6.THE ROLE OF REGULATED PROTEINS RELATED TO CELL CYCIE IN CARCINOGENESIS.

The cell cycle is composed of G1, S, G2 and M phase. The transitions between different phases are regulated at checkpoint such as Start(restriction), S phase and mitotic checkpoint. These checkpoints are regulated by specific cyclins and Cdks(cyclin-dependent kinases). Especially, Start checkpoint in late G1 is though to be very important in control of cell cycle. In this study, it was shown various CDKN2(p16ink4A) alteration, including deletions, mutations, down regulations, and performed differential expression of p53, Cdk4, PCNA and pRb in stomach cancer tissues. 1. The frequency of CDKN2 mutations was not observed in the 19 primary stomach cancer tissues. In contrast to the mutations of CDKN2, mRNA levels was showed by Northern blot analysis that expression of CDKN2 was absent or decreased in 10 of the 19(53%) primary stomach adenocarcinoma. Western blot analysis was performed to determine the differential expression of p53, Cdk4, PCNA related to Start checkpoint. Overexpression of p53 was shown 38%, Cdk4 was expressed in all each specimens, and expression of PCNA was not shown. 2. As the other method to determine the differential expression of p53, Cdk4, PCNA and pRb, immunohistochemical analyses were performed on each 14 formalin-fixed and paraffin embedded tumor tissues of stomach adenocarcinoma. p53 overexpression was showed to clear nuclear staining only in tumor cells not in nonneoplastic cells. In staining for cdk4, the tumor was considered to be cdk4 positive if there was nuclear staining in tumor cells, regardless of cytoplasmic staining. PCNA staining for carcinoma tissues showed more intense nuclear staining in tumor cells than in nonneoplastic cells. pRb overexpression was show in tumor cells. Significant differences were observed in the expression of the proteins among the cancers from different anatomic site. Overexpression of adenocarcinomas had high rate of p53(57.1%) and pRb(71.4% ), and low late of cdk4(7.1% ) and PCNA(14.3% ), As these results, deletion of CDKN2 gene in human stomach cancer was not observed but mRNA expression was down regulated in restriction checkpoint, G1 phase. Inactivation of the CDKN2 gene due to hypermethylation may play an important role in development of cancer. And one of the abnormalities in p53, Cdk4, PCNA or pRb function occurs very common in various cancers, especially oral adenocarcinoma, osteosarcoma and squemous cell carcinoma, suggest that components in restriction checkpoint also play an critical role in the carcinogenesis and progression of cancers.

7.Clinical research of Shoulder Injuries Developed in Volleybal Players Spking at a Posterior Angle

There is no sport in which the shoulder does not play an integral role. There are many patients with shoulder joint problems in volleyball, baseball, tennis and golf. In volleyball, a quick strong spike is necessary. Therefor, the treatment of shoulder injuries as well as the protection of the shoulder is a great concern of the player. The authors have done and analysis of the playing history and the clinical studies of the volleyball players who have been recorded as patients of the department of orthopedic surgery, Korea National Police hospital, from January 1977 to December 1980. Results of the study are as follows: l. Of the ages between 17 and 25, the average age was 19.9. 2. Out of 228 case studies 96 complained of shoulder pain or 42.1%. 3. The 84, or 87.6% of the players used the posterior angle spike. The anterior angle spikers and middle angle spikers were 6 players or 6.2% each. 4. Of 60 posterior angle spikers attempted to change spiking angle, 46 or 76% were able to alieviable the shoulder pain. 5. In the posterior angle spikers the lever arm is at its shortest extent so the cases of pain were analyzed as due to repeated impingement of supraspinatus tendon.

8.Calcaneus Fractures: Clinical Study of Axial Fixation

There is a great difference of opinion regarding the treatment of intraarticular fractures of the calcaneus. Many authors suggested that there are essentially four basic methods of treatment, but the choice of treatment depends on the surgeons preference, the type of fracture, and the age of the patient. We obtained the following results in 11 cases of calcaneal fractures involving subtalar joint which were treated by axial fixation at our hospital from 1977 to 1981. 1. The favourable results were obtained in 5 cases of 6 feet of tongue type fractures and in 3 cases of 5 feet of joint depression fractures. 2. Even if the joint depression type fracture which is not severely comminuted, the treatment with axial fixation is thought to be good method. 3. Axial fixation must be tried in three faces of reduction which are reestablishment of Bohler angle and subtalar joint and reduction of lateral bulging of calcaneus. The results were more excellent in which reduction is achieved near anatomical position.

9.Ultrasonographic determination of fetal gender

Sonographic determination of fetal gender was attempted prospectively in most pregnancies of more than 26 weeks. We stuided 193 cases of pregnancies with ultrasound for recent 9 months from June 1984 to Feb. 1985 atdepartement of radiology, Soonchunhyang university, Soonchyunhyang chunan hospital, and analysed ultrasonographicfinding of fetal gender. The results were as follows; 1. Overall accuracy rate for fetal gender is 90%. 2.Accuracy rate for male fetus is 97.8%. 3. Accuracy rate for female fetus is 88.2%.

10.Clinical Observation on Injury of Genitourinary Tract in Childhood.

Clinical observation was made on the injuries of genitourinary tract of in patient under 15 year in the department of urology, Hart Yang University Hospital during the period from May, 1972 to April, 1983. The results were as follows: 1. Of 3140 cases hospitalized, 30 cases were injury of genitourinary tract in children giving ratio of 0.96%. 2. The favorable age was from 7 to 9years for 40%. The sex ratio, male to female, was 4:1.3. Traffic. accident was the most frequent cause of injury (53.3%) and the ,next was fall-down (43.3%), especially 8 cases of fall-down were from playing. 4. The kidney was involved most frequently in 35.1% (8 cases were fight side) and urethra was next in 32.4%,external genitalia 21,6% bladder in 10.8%. 5. The fracture was the most frequent associated injury (71.4%) and it's sites of fracture were pelvis, femur, ribs, and etc. 6. Treatment of the renal injury was performed as conservative treatment frequently (66.7%) and 5 cases were undergone nephrectomy. Posterior urethra was involved more commonly and. preferable treatment was primary realignment with interlocking sound.
