1.Seborrheic Keratosis: A Clinical and Histopathological Study.

A total of 127 cases of seborrheic keratosis examind at the Dept. of Pathology, Yonsei University School of Medicine during 1980~1986 was reviewed. The results were as follow; 1) Sex distribution showed without preponderance with male; Female ratio of 1.05:1. 2) Age distrubution were 10~19 years 0.8%, 20~29 years 4.7%, 30~39 years 14.2%, 40-49 years 25.2%, 50~59 years 29.1%, 60~69 years 17.3%, 70~79 years 6.3% and neck (3.7%). 3) The most common site was face (44.8%), followed by trunk (29.8), lower extremities (10.4%), scalp (6.7%) and neck (3.7%). 4) The clinical diagnoses were sebrorrheic keratosis (47%), nevus (15.4%), verruca (12%), malignant melanoma (7.7%), actinic keratosis (3.4%), pyogenic granuloma (2.6%), mass (2.6%), basal cell epithelioma (0.9%), corn (0.9%), leproy (0.9%), and angiokeratoma (0.9%). 5) The distributions of the light microscopic diagnosis were acanthotic 52 cases (42.9%), hyperkeratotic 33 cases (30%), irritated 17 cases (13.4%), adenoid 15 cases (11.8%), clonal 3 cases (2.4%), melanoacanthoma 1 case (0.8%) and mixed 6 cases (4.7%). 6) The lesions measured from 0.2 cm to 3 cm in maximum diameter and its color was light brown to black with sharply demarcated papule, pladque or nodule.

2.Lectin Histochemistry on the Surface Mucous Cells, Mucous Neck Cells and Chief Cells in the Gastric Mucosa of Developing Rat.

To localize glycoconjugates of surface mucous cells, mucous neck cells and chief cells in the developing rat, nine of biotinylated lectin(SBA, DBA, PNA, BSL-1, RCA-1, sWGA, UEA-1, Con A and LCA) were applied with ABC method. In the surface and gastric pit epithelium of body of the stomach, DBA affinity was not demonstrated. Although Con A and LCA affinity were slightly increased after birth, these affinities with RCA-1 and sWGA maintained constantly from fetal to adult rat. And UEA-1 affinity gradually increased from the end of suckling period. BSL-1 and PNA affinity showed a tendency to decrease and was not observed in most cells from the suckling and weanling period respectively. In the gastric gland proper, mucous neck cells and chief cells were not distinguished until the early weanling period. All affinities examined except DBA and BSL-1 were observed and increased in the gland of postnatal rat. With the approach of weanling period, more intense affinity for PNA, RCA-1, sWGA and UEA-1 were found on lower portion of the gastric gland proper and more intense affinity for SBA on upper portion. The mucous neck cells showed a similar affinities as gastric gland proper from the weanling period and two affinities for PNA and Con A were detected in the chief cell.

4.The Effect of Environmental Humidity on LASIK Results.

It is known that humidity in the laser room might affect the outcomes of excimer laser refractive surgery. This study evaluated the outcomes of laser in situ keratomileusis (LASIK)according to humidity changes in laser room. Hinged flap was made by SCMD turbokeratome and photoablation was performed by utilizing VISX 20/20 excimer laser system. The differences between targeted refraction and actual refractive error, namely, refractive correction errors (defined by authors)were calculated. We have divided LASIK patients into two groups. Group A consisting of 61 eyes of 31 patients underwent LASIK when laser room humidity was over 70% (average 73%) and Group B includes 49 eyes of 28 patients when laser room humidity was under 40% (average 35%). Each group was broken into two subgroups, that is high myopia (> or= -6 D)and mild to moderate myopia (< -6 D). The mean refractive correction errors in the high myopia (> or= -6 D)was 1.28+/-1.42 D in the group A (n=46 eyes)and 1.99 +/-1.38 D in the group B (n= 26 eyes). In both groups, the difference between two subgroups was statistically significant and high myopes group B was overcorrected compared with ones in group A (t-test, p<0.05). The mean refractive correction errors in the low to moderate myopia (< -6 D)was 0.38 +/-0.49 D in the group A (n=15 eyes)and 0.4 +/-0.38 D in the group B (n=23 eyes). In both subgroups, there was no statistically significant difference in mean refractive correction errors (t-test, p>0.05). The proper room humidity is critical for LASIK results, particularly in the high myopia (> or= -6 D)because humidity can affect outcomes of LASIK. Therefore, the humidity should be maintained properly for enhancement of surgical outcome predictability in the high myopia.

Non-Hodgkin's malignant lymphoma is a relatively frequent lymphoreticular malignancy, and has been reported to constitute up to 5.2% of all malignant tumors in Korean patients. Various morphologic classifications of non-Hodgkin's lymphoma have been proposed, and among them, the Rappaport's classification has been most widely accepted. In 1982, a National Cancer Institute sponsored study on classification led to the creation of the Working Formulation in an attempt to resolve the controversy anddebate regarding the various classifications of non-Hodgkin's lymphoma. Angioimmunoblastic lymphadenopathy with dysproteinemia and polymorphic reticulosis are lymphoreticular proliferative disorders which have reported to transform to malignant lymphoma. The purpose of the present study is to reclassify non-Hodgkin's lymphomas according to the Working Formulation and to investigate the histopathological and immunocytochemical characteristics of angioimmunoblastic lymphadenopathy with dysproteinemia and polymorphic reticulosis. This study reviewed 300 cases of nodal and extranodal non-Hodgkin's lymphoma, 26 cases of polymorphic reticulosis, and 7 cases of angioimmunoblastic lymphadenopathy with dysproteinemia examined in the Departments of Pathology, Yonsei University College of Medicine, Youngdong Severance Hospital and Yonsei University Wonju College of Medicine from January 1977 to December 1986. In non-Hodgkin's lymphoma, each case was classified according to the Working Formulation and the Rappaport classification. All angioimmunoblastic lymphadenopathy with dysproteinemia and polymorphic reticulosis cases were subjected to histopathological analysis and a review of the clinical records. Immunocytochemical studies were done using kappa and lambda chains for B-cell markers and alpha-1-antichymotrypsin for histiocytic marker. The results obtained were as follows; 1) Among 300 cases of non-Hodgkin's lymphoma, the primarily involved tumor sites were the lymph nodes (141 cases), the gastrointestinal tract (67 cases), and the tonsils (32 cases) in descending order of frequency. 2) Using the Working Formulation, intermediategrade lymphomas occurred in 66.4% of the patients, and the most common subtype was "diffuse, large cell" (32.7%). By the Rappaport classification, 3 patients had nodular lymphomas, and "diffuse, histiocytic" was the most common subtype. 3) Infarction was present in 32 cases in which the "diffuse, lagre cell" type was most frequently associated. 4) In immunoperoxidase stains of 7 cases of angioimmunoblastic lymphadenopathy with dysproteinemia, proliferating immunoblasts revealed a polyclonal positivity for kappa and lambda chains. Atypical reticulocytes present in 26 cases of polymorphic reticulosis revealed a negativity for kappa, lambda and alpha-1-antichymotrypsin.

8.Reproducibility of the Peripapillary and Retinal Perfusion Measurement with Confocal Scanning Laser Flowmetry.

Confocal scanning laser flowmetry(HRF: Heidelberg Retina Flowmetry) is a new kind of instrument which enables non-invasive two-dimentional mapping of perfusion of the retina and optic disc and to evaluate the clinically available reproducibility of the peripapillary and retinal perfusion measurement with the confocal scanning laser flowmetry, the authors calculate the coefficient of variation (CV) from the parameters(volume, flow, velocity). The authors indicate two region of interest in the peripapillary region and retina, not overlapping the large vessels and measure the parameters three times consecutively(one session) while visiting three times intervalled(three session), thereafter calculate the mean CV of parameters in ten healthy eyes of ten subject. The CV of parameters obtained with the confocal scanning laser flowmetry was low relatively and therefore reproducibility was high as the CV lower, the reproducibility higher. Comparing the CV between one session and three session measurement to know the clinically available measurement frequency, the CV of three session (volume 13%, flow 16.7%, velocity 16%) was lower than the CV of one session (volume 16.7%, flow 20.7%, velocity 20.1%) in the peripapillary region(Students t test; volume p=0.009, flow p=0.03, velocity p=0.016) and the CV of three session (volume 16.7%, flow 20.7%, velocity 17%) was lower than the CV of one session (volume 18.2%, flow 21.8%, velocity 21.7%) in the retina(Students t test; volume p=0.017, flow p=0.0009, velocity p=0.0004). Above the results, the CV of parameters(13~21.8%) in one and three session had relatively high reproducibility. Also the CV between two measurements had a difference statistically but the difference was not so high, the authors concluded the confocal scanning laser flowmetry which have high resolution measure the peripapillary region and retina reproduciblly with only one time visit, three consecutive measurements.

9.Effects of Electroacupuncture on the Expression of Spinal Neuronal Nitric Oxide Synthase in Adjuvant-induced Arthritic Rat Model.

To examine the effects of electroacupuncture (EA) on central nociceptive modulation, expressional changes of spinal neuronal nitric oxide synthase (nNOS) were investigated in complete Freund's adjuvant (CFA)-injected rats. Inflammation was induced by an intraplantar injection of CFA into the hindpaw of male Sprague-Dawley rats. Bilateral EA stimulation at 2 Hz, 15 Hz and 120 Hz was applied at those acupoints corresponding to Zusanli and Sanyinjiao in man with 3-day intervals for 30 days. At 30 days after CFA-injection, effects of EA on nNOS expression were observed in the dorsal horn of the spinal cord using immunohistochemical methods. The mean integrated optical density of nNOS immunoreaction was significantly increased in the dorsal horn throughout L1 to L5 lumbar segments in CFA-injected rats. The nNOS expression was attenuated in all regions of the dorsal horn by all types of EA. Especially, these reaction was markedly decreased in the superficial laminae and nucleus proprius of L1 and L3 lumbar segments by three types of EA, but a marked decrease in the neck of the dorsal horn was observed only in 2 Hz stimulation. The marked decrease of nNOS also showed in nucleus proprius and the neck of L5 lumbar segments in 2 Hz and 15 Hz EA stimulated rats. It is concluded that EA treatment can attenuate chronic inflammatory process in CFA-injected rats through modulating expression of nNOS in the dorsal horn of the spinal cord.

10.A Case of Pelvic Lipomatosis.

Pelvic lipomatosis is a self-limiting disease of unknown etiology. It has been defined as a nonmalignant overgrowth of normal fatty tissue limited. Primarily to the perivesical and perirectal space in the pelvis which may be to go obstructive uropathy. Typically, urinary tract roentgenographys show a high, gourd-shaped bladder. Computed tomography provided definitive proof that pelvic organ displacement was due to excessive fat. We present a case of this pelvic lipomatosis.
