1.The Effect of Fractures on the Longitudinal Bony Growth of a Growing Long Bone in Raddits

Since Duhamel (1739), numerous experimental studies on the longitudinal growth of growing long bone have been reported. In Hales(1747) experience, growing long bone showed overgrowth after drilling in the tibia of chickens. David(1924) observed the fact that various degrees of shortening disappeared within 15 months after fracture of the femur in children and he concluded that fractures in children should be treated with the caution that the growing long bone has a tendency to increase in length after injury. Bisgard(1936) concluded that shortening from overriding of fragments in fractured extremities of children will frequently, but not invariablly, become partially or totally eliminated by the acceleration of growth which incidentaIly results from the inflammatory process, incited by trauma and fracture repair. Hass(1926) reported that interstitial proliferation of osseous tissue plays no part in the elongation of a growing bone, either in the mature bone or the young osteoid tissue bordering on the epiphyseal cartilage plate, and length growth of bone is entirely dependant on the purposeful multiplication of cartilage cells of the epiphyseal cartilage plate. Ham(1952) reported that the growth of long bone depends upon the interstitial growth of cartilage. This study was initiated to investigate the effects of fractures in the longitudinal bone growth of growing bone. Artificial fractures were made at two different sites in the diaphysis of the tibia in young rabbits. The operation for artificial fracture was performed under general anesthesia with ether. The type of fracture was an incomplete one, leaving a part of the posterior cortex of the tibia, in order to prevent the displacement of fractured fragments. In all of the cases, no cast immobilization was applied. After the fractures, follow up observations were made weekly till the 4th week, and thereafter at two week intervals, with roentgenographic, lethal measurement and histological examination. The results were as follows; 1) Longitudinal bone growth was increased from fracture of the diaphysis of growing long bone. 2) Compared with the control limb, tendency to overgrowth of growing bone was noted from the first week after the operation. 3) In follow up measurements, no growth inhibition was observed till the 10th week after the operation, but, thereafter, a tendency to reduce the proliferation of cartilage cells was noted. 4) In cases of fracture of the mid 1/3, the gorwing activity in the epiphyseal plate was increased more than from proximal 1/3 fractures. 5) The bony growth, accelerated by fractures in diaphysis of growing long bone without displacement was transient, and the prolonged overgrowth was compensated by the reduction of growing activity and premature closure of the epiphysis.

2.Compliance of Nasal Continuous Positive Airway Pressure in Patients with Obstructive Sleep Apnea Syndrome.

OBJECTIVES: Nasal continuous positive airway pressure (nCPAP) is the treatment of choice and has been shown to reduce the frequency of nocturnal respiratory events, improve sleep architecture, and decrease daytime sleepiness in patients with obstructive sleep apnea syndrome (OSAS). However, little is known about the compliance of nCPAP treatment in Korea. Our objective was to look into the nCPAP compliance and examine the factors influencing it. METHODS: We reviewed the records of one hundred and twenty consecutive patients with OSAS referred for nocturnal polysomnography with nCPAP pressure titration during the period of January 1995 through April 1999 to the Seoul National University Hospital. We performed a telephone interview and obtained data from eighty-three patients. RESULTS: In sixty patients who had accepted nCPAP treatment, twenty-six patients (43.3%) were still using nCPAP device, while thirty-four patients (56.7%) stopped using it. Fifteen patients (25%) were using nCPAP device everyday. In thirty-four patients who discontinued nCPAP use, twenty-five patients (73.5%) did within the first three months, and thirty-one (91.2%) within the first year. Significant predictor of long-term nCPAP use was the presence of subjective daytime sleepiness before nCPAP application. CONCLUSIONS: Long-term compliance with nCPAP treatment appears to be associated with the presence of subjective daytime sleepiness before nCPAP application. Long-term compliance with nCPAP may be mostly predicted from the usage pattern within the first three months of use.

3.Two Cases of Hyperkeratosis of the Nipple and Areola.

We report two cases of hyperkeratosis of the nipple and areola which occurred in 22-year-old woman and 13-year-old girl. The former was suffered from mild pruritic, verrucous thickening and brownish discoloration of both nipples and areolae for 5 years. The latter had asymptomatic, verrucous thickening and dark brownish hyper- pigmentation of the right nipple and areola for 3 years. Both showed mild to marked hyperkeratosis and papillomatosis with increased pigmentation in the basal cell layer histopathologically. In both cases, the lesions were not responsive to the topical medications including steroids and keratolytic agents. Therefore we recommended surgical excision because the patients are greatly disturbed by the appearance of the lesions.

5.Clinical values of CT and dynamic CT in brain infarction

With the advent of faster scan time and new computer program, a scanning technique called “dynamic computedtomography” has become possible. Dynamic CT consists of performing multiple rappid sequence scans after injectionof contrast material. The authors have evaluated the clinical usefulness of CT and dynamic CT of 93 patients withbrain infarction and/or ischemia during the period of 17 months from April 1983 to Aug. 1983 to Aug. 1984 inDepartment of Radiology, Chonbuk National University Hospital. The results were as follows; 1. The agedistribution ranged from 18 years to 78 years. Among them the most common age group was between 50 years and 59years(40.9%). 2. The sites of brain infarction were cerebral lobes(63 cases,68), basal ganglia(15 cases, 16.1%)and mlultiple sites(6 cases, 6.4%). The common affected site was middle cerebral artery territories. 3. Thecontrast enhancement of acute infarction was noted in 14 cases(17.5%) which occured commonly between 3 days and 2weeks from ictus. 4. The patterns of time-density curve in brain infarction and/or ischemia were as follow: a .Depression of slow wash-in phase was 20 cases(59%). b. Lower peak concentration was 17 cases(50%), c. Lower anddelayed peak concentration was 7 cases(21%), d. No definite peak concentration was 6 cases(18%). First threepatterns of time-density curve were thought as relatively characteristic curve of brain infarction and/orischemia. 5. Two cases that showed negative findings on precontrast CT scan appeared to be positive findings ashypodensity on postcontrast CT scan and were confirmed as brain infarction by dynamic CT. 6. The diagnostic entityof dynamic CT scan were as follows: a. large artery thrombotic infarction were 23 cases (58%). b. lacunarinfarction were 6 cases (15%). c. ischemia were 5 cases (13%), d. normal were 5 cases(13%), In six cases oflacunar infarction which was doubtful hypodensity on pre-and postcontrast CT scan had a marked difference in CT#(HU) on absolute scale graph of dynamic CT, so diagnosis of lacunar infarction could be made easily. 7. Theclinical values of dynamic CT consist in not only diagnosis of lacunar infarction but also evaluation ofeffectiveness of medical or surgical treatment.

6.The Effect of Self-efficacy and Depression on Sense of Family Coherence in Cancer Patients Undergoing Chemotherapy and Primary Caregivers in Day Care Wards: Using the Method Actor-partner Interdependence Model

PURPOSE: The objective of this study was to determine the effects of self-efficacy and depression on the sense of family coherence in both cancer patients and their caregivers using the Actor-Partner Interdependence Model (APIM) analysis.METHODS: A total of 274 patients were registered in the study, including 137 cancer patients who were undergoing chemotherapy in a day care ward and 137 primary caregivers. The data was collected from December 30, 2017 to July 30, 2018. Data were analyzed by the data analysis method using SPSS/WIN 20.0 (SPSS Korea Data Solution Inc) and AMOS 21.0 (SPSS Korea Data Solution Inc).RESULTS: Self-efficacy of cancer patients had a direct effect on patient depression (β=−.24, p=.008). Self-efficacy of primary caregivers also had a direct effect on patient depression (β=−.51, p < .001). Factors significantly affecting cancer patient FSOC were cancer patient self-efficacy (β=.34, p < .001) and cancer patient depression (β=−.42, p < .001). Factors significantly affecting primary caregiver FSOC (Family Sense of Coherence) were cancer patient self-efficacy (β=.13, p=.026) and caregiver depression (β=−.29, p=.008).CONCLUSION: It is thought that self-efficacy and depression should be controlled to improve the family cohesion of cancer patients and their primary caregivers.

7.Effects of a Thermoelectric ElementBandon Venipuncture-associatedPain and Anxiety: A RandomizedControlledTrial

Purpose:
Venipuncture is an invasive procedure for diagnosis and treatment, which is often attributed to pain and anxiety. In this study, a thermoelectric element (TEE) band was developed to apply heat therapy (40∼45°C), cold therapy (0∼10°C), or thermal grill illusion (TGI) therapy (40∼45°C, 0∼10°C) to cause an illusion of pain by simultaneously applying heat and cold. This band was subsequently used to investigate its effect on patient pain, anxiety, and satisfaction.
Methods:
This was a randomized controlled study. Participants, who were to undergo venipuncture, were randomly assigned to the heat therapy, cold therapy, TGI therapy, or control groups. Each group had 30 participants. The interventions were employed for 10 seconds during venipuncture, and the pain, anxiety, and satisfaction were measured before and after the procedure.
Results:
Subjective pain, anxiety, and physiological responses after TEE band intervention were not significantly different between the four groups. However, there was a significant difference in satisfaction (F = 4.21, p = .007) between the four groups, and the cold therapy group showed the highest satisfaction.
Conclusion
In this study, when heat, cold, and TGI therapy were applied with a TEE band, pain and anxiety relief effects were not confirmed, but satisfaction was high. TEE band is a newly developed product that can easily apply hot and cold treatments without using ice packs or hot water packs. Further studies with various individual characteristics of chronic pain or repeated venipuncture are warranted to evaluate the effect of TEE.
8.Effect of Erythromycin on Pro-inflammatory Signalings by Particles.

Purpose: In periprosthetic osteolysis, cytokines, which are secreted from macrophages by the stimulation of particles, up-regulate the signaling for osteoclast activation through RANKL (Receptor activator of Nuclear Factor Kappa-B Ligand). This study compared the reaction to the particles and RANKL in the macrophages by examining the changes in the pro-inflammatory signals. In addition, because erythromycin has an anti-inflammatory effect, the effect of erythromycin on the pro-inflammatory signals by particles and RANKL was also analyzed to clarify the mechanism for the anti-resorptive effect with particles. Materials and Methods: The Raw 264.7 cell line (murine macrophage cell line) was used for these experiments. The particles were made from PMMA (poly-methyl-meth-acrylate) and UHMWPE (ultra high molecular weight polyethylene) to enhance their stimulatory effects. Under the same culture conditions used for macrophages, the cells were treated with either particles or RANKL. The differences in the production of TNF-α, activities of MAP kinase, I-κB and reactive oxygen species (ROS) between the particle and RANKL treated macrophages were examined. The influence of erythromycin on these models was also observed. Results: Erythromycin inhibited ERK and p38 phosphorylation in both models, and suppressed the increase in H2O2 production in the particle-treated macrophages. However, erythromycin inhibited neither the production of TNF- in both models nor the production of H2O2 in the RANKL-treated macrophages. In addition, erythromycin reversed the suppression of I-κB by the particles. Conclusion: For the response of macrophages, erythromycin mainly suppresses the particle induced ROS and NF-κB activation compared with RANKL-induced osteoclastogenesis signaling. Erythromycin might suppress particle-induced osteolysis through these anti-inflammatory effects. Therefore, further studies on the downstream signals of osteoclastogenesis will be needed.

9.Congenital Lobar Emphysema.

We had experienced a case of congenital lobar emphysema in a 3 months old male infant. Chief symptoms included tachypenea, respiratory difficulty, cyanosis, Chest X-ray or chest CT scan revealed extensive emphysematous changes of the right upper and middle lobes, compression of the right lower lobe and shifted of mediastinum to the left side. This condition was appeared in the absence of infection and foreign body in the bronchus and its failure to respond to conservative treatment. This patient was treated by the right upper and right middle lobes pneumonectomy. A brief review of literature was made.
