1.Treatment of seborrheic keratosis by flat-cutting:a clinical observation

Objective To compare the curative effect,patient's satisfaction degree,and adverse reaction on seborrheic keratosis by level-cutting and frozening with liquid nitrogen.Methods 68 cases were randomly divided into liquid nitrogen frozening group(32 cases,group A)and flat-cutting group (36 cases,group B).Liquid nitrogen was spurted until freeze-thaw status in group A,and the ramnant skin lesion needed for other treatments,but no more than 3 times.The skin lesion in group A was completely resected with scalpel following its basilar part after local anesthesia,and elaeoyarn was pressurized to stop bleeding on wound,which was removed afer 5 days.The effects,satisfactory degree and adverse reaction were analyzed after 3 mouths of treatment.Results The effect and satisfactory degree in group B was significantly better than that in group A(P<0.01),but the adverse reaction in group B was significantly lower than that in group A(P<0.05).Conclusion Compared with liquid nitrogen frozening,the therapy of flat-cutting has advantages to seborrheic keratosis,with good efficacy,high patient's satisfaction and low adverse reaction.

Objective Diammonii glycyrrhizinatis was shown to exert a protective effect on myocardium against ischemia-reperfusion injury. The purpose of this study was to evaluate the effects of diammonii glycyrrhizinatis on acute lung injury induced by cardiopulmonary bypass(CPB), Methods Twenty-four ASA Ⅱ - Ⅲ patients (9 male, 15 female) aged 20-60 yr, weighing 45-75 kg scheduled for elective value replacement were divided into two groups at random: control group (group C, n = 12) and diammonii glycyrrhizinatis group (group G, n = 12) . Patients with infections diseases or immunodiflciency and those who were taking corticosteroid and drugs which may affect immune function were excluded. Anesthesia was induced and maintained with midazolam, fentanyl and vecuronium. In group G diammonii 2.5 mg?kg-1 10% glucose 50 ml was infused during the interval between intubation and skin incision. Blood samples were taken from arterial line at 10 min after induction (T0), 30 min after start of CPB (T1 ), 30 min after aortic unclamping (T2), at the end of surgery (T3), 4 h and 24 h after operation (T5, 6) for determination of plasma TNT-? IL-?, IL-10, MDA levels and Mn-SOD activity. Respiratory index (RI) PA-aDO2/PaO2 ] was calculated at 10 min after induction of anesthesia (T0), 10 min and 30 min after CPB (T1, 2) and at the end of surgery. Results There was no significant difference in age body weight, CPB time and aortic cross clamping time between the two groups. The TNF-?, IL-?, IL-10, MDA levels and Mn-SOD activity were not significanttly different at T0 between the two groups, and increased significanttly after start of CPB in both groups ( P
4.Clinical evaluation of compound betamethasone combined with anisodamine in treatment of eyebrow alopecia areata

Objective To observe the curative effect, safety and patient's satisfaction degree of combining compound betamethasone with anisodamine injection in the blocking treatment of eyebrow alopecia areata.Methods A total of 66 cases were randomly divined into two groups: patients were treated by compound betamethasone ( group A) and combining compound betamethasone with anisodamine ( group B). The drugs were injected from the middle of eyebrow alopecia areata to two sides by the standard of 0. 2 ml/cm2 in both groups one time a month. The curative effect of the all cases were observed at 1, 2 and 3 months after treatment. The safety of them were evaluated based on incidence rate of the side effect and their satisfactory degree were evaluated based on subjective reception 3 months after treatment. Results The effective rates in the group B were 58.82 %, 73.53 % and 88.23 % by turns at 1,2 and 3 months after treatment, and those in the group A were 31.25 %, 53.13 % and 71.88 %, respectively. The curative effect in the group B was better than that in the group A (P < 0. 05), and the incidence rate of side effect in the group B was similar to that in the group A (P>0.05) and the patient's satisfaction degree in group B was better than that in group A (P <0. 05) 3months after treatment. Conclusion The compound betamethasone combined with anisodamine injection in the treatment of eyebrow alopecia areata has superior curative efficacy, more safety and higher satisfactory degree in the patients.
5.Expressions of phosphatidylinositol 3 kinase and phosphorylated Akt in condyloma acuminatum and cervical squamous cell carcinoma

Objective To investigate the roles of phosphatidylinositol 3 kinase (PI3K) and phosphorylated Akt (P-Akt) in the pathogenesis of cervical squamous cell carcinoma and condyloma acuminatum.Methods Immunohistochemistry and Western blot were used to detect the expressions of PI3K and P-Akt in tissue specimens from the lesions of 30 cases of cervical squamous cell carcinoma,30 cases of condyloma acuminatum and the prepuce of 15 normal human controls.The average optical density and gray scale values were calculated and analyzed by t test and F test respectively.Results The expressions of PI3K and P-Akt were observed in only the basal layer of the epidermis of control specimens,but in the whole epidermis of condyloma acuminatum tissue specimens.Cervical squamous cell carcinoma tissue specimens displayed a stronger expression of PI3K and P-Akt compared with the control and condyloma acuminatum tissue specimens.As immunohistochemistry revealed,the average absorbance value for PI3K and P-Akt was 0.28 ±0.05 and 0.20 ± 0.07 respectively in cervical squamous cell carcinoma tissue specimens,0.22 ± 0.04 and 0.17 ± 0.03 respectively in condyloma acuminatum tissue specimens,and 0.16 ± 0.04 and 0.10 ± 0.02 respectively in the control tissue specimens; significant differences were observed in the expressions of PI3K and P-Akt among the three groups of tissue specimens (F =44.87,20.64,respectively,both P < 0.01 ).The results of Western blot were consistent with those of immunohistochemistry,and there was a significant difference in the gray scale value for PI3K and P-Akt between cervical squamous cell carcinoma,condyloma acuminatum and control tissue specimens (3.48 ± 0.48 vs.1.99 ± 0.11 vs.1.00 ± 0.03,F=354.83,P< 0.01; 3.33 ± 0.26 vs.1.96 ± 0.11 vs.1.00 ± 0.03,F=302.33,P< 0.01 ).Conclusions The PI3K/Akt signaling pathway is abnormally activated in condyloma acuminatum and cervical squamous cell carcinoma,and human papilloma virus may cause the abnormal proliferation of infected epithelium likely by affecting the upregnlated expression of PI3K/P-Akt.
6.Management of huge defects following extensive abdominal wall neoplasm resection: classification and immediate reconstruction

Objective To evaluate the effect of extensive resection and immediate reconstruction based on classification of abdominal wall defects for patients with abdominal wall neoplasms.Methods From Jan 1999 to May 2016,112 patients with abdominal wall neoplasms were treated with extensive resection,including Type Ⅰ (n =20),Type Ⅱ (n =45) and Type Ⅲ (n =47).Immediate abdominal wall reconstruction comprised primary sutures or free skin graft for Type I defects,component separation (CST) with or without a prosthetic or biological mesh reinforcement for Type Ⅱ defects and pedicled or vascularized myocutaneous flap with or without a prosthetic or biological mesh or prosthetic + biological mesh with or without CST for Type Ⅲ defects.Results The average follow up was 76.86 ± 21.22 months,3 patients developed flap necrosis,9 patients suffered from wound infection.Local recurrence was observed in 20 patients,35 patients developed distant metastasis.Conclusions The optimal strategy based on the abdominal wall defect classification for immediate reconstruction of huge abdominal wall defects is safe and effective after resection of abdominal wall neoplasms.
7.Effects of propofol and fentanyl on myocardial ischemia-reperfusion injury in isolated rat heart

Objective To investigate the protective effect of propofol and/or fentanyl in myocardial ischemia-reperfusion injury. Methods Thirty-two SD rats weighing 250-300g were anesthetized with intraperitoneal ketamine 80 mg. Their hearts were excised and perfused with oxygenated (95%O2 and 5% CO2) K-H solution in a Langendorff apparatus at a perfusion pressure of 7.8 kPa. The temperature of perfusate and heart were kept at 37 ℃ . The experiment was divided randomly into four groups: A control group was perfused with K-H solution containing intralipid 90 ?g?kg-1 ( n = 8); B propofol group was perfused with K-H solution containing propofol 5?g?kg-1 ; C fentanyl group was perfused with K-H solution containing fentanyl 10 ng?kg-1; D propofol-fentanyl group was perfused with K-H solution containing propofol 5?g ?kg-1 and fentanyl 10 ng?kg-1. The isolated heart was perfused with above mentioned perfusate for 10 min followed by 50 min global normothermic (37℃) ischemia and 30 min reperfusion. The left ventricular developed pressure (LVDP) and ?dp/dtmax and coronary flow were measured 1 min before propofol and/or fentanyl perfusion, 1 min before global ischemia and at the end of 30 min reperfusion. The lactate dehydrogenase(LDH) activity in the total coronary effluent collected during the 30 min reperfusion was measured. Results The recovery of LVDP and ? dp/dtmax and coronary flow at the end of 30 min reperfusion were significantly better in group B, C and D than those in control group and the recovery was best in group D. The release of LDH in coronary effluent decreased significantly at the end of 30 min reperfusion in group B, C and D as compared with that in control group. LDH release was least in group D. Conclusions Both propofol and fentanyl can protect myocardium against ischemia- reperfusion injury in isolated rat hearts. The protective effect of propofol and fentanyl can be added and better protection can be provided.
8.Role of the autophagy in the treatment of neuropathic pain with pulsed radiofrequency

Objective:To detect the effect of pulse radiofrequency (PRF) treatment on the neuropathic pain established by L5-spinal nerve ligation (SNL) on rats,and to investigate if PRF treatment would affect the expression of autophagy related protein LC3 and autophagy related receptor P62 at the dorsal horn.Methods:A total of 36 male Sprague-Dawley rats were randomly divided into 3 groups:a Sham group,a SNL group,and a SNL+PRF group.The 50% paw withdrawal mechanical threshold (PWMT) was detected at 1 day before and 1,3,7,14 and 28 days post-operation by using Von-Frey filaments.The autophagy related protein LC3 and autophagy related receptor P62 were investigated by Western blot.Results:Compared with the Sham group,the PWMT significantly decreased in the SNL group at each time points (P<0.05);in SNL+PRF group,PRF treatment could elevate the PWMT at the 1st day post-operation and lasted for 28 days (P<0.05).What's more,SNL could elevate the LC3-Ⅱ and P62 levels at the 7th day post-operation (P<0.05),which were decreased by the PRF treatment (P<0.05).Conclusion:PRF treatment could improve SNL-induced the neuropathic pain,which might be partly due to the regulatory effects on the autophagy levels at the spinal dorsal horn.
9.Study on Microbes on Skin Lesions and a Controlled Topical Treatment Trial in Atopic Dermatitis

Objective To study microbial flora on skin lesions and their antim icrobial susceptibilities and to conduct a topical treatment trial in atopic der matitis (AD). Methods Seventy one patients with AD were diagnosed according to the criteria of Hanifin-Rajka′s in departments of dermatology in China-Japan Friendship Hospital and Beijing Children Hospital during November,2001 to March, 2002. Bacteria and fungi were examined by culture and microscopy. Among 71 case s, 66 were divided into two groups randomly with one group receiving 1%econazol e+0.1%triamcinolone acetonide cream topically twice daily for 4 weeks, and the control group receiving 0.1%hydrocortisone butyrate ointment. Severity of AD w as measured by SCORAD index. Results It was shown that AD mainly affected infant s and children with a prevalence of 73.24%in patients less than 12 years old. T he rates of bacterial and fungal colonization were 53.51%, 1.41%, respectively on skin lesions. S.aureus was detected in 35.21%of patients with AD. Antimicro bial susceptibility test indicated that S.aureus was sensitive to rifampin, vanc omycin, amikacin, ciprofloxacin, cefazolin, cefuroxime, etc. It was found that 1 %econazole+0.1%triamcinolone acetonide cream had significantly more advantage over hydrocortisone butyrate ointment in the therapeutical effects (P
