2.A Case of Low-Grade Endometrial Stromal Sarcoma of the Uterus (So-Called "Endolymphatic Stromal Myosis").

Uterine sarcomas are relatively rare tumors of mesodermal origin that constitute 2-6% of uterine malignancies. Uterine sarcomas include endometrial stromal sarcoma (ESS), leiomyosarcoma (LMS), malignant mixed m llerian tumor (MMMT) of both homologous and heterologous type, pure heterologous sarcomas, blood vessel sarcomas, and lymphoma. Endometrial stromal tumors are divided into three types: 1) endometrial stromal nodule, 2) low-grade stromal sarcoma or endolymphatic stromal myosis, and 3) endometrial stromal sarcoma. The low grade endometrial stromal sarcoma is very rare. It shows occasional local recurrence, which might arise from endometrial stroma, from adenomyosis, and rarely from endometriosis. We have experienced a case of low grade endometrial stromal sarcoma of the uterus in a 54-year-old woman, which is presented with a review of literatures.

3.Acute Gastric Volvulus due to Diaphragmatic Eventration.

We present the case report of a 72-year-old female in whom diaphragmatic eventration and secondary gastric volvulus developed 10 years after a left partial pneumonectomy for a bronchiectasis. Eventration of the diaphragm is defined as an abnormal elevation of an intact diaphragm. The abnormally wide subdiaphragmatic space provides the potential for a gastric volvulus, which results from the strong negative intrathoracic pulling force created by the potential subphrenic space and paradoxical movement of the diaphragm. Unless this strong negative force is first eliminated, gastropexy alone will lead to recurrence. Obliteration of the subphrenic space by colonic displacement is a simple and effective way of abolishing this negative subdiaphragmatic pulling force. Treatment of gastric volvulus requires immediate surgical repair to prevent subsequent necrosis and perforation, with surgical correction of the underlying anatomic abnormality being the treatment of choice for gastric volvulus. We experienced a case of gastric volvulus due to diaphragmatic eventration who was treated with colonic displacement.

4.Change of Serum IGF-l level in Knee Osteoarthritis.

BACKGROUND: To examine serum Insulin-like growth factor-I(IGF-I) level in patients with knee osteoarthritis(OA) and to determine the relationship between the serum level of IGF-I with pain intensity and severity of OA knee. METHODS: Serum IGF-I levels measured by standard radioimmunoassay techniques were obtained from 30 knee OA patients and 30 normal controls matched by age and gender. Pain intensity was measured by pain rating scale(PRS) and visual analogue scale (VAS). Kellgren grade also used for assessing the severity of knee OA. RESULTS: No difference was found in serum IGF-I levels between the cases and controls(cases: 279+/-128 ng/dL, controls: 282+/-113 ng/dl). There was no correlation between pain intensity and serum IGF-I levels. There is no significant association between IGF-I level and Kellgren grade. CONCLUSION: Serum IGF-I level does not decrease in knee OA. The serum IGF-I levels can not indicate pain intensity and severity of knee OA.

6.Study of Motion Effects in Cartesian and Spiral Parallel MRI Using Computer Simulation.

PURPOSE: Motion effects in parallel magnetic resonance imaging (MRI) are investigated. Parallel MRI is known to be robust to motion due to its reduced acquisition time. However, if there are some involuntary motions such as heart or respiratory motions involved during the acquisition of the parallel MRI, motion artifacts would be even worse than those in conventional (non-parallel) MRI. In this paper, we defined several types of motions, and their effects in parallel MRI are investigated in comparisons with conventional MRI. MATERIALS AND METHODS: In order to investigate motion effects in parallel MRI, 5 types of motions are considered. Type-1 and 2 are periodic motions with different amplitudes and periods. Type-3 and 4 are segment-based linear motions, where they are stationary during the segment. Type-5 is a uniform random motion. For the simulation, Cartesian and spiral grid based parallel and non-parallel (conventional) MRI are used. RESULTS: Based on the motions defined, moving artifacts in the parallel and non-parallel MRI are investigated. From the simulation, non-parallel MRI shows smaller root mean square error (RMSE) values than the parallel MRI for the periodic (type-1 and 2) motions. Parallel MRI shows less motion artifacts for linear (type-3 and 4) motions where motions are reduced with shorter acquisition time. Similar motion artifacts are observed for the random motion (type-5). CONCLUSION: In this paper, we simulate the motion effects in parallel MRI. Parallel MRI is effective in the reduction of motion artifacts when motion is reduced by the shorter acquisition time. However, conventional MRI shows better image quality than the parallel MRI when fast periodic motions are involved.

7.Testicular Yolk Sac Tumor in Beckwith-Wiedemann Syndrome.

The Beckwith-Wiedemann syndrome, which included congenital anomalies such as macroglossia, exomphalos, postnatal somatic gigantism, have a substantially increased risk for the development of tumor. We report a case of testicular yolk sac tumor associated with Beckwith-Wiedemann syndrome, a previously unreported association. Pathologic examination showed Schiller-Duval body with evidence of testicular yolk sac tumor. This finding appears to represent a previously unreported association between Beckwith-Wiedemann syndrome and testicular yolk sac tumor.

9.The Effect of Diterpenoid Extracted from Acanthopanax Koreanum on the Collagen Indkuced Arthritis in DBA/1 Mice.

OBJECTIVE: No medication currently available for the treatment of rheumatoid arthritis is universally effective, and all can produce adverse effects. It is well known that the Acanthopanax koreanum extract has an anti-inflammatory action without any adverse effects reported. It is also reported that diterpenoid which is a partially purified product ectracted from Acanthopanax koreanum has an anti-inflammatory effect. We conducted this study whether diterpenoid extracted from Acanthopanax koreanum has a regressive effect of collagen induced arthritis in DBA/1 mice. METHODS: Four groups of DBA/1 mice were immunized by intradermal injection of 1mg/kg chicken type II collagen with complete Freund's adjuvant. Group II received 5mg/kg of diterpenoid extracted from Acanthopanax koreanum orally daily and Group III receive 5mg/kg of phenylpropranoids extracted from Acanthopanax koreanum orally daily. Group IV received 1mg/kg dexamathasone intraperitoneally twice weekly. Group I received no treatment. The prevalence of arthritis were assessed twice weekly. Seoum anti-collagen antibody was measured by ELISA. Stimulation index of caltured splenocytes were measured. Tumor necrosis factor-alpha and interleukin-10 levels in the supernatant of cultured splenocytes stimulated with Con. A were also measured by ELISA. RESULT: Collagen induced arthritis(CIA) started to develop 5 weeks after collagen injections. In diterpenoid treated group, CIA seemed to be regressed, but in phenylpropranoid group, regressive effect of artiritis was not observed. In dexamathasone treated group, both of suppression of CIA could be observed. Levels of anti-collagen antibodies were reduced in dexamethasone treated group, but not in both diterpenoid and phenylpropranoid group. No significant differences of splenic mononuclear cell SI among the groups was observed. There was an increased secretion of interleukin-10 in the supernatant of cultured splenocytes stimulated by Con A of the diterpenoid treated group. CONCLUSION: These findings showed that the diterpenoid extracted from Acanthopanax koreanum have an effect on supression of CIA.

10.A Socio-Psychiatric Study on Family Violence by Delinquent Adolescents.

OBJECTIVES: This research was performed to identify family violence by male adolescencts, and to examine the characteristics of male adolescent delinquents in terms of domestic viloence. METHODS: We surveyed 677 male adolescents including 238 delinquents from BBS(Big brothers and sisters) in Pusan, 209 criminal adolescents from adolescent correctional institution in Pusan, 230 high school students through questionnaire, MMPI(Minnesota Multiphasic Personality Inventory), and PBI(Parental Bonding Instrument), and then compared these three groups by Chi-square test, T-test, and Anova. RESULTS: In most cases, male delinquent adolescents were placed in poorer socio-economic conditions than those of students, but in better socio-economic conditions than those of criminal adolescents. In general, the assaulters were senior family members, physically stronger than sufferers, and have bad brotherliness. Compared with normal students, the serious problem of violence including violence to the ancestors in delinquent and criminal adolescents. On results of MMPI and PBI test, there were significant difference between violent group and non-violent group on the Sc score and Passive aggressive index in adolescent delinquents and parental overprotection in normal students. CONCLUSION: Generally the characteristics of family violence by male delinquent adolescents were intermediate between those of criminal adolescent and students, these results were same as those of previous socio-psychiatric study, and so the intervention in criminal adolescents is needed for the pre-vention of family violence.
