4.Primary Cutaneous Cryptococcosis.

We report a case of primary cutaneous cryptococcosis on the left knee of a 67 year-old woman. She had a large ulcerated and indurated plaque with yellowish purulent exudates on her left knee. A histopathological examination from the lesion showed numerous encapsulated, round spores. Cultures from the lesion showed the presence of Cryptococcus neoforrnans. This may have resulted from an immunosuppressive state due to long-term use of oral corticosteroids.

5.The Role of Percutaneous Balloon Pericardial Window Formation for Malignant Pericardial Effusion.

BACKGROUND: There are several ways to treat for recurrent pericardial effussion and cardiac tamponade due to malignancy. They are repeated pericardiocentesis, pericardial instillation of sclerosing and chemotherapeutic agents, surgical creation of a pericardial window and transthoracic pericardiectomy. Surgical techniques are usually effective but bear a significant morbidity and mortality especially in chronic debilitating cancer patients. So percutaneous balloon pericardial window as an alternative to surgery in these patients. METHODS: After pericardiocentesis was performed, a 0.035 inch J-tip guidewire was advanced into the pericardial space. And a pigtail catheter was advanced over the wire. A moderate amount of pericardial fluid were removed. A nd then the pigtail catheter was withdrawn and 8F sheath was inserted. A20mm diameter, 4cm long(Single balloon method) or two 10mm diameter, 4cm long balloon dilating catheter(Double balloon medium) was advanced over the wire to straddle the parietal pericardial border though the sheath. Several inflations of the balloon with a solution containing 50% radiographic contrast medium were performed until disappearance of the balloon waist. After balloon dilation, contrast medium from the pericardial space to subcutaneous tisse suggesting successful PBPWF. Single ballon method was employed in 4 patients and Dould balloon method in 2 patients. RESULT: We performed percutaneous balloon pericardial window formation in 6 patiemts with malignant pericardial effusion. We did percutaneous balloon pericardial window formation successfully in 5 patients and failed due to adhesion of parietal pericardium in 1 patient. One patient developed recurrent pericardial effusion with tamponade at a mean follow-up of 11.49.6 months(1.5-26 months). Conclusion: These results suggest that PBPWF is an alternative method less invasive than subxiphoid surgical windowing, espesially in critically ill patients with recurrent malignant pericardial effussion. It carries less risks and has more constant effect than repeated pericardiocentesis.

6.Analysis of Treatement Results of patients with Tinea Unguium and Assessment of the Real Effectiveness of Antifungal Agents and Patient Compliance.

BACKGROUND: We have little information about long-term treatment results of tinea unguium based on the treatment methods and oral antifungal agents used in dermatology. OBJECTIVE: The purpose of this study was to assess the confirmed effectiveness and side effects of antifungal agents and to investigate the ratio of patients who completely finished the course of treatment in the practical area, not in the experimental studies. METHODS: The medical records of 684 patients diagnosed with tinea unguium from January 1985 to December 1996 in Yeungnam l.Jniversity Hospital were analysed for the results. RESULTS: The number of patients who were diagnosed with tinea unguium during this period was 684. In this study, we segregafed the patients whose outcome could be confirmed and calculated the percentage of completely treated and improved cases according to the antifungal agents used (griseofulvin-7.0%, ketoconazole-8.2%, itraconazole-9.0%, terbinafine-13.7%). We also calculated the percentage of patients who had completely finished the course of treatment (griseofulvin-6.8%, ketoconazole-7.4%, itraconazole-32.S%, terbinafine-31.8%). Among the patients whose results were unknown, more than half of patients(51.7%) had discontinued treatment within 3 weeks. The frequency of side effects of griseofulvin was 5.27a, ketoconazole 11.1%, itraconazole 6.7%, and terbinafine 6.1%. CONCLUSION: The treatment results of tinea unguium in clinical fields are different from results of previously reported studies. The percentages of completely treated and improved cases and those of patients who had completely finished the course of treatment are much lower than the results of previous experimental studies. Jt may be due to low compliance of patients and the long-term treatment course required. To increase the patients compliance and improve the results of treatment, we should explain to the patients about the disease course and duration of the treatment.

7.Experimental Iridocyclectomy.
Journal of the Korean Ophthalmological Society 1976;17(4):437-441
Management of the tumor of the iris and ciliary body is a difficult prablem. 244 cases of iridocyclectomy rather than enucleation had been reported from 1912 to 1970. But there seems to be no histopathological report. The author performed experimental iridocyclectomy in 20 albino rabbits, and observed clinically and pathologically one week interval for 6 weeks post-operathively. Surprisingly only a few cases of complications such as hyphema, vitreous hemorrhage and retinal detachment were observed. Post-operative wound healing was good. So the author concluded, "Iridocyclectomy itself isa delicate and difficult surgery, but skillful technique in a definitely indicated case using a operating microscope will get good result".

OBJECTIVE: To evaluate the efficacy of low dose gonadotropin releasing hormone agonist(GnRH-a) therapy combined with high dose human menopausal gonadotrpin(hMG) following estrogen & progesteron therapy for poor responders. METHODS: From May 1997 to Feb 1999, 36 patients who were defined as poor responders on previous consecutive two and more superovulation cycles were randomly allocated to lowdose GnRH-a short protocol with high dose hMG protocol pretreated with estrogen & progesterone(E/P therapy)(n=16)(study group) and the clomiphene citrate with hMG(n=20)(control group). All patients were planned to undergone in-vitro- fertilization(IVF) and embryo transfer(ET) after controlled ovarian hyperstimulation(COH). RESULTS: Two groups were similar with respect to clinical features and basal FSH and E2 levels. The mean level of E2 on day 5, 304.3+/-148.ng/ml in study group was significantly higher than that in control group, 182+/-34.9ng/ml. The mean levels of E2 on hCG day was also significantly higher in study group than control group(1324+/-320ng/ml, vs 414+/-168ng/ml). The mean day of hCG day in study group, 12.3+/-0.3 was shorter than that in control group, 13.8+/-0.4. The concellation rates of cycles were significantly lower in study group than control group(13.2% vs 84.2%). But clinical pregnancy rates did not showed the significant difference between two groups. CONCLUSION: The study suggested that a lowdose GnRH-a short protocol with high dose hMG pretreated with estrogen & progesterone can improve the ovarian response in poor responder group.

9.The Effect of Digitalis on the Ciliary Muscle.

Giiiary muscle strongly resembles the heart muscle in its functional behaviour, although from the anatomic point of view, it can be considered part of the smooth or involuntary muscle system. Visual acuity, refractive state, near point of accommodation and accommodative-convergence/ accommodation were studied in 30 persons before and after instillation of 2 drops of 0.01% digitalis solution. The results showed that the chapge of near point of accommodation was significant. So the author concluded, "Digitalis favorably influences the accommodation".

10.Study on the Effect of Hydrosalpinx Fluid on Sperm Motility.

This study was performed to identify the effect of the hydrosalpinx fluid on sperm motility. It has been reported that the patients with hydrosalpinx show the outstandingly lower success rate than other patients having infertility by different factors. It is unclear that the cause of it is influenced by hydrosalpinx fluid directly or by secondary chronic inflammation of endometrium. We wanted to know if the hydrosalpinx fluid influences sperm motility parameters directly such that it is related to the development of infertility. Therefore, using computer assisted semen analyzer (CASA), we observed, from February to July, 1997, how sperm motility, sperm progressive motility, sperm curvilinear velocity, sperm lateral head displacement, sperm straightness and sperm linearity change after treating normal sperm with hydrosalpinx fluid to evaluate sperm function on infertility. The result was that the study group (n=32) has the tendency to differ from the control group (n=32) on sperm motility, progressive motility, curvilinear velocity, lateral head displacement, straightness and linearity. We concluded that the hydrosalpinx fluid, with varying degree, directly has the harmful effects on sperm motility parameters, that is, curvilinear velocity, lateral head displacement and linearity of sperm which are related to the hyperactivation, hence decreased capacitation.
