1. New Quality Problems and Control Methods for Imported Draconis Sanguis

OBJECTIVE: To explore the new quality problems of Draconis Sanguis imported from Indonesia. METHODS: According to the quality standard of imported medicinal materials and Ch.P method, exploratory analysis was carried out on 10 batches of imported Draconis Sanguis and three batches of Draconis Sanguis fruits. RESULTS: The dracohodin content in three batches of Draconis Sanguis fruits was 1.2%-2.4%. Three batches of imported Draconis Sanguis were obtained by directly pulverization of Draconis Sanguis fruits, and loureirin A and loureirin B were detected in one batch of Draconis Sanguis, suggesting that dracaena cochinchinensis was added. CONCLUSION: Among the 10 batches of imported Draconis Sanguis, three batches are determined to be unqualified due to character inconformity, and the other batch is judged to be unqualified due to the addition of dracaena cochinchinensis.

Objective To evaluate the of feasibility of Roche e601 for detecting hepatitis B virus surface antigen(HBsAg).Meth-ods To evaluate the feasibility of Roche e601 for detecting hepatitis B virus surface antigen(HBsAg).Results The coefficient of variation(CV)of inter-run and between-run from low and high value specimens was lower than the requirements of manufacturers;the negative and positive coincidence rates in detecting 60 external quality assessment controls were 100%(40/40)and 100%(20 /20),respectively;the detection threshold values of the system was 0.01 IU/mL;the results detected by this detecting system in 40 samples,in which the concentration of HBsAg was 0.8
3.Study on the molecular characteristics of multidrug-resistant Acinetobacter baumannii

Objecfive To investigate antibiotic resistance,carbapenemase genotype and the molecular epidemiology of multidrug-resistant Acinetobacter baumannii (Aba) collected from 3 military hospitals in China.Methotis The minimum inhibitory concentrations (MIC) were examined by ager dilution method.Genotypes of carbapenemases were amplified by multiplex PCR and its products were sequenced.PCR was used to detect per gene,Homology of the resistant isolates was analyzed by pulse-field gel electrophoresis(PFGE).Results Among the 64 MDRA strains,78.1%(50)strains possessed blaOXA-23 gene,89.1%(57) carried Class 1 integrase gene,39.1% (25) with blaPER-1 gene,and 1 strain with blaOXA-58-like gene.PFGE showed that 13(A,B,C,D,E genotype) different clones were identified in these strains.A,B,and U clones were the predominant clones in three hospitals,respectitively.Conclusion OutbreaEs of muitidrug-rcsistant Aba occurred at 3 military hospitals with the most prevalent carbapenemase as OXA-23 enzyme.OXA-58 type of carbapenemase and per-1 in Aba were also isolated.
4.The analysis of different treatment methods for 184 cases of soft tissue sarcoma.

OBJECTIVETo study the proper way of using combined postoperative chemo-radiotherapy and prognostic factors of soft tissue sarcoma.
METHODSThe clinical data of 184 patients were retrospectively reviewed. These patients were devided into surgery group (S, 94 patients), surgery plus postoperative radiotherapy group (S + R, 62 patients) and surgery plus chemotherapy group (S + C, 28 patients).
RESULTSThe 5-year survival rates of S, S + R and S + C groups were 39.4%, 48.4% and 28.6%, respectively. Combined multitherapy was the key to improve survival rate and life quality. Clinical stage, pathological type and therapeutic method were also important prognostic factors for the long term survival.
CONCLUSIONSurgery plus postoperative radiotherapy can improve the 5-year survival rate of soft tissue sarcoma.

5.Correction of the epicanthal fold and angulus oculi using the Z-epicanthoplasty.

OBJECTIVETo explore a new procedure for aesthetic correction of the medial epicanthal fold aim at the etiopathogenesis.
METHODSThe new Z-epicanthoplasty devise the upper and inferior margin of angle of eye medial as one angle of the Z.
RESULTSFrom 2004 to 2006, 129 patients were treated by using the method. Follow-up 6 to 24 months, all patients were satisfied by eliminating the medial epicanthal fold without obvious scar.
CONCLUSIONSThe method is more effect than traditionally Z-plasty. Our technique is a simple, advanced procedure that can be performed widely.

6.Analysis of potential risk factor of plague in Wanzhou section of the There Gorges Reservoir area

Objective To find out the potential risk factor of plague in Wanzhou section of the Three Gorges Reservoir area, and to provide scientific basis for prevention and control of plague. Methods Rodents were captured by rat traps/cages at night and identified into species in Wanzhou section of the Three Gorges Reservoir area from 2001 to 2009. Flea was counted and serum antibodies against plague F1 of rats, cats and dogs were detected by indirect hemagglutination (IHA). Plague surveillances were performed in human beings and rats. Results The rodents captured belonged to 9 species, 2 families, 2 orders and 1 classes. The average indoor rodent density was 1.16% (961/82 558), and was 1.12% (1345/119 671) outdoors. Rattus norvegicus was the dominant species,accounting for 50.37%. The proportion of R. Flavipectus was 3.80% in 2004, 4.50% in 2008 and 10.12% in 2009,showing an increasing trend year by year. There were three kinds of mice infected fleas in Wanzhou, which including Xenopsylla cheopis, Leptopsylla segnis and Ctenocephalides felis. The average rate of flea infected mice was 1.18%(82/6959) and the total flea index was 0.036. No F1 antibody against plague was detected in 6959 dogs and 160 cats serum samples. Conclusions No plague is found in Wanzhou section of the Three Gorges Reservoir area. But R.Flavipectus, Xenopsylla cheopis and Leptopsylla segnis are dominant species in Wanzhou section, and the proportion of which shows an increasing trends year by year. There is a potential risk of plague outbreaks in Wanzhou section of the Three Gorges Reservoir area.
7.Nosocomial bloodstream infection in patients caused by Staphylococcus aureus: drug susceptibility, outcome, and risk factors for hospital mortality.

BACKGROUNDPrevious studies have different viewpoints about the clinical impact of methicillin resistance on mortality of hospital-acquired bloodstream infection (BSI) patients with Staphylococcus aureus (S. aureus). The objective of this study was to investigate the mortality of hospital-acquired BSI with S. aureus in a military hospital and analyze the risk factors for the hospital mortality.
METHODSA retrospective cohort study was performed in patients admitted to the biggest military tertiary teaching hospital in China between January 2006 and May 2011. All included patients had clinically significant nosocomial BSI with S. aureus. Multivariate Logistic regression analysis was used to identify the risk factors for hospital mortality of patients with S. aureus BSI.
RESULTSOne hundred and eighteen patients of more than one year old were identified as clinically and microbiologically confirmed nosocomial bacteraemia due to S. aureus, and 75 out of 118 patients were infected with methicillin-resistant S. aureus (MRSA). The overall mortality of nosocomial S. aureus BSI was 28.0%. Methicillin resistance in S. aureus bacteremia was associated with significant increase in the length of hospitalization and high proportion of inappropriate empirical antibiotic treatment. After Logistic regression analysis, the severity of clinical manifestations (APACHE II score) (odds ratio (OR) 1.22, 95% confidence interval (CI) 1.12 - 1.34) and inadequacy of empirical antimicrobial therapy (OR 0.25, 95%CI 0.09 - 0.69) remained as risk factors for hospital mortality.
CONCLUSIONSNosocomial S. aureus BSI was associated with high in-hospital mortality. Methicillin resistance in S. aureus has no significant impact on the outcome of patients with staphylococcal bacteremia. Proper empirical antimicrobial therapy is very important to the prognosis.

8.Security evaluation of exosome derived from human umbilical cord mesenchymal stem cells

Objective To explore the security of exosome derived from human umbilical cord mesenchymal stem cell(huc-MSC-exosome)as a kind of biological product on clinical application.Methods huc-MSC-exosome were separated by ultea centrifuge. CD9 and CD63 expression of huc-MSC-exosome were tested by Western-blot.Nanoparticle analysis was used to detect the size and concentration of huc-MSC-exosome.Hemolytic experiment,systemic anaphylaxis and blood routine test were used to verify the self-safety of huc-MSC-exosome.Results huc-MSC-exosome expressed specific markers of exosome,CD9 and CD63.Moreover,huc-MSC-exosome cannot result in hemolysis,systemic anaphylaxis reaction and abnormality of blood routine test.Conclusion Exosome derived from human umbilical cord mesenchymal stem cell has general signs of exosomes and possesses the certainly security,which may provide the experimental evidence for huc-MSC-exosome clinical application.
9.Transmission and molecular characteristics of carbapenem-resistant Acinetobacter baumannii.

OBJECTIVETo study the mode of transmission and molecular characteristics on carbapenem-resistant Acinetobacter baumannii strain. Strains were isolated from different parts of samples in various patients.
METHODSClinical information of carbapenem-resistant Acinetobacter baumannii isolates were stored and analyzed by WHONET 5.4 software. The transmission and pathopoiesis of the strains were learned through case file review. Genotypes of isolates were identified by pulse-field gel electrophoresis (PFGE) and genes of carbapenemase were detected by multiple PCR, in order to find molecular characteristics and relatedness between strains.
RESULTS29 stains of Acinetobacter baumannii resistant to carbapenem were isolated from 2 or more kinds of samples among 13 patients'. Two genotypes were identified by PFGE: genotype A was obtained from 22 isolates in 11 patients and genotype B was obtained from 7 isolates in 4 patients. PCR amplification showed that all strains possessed OXA-23 gene except 1, and all strains possessed Integrase gene I except 3.
CONCLUSIONThere were 2 different genotypes from 29 strains of carbapenem-resistant Acinetobacter baumannii with Genotype A as the main type. OXA-23 carbapenemase gene and integrase gene I were detected from most of the isolates. All the strains could be easily transmitted in the body of the patients and among patients, hence becoming the epidemic pathogen of iatrogenic infection.

10.The loss of interproximal contact between posterior fixed implant prostheses and adjacent teeth:a retrospective study

Objective To investigate the interproximal contact(IC) loss rate and the associated risk factors of IC loss between posterior fixed implant prostheses and the adjacent teeth.Methods Ninety-seven posterior fixed implant prostheses including 165 IC in 78 patients(34 men, 44 women) between 23 and 76 years of age were investigated.The period after restoration ranged from 1 to 44 months.The IC between implant prostheses and adjacent teeth was evaluated using a 30-μm-thick contact gauge and dental floss, and the IC was regarded as lost when the gauge or dental floss was inserted into an IC without any resistance at follow-up visits.The state of IC, region of implant prostheses, the condition of opposing dentition, the vitality of the adjacent teeth and the time interval of tooth loss were also confirmed for correlation analysis.Results All 78 patients finished follow-up examination.Forty-eight of 165 IC(29%)were regarded as lost.The IC loss rate at the mesial aspect[75%(36/48)] was significantly greater than that at the distal aspect[25%(12/48)](x2=7.344, P=0.007).The difference between IC loss rate in the maxilla[46% (22/48)] and that in the mandible[54%(26/48)] was not significant(x2=2.992, P=0.084).Free-end[hazard ratio (HR)=2.870], and the time interval of tooth loss(HR=0.919) affected the loss of IC significantly(P<0.05, Cox regression analysis).Conclusions Loss of IC between posterior fixed implant prostheses and adjacent teeth was one of biological complications, which was observed frequently at follow-up visits.Patients who have the free-end fixed implant prostheses, or undergo short time interval of tooth loss before restoration may have high risk of the IC loss.
