4.Arthroscopic Meniscectomy in Bucket Handle Tear of the Meniscus

Recently, the frequency of meniscal injury of knee has been increasing due to increase of sports activities. It has been known that bucket handle tear is the most common type of the entire injuries of the meniscus. We found 127 cases of meniscal tears and experienced 42 cases of bucket handle tears in the arthroscopic findings from Jan. 1991 to April 1994 in our hospital. We analysed 31 cases which were followed up for at least 1 year. The purpose of this study is to analyse the locking history and frequency, to discuss the several types of bucket handle tear in arthroscopic view, to introduce three portal techniques using posteormedial or posterolateral portal to resect the meniscus, and to analyse our clinical results. So we concluded as follows. 1. Among the 31 cases, 23 cases(74%) had an episode of locking. 2. We found many different types of bucket handle tears in arthroscopic view and the most common type was complete tear in a classic type(11 cases). 3. We were able to resect the meniscal fragment very easily using posteromedial or posterolateral portal. 4. We obtained satisfactory results in 24 cases(77%).

We report a rescently experienced case with atypical radiologic manifestation of solid and papillary epithelial neoplasm of the pancreas in a 37 years old female patient. The tumor had heavy calcification on its capsule wall and septa. Instead of the usual encapsulated lesion with partly solid and partly cystic-hemorrhagic components, the lesion consisted of numerous fine cavitations containing air without fluid component except focal abscess fluid.

7.Clinical Study of the Correlation of Tumor Necrosis Factor alpha and the Proteinuria of Henoch-Schonlein Nephritis and Idiopathic Nephrotic Syndrome.

PURPOSE: It is not clear that the development of glomerular injury and aggravation by tumor necrosis factor alpha (TNF-alpha) is related to intrarenal or serum concentration of TNF-alpha. So, we studied the relationship between the concentration of TNF-alpha and aggravation of glomerular damage in the Henoch-Schonlein nephritis(HSN) and idiopathic nephrotic syndrome(INS). METHODS: We collected the sera and urines of 21 patients with Henoch-Schonlein purpura(HSP) and 22 patients with INS visited Chungbuk National University hospital from March 1998 to March 2001. The concentration of TNF-alpha in the sera and urines were measured by sandwich ELISA. RESULTS: Serum TNF-alpha levels in the HSP patients with renal involvement were significantly higher than those without renal involvement(P=0.009). But urine TNF-alpha levels have no correlation with renal involvement(P=0.088). In the HSN patients, proteinuria have a significant correlation with serum TNF-alpha levels(P=0.004) but less correlation with urine TNF-alpha levels(P=0.053). Otherwise, proteinuria have no correlation with serum TNF-alpha levels(P=0.763) but have a significant correlation with urine TNF-alpha levels(P=0.007) in INS. CONCLUSION: These result suggest that the serum concentration of TNF-alpha would be important to glomerular involvement in HSP. And, it is interesting that proteinuria shows a significant relation with serum TNF-alpha levels in the HSN, but with urine TNF-alpha levels in the INS. This means the major production of TNF-alpha may be originated by extrarenal inflammation in the HSN and by intrarenal tubulo-interstitial damage due to proteinuria in the INS.

10.A Study of Visual Event-Related Potential P300 in Schizophrenia.

OBJECTIVE: Event-related potentials(ERPs) are electrical changes recorded at the surface of the scalp in response to stimulus presentation, and their latency and amplitude change according to cognitive processes. Through past studies of the auditory ERP in schizophrenia, the P300 has been reported to be statistically smaller and delayed in schizophrenia than comparison groups. However, studies of the visual ERP have not been systematically examined. The present study was designed to investigate the visual P300 in patients with schizophrenia and normal controls and to compare the pattern of P300 between them. METHODS: The subjects were composed of patients(N=22) with schizophrenia by DSM-IV and normal controls(N=22). The visual ERPs were measured by the visual continuous performance test. P300 amplitude and latency measured on 5 scalp electrodes(Fz, Cz, Pz, T7, T8) were compared between patients and controls. RESULTS: The P300 latencies measured on Fz, Cz, Pz, and T7 electrodes were significantly longer in patients than controls(p<0.05). The P300 amplitudes in patients were smaller than controls. However, the difference between them was not statistically significant. CONCLUSION: Analysis of the visual ERPs showed that the P300 latency is significantly delayed and the P300 amplitude is slightly smaller in patients than controls. These results are similar to established studies of the auditory P300 in schizophrenia.
