Superior mesenteric artery (SMA) injuries remain a challenge to most trauma surgeons and continue to result in significant mortality despite aggressive management. We report successful management of a proximal SMA injury through endovascular treatment in a 56-year-old man. The patient presented with hypotension due to a stab wound after the epigastrium. He underwent emergency laparotomy and repair of the penetrated wall of the stomach. Right retroperitoneal hematoma at the initial laparotomy expanded and ruptured, requiring a massive transfusion. At the second laparotomy, we performed lateral arteriorrhaphy of the proximal SMA. Follow-up abdominal computed tomography angiography on post-injury day 7 showed a pseudoaneurysm on the left side of the SMA trunk. The patient was treated successfully with an endovascular stent graft.

PURPOSE: The purpose of this study was to evaluate US and CT features of choroidal osteoma. MATERIALS AND METHDOS: US and CT scans of seven cases of choroidal osteoma occurring in six patients were retrospectively analyzed. We analysed US and CT findings with particular attention to the location, size, and shape of calcification associated with choroidal osteoma, and sought the possible cause of the tumor, if any. RESULTS: None of six patients had any possible cause related to choroidal osteoma. All of seven cases of choroidal osteoma were manifested as calcified mass which were located in the posterior wall of the eyeball near the juxtapapillary region. Calcification ranged in size from 1 to 2 cm and had curvilinear shape. Both US and CT were equally useful to evaluate choroidal osteoma. CONCLUSION: By depicting the characteristic calcification, US and CT are useful imaging modalities in evaluating choroidal osteoma.

Lesions of the skull vault are often incidentally encountered during in plain radiography, CT, and MR imaging of the brain and benign lesions are more common than primary malignancies. The usefulness of plain skull films is limited, but when combined with CT or MRI, there is a high probability of accurate diagnosis. The aim of this essay is to describe a wide range of cranial lesions and to illustrate their distinguishing features.

This study was done to evaluate the effect of fixed orthodontic patients on the level of oral streptococci, Streptococcus mutans, lactobacilli, yeasts in saliva. 35 patients wearing bands were compared with age-matched 35 non-banded control group by colony counting method on the specially designed culture medium. The following results were obtained; 1. The colony forming unit(CFU) of total streptocci per militer of saliva in subjects with or without orthodontic treatment showed no significant statistical difference between them(p>0.05). 2. The colony forming unit(CFU) of total Streptococcus mutans per mililiter of saliva in subjects with orthodontics treatment showed significantly higher than those without orthodontic treatment(p<0.05). 3. The colony forming unit(CFU) of total lactobacilli per mililiter of saliva in subjects with or without orthodontic treatment showed no significant statistical difference between them but higher tendency in those with orthodontic treatment(p=0.052). 4. The colony forming unit(CFU) of total yeasts per mililiter of saliva in subjects with or without orthodontic treatment showed no significant statistical difference between them(p>0.05).

In this study, the effect of kanamycin sulfate was compared with fortified penicillin G in the treatment of uncomplicated penicillinase negative Neisseria gonorrhoeae(nonPPNG) urethritis in male. The subjects were 250 male patients with uncomplicated non-PPNG urethritis, at the Venereal Disease Clinic of Choong-ku Public Health Center in Seoul from May 1982 to August 1982. Among 103 patients treated with kanamycin sulfate, 2. 0 gm, IM, 92 recovered and 11(10. 7%) failed. Among 100 patients treated with fortified penicillin G, 6 megaunits IM, plus probenecid, l. 0 gm, 92 recovered and 8(8%) failed. No significant difference in the effect was found between these two regimens for non-PPNG urethritis. It is concluded that kanamycin sulfate as well as fortified penicillin G. have a good effect in the treatment of non-PPNG urethritis.

PURPOSE: Response time is an important factor in determining the quality of prehospital Emergency Medical Services (EMS). Our objective was to analyze the Daegu Korean Fire Department's ambulances' response time by use of the Geographic Information System (GIS) and to suggest general factors for quality improvement of EMS. METHODS: We retrospectively reviewed computerized ambulance calls of the Daegu Korean Fire Department. During the period from July 2006 to June 2007, computerized ambulance calls could be pinpointed geographically by the GIS. Patients were divided into injury and disease groups. And each group was subdivided into emergency and non-emergency groups at triage. We reviewed the EMS response time, response velocity, distance from 119 safe center to scene, and distance from scene to hospital. RESULTS: Of 46,606 patients, 27,825 patients could be pinpointed geographically by the GIS. The mean response time was 5.5+/-18.9 minutes. Among injury patients, there were no significant differences between emergency and non-emergency groups in mean response time and mean response velocity. In disease patients, however, emergency group response time was shorter than for the non-emergency group, and response velocity was faster for the emergency group. Distance from 119 safe center to scene and from scene to hospital was greater among all total patients for the emergency group than for the non-emergency group. CONCLUSION: Using GIS, we evaluated response velocity as a measure of the quality of prehospital EMS. We found that in injury patients, prehospital triage and transportation were not properly managed. We suggests that EMS could be more effective if GIS is used as a tool for the improvement of EMS quality.

Amyloidosis is a disease complex associated with deposition of insoluble filbrillar protein in various tissues of the body. Cutaneous lesions are present in up to 40 % of patients with primary and myeloma associated systemic amyloidosis. We present a case of systemic amyloidosis associated with multiple myleoma occuring in 54-year-old male who had waxy papules with petechiae on nasola bial fold, postauricular area, and cheek. The diagnosis was confirmed by Congo red stain, electron microscope, immunelectrophoresis, Bence-Jones proteinuria, immunoperoxidase stain, and bone marow biopsy.
