1.Extramedullary Versus Intramedullary Alignment Guide Systems in Total Knee Arthroplasty.

Fifty four consecutive total knee arthroplasties were reviewed to compare the accuracy of extramedullary versus intramedullary tibial resection guides. An extramedullary guide ( Group I ) was used in 25 cases and an intramedullary guide ( Group 2 ) was used in 29 cases. Group 1 system included the LCS knee system and Group 2 system included the Whiteside Ortholoc Advantim total condylar knee system. Preoperatively, the two groups were similar, with no statistical significant differences observed in diagnosis, alignment, and patient age. Postoperative tibial component alignment angles were similar in both group (Group 1, 0.8degrees varus; Group 2, 1.1degrees varus ). In group 1, 84% of tibial components were aligned within 2degrees of the 90degrees goal and in group 2, 83% of tibial components were aligned within the same range ( p > 0.05 ). In all cases, an intramedullary guide was used to prepare the distal femur. In femorotibial angles, with group 1 averaging 4.2degrees valgus versus 4.7degrees valgus in group 2 (p > 0.05). If the optimal femorotibial angle was defined by the range from 5+/-2, it was achieved in 76% in Group 1 and 72% in Group 2 ( p > 0.05 ). This study demonstrates no significant differences between the extramedullary and intramedullary group not only in optimal tibial component alignment angle but also in optimal femorotibial angle. This means that each system is satisfactory for the tibial side, but it is more important for the surgeon to appreciate that the indications, potential limitations and sources of errors are unique to each system and to decide which system to use according to the particular case in question.

3.Clinical research of Shoulder Injuries Developed in Volleybal Players Spking at a Posterior Angle

There is no sport in which the shoulder does not play an integral role. There are many patients with shoulder joint problems in volleyball, baseball, tennis and golf. In volleyball, a quick strong spike is necessary. Therefor, the treatment of shoulder injuries as well as the protection of the shoulder is a great concern of the player. The authors have done and analysis of the playing history and the clinical studies of the volleyball players who have been recorded as patients of the department of orthopedic surgery, Korea National Police hospital, from January 1977 to December 1980. Results of the study are as follows: l. Of the ages between 17 and 25, the average age was 19.9. 2. Out of 228 case studies 96 complained of shoulder pain or 42.1%. 3. The 84, or 87.6% of the players used the posterior angle spike. The anterior angle spikers and middle angle spikers were 6 players or 6.2% each. 4. Of 60 posterior angle spikers attempted to change spiking angle, 46 or 76% were able to alieviable the shoulder pain. 5. In the posterior angle spikers the lever arm is at its shortest extent so the cases of pain were analyzed as due to repeated impingement of supraspinatus tendon.

4.Calcaneus Fractures: Clinical Study of Axial Fixation

There is a great difference of opinion regarding the treatment of intraarticular fractures of the calcaneus. Many authors suggested that there are essentially four basic methods of treatment, but the choice of treatment depends on the surgeons preference, the type of fracture, and the age of the patient. We obtained the following results in 11 cases of calcaneal fractures involving subtalar joint which were treated by axial fixation at our hospital from 1977 to 1981. 1. The favourable results were obtained in 5 cases of 6 feet of tongue type fractures and in 3 cases of 5 feet of joint depression fractures. 2. Even if the joint depression type fracture which is not severely comminuted, the treatment with axial fixation is thought to be good method. 3. Axial fixation must be tried in three faces of reduction which are reestablishment of Bohler angle and subtalar joint and reduction of lateral bulging of calcaneus. The results were more excellent in which reduction is achieved near anatomical position.

6.Treatment outcome of ductal carcinoma in situ patients treated with postoperative radiation therapy.

PURPOSE: To evaluate the outcome of ductal carcinoma in situ (DCIS) patients who underwent surgery followed by radiation therapy (RT). MATERIALS AND METHODS: We retrospectively reviewed 106 DCIS patients who underwent surgery followed by postoperative RT between 1994 and 2006. Ninety-four patients underwent breast-conserving surgery, and mastectomy was performed in 12 patients due to extensive DCIS. Postoperative RT was delivered to whole breast with 50.4 Gy/28 fx. Tumor bed boost was offered to 7 patients (6.6%). Patients with hormonal receptor-positive tumors were treated with hormonal therapy. RESULTS: The median follow-up duration was 83.4 months (range, 33.4 to 191.5 months) and the median age was 47.8 years. Ten patients (9.4%) had resection margin <1 mm and high-grade and estrogen receptor-negative tumors were observed in 39 (36.8%) and 20 (18.9%) patients, respectively. The 7-year ipsilateral breast tumor recurrence (IBTR)-free survival rate was 95.3%. Resection margin (<1 or > or =1 mm) was the significant prognostic factor for IBTR in univariate and multivariate analyses (p < 0.001 and p = 0.016, respectively). CONCLUSION: Postoperative RT for DCIS can achieve favorable treatment outcome. Resection margin was the important prognostic factor for IBTR in the DCIS patients who underwent postoperative RT.

8.Craniofacial malformation treatment: craniosynostosis and positional plagiocephaly.

After the publication of the modern Virchow's suture fusion hypothesis regarding craniosynostosis, various types of linear craniotomy have been developed. However, after the Moss's functional matrix hypothesis became known, extensive cranial remodeling surgical procedures have emerged. However, a recent view that the cause of craniosynostosis may be due to gene mutation has led to a tendency toward treating craniosynostosis with minimally invasive surgery including endoscopic surgery and distraction procedures that utilize springs or distractors. As nonsyndromic craniosynostoses are accompanied by unilateral coronal or lambdoid craniosynostosis, and syndromic craniosynostoses are accompanied by facial anomalies, it is presumed that cranial anomalies are accompanied by facial anomalies. However, the "back to sleep" campaign that was initiated in the 1990's in order to prevent infantile death syndrome led to research in the dramatic increase in the incidence of craniofacial anomalies, which resulted in the establishment of the positional plagiocephaly concept, which has also been ascertained in animal experiments. Despite these advances, the basic problem of whether craniosynostosis is simply a cosmetic anomaly or whether it is a neurological disease that is accompanied by complications such as increased intracranial pressure has not been resolved. The consequent confusion has prevented establishment of the optimal timing for surgery and the type of surgical procedure. The authors of this study review the history of craniosynostosis treatment and attempt to clarify the situation pertaining to the surgical treatment concepts and limitations.

9.Analysis of DNA Ploidy Pattern of Hepatocellular Carcinoma with Comparison between Flow Cytometric and Image Cytometric Study.

Fifty cases of hepatocellularcarcinoma were studied using retrospective flow cytometric(FCM) and image cytometric(ICM) DNA analysis to determine the prevalence of aneuploid cell population and whether they were associated with any particular clinico-pathologic findings. At the same time, we compared the difference between FCM and ICM. The materials were prepared from 50 micrometer cut of paraffin embedded blocks. The DNA modal values, which could be defined in 42 cases by FCM with 74% aneuploidy and in 50 cases by ICM with 76% aneuploidy. So 95% of the cases had concordant DNA ploidy results by both techniques of FCM and ICM. Abnormal DNA pattern was correlated with age(<50), presence of cirrhosis, pathologic grade and some pathologic types(p<0.05), but was not correlated with presence of HBsAg, sex, alphafeto protein, and alcohol history(p>0.05). Also we found that ICM technique was easier to perform and interprete.
