1.Percutaneous transcatheter biliary biopsy with a biotome.

For the purpose of the precise diagnosis and proper treatment planning of obstructive jaundice, various techniques to obtain tissues from biliary strictrue sites have been proposed. We performed perutaneous transcatheter biopsies of biliary strictrues with a biotome in six patients with obstructive jaundice. The sites of biliary stricture were distal common bile ducts (n=3), common hepatic duct (n=1), and confluence of both intrahepatic ducts (n=2). Their histologic diagnoses were adenocarcinoma (n=2), chronic choledochitis (n=3), and atypical cell suspicious of malignancy (n=1). False positive or false results were not documented by other means (including laparotomy),when regarding atypical cell suspicious of malignancy as true positive for malignancy. Percutaneous transcatheter biliary biopsy with biotome is easy to perform in conjunction with percutaneous transhepatic biliary drainage procedure, and can be able to obtain specific tissues for correct diagnosis.

3.Salmonella Osteomyelitis of the patella: A Case Report

A 48 years old male was visited with complaining of painful swelling of the left knee joint for 2 weeks duration. On radiological finding of the patella, there was found local rarefaction and osteolytic change in the inferior pole. It was diagnosed as Salmonella osteomyelitis of the patella and was confirmed by bacteriological study. Osteomyelitis of the patella caused by Salmonella group C as an etiological factor has not been reported. It was treated by conservative method with satisfactory result.

7.Stress-related Physical Symptoms and Stress Managements among the Acarins.

BACKGROUND: A worshipper lives a well-regulated life with rigid discipline. This study aims at measuring the stress level of the acarins (preliminary monks) using BEPSI-K (Brief Encounter Psychosocial Instrument-Korea) and finding out their stress management. METHODS: One hundred forty six male acarins and 111 female acarins who participated in an educational program at a local temple in Sep. 2001 are included in this survey. Using BEPSI-K, we asked them how they use their spare time, their stress management and physical symptoms. SPSS ver. 10.1, one-way ANOVA and independent t-test were used for statistical process. RESULTS: Average BEPSI-K score was high among the age group of 15-19 by age (P<0.05). In academic career, the lower the average BEPSI-K score, the higher in education (P<0.01). In physical symptoms, the average score of headache (P<0.05), epigastric soreness (P<0.05), fatigue (P<0.01), GI problem (P<0.01) were significantly high. Prayer, Zen meditation, bowing, sutras study, and sound a wooden gong were counted as the stress management, which were not significant. CONCLUSION: The stress level of the acarins was lower than normal population. The physical symptoms from stress were significant. Zen meditation and prayer were similar to meditation as the stress management. Further study for releasing stress is needed.

10.MR Features of Common Hepatic Duct Cholangiocarcinoma.

PURPOSE: The role of MRI in the detection of cholangiocarcinoma of the common hepatic duct has rarely been described. This study was attempted to ascertain characteristic MR features of cholangiocarcinoma. MATERIALS AND METHODS: The T1(500/30, TRs/TEs) and T2 weighted(200/80, TRs/TEs) MR images were obtained from nine patients with cholangiocarcinoma of the common hepatic duct. The common hepatic lesions were detected in seven cases. MR features of cholangiocarcinoma were analysed according to the signal intensity and associated findings. RESULTS: The seven cases showed lower signal intensity than the surrounding normal liver parenchyma on T1 weighted image. On T2 weighted image, the tumor showed isosignal intensity and hyperintensity in four and three patients, respectively. T2 image also revealed diffusely increased signal intensity in some hepatic lobe or segment in four cases, and Intrahepatic periductal high signal intensity in one case. Other associated MR findings were intrahepatic metastasis, and intratumoral hemorrhage, each noted in a different case. CONCLUSION: T2 weighted Image appears to be effective in the detection of cholangiocarcinoma of the common hepatic duct and evaluation of its associated intrahepatic lesions.
