1.A Case of Persistent Pupillary Membrane.

Persistent pupillary membrane ia a congenital anomaly which waa resulted from incomplete atrophy of the fetal vascular arcades and its associated mesodermal tissue derived from the primitive annular vessels. Authors experienced a case (female, 17 years old) of thick persistent pupillary membrane of both eyes which were required complete removal of these membranes under operating microscope. Her corrected vision has improved up to 0.5 (Rt. eye) from 0.1 and 0.7 (Lt eye) from 0.2 without diplopia after operation.

2.An analysis of HPV subtype in the uterine cervix and clinical usefulness of HPV DNA chip test.

OBJECTIVE: Human papillomavirus (HPV) has been identified more than 100 HPV subtypes. The distributions of subtypes are different according to nations and regions. We analysed subtype of infection with HPV among women who live in Pusan and surburbs of Pusan. We accessed the clinical usefulness of HPV DNA chip test as a supplementary method of Pap smear in the evaluation of cervical lesion. METHOD: This study was undertaken from January 2002 to January 2005 and the samples were collected from the patients who had abnormal Pap smear. We analysed subtypes of 143 positive cases with HPV DNA chip (Biomedlab) test and estimated pathologic reports of 115 patients except 28 patients who had not biopsy. We investigated pathologic results of 54 of 115 patients who had atypical squamous cells / low grade squamous intraepithelial lesion (ASC/LSIL) in Pap smear and examined high risk HPV in 54 pathologic results. RESULTS: The prevalence of HPV subtypes was 42 cases of HPV-16, 20 cases of HPV- 58, 16 cases of HPV-52, 10 cases of HPV-35, 9 cases of HPV-56, 7 cases of HPV-51, 6 cases of HPV-18 in descending order of incidence in high risk HPV group and 3 cases of HPV-6, 3 cases of HPV-42, 2 cases of HPV-34, 2 cases of HPV-43 in descending order of incidence in low risk HPV group. The results of HPV DNA chip test and 115 pathologic reports were estimated by comparative study. A pure infection with low risk HPV group was detected in low grade lesion. Infection with high risk HPV group was also detected in low grade lesion but was mainly detected in high grade lesion. The pathologic results of 54 patients who had ASC / LSIL in Pap smear were 13 patients had above high grade lesion include 2 cases of invasive carcinoma so false negative rate of Pap smear in the detection of high grade lesion was 24%. CONCLUSION: HPV subtypes were detected HPV 16, 58, 52, 35, 56, 51, 18 types in descending order of incidence and prevalence. Mass study and integrated data from larger population and various regions in many hospitals will be needed. And the supplementary use of HPV DNA chip test may provide clinical usefulness because it can reduce the false negative rate of Pap smear and improve the positive predictive value in the detection of high grade cervical lesion and it enables to decrease the incidence of cervical cancer.

6.Distribution of Neuropeptide Y-Immunoreactive neurons in the Olfactory Bulb of Adult and Aging Rat.

A detailed study of the distribution of neuropeptide Y(NPY) in the olfactory bulb of the male adult and aging rat brains was undertaken by means of immunohistochemistry. Adult rats(Sprague-Dawley, 12-14 weeks old) were used in the control group. And aging rats(30 months old) were used in the experimental group. Brain tissues of both rat groups were obtained from each animal which were perfused transcardially with 0.9% NaCl followed by a 4% paraformaldehyde solution. Coronal serial sections(30nm) were cut on a freezing microtome, and were immunostained with the rabbit-raised antiserum to neuropeptide Y. In the control group, neuropeptide Y-immunoreactive(NPY-IR) neurons were mainly found in the white matter and deep internal granule cell layer, and rarely in the external plexiform layer of main olfactory bulb. In case of accessory olfactory bulb, a few of NPY-IR neurons were found in the internal granule cell layer. Virtually most neurons were medium-sized(10-20nm), and appeared to be bipolar or bitufted in shape. And small numbers of large-sized multipolar neurons(20-30nm) were found in the internal granule cell layer of main olfactory bulb. In the experimental aging rats, the numbers of NPY-IR neurons were decreased, that is to say, NPY-IR neurons in aging group were found average 2.5 neurons per each section compared with average 4.5 per each section in the control group. Also many perikarya of NPY-IR neurons appeared shrinkage, oval or round in shape. In brief, NPY-IR neurons of the olfactory bulb seem to be decreased in number of the neurons in aging rat. These findings may concern with reduction of olfactory function.

8.Angiographic Characteristics of the Intracranial Saccular Aneurysms to Predict the Rupture.

OBJECTIVE: During the last two decades, detection of unruptured intracranial aneurysms has increased because of the improving diagnostic methods, but the management of unruptured intracranial aneurysm is still controversial. We analyzed the angiographic characteristics to compare ruptured aneurysms with unruptured aneurysms. METHODS: The patients were retrospectively selected for this study based on the availability of angiograms and a clear diagnosis of an unruptured or ruptured aneurysm. One hundred sixty nine patients with 209 aneurysms were included in the study. Sixty-one patients harbored only unruptured lesions, 85 only ruptured lesions and 27 had both ruptured and unruptured lesions. RESULTS: The mean age of all the patients was 55.3 years, and it was 53.34 years for those with ruptured aneurysms. It was found that 42.0% of the ruptured aneurysms were on the anterior communicating artery, compared with 10.3% of the unruptured aneurysms. None of the ophthalmic artery aneurysms were ruptured. The mean dome size, depth and aspect ratio of the ruptured aneurysms, except for the neck size, were significantly larger than that of the unruptured aneurysms, but the mean neck size of the ruptured and unruptured aneurysms showed no significant difference. A daughter sac was present in 7.2% of the unruptured aneurysms and in 17.0% of the ruptured aneurysms. CONCLUSION: We suggest that the morphologic features and location of aneurysms should be considered when making decisions regarding whether to treat unruptured aneurysms.
