2.Effects of Age and Left Ventricular Hypertrophy on Left Ventricular Diastolic Function in Normotensives and Hypertensives.

Transmitral left ventricular diastolic inflow velocities determined by pulsed Doppler in 47 normotensives, 80 hypertensives and 44 age-matched hypertensives were measured to evaluate the effects of age and left ventricular hypertrophy on left ventricular diastolic fuction in normotensives and hypertensives. The results were as follows: 1) The peak E velocity in normotensives(77.82+/-13.53cm/sec) was significantly different from those of hyeprtensives(58.13+/-11.80cm, p<0.01) and age-matched hypertensives(59.31+/-10.94cm/sec, p<0.01). The peak E/A in normotensives(1.31+/-0.45) was significantly different from those of hypertensives(0.83+/-0.21,p<0.01) and age-matched hypertensives(0.83+/-0.20, p<0.01). 2)In normotensives, simple linear regression analysis revealed strong inverse correlations of peak E with age(r=-0.758, p<0.01), peak E/A with age(r=-0.748, p<0.01), but no significant correlation of peak E and peak E/A with LVMI(p>0.05). 5.A Case of Embryonal Carcinoma Developed in a Cryptorchid Testis.

A case of testicular embryonal carcinoma developed in s cryptorchid testis is presented with a review of the literature. According to Campbell's series, the incidence of malignant cryptorchid testis was 11.6%. and his statistical evaluation was similar to Gilbert and Hamilton's series of 7,000 cases of testicular neoplasms where 840 tumors were developed in undescended testes(12%). Both authors agree that the chance of neoplastic development in a cryptorchid testis is approximately 48 times greater then that developing in a normally descended testis. Several cases of testicular tumor were reported sporadically in Korea, but malignant tumors in cryptorchism were rarely seen in the literatures Only two cases of malignant cryptorchid testis were already reported in Korea.

6.Percutaneous K-wire Fixation of Colles' Fracture A Prospective Study of Twenty Seven Cases

27 Colles' fracture were treated with closed reduction and percutaneous k-wire fixation under the C-arm fields, from July 1980 to September 1984 at the Department of Orthopedic Surgery of Eul Ji Hospital. The average duration of the follow up was 4.9 Months. A prospective study was made and evaluated under the subjective and objective criteria of Gartland and Werley, and the objective criteria of Scheck. The results of this study were as follows: 1. The incidence was high in 4th and 5th decade as 66. 5%, not significant between females and males, 45% of the cause of injury was falling accident. 2. In 27 cases, showed the satisfactory result about 92.6%, but, in the eomminuted intraarticular fracture group, 2(15.4%) out of 13 cases showed unsatisfactory result. 3. Better result was obtained with more secure reduction and skillful technique, in certain case, considered that the open reduction and internal fixation with k-wires would be better than percutaneous k-wire fixation. 4. In the aspect of hand motion and the concept of improved anatomical restoration and maintenance would lead to improved function, ideal method was percutaneous k-wire fixation and casting after closed reduction under the C-arm fields.

7.A Clinical Study of the Severe Ankle Fracture

The primary goal of treatment of the adult ankle fracture is to effect the returning of normal func tion that is accomplished by the accurete reduction and rigid internal fixation followed by the early institution of motion. The authors have reviewed a series of 41 cases of the bimalleolar and trimalleolar ankle fractures that treated in Orthopedic Department of Eulji General Hospital, Seoul, from January 1982 to June 1986. With the duration of follow-up of average 13.2 months, we summarized the obtaind results as followings. l. Among the forty one patients, thirty one was male patient(M: F =3.1: 1). The average age was 33.2 years old. 2. The main cause of the fracture was traffic accident, 18 cases(43.9%) and other causes were slipping down(24.4%), fall from a height(14,6%), industrial injury(12.2%) and other injuries(4.9%). 3. According to the modified Lauge-Hansens classification, the supination-external type was most common (41.5%). 4. As a diagnostic method, the computerized tomography was also available. 5. All cases were treated by open reduction and internal fixation with the good radiologic results of 91.3% in bimalleolar fractures and 88.9% in trimalleolar fractures. 6. Of the bimalleolar fractures, ten patientz showed normal range of motion in 2 to 4 weeks, average 18.4 days, by early joint exercise.

9.The Usefulness of Myocardial SPECT for the Preoperative Cardiac Risk Evaluation in Noncardiac Surgery.

PURPOSE: We investigated whether myocardial SPECT had additional usefulness to clinical, functional or surgical indices for the preoperative evaluation of cardiac risks in noncardiac surgery. MATERIALS AND METHODS: 118 patients (M: F=66:52, 62.7+/-10.5 years) were studied retrospectively. Eighteen underwent vascular surgeries and 100 nonvascular surgeries. Rest T1-201/stress Tc-99m-MIBI SPECT was performed before operation and cardiac events (hard event: cardiac death and myocardial infarction; soft event: ischemic ECG change, congestive heart failure and unstable angina) were surveyed through perioperative periods (14.6+/-5.6 days). Clinical risk indices, functional capacity, surgery procedures and SPECT findings were tested for their predictive values of perioperative cardiac events. RESULTS: Peri-operative cardiac events occurred in 25 patients (3 hard events and 22 soft events). Clinical risk indices, surgical procedure risks and SPECT findings but functional capacity were predictive of cardiac events. Reversible perfusion decrease was a better predictor than persistent decrease. Multivariate analysis sorted` out surgical procedure risk (p=0.0018) and SPECT findings (p=0.0001) as significant risk factors. SPECT could re-stratify perioperative cardiac risks in patients ranked with surgical procedures. CONCLUSION:: We conclude that myocardial SPECT provides additional predictive value to surgical type risks as well as clinical indexes or functional capacity for the prediction of preoperative cardiac events in noncardiac surgery.

10.A Survey of Health-related Lifestyle of the ROKAF Pilots.

BACKGROUND: The present study was undertaken to know health-related lifestyles of Republic of Korea Air Force (ROKAF) pilots and to investigate prevalence of obesity of them.METHODS: 1284 KAF pilots were checked their height, weight, and body composition (% body fat). We also tried questionnaires on their health-related lifestyles which included the effort to be healthy, whether they know weight training is beneficial to G-tolerance, exercise habit and smoking history. RESULTS: Pilots' attitude for health promotion was relatively passive. Many of them know that weight training is good for their G-tolerance, but only 8.1% of the pilots carried out that training. They engaged much more in aerobic training, and also fighter pilots are less participated in weight training than transport pilots. The prevalence of obesity (29.4%) determined by bioelectrical impedance analysis was much higher than that assessed on the basis of height and weight (BMI, body mass index). It is suggested that many pilots who were normal in BMI have abdominal or visceral obesity. CONCLUSION: The health promotion program and physical conditioning program related to increasing G tolerance of KAF pilots flying high-performance aircraft are very important. These programs must be organized and progressed with flight surgeon and Aeromedical center in Chung-ju as leaders.
