2.A Study on Blood Lead Levels of Children in Ulsan.

PURPOSE: Lead toxicity affects every organ system, most importantly, the central and periphral nervous systems, kidney, and blood. Not yet, there are not so many data about blood lead levels of children in Korea. The children residing in Ulsan have some risk exposured to lead because of air pollution resulted from rapid industrialization and some lead-using industries. So we estimated the blood lead levels of children in Ulsan and tried to abtain the predict values of them. METHODS: Venous blood specimens were obtained for lead levels from two groups of 60 primary school children(30 residing in suburb, near-industry, and 30 in inner-city), aged 11-12 years. We also determined serum iron, ferritin, and hemoglobin. We compared blool lead levels between two groups according to nature of housing, age of housing, using the paint, condition of paint, parent's occupation. RESULTS: 1) The mean blood lead levels of 60 chiddren was 6.20+/-2.29 g/dl. 2) The children residing in inner-city had the higher blood lead levels(7.02+/-2.29 g/dl) than near-industry, suburban children (5.38+/-1.99 g/dl) with statistical significance (p<0.01). 3) Four children had the blood lead level over 10 g/dl, but all of them below 15 g/dl, and no intellectual deficit. 4) Air lead in classroom of 2 schools was not detected. 5) The serum iron, ferritin and hemoglobin level were all within the normal range. 6) There were no significant difference of blood lead levels between two groups according to nature of husing, age of housing, using the paint, condition of paint, parent's occupations. CONCLUSIONS: The mean blood lead levels of some primary school children in Ulsan was 6.20+/-2.29 g/dl, within the acceptable ranges. Children residing in inner-city had slightly higher blood lead levels than near-industry, suburban children.

There are numerous theories of pathophysiology in intracranial aneurysm, but currently accepted hypothesis was congenital defect of medial layer of vascular wall and the major developing factor was hemodynamics. Almost physicians were belived that treatment of intracranial aneurysm was completely through clipping of aneurysmal neck. But many reports were emphasized newly deloped intracranial(De Novo) aneurysm after clipping of initially identified intracranial aneurysm. The authors have same experienced of 2 cases intracranial "Do Novo" aneurysms. We are stress there were never completed treatment of aneurysm that clipping of initially identified intracranial aneurysm and should be attention to "De Novo" aneurysm.

4.A Study of the Trends in Korean Nursing Research on Critical Care in the Last 10 Years (2008–2017) Using Integrated Review and Key Word Analysis

PURPOSE: The purpose of this study was to examine the possible direction of critical care nursing research in the future by analyzing the trends of recent Korean studies.METHOD: Using a database search, we selected 263 articles on critical care nursing that were published in Korean journals between 2008 and 2017. Then, we conducted an integrative review of the contents of the selected articles and analyzed the English abstracts using the relevant packages and functions of the R program.RESULTS: The number of studies concerning critical care nursing has increased over the 10-year period, and the specific topic of each study has diversified according to the time at which it was conducted. In terms of quality, the majority of the research was published in high-level academic journals. The key words regularly studied over the past decade were: knowledge, delirium, education, restraint, stress, and infection. Studies related to vancomycin-resistant enterococci infection, compliance, and standards have decreased, while studies related to death, communication, and safety have increased.CONCLUSION: Randomized controlled trials and protocol research for evidence-based critical care need to be conducted, as does research on family involvement. The key word analysis of unstructured text used in this study is a relatively new method; it is suggested that this method be applied to various critical care nursing research and develop it methodologically.

5.Previous Infection with Plasmodium berghei Confers Resistance to Toxoplasma gondii Infection in Mice

Both Plasmodium spp. and Toxoplasma gondii are important apicomplexan parasites, which infect humans worldwide. Genetic analyses have revealed that 33% of amino acid sequences of inner membrane complex from the malaria parasite Plasmodium berghei is similar to that of Toxoplasma gondii. Inner membrane complex is known to be involved in cell invasion and replication. In this study, we investigated the resistance against T. gondii (ME49) infection induced by previously infected P. berghei (ANKA) in mice. Levels of T. gondii-specific IgG, IgG1, IgG2a, and IgG2b antibody responses, CD4+ and CD8+ T cell populations were found higher in the mice infected with P. berghei (ANKA) and challenged with T. gondii (ME49) compared to that in control mice infected with T. gondii alone (ME49). P. berghei (ANKA) + T. gondii (ME49) group showed significantly reduced the number and size of T. gondii (ME49) cysts in the brains of mice, resulting in lower body weight loss compared to ME49 control group. These results indicate that previous exposure to P. berghei (ANKA) induce resistance to subsequent T. gondii (ME49) infection.

6.Virus-Like Particles Expressing Toxoplasma gondii Rhoptry Protein 18 Induces Better Protection Than Rhoptry Protein 4 against T. gondii Infection

Toxoplasma gondii is a ubiquitous protozoan parasite responsible for causing toxoplasmosis. Preventive measures for toxoplasmosis are currently lacking and as such, development of novel vaccines are of urgent need. In this study, we generated 2 virus-like particles (VLPs) vaccines expressing T. gondii rhoptry protein 4 (ROP4) or rhoptry protein 18 (ROP18) using influenza matrix protein (M1) as a core protein. Mice were intranasally immunized with VLPs vaccines and after the last immunization, mice were challenged with ME49 cysts. Protective efficacy was assessed and compared by determining serum antibody responses, body weight changes and the reduction of cyst counts in the brain. ROP18 VLPs-immunized mice induced greater levels of IgG and IgA antibody responses than those immunized with ROP4 VLPs. ROP18 VLPs immunization significantly reduced body weight loss and the number of brain cysts in mice compared to ROP4 VLPs post-challenge. These results indicate that T. gondii ROP18 VLPs elicited better protective efficacy than ROP4 VLPs, providing important insight into vaccine design strategy.

7.New Chronic Osteomyelitis Model in White Rabbit: A Preliminary Report.

The purpose of this study was to develope a rabbit model for the chronic osteomyelitis, which is reproducible, controllable in quantity of bacteria and suitable for toxicologic research and therapeutic intervention studies. Osteomyelitis was induced in white rabbits by injecting varying numbers of S. aureus(ATCC 19636, 49230) and Alginate-CaCl2 into the proximal metaphysis of tibia. Three rabbits were used in each number of S. aureus respectively. The tibia were harvested at 8 weeks later and evaluation was done by clinical, radiological and histological findings. Clinical sings of infection consisted of soft tissue swelling and limping in rabbits, Radiologic findings were periosteal reaction, osteolysis, new bone formation in proximal tibias. Histology showed chronic active inflammation, debris of alginate, clusters of bacterial and granulation tissue. In ATCC 19636(more than 6X105) inoculated rabbits, osteomyelitis was established consistently in all three rabbits. Using Alginate-CaCl2 and ATCC 19636 Staphylococcus aureus, we made a new chronic osteomyelitis model, reproducible and controllable in quantity of bacteria.

8.Increased Vascular Endothelial Growth Factor in the Ventricular Cerebrospinal Fluid as a Predictive Marker for Subsequent Ventriculoperitoneal Shunt Infection : A Comparison Study among Hydrocephalic Patients.

OBJECTIVE: The aim of this study is to determine the association between the cerebrospinal fluid (CSF) biomarkers and inflammation, and the predictive value of these CSF biomarkers for subsequent shunt associated infection. METHODS: We obtained CSF samples from the patients with hydrocephalus during ventriculoperitoneal (VP) shunt operations. Twenty-two patients were enrolled for this study and divided into 3 groups: subarachnoid hemorrhage (SAH)-induced hydrocephalus, idiopathic normal pressure hydrocephalus (INPH) and hydrocephalus with a subsequent shunt infection. We analyzed the transforming growth factor-beta1, tumor necrosis factor-alpha, vascular endothelial growth factor (VEGF) and total tau in the CSF by performing enzyme-linked immunosorbent assay. The subsequent development of shunt infection was confirmed by the clinical presentations, the CSF parameters and CSF culture from the shunt devices. RESULTS: The mean VEGF concentration (+/-standard deviation) in the CSF of the SAH-induced hydrocephalus, INPH and shunt infection groups was 236+/-138, 237+/-80 and 627+/-391 pg/mL, respectively. There was a significant difference among the three groups (p=0.01). Between the SAH-induced hydrocephalus and infection groups and between the INPH and infection groups, there was a significant difference of the VEGF levels (p<0.01). However, the other marker levels did not differ among them. CONCLUSION: The present study showed that only the CSF VEGF levels are associated with the subsequent development of shunt infection. Our results suggest that increased CSF VEGF could provide a good condition for bacteria that are introduced at the time of surgery to grow in the brain, rather than reflecting a sequel of bacterial infection before VP shunt.

9.Squamous Cell Carcinoma Arising in Mature Cystic Teratoma of the Ovary: a Report of Six Cases and Immunohistochemistry of the p53 Protein and p21WAF1/CIP1.

BACKGROUND: Mature cystic teratoma is a common type of ovarian tumor. Although squamous cell carcinoma (SCC) is the most common carcinoma in malignant transformations of ovarian mature cystic teratomas, SCC arising in a mature teratoma is rare. METHODS: This paper reports four cases of invasive SCC, a case of an adenosquamous cell carcinoma and a case of a pure in situ SCC arising in a mature cystic teratoma including a clinicopathological evaluation and an immunohistochemical study of the p53 protein and p21WAF1/CIP1. RESULTS: The mean age of the patients was 60 years. The sizes of the mature cystic teratomas in all cases were greater than 7.5 cm in the largest diameter. Five cases showed the nuclear accumulation of the p53 protein with no p21WAF1/CIP1 immunoreactivity. The other case showed the nuclear accumulation of p21WAF1/CIP1 without p53 expression. There was a significant inverse relationship between the p53 protein level and p21WAF1/CIP1 expression. CONCLUSION: A clinicopathological evaluation showed that a SCC arising from a mature cystic teratoma must be included in a differential diagnosis when the patient is over 42 years of age and the size of a mature cystic teratoma is greater than 75 mm in the largest diameter. It is suggested that p53 overexpression is implicated in the malignant transformation, and the p21WAF1/CIP1 expression level is dependent on alterations in the level of the p53 protein in these tumors.

10.Toxic Hepatitis Associated with Carp Juice Ingestion.

The potential hepatotoxicity of herbal remedies and/or health foods is usually ignored in daily life. There have been cases showing the toxic hepatitis and renal failure associated with the ingestion of raw carp bile. We experienced a case of toxic hepatitis without any evidence of renal failure that was associated with carp juice ingestion. The clinical manifestations were characterized by nausea and vomiting after the ingestion of carp juice for 3 months. The diagnosis of toxic hepatitis was made on the basis of the patient's history, laboratory data, RUCAM (Russel Uclaf Causality Assessment) and the results of ultrasonography guided liver biopsy. The patient showed rapid improvement after instituting supportive therapy.
