1.Molecular epidemiology of infectious bursal disease virus in Guangxi during the period of 2000 to 2007.

Tissue samples of Fabricius' bursa collected from Nanning, Yulin, Beihai and Wuzhou in the provinces of Guangxi in China during the years of 2000-2007, were detected by a established reverse transcriptase polymerase chain reaction (RT-PCR) technique for IBDV. Viral isolation was performed on the positive samples by chicken embryo inoculation via chorio-allantoic membrane (CAM). Results showed that 27 isolates of IBDV were obtained. A set of primers were designed to amplify the vVP2 of 27 isolates by RT-PCR and the PCR products were sequenced. The sequences of all the isolates and reference viruses were analyzed and compared, and their phylogenetic trees were constructed based on the nucleotide sequences. The results indicated that isolate BH11, TZ(3), 050222, YL051, NN0603, NN0611and QX0602 etc, altogether 17 isolates, which accounted for 62.96 percent of total isolates, were identified to be very virulent IBDV (vvIBDV) and have the highest homology to vvIBDV reference strains. In the phylogenetic analysis, they are divided into 3 groups and have a long distance to commonly used vaccine stains. Isolate NN040124 and YL052 were identified as intermediate-plus virulent strains and showed a highest homology to classical strains of 52-70 and STC. 8 isolates of YLZF2, 040131 etc were identified as attenuated vaccine strains and showed a highest homology to classical strain of CU1. The results from the study demonstrated that the viruses prevailing in chickens in these 4 regions in Guangxi province in the recently 7 years were vvIBDV and their origins were complex. The antigenicity of some isolates may have been drifted.

Through researches of channels and collaterals and clinical practice of many years, the authors understand that Chinese medicine, which considers the human body as an interrelated, mutual constraints, whole, dynamic living system, has gradually become an important part of modern medicine. Channels and collaterals are a closed loop system which is communicated and linked by energy and information in the form of electromagnetic oscillation, reflecting many characteristics similar to quantum. Channels and collaterals are not a fixed organizational structure. Studies on channel and collaterals find that the track of the propagated sensation along channels (PSC) have the phenomenon drifting about. This exactly reflects the law of channels dynamically running. By information triggering and living resonance, channels and collaterals bring into play entirely regulative action. The innovative treatment of channels and collaterals followed by characteristics and laws of quantum can get a better curative effect. Theory of channels in the position of quantum information medical science provides an important breach for modernization of Chinese medicine.

3.Identification of circulating type II pre-dendritic cells (pDC2) and its clinical significance in chronic hepatitis B virus infection.

OBJECTIVETo investigate the characteristics of circulating type II pre-dendritic cells (pDC2) and evaluate its role in patients with chronic hepatitis B virus infection.
METHODSThe quantitative alterations of pDC2 in 27 chronic HBV-infected patients as treated group and 15 healthy individuals as a control group were analyzed by using flow cytometry based on the comparison of CD4+/CD8+ ratios of T lymphocyte subsets between the two groups. The IFN-alpha-producing ability of pDC2 after incubation was determined by ELISA.
RESULTSThe percentage of pDC2 (0.096 +/- 0.086) from the peripheral blood in chronic HBV-infected patients were significantly lower than that (0.304 +/- 0.093) from the normal controls (P less than 0.001) while the CD4+/CD8+ ratios were higher than those in normal controls (P less than 0.01). The values of IFN-alpha-producing function and IL-12 of circulating pDC2 in chronic HBV-infected patients group were significantly lower than those in healthy subjects (P < 0.001). The percentage of pDC2 and CD4+/CD8+ ratios were higher in the patients positive for HBV DNA in sera than those in patients negative for HBV DNA in sera (P < 0.01).
CONCLUSIONThe decreased number of circulating pDC2 and IFN-alpha-producing function from peripheral blood in patients with chronic hepatitis B virus infection may result in the decline of host immune response, which may partially contribute to the disease progress of HBV infection and existence of viral genomic DNA in patient's sera.

4.Effect of acupuncture on serum insulin level in the patient of simple obesity.

OBJECTIVETo compare therapeutic effects of acupuncture plus ear point tapping and pressing therapy and simple medicine for slimming and to probe the mechanism.
METHODSFifty cases of simple obesity were randomly divided into an acupuncture group and a medication group, 25 cases in each group. The acupuncture group were treated with body acupuncture and electroacupuncture at Tianshu (ST 25), Guanyuan (CV 4), Sanyinjiao (SP 6), Feng-long (ST 40), Zusanli (ST 36), etc., combined with ear point sticking and pressing at Shenmen, Nei-fenmi (endocrine), Pi (spleen), Wei (stomach), Sanjiao (triple energy), Dachang (large intestine), etc.. The medication group were treated with oral administration of Sibutramine, once each day, 10 mg each time. Serum insulin contents before and after treatment were detected, and the therapeutic effect for slimming was assessed.
RESULTSThe total effective rate was 88.0% in the acupuncture group and 80.0% in the medication group, with no significant difference between the two groups (P > 0.05); after treatment, the serum insulin levels in the two groups significantly decreased (P < 0.01), and the decrease of insulin level in the acupuncture group was significantly better than that in the medication group (P < 0.05).
CONCLUSIONAcupuncture combined with ear point tapping and pressing therapy has a similar therapeutic effect to western medicine for slimming, but the former is better than the later in improving serum insulin level.

5.Effects of ulinastatin on cerebral inflammatory response during cardiopulmonary bypass.

OBJECTIVE:
To investigate the effects of ulinastatin (UTI) on cerebral inflammatory response during cardiopulmonary bypass (CPB).
METHODS:
Twenty-four NYHA II-III patients (13 males and 11 females) aged 23-45 years, undergoing elective cardiac valve replacement under hypothermic CPB were randomly divided into 2 groups: ulinastatin group (Group U, n=12) and control group (Group C, n=12). In group U, UTI (1.2 x 10(4) U/kg) was given intravenously after the induction of anesthesia, 0.6 x 10(4) U/kg UTI was added to the priming solution, and 0.6 x 10(4) U/kg UTI was given about 5 min before the aortic decamping. In Group C, normal saline was given instead of UTI. Internal jugular vein was cannulated and the catheter was advanced retrogradely till jugular bulb. Blood samples were taken simultaneously from artery and jugular bulb after induction of anesthesia (T1), 60 min (T2) and 6 h (T3) after discontinuation of CPB for determination of TNFalpha, IL-6, IL-8 and IL-10. The juguloarterial gradients of these cytokines (deltaTNFalpha, deltaIL-6, deltaIL-8, and deltaIL-10) were calculated.
RESULTS:
In Group C, arterial levels of TNFalpha, IL-6, IL-8, IL-10 at T2 and T3, deltaTNFalpha, deltaIL-8 and deltaIL-10 at T2, deltaTNFalpha, deltaIL-6 and deltaIL-10 at T3 significantly increased (P < 0.01). deltaIL-8 increased at T3 (P < 0.05). In Group U, arterial levels of IL-6, IL-8, IL-10 at T2, arterial levels of IL-6, IL-8,IL-L-10 and deltaTNFalpha, deltaIL-8 at T3 significantly increased (P < 0.01). Arterial levels of TNFalpha at T2 and T3, deltaTNFalpha, deltaIL-10 at T2, deltaIL-6 at T3 increased (P < 0.05). Arterial levels of TNFalpha, IL-6 and deltaTNFalpha, deltaIL-8 at T2, arterial levels of TNFalpha and deltaIL-6 at T3 in Group U were lower than those in Group C (P < 0.05). Arterial levels of IL-6 at T3, IL-8 at T2 and T3 in Group U were significantly lower than those in Group C (P < 0.01). Arterial levels of IL-10 and deltaIL-10 at T3 in Group U were higher than those in Group C (P < 0.05).
CONCLUSION
Systemic and cerebral activation of inflammatory response during CPB can be alleviated by ulinastatin.

6.Analysis of factors leading to prescheduled analgesic withdrawal in patients with postoperative epidural analgesia.

OBJECTIVETo analyze the factors leading to prescheduled analgesic withdrawal in patients with postoperative epidural analgesia.
METHODSA retrospective study of 4876 patients with postoperative epidural analgesia was conducted and the effect of analgesia and incidence of prescheduled analgesic withdrawal were recorded. The factors precipitating the occurrences of analgesic withdrawal and complications were analyzed.
RESULTSEarly analgesic withdrawal occurred in 113 cases (2.3%), among which 74 (0.5%) were due to factors irrelevant to analgesic complications. Analgesia-related complications occurred in 578 patients, but only 39 (0.7%) of them needed discontinuation of the analgesics.
CONCLUSIONPrescheduled analgesic withdrawal is predominantly due to technical inadequacies rather than complications arising from the analgesics, and improvement of the operation skills for postoperative analgesia may reduce early analgesia discontinuation and enhance the patients' satisfaction.

7.Endoscopic submucosal enucleation for gastric submucosal tumors originated from muscularis propria layer: clinical analysis of 116 case.

OBJECTIVETo explore the efficacy and safety of endoscopic submucosal enucleation (ESE) for gastric submucosal tumors (SMTs) originated from muscularis propria.
METHODSA total of 116 patients with gastric SMT originated from muscularis propria underwent ESE in Department of Gastroenterology of the Taizhou Hospital between July 2006 and March 2011. The occurrence of intra-operative and post-operative complications and corresponding treatment were recorded. After the treatment of ESE, the patients were followed up endoscopically.
RESULTSThe success rate of operation was 96.6%. The mean time of the procedure was (51.9±16.3) min. Complications included intra-operative bleeding (n=9, 7.8%), perforation (n=20, 17.2%), and post-operative bleeding (n=3, 2.6%). Among them, 5 cases (4.3%) required surgical intervention. None of patient had other complications such as peritoneal abscess or peritonitis. The mean hospitalization time after ESE was 6.1 days. The median follow-up period was 12 months (range, 3-48 months) and there was no residual tumor or recurrence.
CONCLUSIONESE is a safe and feasible treatment for patients with gastric SMT originated from muscularis propria.

Objective To learn the status of human resources of vaccination clinics in Zhejiang province and to give suggestions for optimizing the allocation.Methods Investigation and the focus group interview were conducted in Zhejiang Province to learn the configuration standards in vaccination clinics.Results The mean number who attended the immunization work every vaccination day in those clinics was five.The number of staff in each clinic differed from county to county but all reached 0.9 persons serving for 10,000 residents.There should not be less than four persons in each vaccination clinic theoretically.It should be necessary to increase one vaccination staff in every 50 injection doses of vaccine workload.Conclusion The staff of vaccination clinics is inadequate and unbalanced in Zhejiang Province.We should increase staff numbers according to workload and served population.
9.Clinical evaluation of efficacy and safety of nateglinide in the treatment of type 2 diabetes.

OBJECTIVETo evaluate the efficacy and safety of nateglinide, a new antidiabetic agent, in the treatment of type 2 diabetes.
METHODSA total of 219 treatment-naïve patients with type 2 diabetes from 6 centers were enrolled in this study and blindly divided into nateglinide group (n = 105) and repaglinide group (n = 114). In all patients, the disease was confirmed for at least three months. The whole observation lasted for 12 weeks. The efficacy indicators measured include glycohemoglobin A1c (HbA1c), fasting blood glucose, and 2 hours postprandial blood glucose, and the safety parameters measured included renal and hepatic function, serum lipids, and blood and urea profiles.
RESULTSSimilar decreases in fasting blood glucose, 2 hours postprandial blood glucose, and HbA1 c were found in both nateglinide group and repaglinide group without significant differences. No severe adverse events were noted. The hypoglycemia event reports were not significantly different between these two groups.
CONCLUSIONNateglinide is an effective and safe drug in treating type 2 diabetes.

10.Early prediction system for acute severe pancreatitis based on machine learning

Acute pancreatitis (AP) is a common disease faced by clinicians. Severe acute pancreatitis (SAP) has a high mortality rate, so early identification of patients who may develop into SAP is of great significance for guiding treatment. Machine learning is a multi-layer representational learning algorithm that analyzes and obtains laws from existing data and uses these laws to make predictions on unknown data. This study established an SAP prediction scoring system based on machine learning, which can predict the SAP risk of patients within 24 hours. The prediction accuracy rate is as high as 87.36% and AUC 94.11%. The model can better assist clinical decision-making and treatment, and guide doctors to make relevant interventions earlier.
