3.Comparison of Lactobacillus casei in Stool between Children with Atopic Dermatitis and Normal Controls.

PURPOSE: As allergy diseases including atopic dermatitis are increasing lately, many kinds of studies about the cause and the pathogenesis are in progress and the secrets of the of organization intestinal microflora and the relationship with inflammatory are reaction actively being revealed. METHODS: In this study, the subjects included 32 atopic dermatitis patients and 23 normal controls who visited the allergy clinics of the pediatrics department in Soonchunhyang University Hospital from June to October, 2003. Lactobacillus casei were cultured and counted. And serum total IgE, IgA and AST/ALT were examined. Also, the differences in lactobacillus counts between the atopic dermatitis patients and normal controls were analyzed. RESULTS: We were not able to find out a statistically significant differences between the atopic dermatitis patients and the control group. We were able to find out a statistically significant inverse correlation of serum total IgE and the number of Lactobacillus casei. CONCLUSION: The significant inverse correlations of serum total IgE and the number of Lactobacillus casei are expected to contribute to precognition and treatment of atopic dermatitis. It is hoped that more researches on the relationship between atopic dermatitis and intestinal flora should be carried out in the future.

4.A Case Report of Giant Posterior Inferior Cerebellar Artery Aneurysm Simulating a Posterior Fossa Tumor.

A 24-year-old female patient presented with a midline extra-axial posterior fossa lesion. We failed to suspect this lesion could be a giant thrombosed aneurysm of PICA(posteior inferior cerebellar artery) preoperatively, and didn't perform preoperative angiogram even after rather typical finding of thrombosed giant aneurysm on MRI. We suggest a giant thrombosed aneurysm should be included in the differantial diagnosis of any round extra-axial mass lesion in the skull base.

5.Clinical Analysis of Sellar Tumors Treated via Transsphenoidal Route.

Between January 1990 and August 1992 sixteen patients with sellar tumors underwent 16 transsphenoidal approaches at Kang-Nam Sacred Heart Hospital. Pathological diagnosis include 14 pituitary adenomas, of which six belonged to microadenomas and the remaining eight cases were classified as macroadenomas. Other pathologies were one craniopharyngioma and one sphenoid mucocele. Thirteen patients underwent TSA as the primary procedures and the remaining three patients were subjected to TSA as the secondary procedures to the primary treanscranial approaches. Of the 13 primary TSAs excellent or good results were achieved in 11 patients and the remaining two patients were subjected to the secondary transcranial approaches to achieve good results. Six patients with microadenomas achieved gross total or subtotal removal of their tumors in all cases(100%) however, satisfactory removal was achieved only in six macroadenoma cases(75%). There was neither surgical mortality nor major morbidity. Transient diabetes insipidus complicated in 6 patients(37.5%) and CSF rhinorrhea complicated in three cases*18.8%), which needed spinal drainage for three to ten days. Meningitis was complicated in one patient with CSF rhinorrhea(6.2%). This preliminary study agrees with that TSA is a safe and effective procedure even in macroadenoma cases.

8.The Effects of Kinesiotaping Applied onto Erector Spinae and Sacroiliac Joint on Lumbar Flexibility.

PURPOSE: The purpose of this study was to investigate the effects of kinesio taping on lumbar flexibility onto erector spinae and sacroiliac joint. METHODS: Sixty healthy adults (male=36, female=24) participated in this study and were randomly assigned to the experimental group that received kinesio taping onto erector spinae and sacroiliac joint (n=30) or the control group that received X-letter placebo taping onto them (n=30). Lumbar flexibility (flexion, extension, lateral flexion, and rotation) was measured using back range-of-motion instrument (BROM) II before and after taping. RESULTS: In the change of lumbar flexibility after taping in the experimental group, there were statistically significant difference in flexion, lateral flexion, and rotation (p < 0.05), but there was no significant difference in extension. There was no significant difference in the change of lumbar flexibility after taping in the control group. CONCLUSION: In conclusion, kinesio taping onto erector spinae and sacroiliac joint improved the joint function. Kinesio taping may reduce the muscle tension and facilitate the circulation of tissue fluid. In light of these results, it is thought that the application of kinesio taping had influence on an increase in lumbar flexibility. Therefore, kinesio taping will be able to be used as the method of the prevention of pain and the treatment in the lumbar region.

9.Spinal Enumeration by Morphologic Analysis of Spinal Variants: Comparison to Counting in a Cranial-To-Caudal Manner.

OBJECTIVE: To compare the spinal enumeration methods that establish the first lumbar vertebra in patients with spinal variants. MATERIALS AND METHODS: Of the 1446 consecutive patients who had undergone computed tomography of the spine from March 2012 to July 2016, 100 patients (62 men, 38 women; mean age, 47.9 years; age range, 19–88 years) with spinal variants were included. Two radiologists (readers 1 and 2) established the first lumbar vertebra through morphologic analysis of the thoracolumbar junction, and labeled the vertebra by counting in a cranial-to-caudal manner. Inter-observer agreement was established. Additionally, reader 1 detected the 20th vertebra under the assumption that there are 12 thoracic vertebra, and then classified it as a thoracic vertebra, lumbar vertebra, or thoracolumbar transitional vertebra (TLTV), on the basis of morphologic analysis. RESULTS: The first lumbar vertebra, as established by morphologic analysis, was labeled by each reader as the 21st segment in 65.0% of the patients, as the 20th segment in 31.0%, and as the 19th segment in 4.0%. Inter-observer agreement between the two readers in determining the first lumbar vertebra, based on morphologic analysis, was nearly perfect (κ value: 1.00). The 20th vertebra was morphologically classified as a TLTV in 60.0% of the patients, as the first lumbar segment in 31.0%, as the second lumbar segment in 4.0%, and as a thoracic segment in 5.0%. CONCLUSION: The establishment of the first lumbar vertebra using morphologic characteristics of the thoracolumbar junction in patients with spinal variants was consistent with the morphologic traits of vertebral segmentation.

10.A Case of Left Ventricular Outflow Obstruction Caused by Mitral Valve Replacement.

The pathophysiology, clinical presentation and prognosis of left ventricular obstruction present an important cardiological problem. Various anatomical and functional abnormality can cause this phenomenon. Rarely, left ventricular outflow obstruction can result after mitral valve surgery. We experienced a case of left ventricular outflow obdtruction 13 years after mitral valve replavement. The diagnosis was made using two-dimensinal Doppler echocardiography and confirmed by cardiac catheterization. The pressure gradient across the left ventricular outflow obstruction was 96mmHg. A second mitral valve replacement was performed. Because severe fibrosis, pannis around the prosthetic mitral valve and a subaortic web were detected during the operation, the subaortic web was removes.
