3.Percutaneous Transhepatic Venous Embolization of Pulmonary Artery Aneurysm in Hughes - Stovin Syndrome.

Hughes-Stovin syndrome is an extremely rare entity. We present a case of a 42-year-old man, who developed deep vein and inferior vena cava (IVC) thrombosis, repeated internal bleeding and pulmonary artery aneurysms (PAAs). The patient presented with massive hemoptysis and with PAAs of a 2.5 cm maximum diameter. We describe the successful percutaneous transhepatic venous embolization of the PAAs due to occluded common vascular pathways to the pulmonary artery.

Venous thrombosis is uncommon in young individuals. Hypereosinophilia is a rare cause of thrombosis that usually involves the heart and small vessels like retinal vessels. We report a case of massive deep vein thrombosis that developed in a young patient with hypereosinophilia who was successfully treated with continuous local infusion of urokinase.

5.The Depressive Effects of Osteoarthritis Symptoms in the Knee Joint Patients.

BACKGROUND: Osteoarthritis(OA) of the knee represents a common chronic disease, especially in the elderly, leading to functional impairment and disability. And OA patients have depressive symptoms frequently. Several factors contribute to the level of depressive symptoms. Authors tried to assess these factors and show effect of these factors to depressive symptoms. Through this study in the future physicians must consider psychiatric problems besides the pain itself in the treatment of OA patients. METHODS: Subjects included in this study were 64 knee joint OA patients. We obtained depressive symptoms from the CES-D(The Korean version of the Center for Epidemiological Studies-Depression Scale), level of pain, stiffness, functional disability from KWOMAC(WOMAC Korean Version 2, The Korean version of Western Ontario and Macmaster Osteoarthritis Index Score) and other factors able to contribute to depressive symptoms. RESULT: We divided the patients into two groups using CES-D cut-off score of 21. Each group consists of 46(CES-D<21) and 18(CES-D> or =21) patients. Frequency of sleep disturbance and pain consistency and level of pain, stiffness and functional disability were different significantly between two groups(p<0.0l). Through the correlation analysis level of pain, stiffness and functional disability were associated with CES-D score significantly(p<0.0l). And through the stepwise multiple regression analysis we demonstratred that the CES-D score influenced by level of functional disability, BMI, disease duration and, pain but only level of functional disability and BML were statistically significant(p<0.0l). Level of functional disability explained 46% of CES-D score(partial R2=0.46) so influenced on depressive symptoms most effectively. CONCLUSION: This study suggested that functional disability was most important factors to explain depressive symptoms of knee joint OA patients.

6.A Case of Hypersensitivity Pneumonitis by Alternaria as a Suspected Etiology.

A 65-year-old man was admitted due to low grade fever, dry coughing, and dyspnea on exertion. The chest radiograph and CT scan showed diffuse ground glass opacities and small nodules in the both lung fields resulting in a diagnosis of severe interstitial pneumonia. Conservative treatment with antibiotics and bronchodilators decreased the symptoms, but the dyspnea and cough reappeared when he returned home. An inspection of his house revealed the presence of fungi under the wallpaper. His symptoms disappeared completely after these were removed. His clinical course raised the suspicion of hypersensitivity pneumonitis and these fungi believed to be the cause of hypersensitivity pneumonitis. The histological findings of a lung specimen by video-assisted thorachoscopy were compatible with hypersensitivity pneumonitis. The fungi were identified as Alternaria.

7.A Case of Complete Resolution of Aortic Dissection in the Descending Thoracic Aorta Treated with Endovascular Stent-Graft Implantation.

The aortic dissection is an acute aortic syndrome, caused by an intimal tear and subsequent splitting of the media by the pulsatile blood flow. Though there would be differences in the origin of aortic dissection and therapeutic modalities, the intermediate and long-term prognoses are poor. Endovascular Stent-graft implantation is a revolutionary technique in the treatment of aortic dissection. The endovascular stent grafting in aortic dissection is less invasive and feasible method and is an effective tool for closing the entry site and promoting clot formation, reducing the size of the false lumen. Therefore, endovascular Stent-graft implantation makes possible the desirable remodelling of aorta. We report 33 year-old male with aortic dissection in the thoracic aorta, which was treated with endovascular Stent-graft implantation. Over the favorable remodelling, his dissection was healed completely by the endovascular treatment using Stent-graft.

8.Brachial Artery Approach for Outpatient Arteriography.

PURPOSE: To evaluate the diagnostic usefulness of brachial approach arteriography for outpatients, withparticular regard to safety and image quality. MATERIALS AND METHODS: The angiographic findings and follow-upmedical records of 131 brachial approach arteriographies in 121 outpatients were retrospectively analysed. 5 Fpigtail catheters were used in 125 cases, and 5-F OCU-A catheters were used in three cases of renal arteriography,and three of upper extremity arteriography without catheter. RESULTS: Except for three cases of brachial arterypuncture failure, all procedures were performed successfully. One hundred and fifteen of 119 lower extremityarteriographies were visualized down to the level of the tibioperoneal artery. The non-visualized cases were threein which there was multiple obstruction at the distal common iliac artery and one with insufficient contrastamount due to renal failure. In four cases there were complications : two involved arterial thrombosis, one was anintramuscular hematoma, and one an A-V fistula. CONCLUSION: For outpatients, brachial approach arteriography canreplace the femoral approach. Its image quality is excellent, there are time-cost benefits, and the rate ofcomplications is relatively low.

9.A STUDY ON HISTOMORPHOMETRICAL CHANGES OF TRABECULA AND EXPRESSION OF OSTEOPONTIN mRNA IN OVARIECTOMIZED RAT FEMUR.

Osteoporosis is a disease characterized by low bone mass and micro- architectural deterioration of bone tissue, leading to enhanced bone fragility and a consequent increase in fracture risk. Women are the most likely to suffer osteoporotic fractures. In the second part of life bone mass decreases slowly, starting at about the age of 40 for cortical bone and somewhat earlier for trabecular bone. The total loss will be about 50% of the cancellous bone and 35% of the cortical bone in women. The imbalance between the bone resorption and bone formation can be caused by a decrease in the amount of new bone formed. The biochemical causes of the negative balance are not known. Therefore we studied to know the cause of negative balance after postmenopausal state by histomorphometry and in situ hybridization(ISH) for osteopontin. 84 female Spraque-Dawley rats, weighing approximately 200g, and fed laboratory chow ad libitum, were used in this study. Among those, 42 rats were sham operated, and 42 rats in experimental group were ovariectomized. They were divided into 7 groups consisting of 6 rats/group, and were serially sacrified on the 3, 5, 7, 10, 14, 30, and 60 days after operation and we obtained both femur of rats. After specimen preparation as usual manner, it was stained with H&E, MT. For histomorphometrical study, the bone structure was evaluated using the following parameters with OPTIMAS soft ware: trabecular bone volume/tissue volume(BV/TV, %), trabecular thickness(Tb.Th., micrometer), trabecular separation(Tb.Sp., micrometer). And in the way of ISH(in situ hybridization) procedure, osteopontin mRNA expressing cell was calculated. Statistical differences between OVX and sham-operated rats were evaluated using the ANOVA. After examination and comparision of all specimens, the results of this study were as follows: 1. BV/TV(%) after ovariectomy was decreased to 46.2+/-6.75%, compared with 58.14+/-8.72% of control group, with no statistical significance. These declination continued to 14 days postsurgery, in 30 and 60 days, it was decresed to 14.01+/-4.07% and 11.4+/-4.73% respectively, compared with 47.2+/-8.79% and 48.8+/-7.54% of control group, with statistical significance detected. 2. Trabecular thickness(micrometer) was decresed from 3 days to the remaining periods, with no statistical significance. 3. Trabecular separation(micrometer) was incresed throughout the study, especially in 60 days postsurgery, control group was 124.2+/-7.34micrometer and experimental group was 319.5+/-15.85micrometer, with statistical significance detected at 60 days postsurgery (p<0.05). 4. Osteopontin mRNA expressing cells was decreased from 3 days postsurgery, compared with 92.3+/-5.25 of control group. And these declination was observed throughout the study. Especially in 60 days postsurgery, control group was 93.25+/-2.72 and experimental group was decresed to 24.3+/-6.24, with statistical significance detected(p<0.05). From above results, after ovariectomy, bone formation was decreased, and it is thought that these bone formation declination was due to supression of osteopontin synthesis, which is the one of the bone matrix proteins.

10.Congenital Chylothorax.

Chyle is the digestive product absorbed from intestinal lymphatics which the component have high concentration of triglyceride and protein. The various pathological circumstances which give rise to intrathoracic effusion of chyle are well documented and so the etiology is clearly defined in most instances. However, the occurrence of chylothorax in early infancy, in the absence of other demonstrable diseases and without apparent birth trauma, suggests the existence of congenital malformation of the thoracic ducts. We lately experienced a 20 day old new born who was proved to have congenital chylothorax by clinical history and laboratory findings. So we report the clinical findings and laboratory findings with the review of the literatures on congenital chylothorax.
