STATEMENT OF PROBLEM: Over the years, resin-bonded fixed partial dentures (RBFPDs) have gone through substantial development and refinement. Several studies examined the biomechanics of tooth preparation and framework design in relation to the success rate of RBFPDs and considered retention and resistance form essential for increase of clinical retention. However, these criteria required preparations to be more invasive, which violates not only the original intentions of the RBFPD, but may also have an adverse effect on retention due to loss of enamel, an important factor in bonding. PURPOSE: The object of this in vitro study was to compare the dislodgement resistance of the new types of RBFPDs, the conventional three-unit fixed partial denture, and conventional design of RBFPD (Maryland bridge). MATERIAL AND METHODS: Fifty resin mandibular left second premolars and second molars were prepared on dentiforms, according to the RBFPD design. After model fabrication (five group, n = 10), prostheses were fabricated and cemented with zinc phosphate cement. After cementation, the specimens were subjected to tensile loading at a cross head speed of 4 mm/min in a universal testing machine. The separation load was recorded and analyzed statistically using one-way analysis of variance followed by Duncan's multiple range test. RESULTS: Group V, the pin-retained RBFPDs, had the highest mean dislodgement resistance, whereas specimens of group II, the conventional RBFPDs, exhibited a significantly lower mean dislodgement resistance compared to the other 4 groups (P < .05). There were no significant differences between group I, III, and IV in terms of dislodgement resistance (P > .05). Group V had the highest mean MPa (N/mm2) (P < .05). There was no significant difference between groups I, II, III and IV (P > .05). CONCLUSION: Within the limits of the design of this in vitro study, it was concluded that: 1. The modified RBFPDs which utilizes the original tooth undercuts and requires no tooth preparation, compared with the conventional design of RBFPDs, has significantly high dislodgement resistance (P < .05). 2. The modified RBFPDs which utilizes the original tooth undercuts and requires minimal tooth preparation, compared with the conventional FPDs, has significantly no difference in retention and dislodgement resistance)(P > .05). 3. The pin-retained FPDs showed a high dislodgement resistance compared to the conventional three-unit FPDs (P < .05).

Erythema multiforme(EM) is related disorders of skin and mucous members which is typically associated with antecedent medications or infections, etc. EM is an erythematous maculopapular cutaneous eruption of variable form. However, the pathophysiology of the EM remains obscure. Treatment at present is symptomatic and supportive. In conclusion, the authors report two cases of EM that complained of the erythematous maculopapular cutaneous eruption following antibiotics treatment with the review of literatures.

We have presented a case of juvenile pilocytic astrocytoma of the third ventricle with the features of the diencephalic syndrome. A 18 month old female child developed progressively lethargy, emaciation, pallor, tremor, right hemiparesis, nystagmus and marked hydrocephalus with symptoms of increased intracranial pressure. CT scan showed large decreased density in the suprasellar area with extension to the left middle fossa and moderate right parasellar extension with irregular contrast enhancement, and dilatation of both lateral ventricles. The growth hormone increased very severely(80ng%). Biopsy was done through the left temporal lobe. Literatures were also reviewed.

Background: Esmolol is rapid hydrolyzed by plasma esterase but may inhibit plasma cholinesterase activity based on its structure. This study was designed to evaluate the interactions between esmolol and succinylcholine or mivacurium which are metabolized by plasma cholinesterase and to determine the inhibitory effect of esmolol on human plasma cholinesterase. Methods: Neuromuscular effects of succinylcholine (1.0 mg/kg) and mivacurium (0.15 mg/kg) with or without esmolol (0.5 mg/kg or 1.0 mg/kg) were compared in 57 adult patients (ASA class I) during O2-N2O-isoflurane anesthesia. Neuromuscular block was monitored by recording the compound electromyogram of the hypothenar muscle resulting from supramaximal train of four stimuli applied to the ulnar nerve. Also plasma cholinesterase activity was measured before and 5, 10 minutes after injection of esmolol. Results: Time from injection to onset of over 95% block, clinical duration from injection to 25% recovery of control twitch, and recovery index defined as from 25% to 75% twitch recovery of succinylcholine or mivacurium were not altered by pretreatment of esmolol. Plasma cholinesterase activity was not decreased after injection of esmolol 0.5 mg/kg, but decreased by 5% after injection of 1.0 mg/kg (p<0.05). Conclusions: It is unlikely that neuromuscular blocking effects of succinylcholine and mivacurium are prolonged by administration of clinical doses of esmolol (0.5~1.0 mg/kg) due to inhibition of plasma cholinesterase activity in human.

PURPOSE: This study was conducted to find out the effects of hospice care by evaluating the spiritual well-being and quality of life in the hospice and nonhospice patients. METHOD: The research design was composed of descriptive study. The data were collected using the questionnaire with interview from 30 hospice patients at three hospice institutes and 30 nonhospice patients at two general hospitals. The tools used for this study were 14-item questionnaire regarding general characteristics, a revised Spiritual Well-being Survey(Paloutzion and Ellision, 1982) and 22-item of revised Mcgill Quality of Life questionnaire. RESULT: The spiritual well-being of the hospice patients was higher than that of nonhospice patients(F=5.52, p=0.023). The global quality of life of the hospice patients was higher than that of nonhospice patients(F=8.84, p=0.004). There was a significant positive correlation between spiritual well-being and quality of life of the hospice patients and non hospice patients. CONCLUSION: The hospice care effects on spiritual well-being and quality of life of the terminal cancer patients.
