1.Application of hospital teaching rounds in clinical teaching practice in hospitals for infectious diseases

Great difficulties were existed in clinical teaching in hospitals for infectious diseases. Therefore,a centralized hospital teaching rounds,a new clinical teaching practice was taken in a hospital for infectious diseases. During seven years' practice,a basic mode of centralized hospital teaching rounds was established,which contains many aspects such as organizational form,time arrangement,case se-lection and operation procedures. Its effectiveness was evaluated accoding to the following aspects:number of people attending hospital teaching rounds,composition of these people,satisfaction of these people, comparison of ward rounds data recorded nowadays and 7 years before and assembling all the rounds records into a book. Results indicated that this measure overcame the limitations of the specialized hospi-tals to some extent and played an important role in improving clinical teaching quality.
2.The disturbance of calcium metabolism of vascular smooth muscle in the late phase of septic shock and the underlying mechanism

AIM: To evaluate the alterations in calcium metabolism of the vascular smooth muscle in the late phase of septic shock and test the hypothesis that nitric oxide might be involved in sepsis-induced vascular hyporeactivity. METHODS: Male Sprague-Dawley rats were subjected to sepsis by cecal ligation and puncture (CLP). 18 hours post CLP,rat aortic rings were employed for measurement of contractile responses by using organ bath technique. RESULTS: In endothelium-denuded aortic rings from CLP rats,concentration-contraction curves to phenylephrine (PE) and KCl were significantly decreased when compared to that from sham control rats. The transient contraction induced by PE in calcium-free Krebs solution and the concentration-dependent contraction to CaCl_2 in KCl-depolarized medium were also markedly reduced. The hyporeactivity was partially reversed by treatment with aminoguanidine,a selective inducible nitric oxide synthase inhibitor. CONCLUSION: An impairment in calcium handling in vascular smooth muscle is involved in the vascular hyporeactivity during the late phase of septic shock,in which an excessive nitric oxide production might be the major mechanism.
3.The effect of HIFU combined with nanoscale ultrasound molecular probes with simple virus thymidine kinase gene on angiogenesis in the nude mouse tumor

Objective To observe the change of the protein and gene expression of hypoxia inducing factor-1α(HIF-1a) and vascular endothelial growth factor (VEGF) in the nude mouse tumor,which has been treated by HIFU combined with nanoscale ultrasound molecular probes with HSV1-TK gene microvascular density.Methods Sixty nude mice were implanted with HepG2 Cells to establish subcutaneous transplanted tumor.Divided this mice into six groups at random after treated by HIFU:MB+ HSV-TK+ GPC3 (group A),MB + HSV-TK (group B),HSV-TK +GPC3 (group C),HSV-TK (group D),MB + GPC3 (group E),PBS (group F).They were injected into the tail vein every after 3 days.Mice in group A,B,D and E were exposed to ultrasound by 2 W/cm2,1 MHz,5 mintues and 0.2 mL ganciclovi(GCV) was intraperitoneally injected at the first 48 hours after injection.After the treatment,immunohistoche were used to detect the microvascular density(MVD),Western blot and immunohistoche was employed to test the protein change of the VEGF and HIF-1α,Q-PCR was used to test the mRNA gene transcription of VEGF and HIF-1α in the tumor tissues.Results After 14 days,the protein expression of HIF-1α and VEGF in group A was significantly lower than that in group B,C,D,E and F (P<0.05),the MVD level in the tumor is also like this,and the difference is statistically significant.Conclusion Anoscale ultrasound molecular probes with HSV1-TK can reduce the the level of VEGF,MVD and HIF-1α in the tumor which has been treated by HIFU,so it can inhibit tumor growth and improve the therapeutic efficacy after HIFU treatment.
4.Protoplasts isolation, purification and plant regeneration of Pinellia cordata.

The main factors which affected the isolation, purification and cultivation of Pinellia cordata protoplasts from leaves were studied. The results indicated that the optimum enzyme solution for P. cordata leaves was 13% CPW + 1.0% Cellulose +0.1% Pectolase, at pH 6.0, temperature (25-28 degrees C ) for 4 h. The sucrose density gradient centrifugation was adopted to purificate the protoplasts collected, when 25% sucrose was used as mediator, centrifugating at 500 rpm for 10 min. When the protoplasts were shallow liquid and liquid-solid double layer cultured on the medium of MS + 0.5 mg x L(-1) 6-BA + 0.25 mg x L(-1) NAA + 13% mannitol at the density of 2.5 x 104 protoplasts/mL, or fed and nursed cultured at the density of 100-500 protoplasts/mL, cell division could be observed for 3 days; granular calli appeared for 30 days. Calli was proliferated on the medium of MS + 0.5 mg x L(-1) 6-BA + 0.25 mg x L(-1) NAA solidified by 0.55% agar, and differentiated and regenerated after 5-6 months. Plant generation of P. cordata is successfully established.

5.Study of alveolar bone defects reparation by combination of bone marrow stromal cells and fibrin glue.

OBJECTIVETo explore the effects and feasibility of the repairing alveolar bone by combination of bone marrow stromal cells (BMSCs) and fibrin glue (FG).
METHODSBMSCs were obtained from the sucking mouse and then separated, cultured to the second generation to get ready for use. The available cells were combined with FG and then immediately implanted into the defect that had been established in one lateral maxilla of 30 SD rats. At the same time pure FG without BMSCs was implanted into the other side. The rats were killed at the 3rd and 6th week after operation. The hematoxylin-eosin staining and Micro-CT were used to observe and evaluate the osteogenesis.
RESULTSNew bone trabecula formed at experiment side of the rats at 3rd week. The repairing seemed better after 6th week of operation than the control side. There were only fibrous tissues in the control side or none.
CONCLUSIONThis experiment showed a feasible approach of repairing the defects in alveolar bone of the rats.

6.Application of micronucleus test of buccal mucosal cells in assessing the genetic damage of workers exposed to acrylonitrile.

OBJECTIVETo investigate the application of micronucleus test of buccal mucosal cells in monitoring the genetic effect of acrylonitrile in the population exposed to the acrylonitrile.
METHODSForty-one healthy male workers in a chemical factory in Shanghai were selected as the low concentration acrylonitrile exposed group while forty-seven healthy male workers in an acrylonitrile factory in Shanghai were selected as the intermediate concentration acrylonitrile exposed group. At the same time, thirty-one male workers who had no toxicant exposure and lived in the same community were selected as the control group. The micronucleus test in buccal mucosal cells and lymphocytes were used respectively for assessing the genetic damage status of these men.
RESULTSThe rate of micronucleus in buccal mucosal cells in both acrylonitrile groups (the low concentration group: 3.68% +/- 2.72%; the intermediate concentration group: 4.00% +/- 2.38%) was significantly higher than that in the control group (2.03% +/- 2.20%) (P < 0.05). The rate of micronucleus in the intermediate concentration group (4.23% +/- 3.34%) was significantly higher than that in the control group (2.48% +/- 1.46%) (P < 0.05). There was the correlation between the micronucleus test of buccal mucosal cells and the micronucleus test of the lymphocytes in the peripheral blood in the acrylonitrile exposed population (r = 0.299-0.359, P < 0.05).
CONCLUSIONThe micronucleus test of buccal mucosal cells replacing the micronucleus test of the lymphocytes in the peripheral blood can be used as one of the screening indexes in the surveillance of the genetic damage in the acrylonitrile exposed population.

7.A study on the influence of parental history of allergy on the level of cytokine in umbilical cord blood

Objective To explore the influence of parental history of allergy on the cytokine level of umbilical cord blood of neonates.Methods A prospective study was conducted which selected the pregnant women attending Yiwu maternal and child healthcare hospital from July to December in 2012 as objects.The umbilical cord blood sample were collected to detect the level of IgE,eosinophile granulocyte,eotaxin,IL -9,IL -6,IL -4,IL -5,IFN -γand IL -10. Results A total of 136 subjects were recruited and divided into three groups according to the status of parental allergy histories.There was no significant difference in social and demographic characteristics among the three groups (P>0.05). Three groups have significantly different median concentration of IL-13 (1.82pg/mL vs.3.24pg/mL vs.6.12pg/mL), eotaxin (29.33pg/mL vs.50.71pg/mL vs.60.15pg/mL),IL-9 (43.75pg/mL vs.111.17pg/mL vs.183.19pg/mL), IL-6 (11.49pg/mL vs.19.35pg/mL vs.26.09pg/mL),IL-4 (3.67pg/mL vs.21.27pg/mL vs.49.51pg/mL),IL-5 (4.26pg/mL vs.7.69pg/mL vs.10.66pg/mL),IFN-γ(338.65pg/mL vs.649.17pg/mL vs.834.14pg/mL),IL-10 (7.13pg/mL vs.10.56pg/mL vs.14.64pg/mL),P <0.05.Conclusion Parental history of allergy could have influence on cytokine level of umbilical cord blood.Early screening and surveillance in neonates whose parents have history of allergy should be taken to guide early interventions.

OBJECTIVETo evaluate the alterations in calcium metabolism of the vascular smooth muscle of rat thoracic aorta in the late phase of sepsis and to investigate the involvement of nitric oxide (NO)/cyclic-GMP(cGMP) signal transduction pathway in the sepsis-induced vascular hyporeactivity.
METHODSMale Sprague-Dawley rats were subjected to sepsis by cecal ligation and puncture (CLP). Eighteen hours post CLP, rat aortic rings were removed for measurement of contractile responses to vasoconstrictors by using organ bath technique.
RESULTIn endothelium intact aortic rings from CLP rats, concentration-contraction curves to phenylephrine (PE) and high KCl were significantly decreased when compared with those from control rats. The transient contraction induced by PE in calcium-free Krebs solution and the concentration-dependent contraction to CaCl(2)in KCl-depolarized medium were also markedly reduced. The hyporeactivity to vasoconstrictors was completely reversed by pretreatment either with aminoguanidine (AMG), a selective inducible nitric oxide synthase inhibitor, or with 1H [1,2,4] oxadiazolo[4,3-a] quininoxalin-1-one(ODQ), an inhibitor of NO-sensitive guanylyl cyclase.
CONCLUSIONA generalized impairment in calcium handling in vascular smooth muscle,including the calcium influx through the voltage-operated and receptor-operated channels and calcium release from intracellular calcium stores, is involved in vascular hyporeactivity during the late phase of sepsis. The NO/cGMP signal transduction pathway might be involved in this defect in vascular smooth muscle.

9.Pharmacokinetics of fudosteine in healthy volunteers.

AIMTo study the pharmacokinetics of fudosteine in healthy volunteers after the single and multiple dose administration.
METHODSThirty-six volunteers were divided into three groups randomly, each group included six men and six women. In the single dose design, the volunteers received either a single dose of 600 mg, 400 mg or 200 mg fudosteine. After a one-week wash out period, the volunteers of 400 mg group participated in the multiple dose design in which each volunteer received 400 mg fudosteine three times a day for five consecutive days. The plasma concentrations were determined by pre-column derivatization HPLC-FL method and the pharmacokinetic parameters of fudosteine were calculated.
RESULTSThe obtained pharmacokinetic parameters of fudosteine in single dose of 600 mg, 400 mg and 200 mg groups were as follows: T1/2 were (2.8 +/- 0.5), (2.7 +/- 0.5) and (3.2 +/- 0.6) h, respectively. T(max) were (0.51 +/- 0.22), (0.59 +/- 0.21) and (0.48 +/- 0.18) h, respectively. C(max) were (16 +/- 4), (11 +/- 3) and (6.1 +/- 1.5) microg x mL(-1), respectively. The AUC(0-10 h) and C(max) correlated linearly with doses, respectively (r > 0.99). The T(max), C(max) and AUC values of fudosteine in healthy male volunteers were smaller than those in female volunteers, and the T1/2 value was longer than that in female volunteers. The obtained multi-dose pharmacokinetic parameters of fudosteine were as follows: C(ss) was (4.1 +/- 0.8) microg x mL(-1); DF was 3.0 +/- 0.7; T1/2 was (2.5 +/- 0.4) h; T(max) was (0.6 +/- 0.3) h; C(max) was (13.2 +/- 1.3) microg x mL(-1).
CONCLUSIONThe values of pharmacokinetic parameters in healthy volunteers were linear in the range from 200 mg to 600 mg. Statistic analysis results showed that the differences of AUC and C(max) between men and women were not resulted from sexual differences, but from the weight differences. There was no significant difference in pharmacokinetic parameters between single dose and multi-dose.
