1.Chest radiographic findings of scrub typhus: An analysis of 160 cases occurred in Ulsan area.

Scrub typhus (Tsutsugamushi disease)is an acute febrile systemic illness caused by Rickettsia tsutsugamushi that is transmitted to humans by the bite of larval-stage trombiculid mites (chiggers). The authors analyzed chest radiographic findings of scrub typhus in 160 patients in Ulsan area. One hundred and eight (67.5%) of160 patients showed abnormal findings which included lung lesions in 108 patients (67.5%), cardiomegaly in 37 patients (23.1%), lymphadenopathy in 25 patients (15.6%) and pleural effusion in 11 patients (6.9%). Among the lung lesions, interstitial patterns were seen in 107 patients (66.9%), mostly fine or medium reticulonodular, and air-space patterns in 14 patients(8.8%) and combined interstitial and air-space patterns in 13 patients (8.1%). Sixty-four patients(40%) had combined chest radiographic findings. The typical chest radiographic findings of scrub typhus would be helpful in evaluation of the causes of acute febrile illness that occur during late fall in the endemic area.

Inappropriate sinus bradycardia associated with degenerative changes in the sinoatrial node has been designated as the sick sinus syndrome. Patients can be asymptomatic but often complain of palpitations and syncopal episodes. A 30 year old parturient, whose pulse rate was about 45-55 beats/min, underwent cesarean section under neuroleptanesthesia. Authors report this case with the review of the relevant literatures.

6.Clinical Analysis of the Factors Affecting the Prognosis of Spontaneous Pontine Hemorrhage.

The prognosis of pontine hemorrhage has been reported to be extremely poor, but the wide-spread use of CT scan and MRI has led to the discovery of ssmall pontinehemorrhage which previously could not have been discovered. We analysed 28 cases of pontine hemorrhage for evaluation of contributing factors to the patients prognosis from January 1994 to December 1996. A good prognosis was achieved in patients with no or mild disturbance of consciousness, 20mm or less in the transverse diameter of the hematoma, small numbers of slices in which the hematoma was found, and the hematoma localized in the tegmentum. Two patients underwent suboccipital craniectomy with removal of hematoma. But all patients died immediate postoperative period. A comatose state on admission, transverse diameter of hematoma over 20mm, large numbers of slices in which the hematoma was found, and the hematoma localized in the basotegmentum were significant poor prognostic factors.

7.Factors affecting the Result of Acute Septic Arthritis of the Hip joint in Children.

PURPOSE: The purpose of this study was to define the factors affecting results in acute septic hip arthritis in children and to reduce the complications. MATERIALS AND METHODS: This is a retrospective study of 27 children with 28 hip joint infections, treated with arthrotomy and followed up for more than 12 months. Results were evaluated according to Merle D'Aubigne's clinical grade and Bennett's radiological grade, and correlated with the time between symptom onset and treatment, the existence of a causative organism, its species, concomitant osteomyelitis, age of onset and normalized time of C-reactive protein (CRP). We analysed results using the Pearson Chi Square method. RESULTS: A delay in treatment, concomitant osteomyelitis of the proximal femur and old age, were factors associated with poor prognosis. Normalized time of CRP, the existence of a causative organism and species were not correlated with the treatment results. CONCLUSION: Shorter duration of disease from clinical onset to the initiation of therapy and concomitant osteomyelitis of the proximal femur are the most important factors in the treatment of acute septic hip arthritis in children. When analysing results correlated with the age of onset, precise prospective study is required in many cases of acute septic hip arthritis in children.

8.Content Analysis of Mothers’ Questions Related to Parenting Young Children in Internet Parenting Community

Purpose:
This study aimed to investigate mothers’ questions related to parenting young children in the internet community.
Methods:
One representative in the internet community was selected and parenting questions were collected over 2 weeks in October 2019. A total of 15,016 questions were analyzed by frequency and proportion after categorizing of 10 themes; health and daily care, connecting and communicating, play and learning, development and growth, poos and wees, sleep, feeding, solid foods, safety, behavior.
Results:
The most frequently asked questions per age ranged from 1 to 3 months (32.2%). The most frequently asked questions by theme per age were about feeding newborn up to 6 months, solid foods from 7 to 12 months, health and daily care from 13 to 24 months, and play and learning from 25 to 71 months.
Conclusion
As these results reflect mother’s need for information and support, it is suggested that more specific and targeted online/offline support per age should be developed.
9.Vulvar Basal Cell Carcinoma in Postmenopausal Women: Two Case Reports

Basal cell carcinoma (BCC) is a major non-melanoma skin cancer, and its incidence is increasing worldwide. Although the main etiology is sun exposure, BCC may develop in sun-protected areas such as the vulva. The sonic hedgehog signaling pathway mutation may explain the mechanism underlying the occurrence of vulvar BCC. Owing to the rarity of metastases, wide local excision is an appropriate treatment option. Here, we report the cases two postmenopausal women with vulvar BCC who were surgically treated.
10.The Effect of Hyaluronidase in Interlaminar Lumbar Epidural Injection for Failed Back Surgery Syndrome.

OBJECTIVE: To evaluate the effect of hyaluronidase in patients with failed back surgery syndrome (FBSS) treated with interlaminar lumbar epidural injection (ILEI). METHOD: Sixty patients suffering from severe low back pain and sciatica were randomly allocated into three groups. Group T received ILEI with 2 ml triamcinolone 40 mg/ml and 5 ml bupivacaine 0.25%. Group H received ILEI with 1500 IU hyaluronidase and 5 ml bupivacaine 0.25%. Group TH received interlaminar lumbar epidural injection (ILEI) with 1500 IU hyaluronidase, 2 ml triamcinolone 40 mg/ml and 5 ml bupivacaine 0.25%. The effect was evaluated using Visual Analogue Scale (VAS) and Oswestry Disability Index (ODI) at pre-injection, 2 weeks, 6 weeks and 12 weeks after ILEI. RESULTS: After 2 weeks and after 6 weeks, patients in both Group T and Group TH had significant effectiveness more than Group H in decrease of VAS and ODI. After 12 weeks, only patients in Group TH had significant effectiveness in decrease of VAS and ODI (p<0.05). In every period, Group TH had the most effectivess in decrease of VAS and ODI after ILEI. CONCLUSION: ILEI for FBSS with triamcinolone and hyaluronidase is considered to have more long term effectiveness to reduce pain and improve function after ILEI than injection with triamcinolone alone or hyaluronidase alone.
