BACKGROUND: Chest pain is a problem that primary care physicians commonly face and often unnecessary work-up is done because of its clinical importance of cardiac origin. There are various studies on the causative disorders of chest pain, but studies on young adult males are relatively few. Therefore, we conducted this study on the cause of chest pain among young policemen. METHODS: Diagnosis and classification of chest pain were made among 106 policemen who visited the outpatient department of family medicine at the National Police Hospital from March 1, 2002 to June 30, 2002. RESULTS: The causes of chest pain were musculoskeletal (55%), psychogenic (17%), others (15%), gastrointestinal (7%) and respiratory (6%), but none of them were cardiovascular origin. The nature of chest pain were stabbing (41%), sharp (22%), pressing (18%), squeezing (16%) and substernal discomfort (3%). The duration of chest pain were below 10 minutes (37%), 10 minutes to 1 hour (15%), several hours (20%) and several days (28%). The location of chest pain were left chest (61%), right chest (18%), precordial (7%), epigastric (6%), substernal (4%) and others (4%). The frequencies of chest pain were more than once a day (23%), once a week (26%), once a month (11%), once more than 1 month interval (17%) and first attack (23%). The aggravating factors of chest pain were physical strain (25%), coughing (22%), psychologic stress (16%), smoking (10%), weather change (8%), drinking (7%), resting (5%), and overeating (6%). CONCLUSION: Our study showed that unnecessary tests should be avoided and adequate treatment matched to their causes through exact and detailed history taking should be done, because the causes of chest pain among young policemen are mostly non-cardiovascular origin.

Intersphincteric resection (ISR) is the ultimate sphincter saving procedure for low rectal cancer. Hemorrhoids are a common benign condition. We present and discuss a case of ISR which developed painful edematous hemorrhoids after ISR. A 62-year-old female with low rectal cancer received neoadjuvant chemoradiotherapy with successful down staging of tumor before undergoing robot assisted ISR with coloanal hand-sewn anastomosis. She had pre-existing external hemorrhoids which were not excised. She developed painful and edematous external hemorrhoids 4 days after surgery. These were treated conservatively before discharge. Many colorectal surgeons performing ISR have experienced similar situations in their patients, but none have reported on this phenomenon. We discuss the possible factors that may contribute to this situation. A possible solution is prophylactic excision of the hemorrhoids during coloanal anastomosis. Painful hemorrhoids may occur after ISR and if managed conservatively, the outcome is skin tags.

OBJECTIVES: New organizational work systems, and their impact on the mental health of employees, are considered to beone of the most important topics in the area of industrial health. This study was conducted to compare job characteristics (job demand and decision latitude) levels, and psychosocial distress of workers in acompany introducing to new organizational work systems, to those of workers managed by traditional work systems. METHODS: A study sample of 627 shipbuilding workers (446the new work organizational system and 181the traditional system) were recruited for this study. A structured-questionnaire was used to assess general characteristics, job characteristics (work demand, decision latitude), and psychosocial distress. RESULTS: The decision latitude was not significantly higher in the new work system compared to the traditional system. However, the job demand was significantly higher in the new work system than in the traditional system. The psychosocial distress was higher within the new work system than the traditional system, but no significant relationships were found. The proportion of increased strain was significantly greater with the new system than the traditional system. CONCLUSION: These results suggest that increases in the decision latitude did not sufficiently compensate for higher job strain or increased work intensity. If the increase in the decision latitude was temporary, with the typical job demand remaining high, such work can be still be considered to have a job strain. Future research should consider psychosocial distress and fatigue as important problems caused by new work organizational systems, and should be performed to assess their impact through out industry.

Clinical evaluation and imaging studies such as magnetic resonance imaging, computed tomography, and/or myelography can localize the pathologic process in most patients with lumbar radiculopathy. However, these studies occasionally fail to demonstrate exactly the cause and site for ongoing radicular symptom. So we evaluated 54 patients for the efficacy of selective nerve root blocks to identify a symptomatic nerve. The lumbar nerve root injections were performed at L3 root in 1 patients, at L4 root in 4 patients, at L5 root in 46 patients, and at S1 root in 3 patients. Among them, forty two patients were done before operation(HNP 32, stenosis 5, spondylolisthesis 5), 9 patients were done after operation due to poor results (root injury 2, infection 2, adhesion 1, incorrect diagnosis 4), and 3 patients had improved radiculopathies following nerve root injection(HNP 2, stenosis 1). The results of the nerve root injections and of the operations were not affected by age and sex. There was a good correlation between the response of the nerve root injection and the result of the operation(p<0.05). The concentration of bupivacaine 0.5% has no additional effect comparing to 0.25%(70%/73.5%) solution in spite of its adverse effect causing transient motor weakness. Additional use of depomedrol(50mg) proved to be no more beneficial. Three patients had Improved symptoms for a long period after nerve root injection which consequently made operation unnecessary. Five patients with previous history of operations had moderate pain relief. In summary, lumbar nerve root injection can be used as a functional diagnostic tool to predict operative result and as a treatment modality following failure of a previous surgery.

Symptomatic vasospasm is still one of the major causes of death and disability in patients who suffer an aneurysmal subarachnoid hemorrhage(SAH). To identify risk factors related to symptomatic vasospasm, to determine the outcome in patients with SAH, and to determine the differences identified risk factors between patients with good and bad outcomes, we performed this retrospective study. From a total of 279 SAH patients who were admitted to our hospital between January 1993 and December 1995, 212 were chosen for study. These were patients who had been admitted within 7 days of SAH, had undergone brain computed tomography(CT) within 3 days of SAH and had survived more than 6 days after SAH. Nine variables were examined as to their relationship to symptomatic vasospasm: age, sex, admission day after SAH, hypertension history, frequency of SAH, clinical grade, CT grade, operation day after SAH, and outcome. Data were analyzed by the univariate and multivariate logistic regression method using the Statistical Analysis System(SAS). Symptomatic vasospasm was demonstrated in 30 cases(14.2%). Univariate analysis showed that admission more than 4 days after SAH(p=0.07), clinical grades III-V(p=0.001), and CT grade III(p=0.00001) were associated with a higher risk of symptomatic vasospasm. When study cases were grouped into either good or bad outcome groups, and multivariate logistic regression analysis was performed, these factors were associated with a higher risk of symptomatic vasospasm only in the good outcome group. This suggests that there is a group of patients with a predisposition to symptomatic vasospasm that is independent of these risk factors, and that in these patients, the outcome may be worse.

Among 875 patients with intracranial aneurysm operated on during the past 14 years, the authors encountered eleven who had experienced recurrent hemorrhage caused by the rupture of aneurysms which had not been noticed at the time of the initial operation and the interval between initial and recurrent hemorrhage varied between 4 and 16 years. Age at the time of initial hemorrhage was relatively young(average 43.7 years). Multiple aneurysms occurred in four cases and hypertension in four others. Clinical grades at the time of the second admission were relatively poor, and in eight patients there were complications with intracerebral hematomas, intraventricular hemorrhages or acute subdural hematoma. Retrospective evaluation of the first angiograms disclosed suspicious tiny aneurysms in five cases, and these grew and ruptured at recurrent hemorrhage. In eight patients, the outcome was good; One remained moderately disabled, and two died. We conclude that the possibility of recurrent hemorrhage, after the clipping of a ruptured aneurysm, should be considered in all aneurysmal patients, especially in those who are young or have multiple aneurysms. To defermine whether or not suspicious tiny aneurysms are present in these patients, their angiograms should be subjected to detailed examination. Late postoperative follow-up angiography to determine the growth or development of another aneurysm might also be needed.

Synovial sarcoma is a rare renal neoplasm that is not easy to diagnose unless SYT-SSX fusion transcripts are identified. We report here on a case of primary renal synovial sarcoma in a 35-year-old woman. A mass was discovered by accident in the lower part of the right kidney when ultrasonography was performed, and it was removed via radical nephrectomy. Grossly, the tumor was a homogeneously tan-brown soft mass that measured 4.5x3.2x3.0 cm, and it was encircled by a well-defined cystic space. The lesion exhibited hypercellularity of the oval or short spindle cells that were arranged in various solid sheets or intersecting fascicles. Immunohistochemically, the tumor showed diffuse positivity for vimentin, bcl-2 and CD99, and it showed focal positivity for epithelial membrane antigen. The SYT-SSX fusion transcripts were detected by reverse transcription-polymerase chain reaction (RT-PCR). Synovial sarcoma should be considered in the differential diagnosis when a spindle cell neoplasm is encountered in the kidney.

OBJECTIVES: This study aimed to reveal the multiple factors that are related to the work-related musculoskeletal disorders (WMSDs) of shipbuilding workers, and to elucidate the relationship between musculoskeletal symptoms and factors such as the change of working conditions, job stress and physical workload. METHODS: The study sample comprised 1,059 shipbuilding workers. A structured-questionnaire was used to assess the general characteristics, job stress, psychosocial well-being index (PWI), physical workload, change of working conditions and information concerning musculoskeletal symptoms. We estimated the relations of job stress, physical workload and intensity of labour to musculoskeletal symptoms using univariate and multiple logistic regression analyses. RESULTS: The symptom prevalence of musculoskeletal disorders in any part of the body was 89.5% by 'criteria 1'in the order of back (58.6%) and shoulder (56.3%). After adjustment for sociodemographic factors, posture factor (Odds ratio [OR]=1.06, 95% confidence interval [ C I ] = 1 . 0 0~1.12), non-posture factor (OR=1.17, CI=1.05~1.31), Borg scale (OR=1.15, C I = 1 . 0 0~1.32), relative work intensity increase (OR=1.92, CI=1.08~3.41), labor flexibility increase (OR=2.04, CI=1.04~4.01), high job demand (OR=2.68, CI=1.48~4.88), and high risk stress group (OR=13.50, CI=3.15~57.97) were all found to be significantly associated with musculoskeletal disorders. CONCLUSION: These results suggest that WMSDs have multiple risk factors such as stress, physical workload and change of working conditions. High job demand, increased relative intensification of work and increased flexibility, especially such as subcontract, outsourcing and importing of contingent work, were very important factors associated with increasing WMSDs.

PURPOSE: Placenta growth factor (PlGF) is a member of the vascular endothelial growth factor (VEGF) family. PlGF is implicated in several pathologic processes, including the growth and spread of cancer and tumor angiogenesis. The aim of this study was to evaluate the expression and the clinical implications of PlGF in colorectal cancer. METHODS: In order to ascertain the clinical significance of PlGF expression in colorectal cancer, the researcher analyzed the expression pattern of PlGF by using an immunohistochemical method and attempted to establish if a relationship existed between PlGF expression and microvessel density (MVD), and subsequently between PlGF expression and the predicted prognosis. A total of 83 patients with colorectal cancer were included for immunohistochemical staining. Clinicopathological characteristics were defined according to the tumor-node-metastasis (TNM) criteria of the Union for International Cancer Control. Clinicopathologic factors, such as age, sex, histological types of tumors, tumor cell grade, TNM stage, lymphovascular invasion, and lymph-node metastasis, were reviewed. RESULTS: In this study, the PlGF protein expression level was significantly correlated with MVD, patient survival, and clinicopathological factors such as lymph-node metastasis, TNM staging, lymphatic invasion and vascular invasion. CONCLUSION: PlGF may be an important angiogenic factor in human colorectal cancer, and in this study, PlGF expression level was significantly correlated with positive lymph-node metastases, tumor stage, and patient survival. These findings suggest that PlGF expression correlates with disease progression and may be used as a prognostic marker for colorectal cancer.

PURPOSE: Along the invasive margin, colorectal cancer may show distinctive morphologic changes characterized by an asymmetrically attenuating tumor gland with loss of polarity. The author coined the term ‘gland attenuation (GA)’ for these peculiar changes. The aims of this study were to compare the immunoreactivity of the epithelial-mesenchymal transition (EMT) markers E-cadherin and β-catenin and thus determine whether EMTs occurs at tumor budding (TB) or GA sites and to assess the association of TB and/or GA levels with clinicopathological parameters and prognosis. METHODS: Expression patterns of E-cadherin and β-catenin in the tumor centers at GA and TB sites were examined in 101 patients with well or moderately differentiated CRCs, and the prognostic significance of TB and/or GA was statistically evaluated. RESULTS: GA foci, as well as TB foci, revealed loss of membranous and cytoplasmic E-cadherin expressions and aberrant β-catenin expression with reduced membranous expression and increased localization to the nucleus, suggesting that EMTs occur in GA as well as in TB. The high-TB and the TB-dominant groups were significantly correlated with advanced invasion depth, presence of lymph node metastasis, advanced pathologic staging and presence of lymphovascular invasion. The high-TB and the TB-dominant groups showed poor overall survival (OS) and recurrence-free survival (RFS), and high TB was an independent prognostic factor in the multivariate analyses for OS and RFS. CONCLUSION: This study showed evidence that EMTs occurs at GA sites as well as TB foci. TB is a strong and independent prognostic factor, and TB-dominance may be an indicator of adverse clinical outcome.
