1.Endoscopic ACL Reconstruction

Endoscopic anterior cruciate ligament reconstruction using central one-third of bone-patellar tendon-bone autografts were performed on 76 consecutive patients. 36 patients out of them were reviewed and evaluated with subjective and functional rating scales according to the Lysholm knee scoring system, physical examination and instrumented anterior laxity test. The average follow-up was 2 years and 1 month, ranging from 1 year and 6 months to 3 years and 8 months and the everage age at operation was 31 years old, ranging from 20 to 49 years old. At follow-up, the average Lysholm knee score was 87.2 compared to the average score of 49.5 prior to reconstruction. Physical examination and instrumented anterior laxity test showed that excellent anterior stability was regained in all patients but two. There were 4 cases of complication, a fibrous nodule anterior to reconstructed ACL, an effusion of knee, a thrombophlebits, and an inadequate placement of screw fixation with protrusion of bone peg out of tibial hole. In summary, endoscopic ACL reconstruction using central 1/3 of bone-patellar tendon-bone seems to be a good procedure, which leaves less operative scar, takes short operation time and gives a constant good result as far as the surgeon is familiar with the technique.

2.Placental Findings of Listeria Monocytogenes Infection in Twin Pregnancy: A Case Report.

A Listeria monocytogenes infection is relatively rare in healthy adults. However, the chance of an infection increases almost 17 times in pregnancy due to changes in the immune function. A Listeria monocytogenes infection results in characteristic gross and microscopic features in the placenta, including multiple yellowish nodules showing microscopic intervillous abscess and intervillositis. We describe the placental findings of a Listeria monocytogenes infection that was complicated by maternal sepsis, myocarditis and congestive heart failure. The infection was discovered in the 34th week of a twin gestation in a 28 year-old woman. This case should emphasize the importance of this condition to pathologists. Antibiotic treatment was started based on the placental histologic findings before a maternal blood culture confirmed growth of Listeria monocytogenes. Both the mother and twin babies were healthy at the time of this report.

3.Measurement of Normal Corpus Callosum with MRI in Korean Adults and Morphological Change of Corpus Callosum by Grade of Hydrocephalus.

PURPOSE: To measure the size of normal corpus callosum in each portion using objective and reproducible method with MRI and evaluation of morphological change of corpus callosum by grade of hydrocephalus. MATERIALS AND METHODS: Midsagittal Tl-weighted MR imaging of the corpus callosum was investigated in 41 volunteers of normal Korean adults and 19 patients with hydrocephalus. Corpus callosum was measured for the anteroposterior length(A), height(B), and the thickness of genu(C), body (D), splenium(E), and the narrowest portion of body(F). And the analysis of morphology and signal intensity of the corpus callosum were also evaluated. Hydrocephalus was graded as mild, moderate, and severe, and comparision of thickness with normal corpus callosum in each portion was done. RESULTS: The mean length and height were 72.3mm, 28.6mm in male, and 70.7ram, 28.9mm in female. And the mean dimention for C, D, E, and F were 13.1 ram, 8ram, 13.2mm, 5.2ram in male, and 12.8mm, 7.5ram, 12.3 ram, 5mm in female. The morphology of normal corpus callosum was "hook" shaped on midline sagittal Tl-weighted image. Narrowing at posterior third portion of body were present on 30 cases(73.2%) and even in thickness of the body in 11 cases(26.8%). The signal intensity of the corpus callosum on midsagittal Tl-weighted spin echo image of normal cases was homogeneous hyperintense as compared with cerebral gray matter. In hydrocephalus, A and B were increased and other portions were decreased in thickhess. Genu and the narrowest portion of body showed significant difference of thickness according to the grade of hydrocephalus. CONCLUSION: The mean dimention of all portion of corpus callosum were larger in male than female except for callosal height but not significant statistically with the exception of splenium. Hydrocephalus lead to morphological change of the corpus callosum. Among the portion of corpus callosum, genu and the narrowest portion of the body were thought to be the most sensitive indicators of degree in hydrocephalus.

Purpura is discoloration of the skin and mucous membranes due to extravasation of red blood cells and may be due to a variety of factors, but it is sometimes difficult to identify the exact cause. Some forms of purpura can frighten the patient and puzzle the physician. Mask phenomenon is an unusual purpura of the relatively loose tissues of the face and neck occuring after prolonged coughing, vigorous vomiting, valsalva's maneuver, parturition, or any other exertion that raises intrathoracic or abdominal pressure. This occurs with acute onset and fades within twenty-four to seventy-two hours spontaneously. A work-up for a coagulation or platelet defect is usually not required. We herein describe five cases of mask phenomenon(unusual facial purpura).

6.Four Cases of Cutaneous Sinus Tract of Dental Origin.

The most common cause of chronic cutaneous sinus tract in the face and neck is chronically draining dental infection. The diagnosis is easily confirmed by dental examination and dental roentgenogram of the involved area, but this disease is frequently misdiagnosed and incorrectly treated in dermatologic department. We report four cases of cutaneous sinus tract of dental origin. First patient had a painful nodule with chronically draining pus and a dimple on the left cheek. Second patient had a dimple with draining bloody fluid on the right chin. Third patient had a pea-sized papule with draining serous fluid on the left chin. Fourth patient had a fluctuant erythematous plaque with draining pus and a dimple on the left buccal area. In all four patients radiologic examination of the maxillary and mandibular regions demonstrated radioluency at the apex of tooth consistent with periapical abscess.

7.Effects of Negative Automatic Thoughts on Depression in Adolescence.

OBJECTIVES: The purpose of this study was to investigate the effects of negative automatic thoughts on depression in adolescence. METHODS: The participants were 1037 middle school students (521 boys and 516 girls) and 872 high school students (415 boys and 457 girls). They completed self administered questionnaires; The CES-D and the K-CATS. The ANOVA tests were conducted with gender (2 levels) and grade (5 levels) as independent variables, and Pearson correlation analysis and stepwise multiple regression analysis also were conducted. RESULTS: The results showed that both depression and negative automatic thoughts are very high in females with significant age differences; highest in the age of 17, the late adolescence. And depression was significantly predicted by the combination of depression cognition, social anxiety cognition, and physical threat cognition, with 51% estimation. Depression cognition and social anxiety cognition significantly predicted depression in males in 45%. In females, depression cognition, social anxiety cognition, and physical threat cognition predicted depression in 57%. CONCLUSION: Negative thoughts have high impacts on depression, and depression cognition has a significant impact particularly on the depressinon of females. The limitations of the present study and directions for the future research were discussed.

10.Borderline Thyroid Tumors: a Surgeon's Perspectives

Borderline thyroid tumors are composed of hyalinizing trabecular tumor (HTT), well differentiated tumor of uncertain malignant potential (WDT-UMP), follicular tumor of uncertain malignant potential (FT-UMP) and non-invasive follicular tumor with papillary like nuclear feature (NIFTP) by World Health Organization (WHO) definition. They have different pathological feature from each other. However, it is difficult to diagnose with diagnostic imaging, fine needle aspiration (FNA) or core biopsy preoperatively. Thus, the diagnosis is usually made after diagnostic lobectomy. Main surgical concerns about borderline tumor are not performing total thyroidectomy because of relatively indolent nature of these tumors. Unfortunately, some of these tumors can be diagnosed as malignant tumor preoperatively. The other surgical concern is performing completion thyroidectomy or not after diagnostic lobectomy. Decision making is difficult even though it is generally considered that lobectomy alone is enough. In this article, we will discuss clinical features of borderline malignant tumors and surgical strategy for these tumors.
