3.Effects of applying antioxidants on bond strength of bleached bovine dentin.

OBJECTIVES: Some antioxidants are believed to restore dentin bond strength after dental bleaching. This study was done to evaluate the influence of antioxidants on the bond strength of bleached bovine dentin. MATERIALS AND METHODS: Thirty incisors were randomly assigned to 10 groups (two unbleached control and eight bleached groups: immediate bonding IB, 4 wk delayed bonding DB, 10% sodium ascorbate treated SA, 10% alpha-tocopherol treated TP groups). Teeth in half of groups were subjected to thermal stress, whereas the remaining groups were not. Resin-dentin rods with a cross-sectional area of 2.25 mm2 were obtained and microtensile bond strength was determined at a crosshead speed of 1 mm/min. Fifteen specimens were prepared for SEM to compare the surface characteristics of each group. The change in dentin bond strength from thermal stress and antioxidant treatment was evaluated using two-way analysis of variance (ANOVA) and Sheffe's post hoc test at a significance level of 95%. RESULTS: The control group exhibited the highest bond strength values, whereas IB group showed the lowest value before and after thermocycling. The DB group recovered its bond strength similar to that of the control group. The SA and TP groups exhibited similar bond strength values with those of the control and DB groups before thermocycling. However, The TP group did not maintain bond strength with thermal stress, whereas the SA group did. CONCLUSIONS: Applying a 10% sodium ascorbate solution rather than 10% alpha-tocopherol solution for 60 sec is recommended to maintain dentin bond strength when restoring non-vitally bleached teeth.

4.Two Cases of Squamous Cell Carcinoma of the Bulbar Conjunctiva.

Squamous cell carcinoma of the conjunctiva is a rare disease and usually arises at the limbus and spreads to the cornea, adjacent bulbar conjunctiva and sclera. We experienced two cases of squamous cell carcinoma of the cornea and bulbar conjunctiva. One patient was a 36-year old male with progressive loss of visual acuity and discomfort in the nasal side of the left eyeball for about 7 months. The other patient was a 67-year old male with excessive tearing, recurrent conjunctival injection and elevated mass on inferonasal side of the limbus of the left eye. Excisional biopsy was carried out and histopathologic examination revealed bulbar squamous cell carcinoma in both patients.

5.A Case of Early-Onset Childhood Sarcoidosis.

Early-onset childhood sarcoidosis is a unique form of the disease characterized by the triad of rash, uveitis, and arthritis in patients presenting before age 4 years. We report a case of early onset childhood sarcoidosis in a 4-year-old girl. She presented with fever, right ankle joint swelling, multiple skin lesions on whole body and was diagnosed as juvenile rheumatoid arthritis in padiatric department. She was transferred to dermatologic department for skin evaluation, histological investigation revealed well-circumscribed, non-caseating naked granuloma with epitheloid cells, histiocytes, lymphocytes on the upper dermis. Laboratory evaluation, chest X-ray, and ophthalmologic evaluation are all within normal limit. She is treated with systemic steroid, methotrexate, non-steroidal anti-inflammatory drugs and follow up evaluation will be planned for possible eye involvement.

6.Clinical Results of Selective Thoracic Fusion by Segmental Pedicle Screws in King type II Adolescent Idiopathic Scoliosis ( AIS ).

STUDY DESIGN: This study analyzes the results of selective thoracic fusion by segmental pedicle screw fixation in King type II adolescent idiopathic scoliosis(AIS). OBJECTIVES: To verify the efficacy of selective thoracic fusion by segmental pedicle screw fixation in King type II adolescent idiopathic scoliosis. SUMMARY OF BACKGROUND DATA: Decompensations following selective thoracic fusion in King type II adolescent idipathic scoliosis have been attributed to relative inability of the lumbar curves to accommodate correction of the thoracic curves. However, with segmental pedicle screw fixation, selective thoracic fusions yielded satisfactory results in most of the patients regardless of the lumbar curve characteristics. MATERIALS AND METHODS: Eighty patients subjected to selective thoracic fusion by segmental pedicle screw fixation were analyzed after a minimum follow-up of 2 years for deformity correction and spinal balance. Deformity correction was performed by derotation of rod contoured to the normal sagittal curve without additional compression or distraction. RESULTS: Mean preoperative thoracic curve of 53.3 was corrected to 20.3 showing a correction of 63.2%. The mean preoperative lumbar curve was 34.4 with a flexibility of 97.2% and mean lumboapical distance(LAD)of 18.0mm, L4 tilt of 8.0, and lumbar apical rotation (LAR) of 10.9. It was corrected to 16.5 showing a correction of 55.5%. All the patients were balanced clinically and radiologically. CONCLUSIONS: Selective thoracic fusion with segmental pedicle screw fixation and rod rotation offers a satisfactory correction of the deformity and maintenance of spinal balance in King type g AIS regardless of lumbar curve magnitude, LAD, L4 tilt or LAR.

The employment of segmental pedicle screw instrumentation has greatly enhanced the correction of scoliotic deformity. However there is still much controversy on the ability of the system to correct the rotational deformity. This prospective study with 12 idiopathic throacic scoliosis patients subjected to segmental pedicle screw instrumentation was performed to determine the effect of the system on the rotational deformity of idiopathic throacic scoliosis. In addition to the standing films to measure the magnitude of scoliotic curvatures, preand postoperative CT were utilized for the measurement and comparison of angle of rotation relative to the sagittal plane(RAsag.), relative rotation with reference to the upper and lower end vertebrae(RAend), relative rotation with reference to the sacrum(RAsac.), frontal translation(TF) and sagittal translation(TS) for the apical vertebrae. Despite the high correction rate obtained in the frontal plane(average 71.1%) and normalization of sagittal contour, the rotational corrections were statistically insignificant in all parameters and showed no significant correlation to the correction of the curvatures nor horizontal plane displacement as represented by the frontal and sagittal displacement. In conclusions, the segmental pedicle screw instrumentation with rod derotation does not produce significant derotation by itself.

8.Chemical Saturation Breath-hold Fast MR Imaging for Characterization of Regional Fatty Changes in Liver.

PURPOSE: To assess the usefulness of breath-hold fast MR imaging of liver with fat suppression (FS) by application of chemical saturation technique in the diagnosis of regional fatty changed suspected in sonography. MATERIALS AND METHODS: Thirteen patients who had focal lesions with diffuse, homogeneous signal changes after FS through chemical saturation technique without additional changes of imaging parameter during MR imaging of liver were selected. T1-weighed fast low-angle shot and T2-weighted turbo spin-echo sequences were obtained with or without FS during each single breath-holding session. Subjective changes of signal intensity between the pre-FS and the FS images were compared with the sonographic findings in each lesion. RESULTS: Seven lesions of decreased signal intensity after FS on T1 or T2-weighted images, including three lesions only at FS T1 images, were regarded as focal fat infiltration. All seven lesions had compatible sonographic findings as homogenously echogenic areas. Another six lesions of subjectively increased signal intensity including two lesions only at FS T2 images were regarded as focal fat sparing. All six lesions had sonographic findings as homogenous echo poor areas suggesting focal fat sparing. In cases regarded as fat infiltration, score changes were more prominent at FS T1 images than FS T2 images(p=0.0002). In cases regarded as fat sparing, score changes were more prominent at FS T2 images than FS T1 images(p=0.042). CONCLUSION: Breath-hold fast T1 and T2-weighted MR imaging with and without chemical saturation pre-pulse may be sufficient for characterization of regional fatty changes in the different ferential diagnosis of focal hepatic lesion found at sonography.

9.Breath-Hold MR Imaging of Focal Hepatic Lesions: Clinical Usefulness of Breath-Hold TSE T2WI Combined by FastLow-Angle Shot (FLASH) MR Imaging.

PURPOSE: To compare the image quality and diagnostic efficacy of turbo spin-echo (TSE) T2WI with breath-holdturbo SE T2WI and to evaluate the clinical usefulness combined breath-hold turbo SE T2WI with FLASH(fast low-angleshot) MR imaging for the evaluation of focal hepatic lesions. MATERIALS AND METHODS: A total of 47 patients with known or suspected hepatic mass were prospectively evaluated using a commercially available 1.5-T MR system. All patients were examined with conventional spin-echo T1WI, TSE T2WI, breath-hold TSE T2WI, and T1-weighted FLASHwith and without Gd-DTPA. The images were compared quantitatively (liver-lesion C/N ; CNR [contrast-to-noiseratio] and lesion detectability) and qualitatively (sharpness of anatomic structure, artifact, and overall imagequality). RESULTS: A total of 69 hepatic lesions were detected in 47 patients. Sixty-seven lesions (97.1%) were detected with Gd-FLASH, 66 (95.7%) with TSE T2WI, 65 (94.2%) with breath-hold TSE T2WI, 62 (89.9%) withnon-enhanced FLASH, and 55 (79.7%) with conventional SE T1WI. The CNR of cysts and hemangiomas was significantly greater on turbo SE T2WI and breath-hold TSE T2WI than on other sequences, but there was no significant difference between turbo SE T2WI and breath-hold TSE T2WI. For solid lesions, CNR was greatest on turbo SE T2WI and was similar on breath-hold TSE T2WI and Gd-FLASH without statistical significance, but was significantly higher than conventional SE T1WI. Breath-hold TSE T2WI and Gd-FLASH were qualitatively superior to other sequences except the vascular pulsation artifact of FLASH. Non-enhanced FLASH was also superior to conventional T1WI for CNR, lesion detectability, sharpness, respiratory motion artifact, and overall image quality. CONCLUSION: Breath-hold TSET2WI may replace turbo SE T2WI, and as well as conventional SE T1WI, FLASH with or without Gd-DTPA may be used for the evaluation of focal hepatic lesions. The combination of FLASH and breath-hold TSE T2WI may be an excellent technique that can be used to rapidly evaluate liver lesions, and at the same time offer superior overall imagequality.

Carcinosarcoma is a rare malignant neoplasm consisting of both epithelial and nonepithelial components, that is, carcinoma and sarcoma in the same tumor. Since Queckenstedt reported the case of carcinosarcoma of the stomach in 1904, only 30 cases have been reported. We present a case of carcinosarcoma and adenocarcinoma of the stomach in 63-year-old man with the review of the literature.
