1.The Treatment of Bilateral Congenital Dislocation of the Hip

There are few reports in the literature on how to care and manage bi!ateral congenital dislocation of the hip. Six patients have been encountered at this hospital from June 1979, of them four children had operative treat ment and the other two conservative. As a result of our experience about the management of bilateral congenital dislocation of the hip. The authors obtained the following conceptions; 1. The patient with bilateral dislocation was rather delayed in diagnosis. Therefore, in comp the treatment of bilateral cases was poorer than that of unilateral case. 2. In the case of the dislocated hip that might be easily reduced and maintained in the stab servative care was usually recommeded even in the older child. Unstable, unreduced hip,; hip in old children, the more definitive operation and internal & external rigid immobilization mandatory to get good result. 3. It might be beneficial to operate the more deformed and resistive hip first, and then to p with the same procedure 2 weeks after the first operation.

3.Bifurcated Stent-Graft(Vanguard) for the Endovascular Treatment of Abdominal Aortic Aneurysm.

PURPOSE: The purpose of this study was to evaluate the safety, feasibility and effectiveness of an endoluminally-placed bifurcated stent-graft (Vanguard) for the treatment of infrarenal abdominal aortic aneurysm (AAA). METHODS: Transluminal endovascular stent-graft placements were attempted in 29 patients (28 male, mean age 69+/-7 years) with AAAs involving the common iliac arteries from Aug. 1997 to Jan. 1999. Endovascular therapy was performed in the cardiac catheterization laboratory with epidural anesthesia. One side of the femoral artery was opened by surgical cutdown for the bifurcated stent-graft entry and the other side was punctured percutaneously for the straight stent-graft. Computed tomography and/or intraarterial angiography were performed during an average follow-up of 10 months (2-18 months). RESULTS: Primary success rate was 75.9% (22 of 29 patients) and the overall success rate was 79.3% with successful correction of one perigraft leak. Twenty patients (69.0%) had significant coronary artery disease. There were two technical failure cases, the one was tortuous iliac vessel with spasm, the other was disconnection of the stent-graft connecting portion. Complications related to procedure occurred in 13.8% of patients (4 of 29 patients) and two of these four patients had procedure-related mortality because of acute renal failure following contrast overdose and sepsis after operation. CONCLUSIONS: Endovascular treatment of infrarenal AAA with bifurcated stent-graft (Vanguard<0A397>) is effective, feasible and relatively safe. However, further investigation for the outcome, complication and long-term follow-up are needed.

5.A clinical analysis of the marjolin's ulcer.

Marjolin's ulcer is a malignant degeneration of cicatrical tissue, especially in chronic burn scar. Since the first description by Jean Nicholas Marjolin(1828), many studies for Marjolin's ulcer have been performed and reported. From 1985 to 1997, we experienced 20 cases in 14 patients diagnosed as Marjolin's ulcer clinically, and we analysed malignant transformation rate, induction period of Marjolin's ulcer and prevention of Marjolin's ulcer, etc. The results are summarized as follows. 1) The sex distribution was predominant in male(64.3%). 2) The most common cause of injury was flame burn(78.6%). 3) The mean age at the time of diagnosis was 45.6 years and the average lag period was 20.6 years. 4) The pathologic types of Marjolin's ulcer were squamous cell carcinoma(57.2%), followed by dyspasia(28.6%), acnthosis and hyperkeratosis(14.2%). 5) The locations of ulcers were frequently on the lower extremity(42.9%), followed by upper extremity(35.7%) and trunk (21.4%). 6) The lymph node involvements were 3 patients out of the 8 patients who were confirmed as squamous cell carcinoma. 7) The modalities of treatment were skin graft (18 cases, 12 patients), myocutaneous flap(1 case, 1 patient), fasciocutaneous flap(1 case, 1 patient).

7.Bilateral temporo-mandibular joint dislocation due to complication of oro-facial dyskinesia

Oro-facial dyskinesia (OFD) is involuntary, abnormal, uncontrolled and stereotyped movements,
consisting of forehead furrowing, eye opening and closing, smacking and pursing of the lips, lateral
deviation and protrusion of the tongue, and occasionally lateral deviation and protrusion of the jaw.1
OFD is known to have various complications including speech difficulty, chewing and eating disorders,
and social embarrassment; facial muscle stiffness, mucosal and gingival traumatic lesions. In addition,
it may leads to cranio-mandibular joint (TMJ) complications in the presence of intense and prolonged
abnormal movements, with pain and degeneration.1,2 There is no previous report of TMJ dislocation
due to OFD. In this report, we describe a patient who developed bilateral anterior TMJ dislocation
due to OFD which occurred following intra-cranial hemorrhage (ICH).

8.The Treatment of Femoral Head Fractures Associated with Posterior Hip Dislocations

Femoral Head fracture associated with posterior dislocation of the hip was once considered a rare industrial injury. However, the advant of high-speed travel has contributed to its increased incidence. Ten cases of fractured femoral head with posterior dislocation of the hip were treated at the Busan Paik Hospital between June 1979 and June 1982. During the same period, a total of 38 posterior hip dislocations were treated. The patients were followed for the average of 19.3 months, the range being 6 months to 3 years. There were 9 men and 1 woman the age ranged from 25 to 56 years. The fractures were classified according to Pipkin and the results were evaluated by Epsteins criteria. 1. The incidence of femoral head fracture was 26.3%, 10 cases of 38 posterior hip dislocations. 2. The better result was noted in open method than in closed method. 3. The absolute surgical indications were considered as followings. a. inadequate closed reduction of head fragment especially the large one. b. the involvement of weight bearing portion of femoral head. c. the hip joint pain caused by impingement of small fragment between joint space. 4. Arthrogram was helpful to find the concealed small fragment when the hip joint pain persisted with negative findings in ordinary x-ray.

9.Utilization of Emergency Department by Children in Korea.

This study was conducted to examine the nation-wide emergency department (ED) utilization pattern by children in Korea. Most referral hospital EDs provide their essential ED information to the National Emergency Medical Center through the National Emergency Department Information System (NEDIS). We analyzed the NEDIS data on pediatric visits (< 19 yr old) during three years, from June 1, 2008 to May 31, 2010. A total of 2,072,664 children visited 124 EDs during the study period, and this patient population comprised 31.2% of the total ED visits. Male patients with a mean age of 5.44 (+/- 5.40 SD) yr comprised 59.1% of the ED pediatric visits. Among all age groups, the 1-4-yr-old group was the largest (42.3%). The most common mode of arrival was non-emergency medical service (EMS) (90.7%), and only 4.2% of patients used EMS transport. Common chief complaints in infants were fever (37.4%), whereas many older children presented to the EDs with abdominal pain (15.4%). The ratio of disease versus injury as the cause of ED visits was 2.5:1. Most patients were discharged (81.2%), and 15.3% were admitted. In conclusion, the most common age group who are brought to EDs in Korea is 1 to 4 yr-old young children, and common chief complaints of the visiting children are age-dependent, such as fever in younger children and abdominal pain and headache in older children. We need more improved and organized emergency medicine service system for children in Korea.

Factors such as senescence,stress and neurodegenerative diseases,including Alzheimer's disease, contribute to the impairment of organs,especially the brain.They also negatively affect normal brain functions such as memory and cognition.In this study,the neuroprotective role of the natural product BF-7 was examined against A beta - induced neurotoxicity in SK-N-SH human neuronal cells.BF-7 significantly attenuated A beta-induced apoptosis as measured by intracellular calcium levels,accumulation of reactive oxygen species,mitochondrial dysfunction,and caspase activity.These results strongly indicate that BF-7 plays an effective and positive role in the improvement of brain functions,including learning and memory,in our model system for Alzheimer's disease.Thus,BF-7 might be useful for developing strategies to protect the nervous system and improve brain function.
