1.A Survey of Positive Rates of HBsAg and anti-HBs in a Rural Area of Korea.

OBJECTIVE: The positive rates of hepatitis B virus (HBV) markers differ according to age, sex, race, locality, sexual behavior, socioeconomic conditions, immunologic and genetic factors. It is a well-known fact that Korea is one of the endemic areas of HBV infection in the world. Past many reports have presented that the positive rates of HBsAg and anti-HBs in Korea were about 6~14% and 30~62%, respectively. However there were a few epidemiological surveys of rural communities on the positive rates of HBV markers. The present study undertook to determine the positive rates of HBsAg and anti-HBs in Hwasun county, a rural area of Korea. METHODS: We performed Population-based cross- sectional study by random cluster sampling of registered residents in Hwasun county. Out of the 2,920 subjects (4.1%) scheduled for the survey, 1,913 residents underwent the actual examination. We tested HBsAg and anti- HBs by reversed passive hemagglutination (RPHA) and passive hemagglutination (PHA), respectively. RESULTS: The positive rates of HBsAg and anti-HBs in all subjects were 5.6% and 20.8%, respectively, and those were 5.4% and 20.4% in 168 pregnant women, respectively. The positive rate of HBsAg was significantly higher in male (8.2%) than in female (4.0%) (p< 0.01). The positive rate of anti-HBs was 21.4% in female, but it was not significanly higher than in male (19.6%). There were no significant differences according to age groups in the positive rates of HBsAg and anti-HBs. CONCLUSION: There are no significant changes in the positive rates of HBsAg and anti-HBs between the present study for Hwasun county and past many reports in Korea. Although the rate of hepatitis B vaccination increased significantly, it was not effective because that the vaccinated total numbers in the rural area of Korea were probably small. So it is very important to undertake a nationwide program of hepatitis B immunization and to educate the residents in rural areas of Korea.

2.The effects of gingival blood flow on pulpal blood flow detection using ultrasound Doppler flowmetry: animal study

Objectives:
This study evaluated the effect of adjacent gingival blood flow on detection of pulpal blood flow (PBF) using ultrasound Doppler flowmetry (UDF) through animal study.
Materials and Methods:
The study included 36 right and left maxillary the third incisors and canines in 9 experimental dogs. The study included 2 main steps: In the first step, the pulse sound level (PSL) was recorded on the cervical part of each tooth without flap elevation (Group 1), with flap elevation (Group 2), and after it was repositioned in place (Group 3). In the second step, the PSL was recorded on the cervical part of each tooth (Group 4), after pulpotomy (Group 5), after partial pulp extirpation (Group 6), after complete extirpation (Group 7), and after canal filling (Group 8). In Groups 5–8, the study was performed with and without flap elevation in the left and right teeth, respectively. The PSL was graded as follows: 0, inaudible; 1, heard faintly; and 2, heard well. The difference between each group was analyzed using Friedman’s test with Wilcoxon signed-rank tests (α = 0.05).
Results:
In step 1, the PSL results were Group 1 > 2 and 3. In step 2, there was no significant difference between the groups when the flap was not elevated, while PSL results were Group 4 > 5 ≥ 6 and 7 ≥ 8 when the flap was elevated.
Conclusions
PBF is affected by gingival blood flow when measured with UDF. UDF measurements require isolation of gingiva from the tooth.
3.Effects of Parental Verbal Abuse Experience on the Glutamate Response to Swear Words in the Ventromedial Prefrontal Cortex:A Functional 1 H-magnetic Resonance Spectroscopy Study

Objective:
Several lines of evidence indicate verbal abuse (VA) critically impacts the developing brain; however, whether VA results in changes in brain neurochemistry has not been established. Here, we hypothesized that exposure to recurrent parental VA elicits heightened glutamate (Glu) responses during the presentation of swear words, which can be measured with functional magnetic resonance spectroscopy (fMRS).
Methods:
During an emotional Stroop task consisting of blocks of color and swear words, metabolite concentration changes were measured in the ventromedial prefrontal cortex (vmPFC) and the left amygdalohippocampal region (AMHC) of healthy adults (14 F/27 M, 23 ± 4 years old) using fMRS. The dynamic changes in Glu and their associations with the emotional state of the participants were finally evaluated based on 36 datasets from the vmPFC and 30 from the AMHC.
Results:
A repeated-measures analysis of covariance revealed a modest effect of parental VA severity on Glu changes in the vmPFC. The total score on the Verbal Abuse Questionnaire by parents (pVAQ) was associated with the Glu response to swear words (ΔGluSwe ). The interaction term of ΔGluSwe and baseline N-acetyl aspartate (NAA) level in the vmPFC could be used to predict state-trait anxiety level and depressive mood. We could not find any significant associations between ΔGluSwe in the AMHC and either pVAQ or emotional states.
Conclusion
Parental VA exposure in individuals is associated with a greater Glu response towards VA-related stimuli in the vmPFC and that the accompanying low NAA level may be associated with anxiety level or depressive mood.
4.Nationwide Estimation for Incidence at Birth of Congenital Polydactyly and Syndactyly in Korean.

INTRODUCTION: The aim was to estimate the incidence at birth of congenital polydactyly and syndactyly to use medical utilization database of the National Health Insurance Corporation in Korea. MATERIAL & METHOD: Base population are selected from the children born in 1994(n=728,515). The diseases were identified by disease code system; ICD-9 755(Other congenital limb-anomalies), ICD-10 Q69(Polydactyly), ICD-10 Q70(Syndactyly), ICD-10 Q66(Other congenital feet-deformities), ICD-10 Q74(Other upper limb-anomalies). The suspicious cases of polydactyly and syndactyly are selected from the base population with those criteria(n=3,497). The 669 cases, 16 university hospitals and 16 hospitals have been examined the name and location of anomaly through medical record review. The diagnostic accuracy by Disease-code system was calculated. The incidence rate at birth was estimated by the class of polydactyly/syndactyly, adjusted for the diagnostic accuracy. RESULT: The incidence per 100,000 birth are estimated; at hand, polydactyly, 93(95% confidence interval 86-100), syndactyly, 29(25-33), polysyndactyly, 13(11- 16); at foot, polydactyly, 40(36-45), syndactyly, 22(19-26), polysyndactyly, 14(12 - 17); at multifocal area(hand & foot), polydactyly, 1(0.6 -2), syndactyly, 2(0.9-3), polysyndactyly, 3(2 - 4). CONCLUSION: These incidence rates may be underestimated and show the minimum incidence rates. The incidences must be discussed in comparison with incidences through registry or prospective follow-up study of congenital anomalies.

5.The Prevalence of Simple Renal Cysts Studied by Ultrasonography, and the Relations to Age and Hypertension.

Simple renal cyst is the most common space- occupying lesion of kidney in adults. However, there were a few systematic Korean reports for the prevalence of simple renal cysts. Our purpose was to estimate the prevalence of simple renal cysts using ultrasonography, and to determine the relations to age and hypertension. The subjects were 504 volunteers (aged over 29 years) of the persons who were enrolled a health check-up program of Naju Hospital, Naju, Korea. Number of male in 504 persons was 334 (66.3%) and female 170 (33.7%). Mean age was 55 years (range; 30-86). The results were as follows: 1) One or more renal cysts were found in 50 individuals of 504 persons aged over 29 years (9.9%). In 331 persons aged over 49 years, the prevalence of simple renal cysts was 12.1%. The prevalence was significantly increased with age (P<0.001), but the difference between male (9.6%) and female (10.6%) was insignificant. 2) Mean value of cyst diameter was 23mm (5-69 mm) and was not correlated with age. However, the cyst diameter was significantly higher in female than in male, 32.4mm vs 18.0mm (P<0.001). Total number of cysts per person tended to increase with age (P<0.001). 3) The correlation between prevalence of simple renal cysts and blood pressure was statistically insignificant. However, the prevalence of hypertension in persons with cysts was higher than in persons with no cyst (27.9% vs 15.3%, P<0.05). Cyst diameter was not correlated with blood pressure. 4)Urinalysis in persons with cysts, showed microscopic hematuria in 47.4% and proteinuria in 26.3 %, however, grade of microscopic hematuria and proteinuria were not correlated with cyst diameter. The concentrations of blood urea nitrogen, serum creatinine, hemoglobin and hematocrit were not affected by presence or absence of simple renal cysts. By comparison with the recent foreign reports, the prevalence of simple renal cysts in Korean was similar to those in foreigners. Our study ascertained that simple renal cysts are age-dependent and total number of cysts per person also increases with age. In most of persons with simple renal cysts, the renal function is not affected and the related symptoms are trivial. However, we suggested that simple renal cysts may cause hypertension. To elucidate the correlation between simple renal cyst and hypertension, further studies are needed.

6.Development of Korean Social Affective Visual Stimuli

OBJECTIVES: The current study aims to develop Korean Social Affective Visual Stimuli (K-SAVS) to arouse social affect. METHODS: K-SAVS is composed of pictures of social situations among East Asians. Each picture contains a negative, positive, or neutral affect. Positive and negative stimuli were presented to one group of subjects. Negative and neutral stimuli were presented to another group of subjects. All subjects were required to fill out Affective Valence and Arousal Manikin. RESULTS: In the initial test, 18 positive affective stimuli pictures (1 set) and 36 negative affective stimuli pictures (2 sets) were chosen from 95 stimuli pictures. Positive affective stimuli pictures showed higher valence (p < 0.001) and lower arousal (p < 0.001) than negative affective stimuli pictures. In the confirmatory test, 18 neutral stimuli pictures (1 set) were additionally selected out of the total of 79 pictures of neutral stimuli pictures and the pictures used in the former experiment. Neutral stimuli pictures showed higher valence (p < 0.001) and lower arousal (p < 0.001) than negative affective stimuli pictures. CONCLUSION: K-SAVS can be a valid and useful tool for inducing specific social affects of Koreans.

7.Outbreaks of nosocomial feline Internal Medicine calicivirus-associated virulent systemic disease in Korea

Objective:
To evaluate the clinical characteristics of a nosocomial FCV-VSD outbreak involving 18 cats in Korea.
Methods:
Medical records of cats diagnosed with FCV-VSD from March to September 2018 at a referral veterinary hospital were reviewed. The patient’s signalment, history, clinical features, diagnosis, treatment, and prognosis were evaluated.
Results:
Two outbreaks involving 18 cats diagnosed with FCV-VSD occurred over a 6-month period at a referral hospital in Korea. Anorexia, lethargy, fever, and limb edema were the most commonly observed clinical symptoms. Lymphopenia and macrothrombocytopenia were the most common hematological findings, and hyperbilirubinemia and increased levels of aspartate aminotransferase, creatine kinase, and serum amyloid A were the most frequent results of serum biochemistry. FCV was detected by reverse transcription polymerase chain reaction in 11 patients and the remaining 7 were suspected with FCV-VSD. The overall mortality rate was 72.2%. The hospital was closed and disinfected twice, and no additional outbreaks have occurred since the last patient.
Conclusions
and Relevance: The clinical and diagnostic characteristics and outcomes of FCV-VSD described in this study can be used to recognize and contain infectious diseases through quick action. To the best of the authors’ knowledge, this is the first report of a nosocomial outbreak of FCV-VSD in Asia.
8.Posterior Tibialis Tendon Dysfunction: Preliminary Report.

PURPOSE: Posterior tibialis tendon dysfunction (PTTD) is one of the most common causes of acquired flatfoot deformity in western countries. But it was known that they were very rare in eastern countries. So we want to report the clinical features and outcomes of 12 patients with PTTD. MATERIALS AND METHODS: We evaluated the results of 12 patients using clinical features and results from March 2000 to January 2007 and mean follow up periods is 32 months. RESULTS: Average age was 45 years, 8 of 12 patients were female, 2 patient with hypertension and 1 with rheumatoid arthritis. 5 patients were overweighted and 5 patients were obese. 4 patients has a history of last trauma. 4 patients experienced progression of flatfoot. On behalf of Johnson and Strom classifications 6 cases were grade I, 4 cases were grade II, grade III, and grade IV was 1 case, relatively. As a treatment we used tenosynovectomy for 3 cases of grade I, additional FDL transfer was done for 2 cases of grade I and 2 cases of grade II. For other 2 patients of grade II flexor digitorum longus transfer and Medial displacement calcaneal osteotomy was done. Pathologic findings of tendon showed degenerative tendinitis. Lastly conservative treatment group was 3 cases of grade I, III, IV each. Average preoperative and postoperative American Orthopedic Foot and Ankle Society's hindfoot/ankle scoreFAS score was 58 and 90. Initial and follow up AOFAS scores of the conservative group was 38 and 57, relatively. CONCLUSION: As a cause of acquired flatfoot in adult, PTTD is not a rare disease any more in Korea. To prevent the disability and progression of flatfoot, careful clinical evaluation and proper treatment is important.

9.Network Structure of Depressive Symptoms in General Population

Objectives:
:Although subclinical depression symptoms are associated with suicidal idea, most research have focused on clinical depression such as major depressive disorder or dysthymia. The aim of this study is to investigate network structure of depressive symptom and to reveal which symptoms are associated with suicidal ideation.
Methods:
:We used part of data from the seventh Korea National Health and Nutrition Examination Survey. Participants were between 19 and 65 years of age (N=8,741). Network analysis with Isingfit model is used to reveal network structure of depressive symptoms and most central symptom and edges assessed by patient health questionnaire (PHQ-9).
Results:
:The most two central symptoms were psychomotor activity and suicidal ideation. The strongest edge was psychomotor activity-suicidal ideation. Suicidal ideation also has strong association with depressive mood and worthlessness.
Conclusions
:These results suggest that psychomotor activity and suicidal ideation can serve as treatment target for subclinical depression and psychomotor activity, worthlessness and depressed mood may be important factor for early intervention of suicidal ideation.
