1.The Effect on Inhibition of Raising Intraocular Pressure during Endotracheal Intubation in Man .

After various anesthetic inductions, the effects of tracheal intubation on mean arterial preasure(MAP) and intraocular pressure(IOP) were studied in 88 random patients who did not have any cardiovascular and ocular diseases properatively. IOP and MAP were measured with a Perkins hand held applanation-tonometer and a Cardi- 8- mini syhygmomanometer respectively. The results were as follows: 1) Both the MAP and IOP were increased in succinylcholine and pancuronium anesthetic induction groups after endotracheal intubation. 2) Rises in,both the MAP and IOP were inhibited in induced hypotensive anesthetic induction with halothane and trimethaphan. 3) The IOP was inhibited in the propranolol group. These results show the superiority of induced hypotensive anesthetic induction with halothane, trimethaphan and propranolol prior to administration for inhibition of raising intraocular pressure.

2.Gender Differences in the Effects of Psychosocial Health of Adolescents with Chronic Illness on Quality of Life

PURPOSE: Recently, considering the large number of adolescents suffering from chronic illness, it is important to identify the factors affecting the quality of life (QoL) in order to develop intervention programs. METHODS: Data were analyzed using IBM SPSS 21.0. The general characteristics, psychosocial health status, and life satisfaction distributions of the participants were analyzed using frequency, percentage, mean, and standard deviation. Cronbach's alpha coefficients were calculated to test the reliability of the instrument. The differences in psychosocial health status and life satisfaction according to gender were tested with t-test and Chi-square test. Additionally, multiple regression analysis was used to examine the effect of psychosocial health status on life satisfaction. RESULTS: The multiple regression analysis revealed that depression, ego resilience, and self-esteem were associated with the QoL in both male and female participants. Economic status, subjective health, peer attachment, and smart phone addiction were associated with the QoL in male participants, while exercise time during physical education classes, social withdrawal, and school adaptation were associated with the QoL in female participants. CONCLUSION: The findings suggest that a health promotion program should be developed, which reflects gender differences, to improve the QoL of adolescents with chronic illness.

3.Autosomal Translocation Patient Who Experienced Premature Menopause: A Case Report.

Premature ovarian failure (POF) is a condition in which the ovarian functions of hormone production and oocyte development become impaired before the typical age for menopause. POF and early menopause are present in a broad spectrum of gonad dysgenesis, from a complete cessation of ovarian function to an intermittent follicle maturation failure. Actually POF has been identified as a genetic entity (especially chromosome X), but data on genetic factors of premature menopause are limited. Until now, several cases revealed that inactivation of X chromosomes has an effect on ages of premature menopause and females with balanced or unbalanced X-autosome translocations can have several reproductive problems. On the other hand, there have been a few data that was caused by autosome-autosome translocation can lead. Therefore we report a relevant case of POF with translocation between chromosomes 1 and 4. She had her first menstrual period at the age of 12, and after 7 years she stopped menstruation. Chromosomal analysis showed 46, XX, t (1;4) (p22.3;q31.3). While evaluating this rare case, we could review various causes (especially genetic factors) of POF. To remind clinicians about this disease, we report a case of POF caused by autosome-autosome translocation with a literature review.

4.Long-term Outcome of Trigeminal Nerve Block with Alcohol for the Treatment of Trigeminal Neuralgia.

BACKGROUND: Recently trigeminal nerve block with alcohol (TnbA) for the treatment of trigeminal neuralgia (TN) has come to be known as a procedure with a short-term effect and high complications. There has been none of report about long-term outcome of TnbA for TN. The objective of this prospective study for the long-term results of TnbA was to analyse the pain free duration and complication after the administration of blocks and compare them in the first block and subsequent blocks. METHODS: From March 1996 to May 2005, 304 consecutive patients with primary trigeminal neuralgia were treated with TnbA including supraorbital nerve block, infraorbital nerve block, maxillary nerve (V2) block, mandibular nerve (V3) block, and V2 and V3 at the same time and were prospectively followed up every two months for 10 years. RESULTS: The mean value of pain free duation of 1st, 2nd and 3rd TnbA were 43, 38 and 48 months, respectively using Kaplan-Meier analysis. The probability of pain recurrence in 1 and 3 years after the 1st, 2nd and 3rd blocks were 25%, 25%, 20% and 53%, 54%, 34%, respectively. The pain free durations of first and subsequent blocks were not statistically different. Complications were reported at 36 (11.8%), 5 (4.2%), and 0 in 1st, 2nd and 3rd blocks. CONCLUSIONS: TnbA showed the relatively long duration of pain free and low incidence of complications. Repeated TnbA has pain free duration as long as the 1st block and less complications as well. TnbA is a valuable treatment of TN as a percutaneous procedure.

5.Changes in Power Spectum of R-R Interval Variability during General Anesthesia: Real Time Analysis and Monitoring.

In general, anesthetic depth is evaluated by experience of anesthesiologist based on the changes of blood pressure and pulse rate. So it is difficult to guarantee the accuracy in evaluation of anesthetic depth. The efforts to develop the objective index for evaluation of anesthetic depth were continued but there was ression in this area. In recent EEG, evoked potential and law esophageal pressure are testad as monitoring tools of anesthetic depth. Heaut rate variabilty provides much infarmation of autonomic activity of cardiovascular system and almost all anesthetics depress the autonamic activity. Novel monitoring system which can simply and exactly analyze the autonomic activity of cardiovascular system will provide important information for evaluation of anesthetic depth. In this aspect, development of real time R-R interval variability analysis system and evaluation of its efficacy for evaluation of anesthetic depth were performed. Newly developed real time R-R interval variability analysis system works by simple adaptation to EKG monitor, displays the color power spectrum every 30 seconds for at least 4 hours. So users can easily understand the whole trends during anesthesia and check the changes of short duration. And for the cutting expenditure of memory of personal computer this system is programmed to discard the calculated data of power spectrum after display on screen and saves only R-R interal of heart rate for off-line analysis. At first, function of real time R-R interval variability analysis system was tested with EKG simulator. Our system was well functioned in getting the signals from EKG, analyzing, displaying and saving the data. In animal experiment the changes in power of high frequency band assaciated with the changes of end tidal concentration of halothane (p<0.05). And during intravenous infusion of propofol the changes of infusion amount induced the changes in power spectrum of each frequency band. After animal experiment clinical application was tried. During induction of N2O-O2-enflurane anesthesia and recovery, power of each frequency band was decreased and increased in trend. Endotracheal intubation and skin incision induced high peak of power of each frequency band. Standard score of power and trend curve using 3RSSH was used for the analysis of trend of power change and provide meaningful information of R-R interval variability.

6.Central Neurocytoma Originated from Atrium with Malignant Trans formation: A Case Report.

Central neurocytoma, a cerebral intraventricular tumor, is usually benign. It frequently develops in the area of the foramen of Monro, and is usually attached to the septum pellucidum. Mild to moderate contrast enhancement is common. We encountered a case of central neurocytoma in a 60-year-old woman; the tumor arose from the atrium of the lateral ventricle, and extraventricular extension and malignant transformation were apparent. CT and MRI revealed a well-defined, slightly heterogeneous mass measuring 2.5x3x5cm with surrounding edema. There was strong contrast enhancement of the mass.

7.Effectiveness of a Pneumonia Severity Index (PSI) as a Predictor of Mortality in Emergency Department Patients with Community-Acquired Pneumonia.

PURPOSE: Pneumonia is the most common cause of death among infectious diseases. Community-acquired pneumonia is the sixth leading cause of death in Korea. This study was designed to analyze the relationship of risk factors and mortality, especially the pneumonia severity index (PSI) in patients with community-acquired pneumonia diagnosed in the emergency department of a referral hospital. METHODS: The medical records of patients admitted to the Yeungnam University Hospital between March 2006 and March 2008 for community-acquired pneumonia were reviewed retrospectively. The demographic data, comorbidity, laboratory results, PSI score and class of PSI, all of which might influence the prognosis of pneumonia, were analyzed. RESULTS: Among 123 patients admitted for community-acquired pneumonia, 18 died (mortality rate of 15%). Laboratory data showed that sodium, glucose, blood urea nitrogen, albumin, platelets, hematocrit and arterial pH were related to the prognosis. For the pneumonia severity index, the mortality rate increased in a step-wise manner from class I through class V. Comorbidities such as neoplasms (p=0.000), cerebrovascular accidents (p=0.005) and liver disease (p=0.003), as well as systolic blood pressure (p=0.003), respiratory rate (p=0.024), sodium (p=0.000), glucose (p=0.000), blood urea nitrogen (p=0.000), albumin (p=0.003), hematocrit (p=0.000) and arterial pH (p=0.042) were the important risk factors for mortality in patients with community-acquired pneumonia. CONCLUSION: The pneumonia severity index could be used as a valuable index for predicting mortality of patients and the prognosis of community-acquired pneumonia in the emergency department.

8.Covered Bronchial Stent Insertion to Manage Airway Obstruction with Hemoptysis Caused by Lung Cancer.

Malignant airway obstruction and hemoptysis are common in lung cancer patients. Recently, airway stent is commonly used to preserve airway in malignant airway obstruction. Hemoptysis can be managed through various methods including conservative treatment, endobronchial tamponade, bronchoscopic intervention, embolization and surgery. In our case studies, we sought to investigate the effectiveness of airway stents for re-opening the airway as well as tamponade effects in four patients with malignant airway obstruction and bleeding caused by tumors or lymph node invasions.

9.B cell activation factor (BAFF) is a novel adipokine that links obesity and inflammation.

B cell activation factor (BAFF) is a novel member of the TNF ligand superfamily, mainly produced by myeloid cells. BAFF has been shown to participate in B-cell survival and B- and T-cell maturation. BAFF expression in adipocytes has been recently demonstrated. In the current study, we verified that BAFF expression is increased during adipocyte differentiation. BAFF expression was augmented by TNF-alpha treatment and was decreased by rosiglitazone treatment. BAFF secretion in lean and in ob/ob mice sera were compared and smaller amount of BAFF was secreted in ob/ob mice. mRNA and protein expression were different between epididymal and visceral adipose tissue. BAFF expression was also increased in ob/ob mouse adipose tissue. We sought to identify known BAFF receptors (BAFF-R, BCMA, and TACI) in adipocytes, and determined that all three were present and upregulated during adipocyte differentiation. However, the expression of TACI was distinct from that of BAFF-R and BCMA under TNF-alpha and BAFF ligand treatment. BAFF-R and BCMA expression levels were upregulated under pro-inflammatory conditions, but TACI was reduced. Conversely, BAFF-R and BCMA expression levels were downregulated by rosiglitazone treatment, but TACI was increased. Taken together, our results suggest that BAFF may be a new adipokine, representing a link between obesity and inflammation.
