3.Chronic Childhood Idiopathic Thrombocytopenic Purpura: Long-term Follow-up.

PURPOSE: The purpose of this study was to evaluate the spontaneous remission rate in children with chronic ITP. METHODS: The medical records of 21 patients diagnosed with chronic ITP and followed for at least 6 months at Hanyang University Hospital between Mar. 1985 and Feb. 1997 were reviewed. Complete remission(CR) was defined as a recovery of platelet count of more than 100x109/L and partial remission(PR) was defined as a recovery of platelet count of 50-100x109/L, maintained for at least 6 months. RESULTS: Of 21 patients, 9 boys and 12 girls, the mean age at diagnosis was 5.5 years (0.4~15.5 years). The mean platelet count at diagnosis was 26.5x109/L(2-86x109/L). Of 21 patients, 5 with persistent thrombocytopenia and severe bleeding tendency underwent splenectomy. With a median follow up of 39.5 months after splenectomy, 4 achieved CR. The time to CR of the 4 patients were 2 weeks, 13.6, 18.6, 31.0 months, respectively. Of 16 patients, excluding 5 splenectomized children, 6 achieved CR and 4 PR. With a median follow-up of 21 months, the predicted spontaneous CR and PR rate at 5 years were 37.5%, and 68.7%, respectively(The percentage of PR includes that for CR). No serious complications including ICH or death were observed. CONCLUSION: Our study indicates that many cases of chronic ITP in childhood may remit without therapy even several years from onset. Splenectomy should be delayed as long as possible and reserved for the symptomatic and severely thrombocytopenic patient.

4.Computerized management of radiology department: Installation and use of local area network(LAN) by personal computers.

There is increasing need for network connecting personal computers(PC) together. Thus Local Area Network(LAN) emerged, which was designed to allow multiple computers to access and share multiple files and programs and expensive peripheral devices and to communicate with each user. We built PC-LAN in our department that consisted of 1) hardware-9 sets of personal computers(IBM compatible 80386 DX, 1 set:80286 AT, 8sets) and cables and network interface cards (Ethernet compatible, 16bits) that connected PC and peripheral devices 2) software - network operating system and database management system. We managed this network for 6 months. The benefits of PC-LAN were 1) multiuser (share multiple files and programs, peripheral devices) 2) real time data processing 3) excellent expandibility and flexibility, compatibility, easy connectivity 4) single cable for networking ) rapid data transmission 6) simple and easy installation and management 7) using conventional PC's software running under DOS(Disk Operating System) without transformation 8) low networking cost. In conclusion, PC-LAN provides an easier and more effective way to manage multiuser database system needed at hospital departments instead of more expensive and complex network of minicomputer or mainframe.

9.A Clinical Study of the Fracture

The talus is a bone with unique biomechanical features and vascular supply. Although fractures of the talus are not common, the complications of the displaced fractures or dislocation are frequent and resulting disabilities are so severe that the importance of proper management is emphasized. Authors analyzed 17 cases of fracture-dislocation of the talus, treated at Soon Chun Hyang University Hospital from January 1983 to December 1988. The results were as follows:l. Among the 17 cases, there were 15 males and 2 females and the average age was 28 years old, ranged from 21 years to 58 years. 2. The causes were fall from height in 10 cases, traffic accident in 6 and others in one. 3. According to Marti-Weber classification, 2 cases were type I, 5 in type II, 7 in type III and 3 type IV. 4. Among the 17 cases, 9 cases in type I and II were treated conservatively, and 8 cases in thpe III and IV were treated operatively. 5. Complications were avascular necrosis in 3 cases, degenerative arthritis in 4 and nonunion in 1. 6. Final results, evaluated by the criteria of Mindell et al., were as follows: excellent in 7 cases, good in 5, fair in 3 and poor in 2.

10.Eosinophilic Cystitis.

Eosinophilic cystitis is a rare inflammatory disease of urinary bladder with unknown etiology. The disease is known to be associated with several different processes such as allergy or injury. Eosinophilic cystitis is similar to interstitial cystitis, vesical tuberculosis and bladder neoplasm in clinical manifestations and should be differentiated from them. A 75 years old male patient underwent TUR-P for benign prostatic hyperplasia. After 2 months, he complained of irritative voiding symptoms. Cystoscopy revealed a polypoid mass, measuring 3 cm in diameter in the right lateral wall of bladder. Pathology of the lesion that was transurethrally resected revealed the diffuse eosinophilic infiltration of the bladder wall without evidence of malignancy.
