2.A Case of Neurilemmoma of the Penis.

Neurilemmoma is the most frequent solitary tumor of the peripheral nerves. It is a tumor from Schwann cells, and it is usually solitary and benign. Treatment of this benign lesion is usually by excisional biopsy. This report concerns a rare case of neurilemmoma developed in the penis in a 51 years old male and the relevant literature has been reviewed.

3.A Case of Primitive Clioma.

Among the intracranial glioma, primitive glioma is very rare. We report a case of primitive glioma involving left parietofrontotemporal lobe of 8-year-old boy who had about 12 months duration of right sided hemiparesis, headache, vomiting and papilledema. CT scan showed a large, well defined round area of low density with peripheral rim of high density and high density small mass suggesting mural nodule in left frontoparietotemporal region. A left frontoparietotemporal osteoplastic craniotomy was carried out. A small nodule and cystic membrane containing pale yellowish colored proteinous cystic fluid were almost totally removed without difficulty. Postoperatively, the right hemiparesis and headache gradually cleared. Pathologic diagnosis was primitive glioma.

4.Giant Fusiform Aneurysm by Circumferential Wrapping with Sutures-Reinforcement.

Adirect intracranial operative approach with clipping the neck of the aneurysm is generally considered to be the ideal treatment for the intracranial arterial aneurysms. Several strategies exist for treating unclippable aneurysms. Intravascular techniques, proximal occlusion of the parent artery or trapping of the aneurysm, microsurgical bypass of the involved arterial segment, and reinforcement or wrapping of the aneurysmal wall are frequently used. The authors report a case with long term(6 years) follow-up results of the circumferential wrapping with sutures-reinforcement for giant fusiform aneurysm of the internal carotid artery. The giant fusiform aneurysm and ideal wrapping materials were discussed with review of literature.

6.Occipital Chondroid Chordoma.

The authors present a case of occipital chondroid chordoma in a 12 year-old boy who presented with occipital mass amd mild tenderness but no neurologic symptoms or signs. On plain skull X-rays, there was an irregular calcific mass at the left occipital area. The CT scan revealed irregular extracranial calcific mass with intracranial extradural lentiform low density lesion at the left occipital area. Also, the occipital bone was thickened. Angiography showed hypervascular mass feeded by occipital artery of left ECA.

One of the least frequently encountered nongliomatous intraspinal neoplasm is the teratoma. Tumors that are not dermoid or epidermoid cyst have been reported as being teratomatous or teratoid when they do not have tissue derived from all three germ layers and when they are clearly most closely related to the trigerminal teratoma. Authors described an illustrated case report of teratoid cyst of the spinal cannal, summarized the previous report cases, and discuss the classification and origin of these tumors.

8.A Case of Astrocytoma in the Third Ventricle.

In children, primary tumors of the third ventricle are uncommon. The majority of the lesion were infiltrating astrocytoma of the anterior portion of the third ventricle. Tumors within the third ventricle were benign, encapsulated, and respectable with good prognosis. But most surgerns have considered this tumors to be inoperable;accordingly shunting procedures followed by radiotherapy has been the treatment of choice. We recently experienced a case of astrocytoma which entirely filled the third ventricle with hemorrhage and reviewed the literatures.

9.A Case of Pineal Germinoma with Drop Metastasis to Cauda Equina.

Pineal germinoma is one of the most radiosensitive intracranial tumors and so recently a regime of radiotherapy combined with ventricular shunting was advocated. But not infrequently seeding through the subarachnoid space and shunting system was observed. We have experienced a pineal tumor, which was treated completely by irradiation following ventriculo-atrial shunt but without evidence of intracranial recurrence spinal metastasis occurred 16 months later. After myelography total laminectomy from L1 to L3 was performed and intradural tumor was removed. Histopathological diagnosis was germinoma. After would healing spinal axis irradiation was performed with symptomatic improvement.

10.Analysis of Femoral Tunnel Position Targeted at Bifurcate Ridge Using Anteromedial Portal Technique in Anatomic Anterior Cruciate Ligament Reconstruction.

PURPOSE: The purpose of this study is to analyze the position of the 10-mm-sized femur tunnel drilled aiming for the bifurcate ridge using anteromedial portal technique with 'Figure of 4 position' by 3-dimensional computed tomography (3D-CT) reconstruction images after anatomic anterior cruciate ligament (ACL) reconstruction and to evaluate it's propriety. MATERIALS AND METHODS: Out of 35 patients who underwent anatomic ACL reconstruction from March 2012 to February 2013, 32 patients who undergone postoperative 3D-CT scans were included in this study retrospectively. Medial surface of the lateral femoral condyle was reconstructed using Mimics, and then the position of the femoral tunnel was evaluated using the Bernard quadrant method and the results were compared with those of published literatures. The mean age of the patients was 32.6 years old. There were 25 cases of double-bundle ACL reconstruction with one femoral-two tibial tunnel. There were 7 cases of single bundle ACL reconstruction with one femoral-one tibial tunnel. RESULTS: The mean distance of the femoral tunnel center was 32.2%+/-2.9% (range, 27.4%-37.6%) along the line parallel to the Blumensaat's line from the posterior condylar surface and 46.7%+/-2.3% (range, 43.5%-51.1%) along the line perpendicular to the Blumensaat's line from the roof of the notch. In comparison with the results of published literature, although the center of the femoral tunnel was presented in the femoral footprint, it was located slightly more shallow and inferior than the center of the ACL footprints. CONCLUSION: The bifurcate ridge may be a good anatomic landmark when making a 10-mm-sized single femoral tunnel in 'Figure of 4 position' using the anteromedial portal technique.
