4.Prognostic Impact of pT3a Components (Perirenal Fat or Adrenal Gland Involvement) in Patients with pT3b Renal Cell Carcinoma.

PURPOSE: To analyze the prognostic impact of the pT3a components, perirenal fat or adrenal gland involvement, in patients with pT3b renal cell carcinoma (RCC). MATERIALS AND METHODS: Between January 1997 and June 2006, a total of 36 patients with pT3b RCC who underwent radical nephrectomy with complete resection of the tumor thrombus were included. The presence of pT3a components was reviewed retrospectively, and disease-specific survival was compared between the pT3b only group and the pT3b with pT3a group by the Kaplan-Meier method. To evaluate the prognostic impact of the pure pT3a and pT3b components, we also compared these results with those of pT3aN0M0 only patients (n=44). RESULTS: After a thorough review by a pathologist, 21 patients were identified as having pT3b only and 15 patients as having pT3b+pT3a. The mean disease-specific survival time in those with pT3b only was significantly longer at 42+/-7 (SE) months compared with 18+/-4 (SE) months in those with pT3b+pT3a. The mean disease-specific survival time in those with pT3aN0M0 was significantly longer than in the pT3bN0M0 group and pT3b+pT3aN0M0 group. In multivariate analyses by use of the Cox-proportional hazard model, the presence of pT3a components (p=0.032), preoperative metastasis (p=0.001), and preoperative ECOG performance status (p=0.001) were recognized as predictors of a poor prognosis. CONCLUSIONS: In patients with pT3b RCC, the presence of pT3a components might be indicative of significantly poor prognosis. Careful preoperative and postoperative evaluation should be performed, and pT3b RCC should be reclassified according to the presence of pT3a components.

6.Acute Acalculous Cholecystitis Associated with Sunitinib Treatment for Renal Cell Carcinoma

A 64-year-old man was treated with sunitinib as a first-line therapy for metastatic renal cell carcinoma. He was given oral sunitinib in cycles of 50 mg once daily for 2 weeks followed by a week off. During the 5th week of treatment right upper quadrant pain developed, but this resolved spontaneously during the 6th week (off treatment). However, on the 8th week of treatment, he was admitted to hospital because the acute right upper quadrant pain recurred with nausea, vomiting, and fever. Acute acalculous cholecystitis was then diagnosed by ultrasonography and CT. In addition, his laboratory findings indicated disseminated intravascular coagulation. Accordingly, sunitinib therapy was discontinued and broad-spectrum antibiotics initiated. He subsequently recovered after emergent percutaneous cholecystostomy. His Naranjo Adverse Drug Reaction Probability Scale score was 7, indicaing a probable association of the event with sunitinib. Suspicion of sunitinib-related acute cholecystitis is required, because, although uncommon, it can be life-threatening.

7.Measurement of inferior vena cava diameter, alpha-human-atrial natriuretic peptide and cyclic guanosine 3':5'-monophosphate for estimation of dry weight and intravascular volume change in hemodialysis patients.

BACKGROUND: A correct estimation of volume status and dry weight in dialysis patients remains a difficult clinical problem. Clinical status and chest X-ray are not sensitive enough, while invasively measured central venous pressures are not routinely available. Recently, the ultrasonographic determination of the diameter and collapse index of the inferior vena cava has been proposed as a noninvasive method for estimating intravascular volume. We tried to evaluate the clinical relevance of this method in dialysis patients by comparing it with alphahuman-atrial natriuretic peptide (alpha-h-ANP) and cyclic guanosine 3:5-monophosphate (cGMP) levels. METHODS: Using echocardiography, the diameter of the inferior vena cava (VCD) and its decrease on deep inspiration (collapse index : CI) were evaluated in 27 hemodialysis patients. Echocardiography of the inferior vena cava (IVC) was performed in the supine position after 10 minutes rest. The transducer was placed in the subxiphoid region and long and short axis views of the IVC were obtained just below the diaphragm in the hepatic segment. VCD was measured before the P-wave on the electrocardiogram to avoid interference with A-wave and V-wave on the venous pressure curve, and corrected for body surface area. Preand post-hemodialysis levels of the plasma alpha-h-ANP and cGMP were measured by radioimmunoassay. The relationship between VCD, CI determined by echocardiography, and alpha-h-ANP and cGMP concentrations were studied. RESULTS: The levels of alpha-h-ANP and cGMP were markedly elevated before hemodialysis and significantly lower values were found after hemodialysis (alpha-hANP : 162.7 102.6 pg/ml vs 90.6 61.0 pg/ml , cGMP : 35.3 8.8 pmol/ml vs 21.3 6.2 pmol/ml). A significant correlation was found between VCD and alpha-h-ANP before (r=0.81, p < 0.05) and after hemodialysis (r=0.65, p < 0.05). No such significant correlation was observed bet ween CI and alpha-h-ANP levels. Furthermore, a significant correlation was observed between VCD before hemodialysis and the change in alpha-h-ANP during hemodialysis (r=0.64, p < 0.05). The relationship between VCD and cGMP before hemodialysis was not significant (r=0.26, p> 0.05) and also no relation was observed between the decrease of cGMP during hemodialysis and VCD before hemodialysis (r=0.12, p > 0.05). A significant correlation between the percent change in body weight and the percent change in VCD during hemodialysis (r=0.91, p<0.05) and also significant relation was observed between the pecent change in body weight and the percent change in alpha-h-ANP levels (r=0.40, p , 0.05). CONCLUSION: Echocardiography of the inferior vena cava allow an estimation of changes of intravascular volume in ESRDpatients without cardiac filling impairment as shown by the correlation to other indices of intravascular volume, such as alpha-h-ANP. In this study, CI and cGMP levels were less informative. Inferior vena cava echocardiography is noninvasive and easily available and serial measurements of VCD and alphah-ANP levels allow an estimation of chages of intravascular volume in ESRD patients on maintenance hemodialysis.

8.Effective Dose of Atracurium in Korean Adults.

BACKGROUND: Atracurium is non-depolarizing neuromuscular blocking agent with intermediate duration of action. The purpose of this study is to determine the effective dose (ED) of atracurium in Korean adults. METHODS: We studied 100 patients, aged 18-59 yr, who had given informed consent and belonged to class I or II of ASA physical status. We excluded patients who had cardiac, pulmonary, renal, hepatic, neurological, psychiatric, muscular, inflammatory or endocrine disease, as were pregnant women and patient with recent exposure (< 72 hr) to medications known to interfere with neuromuscular transmission. In all patients, anesthesia was induced with intravenous propofol 2 mg/kg and fentanyl 2microgram/kg. Anesthesia was maintained with continous infusion of propofol 5 mg/kg and 100% oxygen through mask, supplemented with fentanyl as required. The electrodes were applied over the ulnar nerve at the wrist. Then, the patients were stimulated with a 50 Hz supramaximal tetanic stimulation for 4 seconds and a 1 Hz single twitch for 5 minutes to stablize muscle contraction and then a control value was obtained with acceleromyography (TOF-Guard(R), Organon Technika BV , Netherland). Thereafter, a 0.1 Hz single twitch of adductor pollicis was kept during the study. The 100 patients were randomly divided to five groups. The patients in each group received 90microgram/kg, 120microgram/kg, 150microgram/kg, 180microgram/kg, 210microgram/kg of atracurium. We measured the maximal response (% reduction) and time to achieve maximal blockade of single twitch. Values were expressed as mean +/- SD or 95% confidence intervals. ED values were obtained from the regression analysis using probit model. A P value of < 0.05 was considered statistically significant. RESULTS: There were no significant defferences in age, body mass index and temperature among dose groups. ED50 and ED95 of atracurium were 128microgram/kg, 236microgram/kg, respectively. For men, ED50 and ED95 were 127microgram/kg, 249microgram/kg, respectively. For women, ED50 and ED95 were 128microgram/kg, 236microgram/kg, respectively. The onset time to maximum blockade was reduced as dose was increased. CONCLUSIONS: ED50 and ED95 of atracurium in Korean adults were 128microgram/kg, 236microgram/kg, respectively. There was no difference between the effective doses of men and women. Onset time of atracurium was reduced as doses were increased.

10.A study on the smile of Korean Youth compared with twenty years ago.

PURPOSE: Teeth are generally exposed when people smiling. Moreover, the exposed teeth and soft tissue when smiling becomes an important guideline for esthetically prosthetic restoration. This research is to compare and find out differences of Korean young men's smile living in presence and twenty years ago. MATERIALS AND METHODS: Subjects, 100 young men (50 male and 50 female), were required about several aspects; normally developed physical condition, no psychological or genetic disorders, a fine face with no loss of teeth, no experience in orthodontic or prosthetic treatment, relatively normal occlusion, aged between 20 - 29. The photos of the subject at rest position and front face when fully smiled were taken three times. 100 photos (50 male and 50 female) were chosen at random from the 240 university students' smile photos taken by Yoon and his colleagues in 1991. By Hulsey's method of measuring smile, several factors; the change of upper lip curvature, the change of the relation between the upper lip and teeth, parallelism between Mx. incisor and lower lip, contact relation between Mx. incisor and lower lip and teeth displayed in a smile, were measured and analyzed. Meanwhile, ten dentists assessed aesthetic evaluation about men and women's smile for twice and recorded and compared smile score. The -test (P<.05) was used to compare the measured value. The difference of smile score was analyzed by t-test (P<.05). RESULTS: The smile score calculated in 2011 (60.22) was higher than that of 1991 (52.80). Among five measurement categories, the noticeable difference was distinguished from two factors; the change of upper lip curvature and contact relation between Mx. incisor and lower lip. CONCLUSION: The Korean young men's smile has been considerably improved for twenty years. And it is found that the change of upper lip curvature plays an important role, that is, the smile formed with an ascended labial commissure has been increased significantly.
