We report a 72-year-old female patient with spontaneous rupture of the left external iliac vein. She visited our hospital for abdominal and back pain. She had the abnormal finding of hemoperitoneum. We performed an emergency operation with diagnosis of left ovarian cyst rupture though she suffered from spontaneous rupture of the left external iliac vein. This case provides insight to the experience of spontaneous rupture of the left external iliac vein.

In many developed countries, for example, USA, respirator fit testing is required before entering specific work environment to ensure that the respirator worn satisfies a minimum of fit and that the user knows when the respirator fits properly. Unfortunately because we have not fit test regulation in Korea, a lot of workers wearing respirators may be potentially exposed to hazards. This study was conducted to evaluate the fitting performance for respirators and correlation fit factors with facial dimensions of wearers. 110 subjects (70 males, 40 females) were fit tested for three quarter masks, i.e., two domestic-made Mask Z, and Y and one foreign-made Mask T using PortaCount 8020. A facial dimension survey of the same subjects was conducted to develop a facial dimension grids for correlation fit factors with facial dimension parameters. A facial dimension grid was developed on the basis of face length and lip length for quarter masks. The results obtained were as follows: 1. Fit factors of Mask T were much higher than those of Masks Z, and Y. 2. Males were fitted more properly than females. 3. Male in box "f" of grid would be adequately fitted Mask Y and male in box "b", "e", "f", "h" of grid would be sufficiently fitted Mask T. Female in box "h" of grid may have a good fitting performance for both Mask Y, and T. But subjects in all boxes of grid would be inadequately fitted Mask Z.

Background: This study aims to introduce the formulation of the regulation for the selection of respirators for accident preparedness chemicals (APCs) according to chemical workplace situations and to determine on-site applicability. Methods: Workplaces were grouped into seven work categories, and APCs were classified into six groups to select adequate respirators. A survey was conducted to enhance the understanding of work situations and adequate respirators. The total number of subjects surveyed in 2018 was 201 managers and handlers, and that in 2019 was 91 handlers and 204 managers. Results: Adequate respirators were allocated to each cell using the matrix method. The study observed an overall lack of understanding of work situations, especially in the operation of open devices, which was the highest at 32.7%. Despite its implementation in 2015, 17.6% and 25.0% of the managers and APCs handlers, respectively, were unaware of the regulations for selecting respirators. Only 70.4% of the APCs handler wore respirators in compliance with regulations. Conclusion The method for selecting respirators according to work situations using the matrix method is considered reasonable. Thus, this study suggests that the development of educational contents and reinforcing education should be essential steps to increasing awareness of regulations.

We experienced 2 cases of caute subdural hematoma with tentorial herniation due to ruptured posterior communicating artery aneurysm. Based on a review of the literature and the clinical course in the present cases, recommend therapy consist of early evacuation of hematoma and clipping of aneurysm because of the not infrequent development of rapid deterioration.

A series of 26 patients of acute axis fractures were treated at the Wonkwang Medical Center, Wonkwang University between January, 1988 and December, 1992. The medical records and roentgenograms of these 26 patients were reviewed in detail. Follow-up data were available for all of the cases. for a median duration of 25.6 months(range 3 months to 5years). There were 8 hangman's fractures(31%), 10 odontoid type II fractures(38%), 6 odontoid type III fractures(23%), 2 miscellanous fractures(8%). The odontoid type II fracture was the most common axis fracture and the most common cause of injury was motor-vehicle accident followed by fall, Hangman's fracture, odontoid type II, miscellanous fracture were effectively treated with external stabilization. In the 10 odontoid type II fractures, 7 patients were treated by external stabilization and 3 were by early surgical stabilization. Among the external stabilization group, the fusion failure occured 2 of the displacement of 4mm or greater in the initial treatment.

Toxoplasma gondii tachyzoites were isolated from the blood of an ocular patient, and have been successfully passaged in the laboratory, for over a year, by peritoneal inoculation in mice. The isolated parasite was designated the Korean Isolate-1 (KI-1) and its characteristics were compared with those of the RH strain, a wellknown virulent strain originating from a child who suffered from encephalitis. The morphology, pathogenicity, infectivity and cell culture characteristics of the KI-1 were similar to those of the RH strain. Both RH and KI-1 antigens were detected by an anti-T. gondii monoclonal antibody (mAb), Tg563, against the major surface protein SAG1 (30 kDa), whereas no reaction was observed against an anti-Neospora caninum mAb, 12B4. The KI-1 was confirmed as an isolate of T. gondii. A long-term laboratory maintenance and characterization of a local T. gondii isolate is reported for the first time in the Republic of Korea.

Plasmodium vivax malaria re-emerged in South Korea in 1993, and epidemics continue since then. We examined genetic variation in the region encompassing the apical membrane antigen-1 (PvAMA-1) of the parasites by DNA sequencing of the 22 re-emerging P. vivax isolates. The genotype of the PvAMA-1, which was based on sequence data previously reported for the polymorphic regions, showed that two haplotypes were present at one polymorphic site. Compared with reported data, the two types, SKOR type I and type II, were similar to Chinese CH-10A and CH-05A isolates, respectively. Thus, the present study showed that two genotypes of AMA-1 genes coexist in the re-emerging Korean P. vivax.

Distal thumb injuries are a common and difficult problem for hand surgeons. Coverage of soft tissue on the fingers may be difficult due to the size of the defect or the limitation of local flap mobilization. However, the variable anatomy of the dorsal hand vascular system sometimes prevents successful flap harvest. The purpose of this study was to clarify the vascular anatomy of the dorsal side of the thumb and the first web for the flaps. Twenty six hands (13 right and 13 left hands) from Korean embalmed cadavers were dissected. A catheter was inserted into the radial artery in the forearm, and the red colored latex (Latex 671, Dupont Industry, France) was injected until the dorsum of the hand was colored. The arrangement of the first dorsal metacarpal artery (FDMA) and its branches were vary and classified into three categories according to their branching patterns; Both dorso-ulnar thumb branch (DUTB) and dorso-radial index branch (DRIB) arose from the FDMA (10 cases, 38.5%). Each DUTB and DRIB arose separately from the radial artery (5 cases, 19.2%). The DUTB and the DRIB originated from the princeps pollicis artery and the radial artery, respectively (11 cases, 42.3%). The typical course of the FDMA and its branches ran overlying the first dorsal interosseous muscle in 17 cases of the 26 specimens (65.4%). However, in nine cases (34.6%) the DRIB ran on the first dorsal interosseous muscle and the DUTB had a deep course within the substance of the first dorsal interosseous muscle. The FDMA flap represents a good option to cover defects for the thumb. These anatomical findings in the present study could provide useful knowledge of flaps for dorsal aspect of the thumb and the first web.

Over 26 kinds of tablets and capsules, such as Tetracycline, Quinidine and Potassium preparations were reported to cause esopbageal ulcerations, eharacterized in various size, shape and number, ind sually in mid-esophagus, Recently authors experienced 10 cases of drug-induced esophageal ulcerations due to antibiotics and antiinflammatory agents such as Tetracycline, Aspirin, etc. Four cases were men and six were women. Four cases were in third dades, three in fourth cleeades, two in fifth decades and one in sixth decades. Presenting symptoras were odynophagia(4/10), dysphagia(3/10), substernal pain(7/10) and epigastric pain(3/10). Endoscopic examination of the esophgus showed single or multiple, small and shallow ulcers on the mid-esophgeal mucosa at the level of 30cm from the ineisor in eight cases, one Iarge and deep ulcer at the level of 40cm from incisor in one patient and one amall, shallow and one large, deep ulcers at the same time. in one patient The shape of alcers were various from a round to a large horseshoe shaped one. The clinical course was mild without complications. It was suggested that drug-induced esophsgeal ulcers with antibiotics and antiinflammatory agents could be found frequently and they had benign and mild clinicalc ourse,

BACKGROUNDS/AIMS: This study focuses on the pathogenesis of inflammatory reaction and cell necrosis in patients with chronic viral hepatitis and examines the possible effects of follicular dendritic cells, HLA-DR and CD8+ presenting lymphocytes by analyzing their expression and the histological activity index (HAI) in liver tissues. METHODS: Liver biopsy specimens were obtained from 59 patients with chronic hepatitis B and from 26 patients with chronic hepatitis C. The expressions of dendritic cells, HLA-DR and CD8+ presenting lymphocytes were determined by immunohistochemical stain. RESULTS: The incidence of lymphoid follicle and/or lymphoid aggregates in portal tracts of the liver was higher in chronic hepatitis C than it was in chronic hepatitis B (84.6% vs. 15.3%, p=0.000). Follicular dendritic cells were exclusively expressed within lymphoid follicles and/or lymphocyte aggregates in portal areas. HLA-DR restricted cells were mainly observed in portal and periportal areas as well as in the area of piecemeal necrosis. CD8+ lymphocytes were diffusely expressed in portal and periportal areas and within intralobular parenchymal sinusoids. The expression of dendritic cell and HLA-DR was more frequently observed in moderate chronic hepatitis than in mild chronic hepatitis. While that of CD8+ lymphocyte expression was more frequent in severe chronic hepatitis with a high HAI score. CONCLUSIONS: The follicular dendritic cells may trap viral antigens in intraportal lymphoid follicle and present them to HLA-DR and CD8+ presenting lymphocytes. It is suggested that the associated expression of dendritic cells, HLA-DR and CD8+ presenting lymphocytes in liver tissues may play one of the biological role in immune injury in chronic viral hepatitis.
