1.Investigation and Analysis of Rational Use of Drugs in Outpatients with International Selected Drug Use Indicators as Benchmark

OBJECTIVE: To investigate the drug use involved in outpatient's prescriptions in a hospital in 2007.METHO-DS: With the International Selected Drug Use Indicators and defined daily dose as indexes,the drug use involved in a total of 5 029 outpatient's prescriptions randomly selected from a hospital were analyzed statistically based on Excel sheet.RESULTS: Of the total 5 029 outpatient prescriptions,60.83% paid the expenses at one's own expense,and the average age of the patiens were 46.9 years.The average number of drugs prescribed for patient's each visit was 2.63 kinds,and the percentages of antibacterial agents,ampuls,and prescriptions of drugs prescribed in generic names were 21.79%,10.44%,and 50.47%,respectively.Quinolones,cephalosporins and macrocyclic lactones ranked at the first three places in DDDs and consumption sum.The drug utilization indexes of cephalosporins and penicillins were on the high side.23.72% of antibacterial agents used in combination and 76.28% alone,20.07% in bigeminy.The antibacterial agents used in trigeminy or more than 3 kinds in combination were all for antiuberculotic treatment.Physician's signature rate after prescription revision was 60.19%,and the rate of prescriptions in which medicines for oral administration or for external use were prescribed alone was 32.88%.CONCLUSION: The drug use for the outpatients in the hospital is rational on the whole,but measures should be taken to tighten control on the drug use by strenthening physicians' consiousness on rational drug use so as to reduce the incidence of irrational drug use.
2.Comparison of tamsulosin versus nifedipine for the management of lower ureteral stones

Objective To compare the efficacy of tamsulosin and nifedipine for the adjunctive expulsive therapy in patients with lower ureteral stones. Methods A total of 180 patients with stones (0.4-1.0 cm in diameter) located in the lower ureter (juxtavesical or intramural tract) were randomly divided into 3 groups (60 cases in each group). Group 1 served as controls; group 2 received nifedipine (10mg, 3 times daily) ; and group 3 received tamsulosin (0.4 mg, once daily). All patients were observed for 2 weeks. Results During 2 weeks, expulsion of stones was observed in 26 cases (43. 3% ) of group 1, in 44 (73. 3% ) of group 2, and 49 (81. 1% ) of group 3. The difference in expulsion rates between groups 2, 3 and group 1 was statistically significant (P 0. 05). Objective To study the clinical characteristics of gastrointestinal (GI) hemorrhage in SARS and factors affecting its occurrence. Methods Retrospective study on clinical data of SARS cases complicated with gastrointestinal hemorrhage admitted to our hospital. Results Of 220 SARS cases gastrointestinal hemorrhage occurred in 20 patients (9% ) , with 13 males, 7 females, age ranged 19 -83 years (50?20) , onset at the 5th to 35th day of illness (22?9) , hematemesis/melena in 5 cases, positive occult blood test in 15. After treated with famotidine, omeprazole, reptilase and thrombin bleeding stopped in 16 cases within 7 to 10 days. Four cases died of respiratory failure. 5.Clinical outcome of cervical arthroplasty in the treatment of spondylotic myelopathy

[Objective]To verify the clinical outcome of cervical arthroplasty in the treatment of spondylotlc myelopathy exclusively.[Method]From January 2004 to August 2003,twelve spondylotic myelopathy patients who underwent cervical arthroplasty were included in this study.Preoperative and final follow-up Nuriek grade,Oswestry neck disability index(ONDI)score,neck and arm pain VAS score of patients were recorded and compared.Surgical outcome was also measured using odom' s criteria.[Result]100% of patients had good or excellent outcomes using Odom' s criteria,while the final follow-up Nurick grade,ONDI score,neck and arm pain VAS score were improved significantly compared with the preoperative valule.[Conclusion]Cervical arthroplasty is effective in the treatment of spinal cord compression secondary to spondylotic disease with interveterbral movement being preserved.
6.Relationship between MDR1 exon 26 genotype and dosage of FK506 and acute rejection following renal transplantation

Objective To evaluate whether the MDR1 exon 26 genotype is associated with the dosage of FK506 and acute rejection in renal transplant patients. Methods One hundred and six patients who administrated FK506 after renal transplantation were retrospectively analyzed. The patients were investigated for MDR1 genotype determined by polymerase chain reaction and restriction fragment length polymorphism (PCR-RFLP) assays. According to the genotype, the patients were divided into 3 subgroups, wild homozygous-CC, mutant homozygous-TT and heterozygous-CT. At 3rd, 6th and 12th month after renal transplantation, enzyme-linked immunosorbent assay (ELISA) was used to measure the FK506 whole blood concentration. Whole blood FK506 concentration per dose/kg/day and acute rejection within one month postoperation were compared among 3 subgroups. Results Of the 106 patients, 32 (30.2 %), 44 (41.5 %) and 30 (28.3 %) were carriers of CC, CT, TT in MDR1 exon 26, respectively. 7.Construction of eukaryotic vector for HN gene of NDV Italien strain and its expression

Objective To construct an eukaryotic expression plasmid containing gene coding for the hemagglutinin-neuraminidase(HN)of newcastle disease virus(NDV)oncolytic strain Italien,and then to express the protein in eukaryotic cell.Method The HN cDNA was synthesized from viral RNA by RT-PCR,and the eukaryotic expression vector of HN gene(named pcDNA3.1-HN)was constructed.The vector pcDNA3.1-HN was transfected into CHO-K1 cell by liposome,and G418 was used to select stable clones expressing HN gene.The expression of HN protein was visualized by Western blot and Immunofluorescence microscopy.Results Restriction analysis and DNA sequencing proved that HN gene was correctly cloned into expression vector.Western blot analysis and immunofluorescence showed that the HN was expressed in CHO-K1 cells.Conclusion The HN cDNA of NDV was successfully cloned into eukaryotic vector which showed good expression of HN protein in CHO-K1 cells.
8.Nerve cells injury in hippocampal region after focal cerebral ischemia-reperfusion for rats with diabetes

Objective To understand the effect of diabetes on the nerve cells injury in hippocampal region by observing the nerve cells changes in this region of ischemia after focal cerebral ischemia reperfusion for rats with diabetes.Methods Sixty healthy male Wister rats were randomly divided into four groups:the control group,sham operation group,normal cerebral ischemia-reperfusion(NIR)group,and diabetic cerebral ischemia-reperfusion(DIR)group.The model of local cerebral ischemia and reperfusion was established by the thread-embolism and cerclage of rats with STZ-induced diabetes.Hematoxylin-eosin(HE)-stained neurons in hippocampal CA1 region were observed by selecting 8 un-overlapped fields under light microscope(?400);the total number of the normal pyramidal neurons with whole cellular organ structure were counted within 100 ?m length in CA1 region.The numbers of positive neurons were presented as mean with standard errors(?s).Significant differences between groups were analyzed by t-test.Results Neuronal loss or apoptotic cells were not found in the control group or sham operation group after observing under light microscope.There were neuronal losses in both NIR and DIR groups,neuronal loss of DIR group being more serious than that of NIR group(P
9.Interleukin-18 upregulates FasL protein expression and its function in colon cancer cell line.

Objective To investigate the modulation of FasL protein in colon cancer cells by interleukin 18 (IL 18) and it's effect on tumor cells counterattacking T lymphocytes. Methods By using Western blotting assay, FasL protein expression was examined in human colon cancer cell line SW620. The ratio of apoptosis of lymphocytes induced by tumor cells was studied by flow cytometry analysis. Results 10 ?g/L and 100 ?g/L IL 18 upregulated FasL protein expression in SW620 cell line, reaching the peak in 18 and 36 hours respectively, and returned to initial levels in 72 hours. The ratio of apoptosis of Jurkat cells induced by tumor cells increased in accordance with the upregulation of FasL protein. Conclusions IL 18 upregulates FasL protein expression in colon cancer cell line and enhances the ability of cancer cells to counterattack T lymphocytes.
