1.Anal Endosonographic Findings of Internal Anal Sphincter in Normal Adult Korean.

PURPOSE: To describe the appearance and average thickness of the internal anal sphincter with anal endosonography in healthy Korean adults. MATERIAL AND METHODS: 184 subjects(male: 96, female: 88) with no history of anorectal disease or surgery were studied with anal endosonography. The average thickness of internal sphincter was meas ured at the mid-anal canal. For the internal sphincter, which is often asymmetric, the thickness of each 4 part(12, 3, 6 and 9 o'clock direction) were measured in left lateral decubitus position. RESULTS: The anal wall was well visualized in 5 layers(mucosa, submucosa, internal anal sphincter, intersphincteric plane, external anal sphincter) with anal endosonography. The anal endosonogram showed the internal anal sphincter as a homogenous, well-defined, hypoechoic, circular band, and slightly asymmetric. The average thickness of the internal anal sphincter in the area of mid-anal portion was 2.0 0.3 mm(range: 1~3 mm). There was no sexual difference; however, a significant positive correlation with age was found in average thickness of the internal anal sphincter. The correlation with lean body mass was not found. CONCLUSION: The internal anal sphincter is well-visualized, best defined structure by anal endosonography. Average thickeness of the sphincter in Korean appeared to be the same as in the Western.

6.A Case of Malignant Histiocytosis Showing Papulosquamous Skin Lesions and Fever as Initial Manifestations.

We report a case of malignant histiocytosis which began with intermittent fever and scaly skin lesions. A 3-year-old girl presented with erythematous scaly papules on the face and the trunk, and high fever for 3 months. The cutaneous lesions consisted of widespread coin-sized erythematous scaly papules with marginal brownish pigmentation. She was anemic and thrombocytopenic and had impairment of the liver function. Histopathologic study of the skin lesions showed non-specfic findings except for hyperkeratosis. However, bone marrow examination revealed an increased number of histiocytes, mostly immature with active phagocytosis of erythroid cells, myeloid cells, and platelets. She was diagnosed as having malignant histiocytosis and treated with cyclophosphamide and vincristine. She died the next day after the treatment had begun.

8.The Effects of Glutathione on Cyclosporine Induced Cytotoxicity in Cultured Insulinoma Cells.

PURPOSE: Cycloporine A (CsA) is a immunosuppressive agent most widely used in organ transplanted patients for preventing immunorejection, but it has some side effects, including hypertension, nephrotoxicity, hepatotoxicity and diabetes. The mechanism of toxicity of CsA was not completely understood, but the reactive oxygen species has been proposed to be involved in the reaction of toxicity of CsA. The purpose of this study is to determine the effects of glutathione, as a physiological antioxidant on CsA induced cytotoxicity in rat insulinoma (RINm5F) cells. METHODS: RINm5F cells were incubated in the presence of CsA (105~108 M) and buthionine sulfoximine (BSO), as a inhibitor of r-glutamyl cysteine synthetase, was added to cultured media (RPMI1640). Twenty four hours of incubation with CsA and BSO, viable cells were determined by MTT method. RESULTS: CsA decreased cell viability in dose response in cultured RINm5F cells and significantly decreased according to redusing glutathione. CONCLUSION: These results suggested that CsA may induce the diabetes and glutathione have some roles in the pathogenesis of CsA-induced diabetes.

During the Period from 22th to November 26th , 1971, An Epidemiological Survey was performed on the Accident which was occurred by Leakage of Chloride Gases in A Chimical Industry, one of Wul San Industrial Areas. The obtained results were as follows ; 1. The largest damage was observed at Yeo Cheon Dong, 5.38 Km2 in Area and 8,192 in the Residents. 2. By the age distribution of Yeo Cheon Dong, The largest damaged area resulting from chloride accident, the age group of between 10 and 39 was highly occupied with 55.2% of all Yeo Cheon residents. 3. The number of patient was 436 totally, 198 in male and 238 in female. 4. By the age group distribution of the patient, in case of male, the largest group was under 20 age (96) and female, between 20 and 40 age group. 5. By the educational level of the patient, No-Education group was 116 (26.6%), and primary school, middle school and high school were respectively 168 (38.5%), 68(15.6%) and 12 (2.8%). 6. By the occupational distribution of the patient, No-Occupation group was 49.7%, students and agriculture were respectively 14.4% and 8.1%. 7. By the complaints of the patient, most of all were for respiratory tract, those are coughing (56.9%), chest tightness (35.6%), sore throat (24.8%), and so forth. 8. By the injured domestic animal, the Dog was most highly injured with 46.2%.
