3.Right Atrial Myxoma Showing Granulomatous Lesion with Pulmonary Infart: A case report.

Cardiac myxoma is the most common primary tumor of the heart. The tumor contains a variety of cell types that are thought to arise from a focus of primitive pluripotential mesenchymal cells in the area of the fossa ovalis. Throughout the myxoid stroma, there are variable amounts of reticular fivers, collagen, elastic fibers and smooth muscle cells. A 38-year-old female had right atrial myxoma with multiple pulmonary infarcts. In this case, we experienced an unusual degenerative change in the tumor of granulomatous lesion consisting of hemosiderin pigments, foreign body giant cells and peculiar, spheroid, semilunar or bamboo-shaped degenerated elastic fibers. Microscopically it resembles Gamna-Gandy nodule seen in the spleen of chronic passive congestion.

4.The Effects of Doxapram on the Pulmonary Function during Total Intravenous Anesthesia with Propofol According to Nalbuphine Pretreatment.

BACKGROUND: Total intravenous anesthesia with propofol can cause respiratory depression and apnea especially during induction of anesthesia. To study the possibility of reversal of respiratory depression during anesthesia with propofol, pretreated with nabuphine or not, the respiratory effects of doxapram to spontaneously ventilating patients were investigated. METHODS: Patients were divided into 4 groups - saline-propofol-saline group (SPS), saline-propofol- doxapram group (SPD), nalbuphine-propofol-saline group (NPS), and nalbuphine-propofol-doxapram group (NPD). After saline or nalbuphine pretreatment, anesthesia was induced with propofol and then doxapram or saline was intravenously injected. Apneic time interval, blood pressure, heart rate, respiratory rate, minute ventilation, end tidal CO2 partial pressure and oxygen saturation were measured in every minutes during induction of anesthesia. Percent changes of each values were compared. RESULTS: There is no differences in apneic time intervals in each groups. The percent change of first minute ventilation in SPD group after doxapram injection unchanged significantly compared with those depressions of SPS, NPS and NPD group (p<0.05). Respiratory rates increased in SPD and SPS groups after laryngeal mask insertion. There is no differences in minute ventilation, respiratory rate and end-tidal CO2 concentration between nalbuphine pretreated groups regardless of doxapram injection. CONCLUSIONS: Doxapram has effect in increasing minute ventilation after propofol induction within first few minutes, but it cannot reverse respiratory depression during propofol induction pretreated with nalbuphine.

5.Mutagenicity of Human Urine Excreted after Ingestion of Roast Beef.

This study was undertaken to observe the mutagenic occurrence in urine excreted after the ingestion of roast beef. Two healthy nonsmoker persons of both sex were selected for this test, employing two strains (TA98, TA100) of Salmonella typhimurium according to Ames' method. The mutagenic activity began to appear in urine of both sex three hours after ingestion of 300 g of roast beef, gradually increasing until 6 hours and declining thereafter.

6.A Histopathologic, Histochemical and Immunocytochemical Study of Cardiac Myxoma.

The immunoreactivities of surgically removed 16 cardiac myxomas were studies for factor VIII-related Ag (F VIII-RA), Ulex europaeus agglutinin I (UEA-I) and desmin alpha1-antichymotrypsin, and this study was accompained by a clinicopathologic review. More than 50% of the patients with cardiac myxomas were in their fourth and fifth decades, and cardiac myxomas were much more common in women than in men. All but on occurred in the left atrium, and the majority were attached to the atrial septum, usually in the region corresponding to the fossa ovalis. In one case, an atrial myxoma recurred 37 months after the initial excision. Microscopically, the myxomas contained a myxoid matrix composed of acid mucopolysaccharides within which were embedded polygonal cells. The cells forming both the surface and complicated vascular like channels throuhout the myxoid stroma tested positive for F VIII-RA and UEA-I. The outer cell layers of the complex vascular structures demonstrated variable staining for F VIII-RA, while isolated bundles of smooth muscle cells were present and stained for desmin. A small number of the so-called myxoma cells, immunoreactive for alpha1-antichymotrypsin which were not laden with hemosiderin pigment but were similar to histiocytes, were present particularly around the areas of hemorrhage. These findings support the current view that cellular and histologic heterogeneity arose from the divergent differentiation of multipotential mesenchymal cells. In particular, it remains to be confirmed by further study whether or not true histiocytic differentiation occurs.

7.Evaluation of Centaur Syphilis, Immulite Syphilis, and Mediace TPLA for Detecting Treponemal Antibodies.

BACKGROUND: We evaluated the efficacy of two chemiluminescence immunoassays that detect treponemal antibodies, Centaur Syphilis and Immulite Syphilis, in comparison with Mediace Treponema pallidum latex agglutination (TPLA). METHODS: The study was conducted in two phases. In the first phase, we tested 1,147 serum samples that were sequentially submitted for routine syphilis serology. In the second phase, we tested a panel of 119 frozen serum samples that had previously tested positive by Mediace RPR. The kappa value, total agreement percentage, and sensitivity and specificity were analyzed. RESULTS: Of the 1,147 random samples, 24 (2.09%) tested positive with Centaur Syphilis, 16 (1.39%) with Immulite Syphilis, and 19 (1.66%) with Mediace TPLA. Of the 119 Mediace RPR-positive samples, 103 (86.6%) tested positive with Centaur Syphilis, 101 (84.9%%) with Immulite Syphilis, and 105 (88.2%) with Mediace TPLA. The percent agreements (kappa values) were 98.8% (0.934) between Centaur Syphilis and Mediace TPLA, 99.0% (0.94) between Immulite Syphilis and Mediace TPLA, and 99.2% (0.955) between Centaur Syphilis and Immulite Syphilis. To measure the sensitivity and specificity of each treponemal test, samples showing agreement in three or four of the tests (three treponemal tests and Mediace-RPR) were regarded as true positive (n=117) or true negative (n=1,142). The respective values for sensitivity and specificity were 100% and 99.6% for Centaur Syphilis, 98.3% and 100% for Immulite Syphilis, and 99.2% and 99.7% for Mediace TPLA. CONCLUSIONS: Results from the three treponemal assays were in good agreement. Greater sensitivity of Centaur Syphilis and greater specificity of Immulite Syphilis were suggested.

9.Cellular Fatty Acid Analysis of Vibrio vulnificus Strains Isolated from Korea.

Vibrio vulnificus infection is one of the most fatal diseases in Korea. This study was undertaken to determine the cellular fatty acid (CFA) compositions of ninety-five clinical strains of V. vulnificus isolated from Korea during 1985-1995. We compared these results with the CFA profile of V. vulnificus in the Microbial Identification System (MIS) (CLIN library version 3.9; Microbial ID Inc., Newark, Del.), and also evaluated the MIS ability to identify V. vulnificus. Subgrouping of V. vulnificus by CFA analysis was performed and its results were compared with those of serotyping. Most of the CFAs in V. vulnificus strains were similar to the CFA profile of V. vulnificus in the MIS, but some distinctive differences were observed. First, means of two major CFAs, 16:0 and 16:1w7c, were 22.16% and 18.26% in this study, but 23.52% and 25.44% in the MIS respectively. Second, all isolates had 11:Oiso3OH, which was not present in the MIS. Eighty-five strains (89.5%) disclosed the first choice identification of V. vulnificus by the MIS, but only two strains (2.1%) were identified with SI values of 0.6. Remaining ten strains (10.5%) showed 'NO MATCH' results. Cluster analysis of CFA could separate V. vulnificus into nine subgroups, and predominant subgroups were subgroup VII (45 strains) and V (36 strains). There was heterogeny between subgroups by CFA and serotypes of V. vulnificus. The strains of 04 serotype which accounted for 80% (76/95) of the isolates were distributed into six different subgroups such as VII (40 strains), V (27 strains), III (4 strains), I (2 strains) and VI (1 strain). These showed that V. vulnificus strains isolated from Korea had different characteristics in the CFA composition in comparison with the MIS V. vulnificus library. Subgrouping by the CFA analysis might be a useful tool for the epidemiological study of V. vulnificus infection in Korea.

10.Korean Women's Causal Perceptions of Hwabyung.

PURPOSE: This study elicited Korean women's perceptions of the life situations that contribute to Hwabyung, a culture bound psychiatric illness in Korea, and delineated a typology of the perceived causative factors for this illness. METHOD: A purposive sample of 21 Korean women was recruited from a church, a senior center in Seoul and a clinic that provides traditional Korean and westernized medical services to patients in the Gyeonggi area. Inclusion criteria were: 1) the diagnosis of Hwabyung by a traditional doctor or "stress reaction and depression" by a westernized doctor; and 2) a score greater than 40 on the Hwabyung Self Report Instrument. Surveys using the HSRI and individual interviews using Q methodology were used. RESULT: The average age of the participants was 53.7 years (range 35-84). The mean score on the HSRI was 46.2 (range = 42-52). Data analysis showed that participants perceived three life situations to cause Hwabyung: vulnerable situation, lowered self esteem, and negative life events. CONCLUSIONS: Korean women, suffering from Hwabyung, perceived at least three different causal patterns for this illness. Nursing care plans should be tailored to meet these differences.
