1.Bone Mineral Density in Well Controlled IDDM.

PURPOSE:Effects of IDDM on bone mineral metabolism are still in controversy. Some reported that bone mineral density in IDDM had inverse relationship with HbA1c, some reported that spine BMD was normal while femur BMD was decreased. Others reported that increased urinary calcium excretion in IDDM induced early trabecular bone mineral loss. We studied the correlation of BMD with diabetic control and body measurements. METHODS:In sixteen IDDM patients, using dual energy X-ray absorptiometry, BMD was measured in lumbar spine as trabecular bone and femur neck as cortical bone. Z-score of BMD was obtained by comparing age and sex matched control data. Correlations between BMD and diabetic control parameters (HbA1c, duration of IDDM) and body measurements were calculated. RESULTS:The body measurements were in normal range in all IDDM patients, the duration of IDDM was 38.4+/-24.0months, HbA1c was in good control state (7.69+/-1.53%), and urinary Ca/creatinine ratio was not increased. The Z-score of BMD was not decreased statistically (lumbar spine: -0.255, femur neck: -0.404), and the Z-score had no correlationship with body measurements and diabetic control parameters. CONCLUSIONS:In well controlled childhood IDDM, BMD was not decreased significantly.

2.Five Cases of Cutaneous Leishmaniasis.

Cutaneous leishmaniasis is a self-limited infection of the skin caused by Leishmania tropica, a protozoan parasite transmitted by Phlebotomus sandfly. This disease is endemic in tropical and subtropical zone, but recently many cases of cutaneous leishmaniasis were reported in Korean among the peoples who had been worked in the Middle East. We experienced five cases of cutaneous leishmaniasis developed in Korean who had been worked in Saudi Arabia and Jordan. Skin lesions of various sized, central ulcerated and marginal elevated, with serosanguinous discharge and some crust were scattered on exposed area, especially both extremities. Histopathologic findings showed hyperkeratosis, acanthosis, pseudoepitheliomatous hyperplasia, dermal infiltration of histiocytes, lymphocytes, plasma cells and neutrophils, and numerous Leishman-Donovan bodies in and around histiocytes. About 1-4 months treatment with trimethoprim-sulfamethoxazole and metronidazole, two cases were healed with hyperpigmented scar and two cases were improved but one case was not responded.

3.Diffuse Pigmented Villonodular Synovitis of the Hip Joint

In 1941, Jaffe and coworkers studied a lesion with histologic appearance of fibrous stroma, pigmented deposition and histiocytic infiltration as well as giant cell, for which they named pigmented villonodullar synovitis, bursitis and tenosynovitis. Thereafter many authors have disscussed etiology, clinical and radiological features, pathology and treatment regarding the disease. Recently we experienced a case of pigmented villonodular synovitis involving an hip joint which was treated by synovectomy and total hip replacement with good result.

4.Quality of Life after Total Knee and Total Hip Replacement.

We reviewed 40 consecutive patients having a primary total hip replacement(THR) and 60 patients having a primary total knee replacement(TKR) for osteoarthritis to compare the qu;dity of hfe(QoL) before and after operation. Bilateral arthroplasties were perfomed 10 cases of THR and 25 cemes of TKR. We used a modified Harris hip score and a knee score of American knee society, the Rosser Lndex Matrix and authors eval- uation system to generate these scores. Quality of life was highly improved by Rossers and authors evalua- tion system after THR and TKR. The median values of QoL scores before and after operation were signifi- cantly different(p<0.05). The median preoperative QoL score in THR was t>etter than in TKR. Postoperative QoL scores for both groups were similar. Quality of life evaluated by Rossers score and authors evaluation system in bilateral THR was better than bilateral TKR. We think better quality of life in bilateral THR over bilateral TKR by authors evaluation system is because the hip joint is mcire stable and has better range of motion than knee joint. We conclude that change of life style including the use of bed, toilet seat elevation, and the use of dinning table will be neccesary after bilateral TKR.

5.A Case of Herpes Gestationis.

Herpes Gestationis is a rare polymorphous, vesiculobullous disease of pregnancy and puerperium. It is characterized by rapidly progressive skin eruption heraled initially by severe generalized pruritus. Erythematous papules and urticaria like plaques rapidly appear followed by grouped, tense, vesiculobullous eruptions. A 23 year-old pregnant woman was admitted to our departrnent of dermatology hecause of erythematous patches with pruritic vesiculobullous and pustular eruption on the whole body since third week of conception, Biopsy specimen of bullous skin lesion showed spongiosis of epidermal cells, mild necrosis of basal cells, subepidermal bulla, edema, of papillary derrnis and perivascular infiltration of inflammatory cells. Ultrastructural findings showed mild necrosis of basal cells, bulla above or below basement membrane zone and partial separation of basal larnina. Direct immunofluorescences of bullous lesion and normal appearing skin demonstrated deposition of IgG in linear pattern along basement rnembra,ne zone. After treatment with systemic corticosteroid and antihistamin, the lesions were cleared, leaving residual hyperpigmentation, and she delivered normal female baby.

6.The Effect of Room Temperature on the VDRL Test Result.

The Venereal Disease Research Laboratory microflocculation (VDRL) test, especially the quantitative test is well standardized, simple, rapid and, at present, the most widely used non-treponernal test in serodiagnosis and follow up sturly of syphilis. However, the UDRL test result could be influenced by room ternperature, especially in summer and winter in Korea in the abscence of proper air-conditioning and heating. This study was undertaken to evaluate the effect of room temperature on the VDRL test result and to determine the critical temperatures which alter the test result. Eighty six VDRL-reactive serum samples collected at the Soonchunhyang Medical College Hospital and the V.D. Clinic of the Chung-Ku Public Health Center in Seoul during the 7 months periods from September 1982 to March 1983 were used. The results obtained were as follows: The change in the VDRL titer was not obscrved at room temperature of between 18 C and 32 C 2. The VDRL titer became weakly reactive on the same titer or lowered to half titer at room temperature of below 18C. R. The VDRL titer increased two fold at room temperature of above 32C.(countinue..)

9.Surgical Management of Infectious Endophthalmitis: Early Vitrectomy vs Late Vitrectomy.

Authors investigated the effect of the timing of vitrectomy for endophthalmitis on the final visual acuity, by retrospectively reviewing the clinical records of patients who have undergone pars plana vitrectomy for the treatment of endophthalmitis from Feb. 1990. Twenty one cases(21 eyes) were included who were followed up for 6 months or longer after surgery and intravitreal injection of antibiotics. 14 cases were male. The age ranged from 28 to 81 years, with mean age of 40.5 years. Ten cases arose from intraocular surgeries, with 7 cases of early-onset and 3 cases of late-onset, the other 10 cases were related to ocular trauma and the remaining one case was associated with a filtering bleb. Intravitreal injections, subconjunctival injections and topical eye instillation of antibiotics were preformed as an initial treatments on cases whose retina were invisible due to severe inflammatory media opacities. Those cases who did not respond to initial treatments were divide into two groups: one which underwent early vitrectomy within 2 days, and the other which underwent late vitrectomy after 2 days. Ten out of 12 cases among early vitrectomy group and 3 out of 9 cases among late vitrectomy group obtained final vision of 0.1 or better. The cases which underwent early vitrectomy showed good visual prognosis with statistical significance(P<0.05). Therefore, authors suggest that management of endophthalmitis with initial severe inflammatory reactions should include early diagnosis, and early vitrectomy with intravitreal infection of antibiotics for recovery of maximal visual acuity.

10.Clinical Experience with Black-Diaphragm Intraocular Lens in Traumatic (Postoperative) Aniridia and Aphakia.

When aniridia is associated with aphakia after trauma, traditional implantation of posterior chamber IOL has some technical difficulties owing to lack of iris support and weakness of zonule, but especially big problem lies in that it cannot satisfy improvement of symptoms such as photo phobia, epiphora and visual acuity simultaneously. For the purpose of correcting these two diseases, specially designed artificial lens (Black-Diaphragm IO-LMORCHER IOL) has been introduced and is being used currently. Black-Diaphragm IOL may have some postoperative complications such as persistent intraocular inflammation, corneal endothelial cell damage, cystoid niacular ederna, and increased intraocular pressure. On 6 patients (4 rnales, 2 females) who had aniridia associated with aphakia after trauma authors implanted Black-Diaphragm IOL into ciliary sulcus by transscleral fixation. During average of 7.3 months follow up periods, we observed corneal ederna, hyphema, vitreous hernorrhage in I eye (I case) but, in the remaining 5 eye.
