1.A case of lichenoid contact dermatitis caused by a color film developer.

Color developers(CD) have been noted to produce lichen planus like eruption as well as eczematous contact dermatitis. The chemicals involved are derivatives of paraphenylenediamine or aniline, i. e., Kodak CD 2, Kodak CD 3, Agfa TSS. We report a 28-year-old male who showed a feature of lichenoid contact dermatitis after exposure to color developer. Patch test revealed positive reactions to CD2, CD3 and CD4. Skin biopsy from a patch site showed lichen planus-like tissue reaction.

3.Radiographic evaluation of dentigerous cyst with cone beam CT.

PURPOSE: The purpose of this study was to accurately analyze the radiographic characteristics of dentigerous cyst (DC) with multiplanar images of cone beam computed tomography (CBCT). MATERIALS AND METHODS: Thirty eight radiographically and histopathologically proven cases of DCs were analyzed with panoramic radiograph and CBCT, retrospectively. The radiographic CT pattern, symmetry of radiolucency around the unerupted tooth crown, ratio of long length to short length, degree of cortical bone alternation, effects on adjacent tooth, and cyst size were analyzed. Relative frequencies of these radiographic features were evaluated. In order to compare the CBCT features of DC with those of odontogenic keratocyst (OKC), 9 cases of OKCs were analyzed with the same method radiographically. RESULTS: DCs consisted of thirty unilocular cases (79.0%), seven lobulated cases (18.4%) and one multilocular case (2.6%). Eight were asymmetric (21.0%) and thirty were symmetric (79.0%). Maxillary DC showed rounder shape than mandibular DC (L/S ratio; maxilla 1.32, mandible 1.67). Alternations of lingual cortical bone (14 cases, 48.2%) were more frequent than those of buccal side (7 cases, 24.1%). CBCT images of DC showed definite root resorption and bucco-lingual tooth displacement. These findings were hardly observed on panoramic radiographs of DCs. Comparison of CBCT features of DC with those of OKC showed several different features. CONCLUSION: CBCT images of DC showed various characteristic radiographic features. Therefore, CBCT can be helpful for the diagnosis of DC radiographically.

4.Propranolol Hcl in Patients with Angina Pectoris.

A double-blind trial of the efficacy of propranolol hydrochloride a new beta adrenergic blocking agent, in the prophylactic treatment of angina pectoris was conducted in 9 patients. Through the trial period of 8 weeks to more than 26 weeks, the patients received propranolol and placebo for equal period. Following are the results. 1. The effective dose of propranolol hydrochloride in the prophylactic treatment of angina pectoris was 80 mg to 120 mg per day. 2. The effectiveness of propranolol hydrochloride was manifested by a reduction in frequency of pain to 40.7%, in nitroglycerin comsumption to 38.8% and by an increase in exercise tolerance. 3. Obseved side effects were transient nausea, vomiting and bradycardia, which were improved with reduction in drug dosage. 4. Further clinical and experimental information may be necessary in order to assess the long-term effects of propranolol hydrochloride.

5.A Statistical Study on Arrhythmias of the Aged.

A statistical study was done on arrhythmia of the aged who were older than 60 during the period 1963~1972 at Seoul national University Hospital. This study included 6,511 patients among 40,000 total patients. The results were as follows. 1) Arrhythmias were observed among 3,058 patients(46.97%). Sinus irregularitis were the most frequent arrhythmia. 2) Except sinus irregularities premature beat (38.3%), atrial fibrillation (16.1%), bundle branch block (10.5%), atriovenricular block (10.5%), atrioventricular escape beat (5.1%), atrioventricular junctional rhythm (4.9%), wandering pacemaker(4.0%) and paroxysmal tachycardia(2.5%) were frequent arrhythmias. 3) Cardiovascular disease was major underlying disease. In cardiovascular disease group, the incidence of arrhythmia was 51.15% which was three times as high as non-cardiovascular and normal group. 4) Most arrhythmias were more frequently observed among the aged except bundle branch block and ventricular paroxysmal tachycardia. 5) The incidence of arrhythmia was similar among the aged except pre-excitation syndrome which was predisposed to woman. 6) Arrhythmia was most frequently observed in coronary heart disease. 7) Among sinus irregularities, simus bradycardia was more frequent in the aged. 8) There were no significant differences in frequency of arrhythmia between non-cardiovascular disease group patients and normal group.

6.Efficacy of Glycopyrrolate in Primary Hyperhidrosis Patients.

BACKGROUND: Primary hyperhidrosis is a disorder of excessive sweating, which shares several features with anxiety disorders and has a negative impact on a patient's quality of life. Oral glycopyrrolate is one of the treatments available. There are a few published studies on the use of glycopyrrolate given orally in the treatment of hyperhidrosis. METHODS: Thies is study was a review of case notes in a series of 36 patients with primary hyperhidrosis. We made a comparison between the Keller's scale score of a pre-glycopyrrolate medication group and the Keller's scale score f a post-glycopyrrolate medication group. The Milanez de Campos score, Short Form_36 (SF-36) score, Beck Depression Inventory (BDI) score, Beck Anxiety Inventory (BAI) score, and autonomic nervous system (ANS) scale score were also compared between the two groups. RESULTS: In the post-glycopyrrolate medication group, there were declines in Keller's scale, and Milanez de Campos scale score and BAI score (P < 0.001). In addition, there were increases in SF_36 score in the post-glycopyrrolate medication group (P = 0.03) However, no changes were seen in, BDI score and ANS score in the post-glycopyrrolate medication group (P < 0.001). CONCLUSIONS: Glycopyrrolate is an effective initial method of treating primary hyperhidrosis that, reduces anxiety and improve patients' quality of life.

7.Comparison of Surgical Parameters and Results between Curved Varus Osteotomy and Rotational Osteotomy for Osteonecrosis of the Femoral Head.

BACKGROUND: Various osteotomies have been introduced to treat osteonecrosis of the femoral head. The purpose of this study was to compare surgical parameters, postoperative limb length discrepancy, and minimum 5-year clinical and radiological results between transtrochanteric curved varus osteotomy (TCVO) and transtrochanteric rotational osteotomy (TRO) for osteonecrosis of the femoral head. METHODS: From 2004 to 2009, 103 consecutive TROs (97 patients) followed by 72 consecutive TCVOs (64 patients) were performed for the treatment of osteonecrosis of the femoral head. Of these, 85 patients (91 hips) in the TRO group and 58 patients (65 hips) in the TCVO group completed minimum 5-year clinical and radiological follow-up. The Kaplan-Meier product-limit method was used to estimate survival. RESULTS: The TCVO group had shorter operation time (p < 0.05) and less estimated blood loss (p = 0.026). Postoperative collapse developed in 26 hips (28.6%) in the TRO group and 7 hips (10.8%) in the TCVO group (p = 0.007). Osteophyte formation was observed in 34 hips (37.4%) in the TRO group and 13 hips (20%) in the TCVO group (p = 0.020). Fifteen hips (16.5%) in the TRO group and 7 hips (10.8%) in the TCVO group underwent conversion total hip arthroplasty (THA). The survival rate at 9 years with radiographic collapse as the endpoint was 68.7% (95% confidence interval [CI], 58.1% to 79.3%) in the TRO group, and 84.7% (95% CI, 71.5% to 97.9%) in the TCVO group. With conversion to THA as the endpoint, the survival rate was 82.2% (95% CI, 73.1% to 91.3%) in the TRO group and 89.2% (95% CI, 81.7% to 96.7%) in the TCVO group. CONCLUSIONS: The comparison indicates that TCVO was better than TRO in terms of surgical parameters including operation time and estimated blood loss while the 9-year survival rates were similar.

9.Improvement of Transformation System in Filamentous Fungus Aspergillus oryzae.

Aspergillus oryzae is a filamentous fungus classified in the group Aspergillaceae Ascomycetes. A. oryzae is an important microorganism for industrial production of enzymes and fermented food products. It secrets large quantities of proteins or enzymes into the culture medium which makes this organism appealing for the production of heterologous proteins. Recently Electric field-mediated transformation method, electroporation, has been applied to fungal transformation. It is fast, simple to handle, and avoids the use of some chemicals. The optimum conditions for A. oryzae were determined with pILJ-16 and ~0.2 x 105 protoplast cell at various field strength. The survived population of protoplasts in the electric field were ~80% of nonprotoplast cell population at 1.3 KV/cm to ~50% at 6.3 KV/cm. The electrotransformation efficiency, expressed as transformants/microgram of input DNA/population of protoplast cells, increased with the increment of the field strength up to 6.3 KV/cm. The highest value, 14.35%, was obtained at 6.3 KV/cm and 1540ohm. Some antibiotics for the dominant selectable makers were applied to A. oryzae and Tolypocladium inflatum. Whereas phleomycin was very effective on T. inflatum, hygromycin B and phleomycin were not effective on A. oryzae. Protoplasts were obtained with hemicellulase and celluclast, instead of novozyme234. More than 104 transformants/microgram of DNA with hemicellulase-treated protoplasts were obtained by using electroporation at the condition of 2,500 voltage, 1,540 ohm and 0.50 capacitance. Less than 102 transformants/microgram of DNA were obtained with Novozyme234- and celluclast-treated protoplasts.

10.The Evaluation of Incidence of Hyphema as Early Complication following Sutureless Cataract Surgery.

Postoperative hyphema following cataract extraction surgery is not usually a serious complication and clears quickly in the most cases. Sutureless cataract surgery was reported to have less incidence of postoperative hyphema than conventional cataract surgery. We analyzed the incidence of postoperative hyphema in each group according to the depth and the length of scleral pocket incision in 500 eyes with the sutureless cataract surgery via superior incision during two years. One day after surgery, 90 cases(18.0%) had hyphema and most of them(74.4%) showed grade 1 severity. The incidence of hyphema was 23.0% in the deep incision group and only 11.9% in the superficial incision group(p<0.01). The percentage of grade 1 hyphema was 66.6% in the deep group and 92.6% in the superficial group, then severity was greatly reduced in the superficial incision group(p<0.01). The incidence of hyphema in the group of 7.0 mm in length was highest among the 6.0, 6.5, 7.0 mm groups(p<0.01), and the shorter the incision length, the milder the serverity of hyphema(p<0.01). The incidence of hyphuma was the lowest(6.2%) in the group of 6.0 mm in length and superficial incision, and it was the highest(28.5%) in the 7.0 mm in length and deep incisional group(p<0.01).
