1.Atrophoderma Vermiculatum Occuring in a Patient with Congenital heart Disease.

A 6-year old girl was diagnosed as atrophoderma vermiculatum on face, especially on cheek and preauricular area, and dorsum of both hands. She had suffered from congenital heart disease which was diagnosed as ventricular septal defect by cardiac: catheterization and cineangiography in final. Three months after patch repair of ventricular septal defect, no further new skin lesion developed. But reticulate skin atrophy was remained.

2.Clinico-pathological Study on the Skin Appendage Tumors.

A clinico-pathological study on the true skin appendage tumors was carried on the accumulated cases during pat 11 years from 1976 to 1986 years in the Department of Pathology, Seoul National University Hospital. The results was as follows; 1) Among various benign skin appendage tumors pilomatricoma was the most common benign tumor consisting of 72 out of 244 cases (30.1%), followed by dermoid cyst 48 cases (20.0%), steatocystoma multiplex 37 cases (15.5%), syringoma 33 cases (13.8%) and trichilemmal cyst 19 cases (0.8%). Other benign skin appendage tumors were rarely noted under 3%. Sebaceous carcinoma was most common among malignant skin appendage tumors and no malignant tumor of sweat gland origin was noted. 2) The sex ratio of male to female of the pilomatricoma was 1 : 2.6 and on the face, neck, and extremeties in 1st and 2nd decades. The sex ratio of dermoid cyst was 1 : 1.2 on the face in 1st decade. The sex ratio of steatocystoma multiplex was 1 : 1.9 and on the trunk in 3rd decade. The sex ratio of syringoma was 1 : 7 and on the face in 5th decade. The sex ratio of trichilemmoma was 1 : 1.6 and on the trunk in 5th decade. Other tumors were not avaiable for analysis because of small number. The sex ratio of sebaceous carcinoma was 1 : 2 and on the eyelid in 6th decade

4.Clinical Analysis of Acute Subdural Hematoma.

The outcome of acute subdural hematoma(SDH) had been generally unsatisfactory. The authors have analyzed the factors influencing the outcome of 70 patients who underwent acute SDH evacuation. Thirty five patients(50%) died and twenty two(31.4%) had a favorable outcome. A significant correlation was found between the outcome and the motor score at operation, the time interval to operation papillary response, type of operation and the Glasgow Coma Scale(GCS) at operation. But the patient's age, the mode of injury, the skull fracture, the degree of midline shift, the hematoma location, the type of brain swelling, the hematoma thickness, and the systolic blood pressure did not correlate with the outcome scale.

5.An Obstetric aspects of RDS of Preterm Infants in Kwang-Ju and Chon-nam.

OBJECTIVES: This study was performed to evaluate obstetric aspects and epidemiologic data of respiratory distress syndrome(RDS) in premature baby(under 37 gestational weeks). We compared the mortality and incidence of RDS in premature baby according to gestational age, birth weight, infant sex, mode of delivery. In addition, this study was searched for demonstrating the influence of hypertensive mother, and effectiveness of surfactant and corticosteoid administration. METHODS: A study was conducted for 2,360 premature baby under 37gestatinal weeks, who were born at four main hospitals in Kwangju and Chonnam from January 1, 1995 to December 31, 1997. Data were collected from hospital records and was analyzed by SPSS computer program. RESULTS: 1. The total incidence of RDS among all the preterm infants under 37gestational weeks, who were admitted in NICU of four hospitals was 22.7%. 2. Neonatal mortality was decreased as the gestational age and or birth weight advanced. (p<0.001) 3. There was no difference in the incidence of RDS according to sex and mode of delivery. (p>0.005) 4. The mortality of RDS infants in this study was 28.1%. 5. There was no statistical correlation between neonatal mortality and use of surfactant. 6. Corticosteroids administration to preterm mother did not decrease the incidence of RDS, but, there was no statistical significance due to insufficient study group. 7. The incidence of RDS among the babies of hypertensive mother was 21.0%. CONCLUSION: Still the RDS in premature baby is the most common cause of perinatal death in this country. In our study, the incidence and mortality of RDS in prematue baby was mainly depends on gestational age of infants. Therefore, we suggested that prevention of preterm labor is the important to decrease the incidence and mortality of RDS.

6.Nonarteritic Anterior Ischemic Optic Neuropathy with an Atypical Visual Field Defect

PURPOSE:
To report a case of nonarteritic anterior ischemic optic neuropathy (NA-AION) with an atypical visual field defect after intraocular surgery.CASE SUMMARY: A 61-year-old male presented with a visual field defect in his right eye 1 day after uneventful cataract surgery with pars plana vitrectomy for epiretinal membrane. His best-corrected visual acuity (BCVA) in the right eye was 20/400 with a relative afferent pupillary defect. A color vision test revealed failure only in the right eye. A slit-lamp examination revealed no abnormality in the anterior part of the eyes. A fundoscopic examination also revealed no abnormality in the posterior part of the eyes, including the optic disc. The Humphrey visual field test revealed a nasal vertical defect in the right eye. Orbital and brain magnetic resonance imaging were normal. After 14 days from the initial symptom, fundus photography and optical coherence tomography revealed an optic disc swelling and splinter hemorrhage. Fluorescein angiography revealed a delayed filling on the temporal half of the optic disc and inferotemporal peripapillary choroid. A diagnosis of NA-AION was made. The patient was treated with oral steroids and aspirin. After 3 months, the BCVA was 20/125. The visual field defect was maintained and segmental atrophy developed on the superior and inferior sides of the right optic disc.
CONCLUSIONS
AION may present as vertical hemianopsia. With the risk factors of ischemic optic neuropathy, the possibility of AION should be considered in the differential diagnoses of postoperative visual impairments or field defects after intraocular surgery.

8.Analysis of Choroidal Thickness and Vascular Density Using Optical Coherence Tomography Angiography after Laser Photocoagulation

PURPOSE: To evaluate the changes in choroidal thickness and superficial vascular density of the macula and optic disc using optical coherence tomography angiography after laser photocoagulation. METHODS: We conducted a retrospective chart review of 25 eyes of diabetic retinopathy patients who underwent panretinal photocoagulation. The macula and optic disc were divided into nine areas, and the vascular density of each area was quantitatively measured using optical coherence tomography angiography. The changes in vascular density and choroidal thickness were analyzed before laser photocoagulation and at 1 week after, 1 month after, and 3 months after treatment. RESULTS: In the panretinal photocoagulation group, the average vascular densities of the macula were 13.5 ± 3.6 mm⁻¹ before treatment, and 14.7 ± 3.1 mm⁻¹ after 1 week, 13.7 ± 2.6 mm⁻¹ after 1 month, and 12.8 ± 3.8 mm⁻¹ after 3 months of treatment. The average vascular densities of the optic disc were 14.7 ± 5.2 mm⁻¹ before treatment, and 14.1 ± 4.7 mm⁻¹ after 1 week, 14.8 ± 5.3 mm⁻¹ after 1 month, and 15.0 ± 4.7 mm⁻¹ after 3 months of treatment. The average subfoveal choroidal thicknesses were 327.5 ± 57.9 µm before treatment, and 334.4 ± 52.5 µm after 1 week, 291.2 ± 52.9 µm after 1 month, and 286.3 ± 44.4 µm after 3 months of treatment. CONCLUSIONS: The vascular density of the macula increased temporarily after 1 week of treatment but decreased afterwards. The vascular density of the optic disc decreased after 1 week of laser treatment but increased over time. The subfoveal choroidal thickness increased after 1 week of laser treatment but decreased afterwards.

9.Perioperative Complications in Elderly Patients Undergoing Lumbar Arthrodesis with Pedicle Screw Instrumentation in the Degenerative Lumbar Disorders: A Comparative Study between Posterolateral Fusion and Posterior Lumbar Interbody Fusion.

STUDY DESIGN: This is a retrospective study that focused on the perioperative complications when performing posterior arthrodesis for treating degenerative lumbar disorders in elderly patients. OBJECTIVES: We wanted to analyze the perioperative risk factors and the complications associated with posterior lumbar decompression and arthrodesis in patients who were sixty-five years of age or older. SUMMARY OF LITERATURE REVIEW: Lumbar arthrodesis is commonly done in elderly patients to treat degenerative spine problems. These patients may be at an increased risk for complications because of their age and their associated medical conditions. MATERIALS AND METHODS: We studied 111 patients who were over 65 years old and they were able to be followed up for more than 3 months after performing posterior arthrodesis during the recent 5 years. Sixty-one patients who had undergone posterolateral fusion (PLF) were classified as group A, and 50 patients who had undergone posterior lumbar interbody fusion (PLIF) using a PEEK cage were classified as group B. We analyzed their rates of complications and the causative factors, according to operative methods and age, and these complications occurred within 12 weeks after the operations. RESULTS: Complications occurred for 43 patients of group A, and for 26 patients of group B. The differences in the rates of complications were statistically significant, and group A had a more complications than did group B. The relations between medical problems and the incidence of perioperative complications were statistically significant for both groups. CONCLUSIONS: Elderly patients with medical problems should be made aware that they are at an increased risk for surgical complications, and attention hould be paid to controlling the blood loss and limiting the operative time. The PLIF using a PEEK cage was identified as a procedure that could reduce the perioperative complications.

10.Five-year Experience of Extracorporeal Life Support in Emergency Physicians.

BACKGROUND: This study aimed to present our 5-year experience of extracorporeal cardiopulmonary resuscitation (ECPR) performed by emergency physicians. METHODS: We retrospectively analyzed 58 patients who underwent ECPR between January 2010 and December 2014. The primary parameter analyzed was survival to hospital discharge. The secondary parameters analyzed were neurologic outcome at hospital discharge, cannulation time, and ECPR-related complications. RESULTS: Thirty-one patients (53.4%) were successfully weaned from extracorporeal membrane oxygenation, and 18 (31.0%) survived to hospital discharge. Twelve patients (20.7%) were discharged with good neurologic outcomes. The median cannulation time was 25.0 min (interquartile range 20.0-31.0 min). Nineteen patients (32.8%) had ECPR-related complications, the most frequent being distal limb ischemia. Regarding the initial presentation, 52 patients (83.9%) collapsed due to a cardiac etiology, and acute myocardial infarction (33/62, 53.2%) was the most common cause of cardiac arrest. CONCLUSIONS: The survival to hospital discharge rate for cardiac arrest patients who underwent ECPR conducted by an emergency physician was within the acceptable limits. The cannulation time and complications following ECPR were comparable to those found in previous studies.
