1.Application of Medial Collateral Ligament Release Technique in Knee Arthroscopic Surgery for Medial Tibiofemoral Compartment Tightness

Because of the factors such as degeneration or congenital dysplasia which leads to medial tibiofemoral compartment tightness, the arthroscopic surgery of this part might have limited vision and narrow operating space, which causes missed diagnosis and iatrogenic injury.Medial collateral ligament ( MCL) release technique is a simple, effective and safe procedure.By releasing part of the ligament fiber, this operation can effectively expand the visual field and operation space.avoiding the risk of iatrogenic injury of cartilage and missed diagnosis.MCL release technique is an effective method to solve the arthroscopic examination and endoscopic operation for medial compartment tightness of the knee joint.This article reviewed the technique of MCL release.
2.Clinical Observation on Termination of Pregnancy Effects of 75 Patients Applied with Guizhi Fuling Capsule Combined with Mifepristone

Objective To observe the effects of termination of pregnancy with Guizlfi Fuling Capsule plus Mifepristone.Methods 75 early pregnant women were randomly recruited into a treatment group and a control group.Women in the control group were applied with Mifepristone plus Misoprostoi.Form the 3rd day,women in the treatmcnt group were applied wilh Guizhi Fuling Capsule(3 times daily,and 3 granules for each time)for successive 7 days oil the basis of medicines used in the control group.Compared and analyzed the therapeutic effects of both groups.Results The average time for ovum evacuation in the control group and the tretment group was(6±1.8)h and(3±2.6)h separately.Besides both bleeding volume and bleeding time in the Izeatment group were lower than the eonlxol group.Conelusion Guizhi Fuling Capsule plus Mifeoristolle therapy will shorten ovum evacuation time and decrease bleeding volume.
4.Genetic Association Research between Mutation in CYP1A2 Gene and Erxian Soup Response

Objective To investigate the genetic association between mutation in cytochrome P450 1A2 (CYP1A2) gene and Erxian suop response. Methods Forty-eight patients with male climacteric syndrome of both Yin and Yang deficiency received a treatment of erxian suop with three months. The severity of symptoms and responses to Erxian suop were quantizated by the method of Department of Psychology in Istanbul Bosphorus University and Clinical Research Guidance Principle on New Drug of Traditional Chinese Medicine published by China. A single nucleotide polymorphism (SNP) at position 2964 (G-A) of CYP1A2 gene were identified by polymerase chain reaction (PCR) in order to analyse the relationship between Erxian response and CYP1A2 single nucleotide polymorphism. Results Statistical analysis showed there was no significant difference among genetype and genetype distribution of CYP1A2 G2964A between the patients and control group. The genotype of the 16 patients among 32 effective patients was G/G (56.25%). The genotype of the 11 patients among 16 ineffective patients was G/A (68.75%). There was no significant difference in symptoms, physical sign between the two groups at the pre-treat and post-treat. Also the two groups had no significant difference among the score of major symptoms and hyposymptoms. Conclusion The patients with genotype of G/G have higher efficiency, while with genotype of G/A have lower efficiency among patients with CYP1A2 G2964A gene when treated with erxian soup.
5.Application of transthoracic echocardiography in interventional treatment of congenital heart diseases

Objective To study the value of transthoracic echocardiography (TTE) in interventional treatment of congenital heart diseases. Methods TTE was used to identify patients with atrial septal defect (ASD, n=26), ventricular septal defect (VSD, n=23) or ductus arteriosus (PDA, n=16). They received interventional treatment with domestic made nitinol occluder. The TTE was then used to monitor the entire process of the surgery and to assess the effect of the surgery. Results The treatment with indigenous nitinol occluder failed in five patients, including two with ASD, two with VSD, and one with PDA. The other sixty-one patients were freated successfully with the occluder. Once the procedure was completed, those patients were found to have neither abnormal valvular regurgination as shown by TTE, nor any shunt flow around the occluder as shown by both TTE and angiocardiography. One month later, TTE demonstrated that the position of all the occluclers remained stable, and no residual leakage or shunt was detected by TTE in all of the sixty-one patients, and the same results were obtained after three months. However, two days after the surgery, ECG showed that three of the patients with VSD suffered from different degrees of atrioventricular block on the second postoperative day, but they eventually recovered after treatment. The successful rate of the procedure was 93.8% (61/65). Conclusions Transthoracic echocardiography may play an important role at three stages (pre-, during- and after), i.e. to select the most suitable patients before the procedure, accurately identify the location, and release of the occluder at the end of the operation; and finally, it serves as a promising method to evaluate the treatment result in patients during their follow-up period.

Objective To study the effects of hypoxic preconditioning on the Fos and Jun expression and apoptosis in cultured rat hippocampal neurons after anoxia\|reoxygenation. Methods The hippocampal neurons cultured for 12d from control and hypoxic\|preconditioning groups were exposed to anoxia environment (0\^90?l/L N\-2+0\^10?l/L CO\-2) for 4h and than reoxygenated for 24h and 72h.The expression of Fos and Jun were revealed immunocytochemically using the antiserum against Fos and Jun.The apoptosis was examied using the terminal deoxynucleotidyl transferese\|mediated biotinylated deoxyuridine triphosphate nickel and labeling(TUNEL) method and flow cytometric analysis. Results The increased of percentage of Fos and Jun positive neurons and apoptosis neurons in cultured hippocampal neurons after anoxia\|reoxygenation than those in befor anoxia.The precentage of Fos and Jun positive neurons and apoptosis neurons were less in the hypoxic\|preconditioning group than that in control aftre anoxia\|reoxygenation.Conclusion\ Hypoxic\|preconditioning increases tolerance of hippocampal neurons to anoxia and decreases the precentage in Fos and Jun expressing neurons and apoptosis neurons in hippocampus aftre anoxia\|reoxygenation in vitro. \;[
7.Effects of alendronate on trabecular bone structural properties

Objective To evaluate alendronate with regards to its effect s on trabecular bone structural properties. Methods Alendronate was administered at a daily p.o. dose of 0.5 mg?kg-1?d-1 over a 12 weeks period in Hound do gs (n=8 + 8 for control and treated group), the structural indices of the lumba r vertebral (L 1 and L 2) trabecular bone were assessed directly from 3 D image s. Results Treatment with alendronate increased bone volume fraction by 9.5% a nd 7.7% respectively, in the L 1 and L 2. Trabecular thickness significantly in creased after alendronate treatment, whereas trabecular separation remained cons tant. The degree of anisotropy for the alendronate treated group was decreased compared with that of the control group. Bone surface to volume ratio declined s ignificantly in the alendronate treated group, whereas alendronate induced a hi gher value of bone surface density. Conclusion Alendronate increased the struct ural properties of canine trabecular bone after short-term treatment at a dose of 0.5 mg?kg-1?d-1. Thus, alendronate could increase the structural properti es of healthy trabecular bone in a short-term period.
8.Biological response modifiers combined with radiotherapy and chemotherapy for stage Ⅲ non-small-cell lung cancer

Purpose:To evaluate the effect of biological response modifiers for Ⅲ stage non-small-cell lung cancer.Methods:Fifty-nine patients were randomized into two groups.Twenty-nine patients were put into radiotherapy and chemotherapy(R+C) groups,thirty patients were into Biological response modifiers and radiotherapy chemotherapy(BRMS+R+C) groups.All patients received routine radiotherapy 2.0 Gy per day,5times a week to a total dose of 60-65 Gy,in 42-45 days.Radiation fields just covered clinical tumors.Chemotherapy consisted of cisplatin 25-30 mg/m 2 ,etoposide 50-70 mg/m 2 on days 1?2and 3 every 4 week,for squamous cell carcinoma.For adenocarcinoma,it was mitemycin 4-6 mg/m 2 on day1 and vindesine 3 mg on day1 and8,and cisplatin25-30 mg/m 2 on day1?2 and 3,every 4 weeks interferon-?(IFN-?)and interleukin-2(IL-2)were used in(BRMS+R+C)groups after the second chemotherapy was over. IFN-? was given on days 1?2?3 and IL-2 on days 2?4?6 to a total of forty-eight day.Results:The overall response rate was 51.7% and 76.6% in(R+C) and(BMRS+R+C) groups.The one.two year survival rates were 62.1% 17 3% and 83 3%,43 3% in (R+C) and (BMRS+R+C) groups respectively.Conclusions:The preliminary results from our study has shown that biological response modifiers combined with radiotherapy and chemotherapy for non -small cell lung cancer has better early response rate and survival rate.
9.Acute non-isovolemlc hemodilution during operation In the elderly patients

35 % and the intraoperative blood loss was expected to be 800-1 600 ml. The operations were performed under combined general-epidural anesthesia. General anesthesia was induced with fentanyl 2-4 ?g?kg-1 , thiopentone 5 mg?kg-1 and succinylcholine 1.5mg? kg-1 and maintained with isoflurane inhalation and intermittent iv boluses of vecuronium. Epidural anesthesia was performed at T5-6 (thoracic surgery), T10-11 (upper abdominal surgery) or L3-4 (lower abdominal surgery) . A mixture of 0.1% lidocaine +0.2% dicaine was used for epidural block during operation. The patients were randomly divided into 2 groups: (1) ANIH group ( n = 18) and (2) AHH group ( n = 20) . Blood volume (BV) was calculated according to following formula: BV (ml) (male) = Height (cm)? 28.5 + Body weight (kg) ? 31.6 - 2 820,BV(ml) (female) = Height(cm)? 16.25 + Body weight (kg) ? 38.46 - 1 369. 1 000-1 200 ml of 6% HES (200/0.5) and 500 ml of lactated Ringer's solution (about 25%-30% of BV) were infused at a rate of 50ml?min-1 when induction of anesthesia was started in both groups. In group I (ANIH) 400-600 ml of blood (about 10%-15% of BV) was removed through radial artery before induction of anesthesia. The removed blood was reinfused at the end of surgery. Vital signs (BP,HR,CVP and ST-T changes) , Hct, oncotic pressure and arterial blood lactate concentration were monitored during operation. Results The vital signs were fairly stable during perioperative period in both groups. Transient hypotension developed in 16.7% (3/8 in group Ⅰ ) and 15.0% (3/20 in group Ⅱ ) of patients during induction of anesthesia. CVP was significantly elevated in both groups but the increase in CVP was significantly higher in group Ⅱthan that in group Ⅰ . In group Ⅰ (AN1H) moderate hemodilution was achieved (Hct = 29.9% 2.9%) while in group Ⅱ (AHH) only mild hemodilution was achieved (Hct = 32.9% ?2.9%) .Hct was significantly higher in group Ⅰ(31.5% ?5.1%) than that in group Ⅱ (27.7% ?3.6%) at the end of surgery. Blood loss was comparable between the two groups, and oncotic pressure and blood lactate level were within normal range in both groups. Conclusion ANIH is more effective and safer than AHH without obvious adverse effects, and can avoid exceedingly high CVP commonly seen in AHH. ANIH is a hemodilution technique of choice in the elderly patients.
