Juvenile Hyalin Fibromatosis is a rare congenital disease that is characterized by large tumors, especially on the scalp, small whitish nodules, hypertrophy of the gingiva, and flexural contractures of the large joints of the limbs. A review of the world literature disclosed 13 similar cases, under various names. This case is the 14th case in the literature and the first case in Korea. A 3 year old girl had multiple large tumors and nodules on various parts of the bodis, hypertrophic gingiva, and flexural contracture of the elbow and knee joints. The histopathologic features of the tumor were characteristic of Juvenile Hyalin Fibromatosis. The tumor cells were embedded in an amorphous eosinophilic ground substance. Diagnosis was confirmed by clinical features and histopathological findings.

The warts are benign skin tumors caused by virus, most frequently seen in childhood, and have atendency to regress before reaching adulthood. Various articles have been suggested that wart involution may be involved with immune response of the host, but definite mechanism has not been found. The author studied the tuberculin sensitivities of the wart patients in order to evaluate immune responses of the hosts, and observed the course of the warts after BCG vaccination to the patients for the facilitation of nonspecific immune reaction of the hosts. The results are as follows; 1. Tuberculin positivity was not decreased, however the sensitivity to the tuberculin was decreased among the wart patients than the non-wart groups. 2. No significant difference was observed between the numbers of wart and the degrees of tuberculin sensitivity. R. BCG vaccination was promoted the invotution of warts without valuable changes of the tuberculin sensitivities.

A clinical and histological study of typical sebaceous epithelioma (basal cell epitheioma with sebaceous differentiation) developed secondarily in lesions of nevus sebaceus of Jadassohn is presented. The patient was 42 years old male. The sebaceous epithelioma tends to have about the same clinical characteristics as undifferentiated basal cell epithelioma but it is more often a yellow color. Sebaceous epitheliova may arise from nevus sebaceus and it is preursor of undifferentiated basal ce]1 epithelioma. Histological]y, the preparation showed massive development of sebaceous gland, papillomatous epidermal hyperplasia and maturation of apocrine glands. The tumor cell nests were mostly composed of large sheets of irregularly shaped cells in palisading pattern at the periphery. There were Iarge number of cells with vacuolated cytoplasm resembling sebaceous cell in certain part of the tumor cell masses and several keratin pearls. Diagnosis confirmed by clinical appearance and histological picture. These lesions could be treated sucessfully by wide excision.

Clinically, benign juvenile melanoma (BJM) is a benign nevus-cell tumor and arise predominantly in children. The lesion usually is solitary and is encountered most commonly on the face and extremities. In most mstances the lesion consists of a dome-shaped, srnall nodule. The histoIogic picture often closely resembles that of a maIignant melanoma and there is no doubt that prior to its recognition as an entity by Spitz in 1948 many cases were misdiagnosed as malignant melanoma. A case of benign juvenile melanoma in 23-year old male is described with brief review of literatures. Hisr.ologic pictures show considerable junctional activity. In the upper dermis edema and ectasia of the various vascular elements are present. The re:us cells are pleomorphic. They are mostly spindle-shaped and are arranged mostly in fairly well circumscribed nests. Giant cells are frequentIy observed. Melanin is scanty and mitotic figures are absent. These are typical histologic pictures of the BJM, To our knowledge, there have been no prior repots of BJM in Korea, based on our own review.

A case of typieal subcorneal pustular derrnatosis in 16-year-old female was presented. She failed to respond to oral administration of sulfone, however responded favorably to prednisolone.

Although the psoralen therapy has been used extensively on vitligo since the : introduction by El-Mofty on 1948, the result has not been satisfactory. Recent investigations showed that the autoimmune mechanism might be the causative factor to the development of vitiligo, and the use of steroid might be expected to be beneficial in the treatment of this disorder. Authors tried the combined treatment of prednisolone and psoralen on vitiligo and the results obtained are as follows; 1. Repigmentations began to develop from injected area of prednisolone as well as frorn hair follicles and peripheral area of the lesions. 2. More satisiactory responses were obtained on the widespread and symmetrical lesions of shorter duration than on the lesions of localized and long duration.

Lidex' cream (Fluocinonide 0.05% in FAPG base) is a new fluorina,ted topical corticosteroid, and its clinical effect was assessed by applying topically three to four times daily to the lesions of 50 patients with a variety of inflamrnatory skin condi- tions. The results revealed excellent effect in 16 patients, moderate effect in 23 patients, slight improvement in 9 patients, and no effect in 2 patients. No unfavorable side effects were noted except blanching. The findings suggest that fluocinonide is an excelIent addition to the topical corticosteroid agents that are available for clinical use at the present time.

Human wart diseases are benign skin tumors caused by viruses of the papova group which usually affect children and almost always regress until adulthood. Various therapeutic modalities have been tried for the treatment of warts, however there is no single satisfactory method and the clinical course as well as the prognosis is always variable. The spontaneous regression of warts may represent an increase in the activity of host immune responses. Recent studies have tended to confirm that serum antiviral antihodies and cellular immune responses occur in relation to regression of warts, but there is no convincing evidence of a correlation between the appmrance of immune responses and resolution of the warts. BCG and some of similar extra,cts have becn known to be capable of initiating a wide range of non-specific immune reactions to cancerous diseases. The aim of the present investigation has been to study no-specific irnmune response of Tubercin-3 in patients with svarts, based on the findings that RCG and some of similar extracts were found to be a potent reticuloendothelial stimulant. Taventy casee attending a, dermatolo-ical out-ptient department consists of eight verrua plana juvenilis, seven verruca vulgaris, one verruca plxntaris and four molluscurn contagiosum were trezted with Tubercin-R. The results revealed complete cure in six(37. 5.) and clinical improvemeat in nine. (56. %) of sixteen cases of various warts, and one of four czses of molluscum contagiosurn showed a slight decrea,se in size and nu!nber of the lesions. There is no eviclence of a correlation among the age of patient, type of verruca and therapeutic effect. On the basis of our findings, we can state that non-specific immune response of BCG .and some of similar extracts may be facilitate the involution of warts. The relationship of thes immune respons to the involution of warts clezrly require further study.

Disfiguring hyperpigmentation of the exposed areas may have impartant and lasting effects on the emotional well-being of the patient. And many attempts to treat areas of hyperpigmentation have been earried out, but all of them are not very successful. Recently 4-isopropylcatechol has been known to inhibit melanin pigmentation with a selective action on melanocytes either destroying or inactivating the melanocytes experimentally. There is no report on its clinical trial till now. In this study, 1% 4-isopropylcatechol cream was topieally applied to the hyperpigmented lesions of 29 patieots of melasma, freckle, and cafe-au-lait spot. The bleaching effect on meiasmas was very satisfactory in all patients but coantact dermatltis in 3 patients and reticular hyperpigmentation with areas of depigmented spots in one patient were developed as the side effects of the drug. Freckle and cafe-au-Iait spots were not bleached.

Hepatie hilar ductal carcinoma is relatively rare, but the prognosis is known to be poor because an early diagnosis is difficult. Because the majority of patients are already infiltrated into adjacent organ by itself at the diagnosis, the rate of resectability is low. The best method of treatment is curative resection, and the range of tumor invasion is very important. The preoperative diagnosis is difficult because the cholangiography may be normal in cases of superficial invasion. Multifocal lesions within the biliary tract may be identified in as many as 10 percent of patients, Especially, the papillary type has the best prognosis and is associated with multiple tumors within the bile duct. We report a case that percutaneous transhepatic cholangiogram showed normal distal common bile duct, but the hepatic hilar confluence and ampulla of Vater lesion was confirmed as adenocarcinoma.
