1.A Case of Leptomeningeal Carcinomatosis on Superficial Spreading Carcinoma of the Stomach.

Meningeal carcinomatosis is an important neurological complication of systemic cancer, and is increasingly being recognized as a cause of reurologic disability in life. Also there is evidenee that the incidence of this form of metastasis is increasing. We reported a case of 52-year-old female patient, who was diagnosed as early gastric cancer by biopsy under gastrofiberscopy. Other Lab. findings were all normal range except high alkaline phosphatase level. And then performed Billroth II operation. On 7th post-operative day, she was encountered severe headache and insomnia. Abnormal neurologic signs and subjective symptoms were developed and progressed, Repeated lumbar punture was done and CSF was examed. There findings were abnormal, but no specific finding of certain disease. She died of severe neurologic deficit and sudden respiratory arrest. Autopsy was done, and confirmed leptomeningeal carcinomatosis.

2.A Case of Long-Term Survivor after Surgical Resection for Undifferentiated Embryonal Sarcoma of the Liver in Adult.

An undifferentiated (embryonal) sarcoma of the liver (USL) is a rare and highly malignant hepatic neoplasm of mesenchymal origin. This tumor almost exclusively affects pediatric patients, with a poor prognosis. The highest incidence is noted in pediatric patients, usually from 5~10 years of age, but only 43 cases of USL have been reported in adults worldwide since it was first as a clinicopathological entity. With a USL in adults, most patients are known to have died within 1 year of diagnosis. The absence of specific symptoms, rapid tumor growth, normality of common tumor markers and the consequential delay in diagnosis often result in significant enlargement of the tumor, with a poor prognosis. Various therapeutic modalities have been attempted in adult patients, but only a few long-term survivors have been reported. Herein, our experience of a 42-year-old female patient who is still alive, 55 months after the first resection for a USL is reported.

3.Serum C-Reative Protein As a Predictor for Cardiac Sequelae in Kawasaki Disease.

PURPOSE: The aim of this study was to identify the predictive factors in the early laboratory findings for cardiac sequelae in Kawasaki disease(KD). METHODES: A retrospective review of the records was conducted of all children with KD who were admitted to the Ulsan Dongkang General Hospital, Masan Samsung Hospital, and Gyeongsang National University Hospital between January 1995 and December 1999. We analyzed and compared the early laboratory findings between the patients with and without coronary artery dilatation. RESULTS: A total of 981 patients were divided into two groups : 826 patients(84.3%) with normal coronary artery and 155 patients(15.7%) with coronary artery dilatation. Age and sex were not significantly different between the two groups. The mean serum C-reactive protein(CRP) in the coronary artery dilatation group and in the normal coronary artery group were 5.0 mg/dl(+/-5.3) and 4.1 mg/dl(+/-5.0), respectively, with a significant difference(P<0.05), whereas the other early laboratory findings had no difference between the groups. CONCLUSION: This study shows that the early serum CRP was higher in patients with KD who had coronary artery dilatation than in those with normal coronary artery. There may be a strong possibility of cardiac sequelae at a high level of serum CRP. However, the cut-off value of serum CRP could not be determined for the prediction of cardiac sequelae in patients with KD.

4.The Relationship between Stress and Eating Habits of University Students.

BACKGROUND: Recently, many studies have reported that university students have been suffering from stress because of their the uncertainty of their future and employment. Eating habits have often been considered as one of health-related behaviors that may be affected by life stress. The purpose of this study was to examine the association between stress and eating habits in university students. METHODS: A cross-sectional study was designed. The subjects were 282 university students which assessed their eating habits and stresses. The total numbs of students in the study 263. Eating habits were scored using a questionnaire consisting of three categories-regularity, balance, and preference. The stress was assessed by modified Korean version of BEPSI. RESULTS: The stress score was 2.4 and the total score of eating habits was 44.2. The regularity among the domains of eating habits was 14.1, the balance score was 16.1, and the preference was 14.0. In multiple regression after adjustment with confounding variables, stress had a negative relationship with eating habits. CONCLUSION: The relationship between stress and eating habits was statistically significant, especially in regularity and balance. It is important to know not only the health status, but also the factors associated with health such as stresses and eating habits in order to improve the health status of the university students. Further research needs to uncover causality and make a generalization.

Crohn's disease is an acute or chronic inflammatory disorder, characteristically non- specific granuloxaatous neerotiziag, cicatrical low grade inflammation, occuring in various portions of the entire oro-gastraintestinal tract. The etiology is not clearly defined and rather uncertain. The disease is a heterogenous & dubious entity with multiple systemic manifestations. The incidence of the disease is geographically unequal in world wide distrihution. And male and female are equally involve. 6.Warfarin Induced Skin Necrosis.

Warfarin is widely used for the prevention of cerebral infarction, especially in patients with atrial fibrillation or artificial valve. Although hemorrhagic problems are well known, skin necrosis is a rare complication. Failures of early diagnosis or management may lead to serious results. We report a case of skin necrosis induced by warfarin therapy.

7.The Use of Sonohysterography in Infertile Patients.

OBJECTIVE: The purpose of this study was to assess the usefulness of sonohysterography in the detection of abnormalities of the uterine cavity in infertile patients, compared with other diagnostic methods, such as transvaginal sonography, hysterosalpingography and hysteroscopy. METHODS: Three intrauterine pathologies including intrauterine synechiae, endometrial polyp and submucosal myoma, that could be possible causes of infertility were diagnosed. RESULTS: Transvaginal ultrasonography and hysterosalpingography were able to detect 41.7% and 83.3% of uterine pathologies respectively comparing with sonohysterography to detect all of the uterine pathologies. CONCLUSION: Our results show that sonohysterography is easy, inexpensive, well-tolerated and non-invasive technique with high sensitivity in the detection of uterine pathologies. Therefore the use of sonohysterography for the diagnosis of intrauterine pathologies in infertile patients is highly recommended.

8.Cardiovascular disease risk differences between bus company employees and general workers according to the Korean National Health Insurance Data

BACKGROUND: Bus drivers are known to be highly at risk of cardiovascular diseases. In this study, we assessed the cardiovascular disease prevalence of bus company employees in Seoul, South Korea, and compared the results to those of general workers. METHODS: We analyzed the 2014 Korean National Health Insurance (NHI) data and defined hypertension, diabetes, dyslipidemia, ischemic heart disease, and cerebrovascular disease based on the KCD-6 medical diagnoses. We used bus company employees as surrogate participants of bus drivers due to the characteristics of Korean NHI data. We identified bus company employees in Seoul based on one’s workplace which the insurance is registered. The prevalence of five diseases was compared between the bus company employees and general workers. We also calculated the odds ratios (OR) of five diseases between the bus company employees and general workers. To compensate the vast demographical differences between the two groups, we performed propensity score matching. RESULTS: Bus company employees have higher OR for having hypertension (OR 1.33, 95% CI: 1.28–1.39), diabetes mellitus (1.14, 95% CI: 1.08–1.22), and dyslipidemia (1.23, 95% CI: 1.17–1.29) than the general workers or propensity score matched controls. However, the OR of having ischemic heart disease were not significant. The OR of cerebrovascular disease were lower in bus company employees than in the general workers after adjusting the covariates, but similar in the propensity score matched model. CONCLUSION: This study showed that the ORs of cardiovascular disease risk factors are high in bus company employees when compared to the general working population. Further studies with the longitudinal design should be conducted to confirm the causal association.

9.A Case of Primary Extracranial Meningioma in the Maxillary Sinus

Meningioma is one of the most common tumors of the central nervous system. However, primary extra cranial meningioma is rare as it consists 1%-2% of meningioma. It is very rare in paranasal sinus and accounts for about 0.1% of the paranasal sinus tumor. The treatment of choice is complete surgical excision of the tumor. Herein, with a review of the literature, we report a rare case of primary extra cranial meningioma in a 38-year-old male.
10.Effect Of Surface Treatments On The Bond Strength Of Denture Base Resins To Dental Alloy.

The purpose of this study is to compare tensile bond strength between Cr-Co alloy and three denture base resins after surface treatment. Following the manufacturer's instructions, 180 bonded specimens were made from three denture base resins (Lucitone 199, Paladent 20, POSS resin) and three surface treatment methods(sandblasting, metal primer, silicoating). 20 samples were made in each group and a half was thermocycled 1000 times between 5 celsius degrees and 55 celsius degrees. The tensile bond strength was measured using an Instron with 5mm/min crosshead speed. Data was analyzed with one-way ANOVA, T-test and Duncan test. The results were as follows: 1. Samples with metal primer coating had significantly high tensile bond strength than the other surface treated groups (p<.05). Significantly low tensile bond strength was shown in sand blasted groups (p<.05). 2. No significant difference was observed in metal primer coating groups before and after thermocycling (p>.05). 3. Tensile bond strength was decreased in silicoated samples after thermocycling (p<.05). 4. Of the surface treated groups with metal primer,Lucitone 199 had the greatest bond strength and POSS resin and Paladent 20 were followed (p<.05). 5. Of the surface treated groups with silicoating, POSS resin and Lucitone 199 had greater bond strength than Paladent 20 (p<.05).
