( CPR ) skills of community nurses in the regional health alliance and its influencing factors . Methods Nosocomial CPR first-aid skills of 104 in-service nurses from 6 community hospitals in the regional health alliance using a convenience sampling method were investigated .Results The score of the CPR skill examination of community nurses in the health alliance was (75.6 ±11.3), which was lower than the score of nurses in the leading hospital .The subjects with the three lowest accuracy were:the technique and the items of the bag safety inspection of the bag valve mask (8.7%, 10.6%), the CE method to hold the mask (15.4%). The multiple linear regression test showed that community nurses′education , working age , weather they had attended related training were influencing factors of the nosocomial CPR skill score of community nurses in the health alliance .Conclusions Nosocomial CPR skills of community nurses in the regional health alliance need improving .The leading hospital in the health alliance should fully integrate resources and strengthen the first -aid skill training of community nurses in the regional health alliance in order to ensure medical safety in the community.

3.Analysis of patients′ cognitive status and influencing factors of intravenous transfusion demerits in health alliance community
Chinese Journal of Modern Nursing 2017;23(19):2462-2465
Objective To explore patients′ cognitive status and influence factors of intravenous transfusion demerits in health alliance community, and to provide evidence for related health education. Methods A total of 589 patients who had received infusions in six health alliance communities outpatients were investigated by questionnaire to get to know their knowledge about infusion. Results The overall average score of intravenous transfusion KAP among health alliance outpatients was (7.21±2.10) with only 93 patients (15.8%) getting full score. Logistic multi-factor regression analysis showed that gender, degree of education, occupation, health education on infusion were the influencing factors of intravenous transfusion KAP.Conclusions Intravenous transfusion related KAP of community patients remains to be improved. More attention should be focused on male patients, patients with lower literacy level,non-government institutions, and those who are lack of transfusion related health education.

Objective Through establishment of prevention and control guideline for radioactive oral cavity mucositis ( radiotherapy-induced oral mucositis, RTOM ) , to guide nursing staff to strengthen the prevention and control for RTOM in patients with head and neck tumor, thereby improving the quality of nursing. Methods Through evidence-based literature retrieval, interview chemoradiation division staff and RTOM cases, the common problems in nursing management of RTOM prevention and control were concluded and analyzed. According to related evidence-based nursing practice, prevention and control guidelines was established. Through establishment of a multidisciplinary team and process for prevention and control of RTOM, strengthening RTOM specialty and related assessment scale training, health education propaganda material and quality control measures were made. Results After the implementation of prevention and control guidance, the incidence of grade Ⅲ RTOM reduced to 16. 7% from 40. 0%; pain score ( Numerical Rating Scale, NRS ) reduced from (3.90±0.76) points to (1.67±1.07) points (P<0.05).Conclusions The prevention and control guideline has better clinical guidance on RTOM in patients with head and neck tumor.

Objective To observe the application effect of procedural sedation and analgesia solution that aiming at shallow sedation in critical patients with mechanical ventilation. Methods Totally 160 patients with mechanical ventilation who accepted endotracheal intubation and without cerebral organic lesions in intensive care unit ( ICU) of our hospital from February 2014 to February 2015 were assigned to the observation group and the control group at random. Patients in the control group received drug sedation and analgesia treatment and the dosage was regulated according to the medication orders; patients in the observation group received the procedural sedation and analgesia solution that aiming at shallow sedation. Doctors made plans and aim of sedation and analgesia, and nurses dynamically evaluated the standard situation of sedation and analgesia and regulated the dosage according to the assessment results. We compared the dosage of sedation drugs, the time of mechanical ventilation, ICU time and the incidence of delirium between two groups. Results The dosage of sedation drugs in the observation group were:disoprofol,(5 690±618.610) mg;dexmedetomidine, (1 642.5± 317.330) μg;the mechanical ventilation time was (4.77±0.740) d, and the ICU time was (5.78±0.750) d. Those data were all lower than the control group (t=9.587,9.523,6.291,6.260;P<0.05). The incidence of delirium was 13.75% in the observation group, and was 26.25% in the control group (χ2=3.91,P<0.05). Conclusions The procedural sedation and analgesia solution that aiming at shallow sedation could effectively reduce the dosage of sedation drugs, shorten the mechanical ventilation time, reduce the incidence of delirium and is worth to popularization and application.

Objective To explore the current situation of knowledge, attitude, practice (KAP) on transitional nursing of nurses in regional medical association so as to provide the base for related training and quality control in regional medical association. Methods A total of 128 nurses in regional medical association were investigated with the transitional care KAP questionnaire for nurses. Results The overall score of the transitional care KAP questionnaire for nurses was (70.11±11.46), which was at the medium level. The score of knowledge, attitude, practice was (64.51±15.88), (78.27±9.47), (63.35±18.02). The pearson correlation analysis showed that there were statistically positive correlations between knowledge and attitude, between attitude and practice, between knowledge and practice (r=0.59, 0.50, 0.55;P<0.01). It is the influencing factor of the overall score of the transitional care KAP questionnaire, knowledge, attitude, practice wether attending related training. Conclusions Nurses in regional medical association have positive attitudes on transitional nursing. However, the knowledge and practice need to be strengthened further. It is imperative to improve the training mechanism of transitional nursing of nurses in regional medical association.

Objective To explore the current status and influencing factors of the fear of disease progression in cancer patients. Methods According to the convenience sampling, a total of 141 cancer patients who underwent radio-chemotherapy in a grade Ⅲ class A hospital in Ningbo from April to October 2016 were collected into this study. The general information questionnaire, fear of progression questionnaire-short form (FoP-Q-SF)and hospital anxiety and depression scale(HADS) were used to survey the patients. The univariate analysis and multivariate Logistic regression analysis were used to investigate the influencing factors for fear of disease progression. Results The score of FoP-Q-SF were (23.60±8.10). Approximately 10.6% (15/141) participants had a dysfunctional level of FoP-Q-SF (≥34 points). The multivariable Logistic regression showed that the influencing factors included age, marital status and anxiety level (OR=0.168, 28.032, 6.844;P<0.05). Conclusions The fear of disease progression in cancer patients should be well concerned and evaluated, especially in patients who at young/middle age, divorced or widowed, or with high anxiety level. These patients should be paid more attention to reduce and control the fear of disease progression.

Objective To investigate the application effect of group therapy in improving the subjective well-being (SWB) of elderly patients with long-term hospitalization. Methods Totally 100 cases of elderly patients admitted in geriatric ward of Ningbo First Hospital for long-time were selected. Newfoundland scale of happiness (MUNSH) scale and the negative life events questionnaire were used to investigate elderly inpatients, and patients with MUNSH ≤12 points received group therapy. Patients were divided into several groups to receive the intervention according to the negative life events. One year after intervention, patients receiving group therapy were evaluated again by using MUNSH scale to evaluate the intervention effect. Results Among 100 elderly patients, there were 32 cases of high SWB,3 cases of low SWB,and 65 cases of medium SWB. 53 cases of patients had sleep disorders, and 62.26% of them had low SWB. After intervention, the scores of negative experience and emotion of elderly patients were lower than those before intervention (P<0.05);the scores of positive emotion and positive experience and the total MUNSH score were higher than those before intervention (P < 0.05). Conclusions According to the different influencing factors, group interventions can significantly improve the SWB level of elderly patients with long-term hospitalization,so as to improve their quality of life.
