barcelona-Clinic Liver Cancer (BCLC) stage C. MATERIALS AND METHODS: This retrospective study included consecutive patients who received sorafenib for BCLC stage C HCC at a single tertiary hospital in Korea. A Cox proportional hazard model was used to develop a scoring system, and internal validationwas performed by a 5-fold cross-validation. The performance of the model in predicting risk was assessed by the area under the curve and the Hosmer-Lemeshow test. RESULTS: A total of 612 BCLC stage C HCC patients were sub- classified into strata depending on their performance status. Five independent prognostic factors (Child-Pugh score, α-fetoprotein, tumor type, extrahepatic metastasis, and portal vein invasion) were identified and used in the prognostic scoring system. This scoring system showed good discrimination (area under the receiver operating characteristic curve, 0.734 to 0.818) and calibration functions (both p < 0.05 by the Hosmer-Lemeshow test at 1 month and 12 months, respectively). The differences in survival among the different risk groups classified by the total score were significant (p < 0.001 by the log-rank test in both the Eastern Cooperative Oncology Group 0 and 1 strata). CONCLUSION: The heterogeneity of patientswith BCLC stage C HCC requires sub-classification of advanced HCC. A prognostic scoring system with five independent factors is useful in predicting the survival of patients with BCLC stage C HCC.

BACKGROUND AND OBJECTIVES: It is necessary to establish the most efficient diagnostic and therapeutic method for benign paroxysmal positional vertigo (BPPV), which is appropriate for Korean healthcare system. We aimed to evaluate current state of Korean clinician's diagnostic and therapeutic approaches for BPPV. MATERIALS AND METHODS: A 16-item survey was emailed to the members of dizziness department of Otology Research Interest Group in the Korean Otologic Society (n=68). 43 were returned and analyzed. RESULTS: All respondents (100%) used Dix-Hallpike test as a diagnostic tool for vertical canal-BPPV. Supine roll test was used for diagnosing lateral canal BPPV in nearly all the respondents (97.7%). Epley maneuver was chosen as otolith repositioning maneuver (ORM) for posterior canal BPPV in all respondents and barbecue rotation (BBQ) was used for treating lateral canal BPPV with geotropic nystagmus in 95.3% of respondents. Extreme variation was noted for therapeutic approach of lateral canal BPPV with ageotropic nystagmus BBQ, with 4 kinds of ORM and adjunctive measures to liberate otolith from cupula, while BBQ was again the most commonly used ORM (76.7%). CONCLUSION: The development of practical and efficient ORM for lateral canal BPPV with ageotropic nystagmus is necessary.
